Preoperative evaluation of P53 and bcl-2 over expression in clinical stage 1 endometrial carcinoma and their correlation with surgico-pathological data and prognosis of patients

Introduction: P53 and bcl-2 over expression was reported to affect the biology and prognosis of patients endometrial carcinoma. Methods: This prospective study included 38 patients with histologically confirmed endometrial carcinoma and staged clinically as stage I. Immunohistochemical staining of the tumor specimens obtained by dilatation and curettage (D&C) with P53 and bcl-2 monoclonal antibodies was done. The surgical, pathological and follow up data of all patients were studied. Results: There were 18 cases (47.4%) with positive P53 over expression. P53 over expression was found to be associated with increasing grade of differentiation, advanced stage, and non endometrioid type (significant), and increased depth of myometrial invasion (non-significant). It was associated with more recurrence rate (22% versus 15%) and shorter mean survival time (33.9 versus 29.2 months). Bcl-2 expression was present in24 cases (63.2%) of the studied group. There was significant decrease in bcl-2expression with poorly differentiated, advanced stage, increased depth of myometrial invasion, and non-endometrioid type. It was not significantly correlated with recurrence rate and mean survival time. Conclusion: P53 over expression in the D&C specimens was associated with adverse surgicopathological criteria, increased mortality rate, and shorter survival time in patients with endometrial carcinoma. A significant decrease in bcl-2 expression was associated with adverse surgicopathological criteria, but it was not significantly correlated with prognosis of the patients.

Impact of sea surface Temperature on COsMO Forecasts of a Medicane over the Western Mediterranean sea

The paper describes and analyzes the sensitivity of an operational atmospheric model to different ssT （sea surface temperature） estimates. The model＇s sensitivity has been analyzed in a Medicane （Mediterranean hurricane） test case. Numerical simulations have been performed using the COsMO （consortium for small-scale modeling） atmospheric model, in the COsMO-ME configuration. The model results show that the model is capable of capturing the position, timing and intensity of the cyclone. sensitivity experiments have been carried out using different ssTs surface boundary conditions for the COsMO forecasts. Four different experiments have been carried out： the first two using ssT fields obtained from the OsTIA （operational sea surface temperature and sea ice analysis） system, while the other two using the ssT analyses and forecasts from MFs （Mediterranean Forecasting system, Tonani et al., 2015; Pinardi and Coppini, 2010）. The different boundary conditions determine differences in the trajectory, pressure minimum and wind intensity of the simulated Medicane. The sensitivity experiments showed that a colder than real ssT field determines a weakening of the minimum pressure at the vortex center. MFs ssT analyses and forecasts allow the COsMO model to simulate more realistic minimum pressure values, trajectories and wind speeds. It was found that MFs ssT forecast, as surface boundary conditions for COsMO-ME runs, determines a significant improvement, compared to AsCAT observations, in terms of wind intensity forecast as well as cyclone dimension and location.

AIM: To investigate the clinical performances of rapidstool test (ImmunoCard sTAT HpsA, Meridian DiagnosticInc.) in the evaluation of eradication therapy of H pyloriand to compare it with a well-known and validatedlaboratory stool test (Amplified IDEA Hp stAR, Dako).METHODs: stool samples of 122 patients wereevaluated after eradication therapy of H pylori. H pyloristatus was assessed by 13C-urea breath test (UBT).stool specimens were tested using either the rapidimmunoassay kit or the laboratory immunoassay ***: Forty-three patients were infected and 79non-infected. sensitivity and specificity of ImmunoCardsTAT and Hp stAR were 58.14% and 76.4%, and97.47% and 98.73%, respectively (P > 0.05). Overallagreement between the two tests was 92.6% (113 of122 cases).CONCLUsION: ImmunoCard sTAT seems to haverather low performances, and it cannot be regarded as areliable tool in the post-treatment setting. Also Hp stARcannot be recommended to confirm H pylori eradicationafter treatment.

A halotolerant, gram positive, motile and rod, was isolated from the E1 Gol6a lake of Ghardaia, Alegria and screened for its antimicrobial potential. The strain showed an inhibitory effect to gram positive and gram negative bacteria against Cladosporium spp. and a slight inhibitory effect against C. albicans using chloroform as the extraction solvent and the nutrient broth as production medium. 16s rRNA sequencing showed that the isolate named LMB3981 is a new taxon in the family of Bacillaceae （with 96% similarity）. The strain is close to Filobacillus milosensis and Bacillus haloalkaliphilum with 95% similarity. The phenotypic study showed differences between LMB3981 and two strains that are near and have confirmed the results of 16s rRNA sequencing by specific metabolic properties of the strain.

Objective:?The aim of this study is to assess any potential relationship between perifollicular vascularity and occurrence of pregnancy in cases of stimulated IUI cycles using the subjective grading system by 2D transvaginal power Doppler ultrasonography.?Design: A prospective cross sectional cohort study. Method: This is a prospective cross-sectional cohort study of 90 stimulated IUI treatment cycles. selected women were prescribed clomiphene citrate combined with highly purified urinary follicle stimulating hormone. All patients underwent serial transvaginal ultrasound scans starting from day 6 to 7 of the cycle. Perifollicular Doppler blood flows were assessed in dominant follicles ≥18 mm. The patients then were categorized into 3 groups (high vascularity group {G3 & G4}, low vascularity group {G1 & G2} and mixed grades group). Other parameters measured included number of follicles ≥ 18 mm in both ovaries, endometrial thickness and estradiol (E2) level . Human chorionic gonadotropin (hCG) injection 10,000 IU IM was given to the patient when the dominant follicle reached 18 mm in diameter. At that time, the endometrium was evaluated as regards endometrial thickness. IUI was carried out using prepared/“washed” semen (husband). All patients received luteal support in the form of progesterone from day of IUI for 14 days. serum Β-hCG was estimated 2 weeks after insemination. Results: In this study, from all 90 cases only 8 cases got pregnant with pregnancy rate of 8.88% (6 cases got pregnant in high grade vascularity group;2 cases in mixed grades group and no cases got pregnant?in low grade group). There was statistically significant difference among the 3 groups as regarding?the pregnancy rate (P value = 0.02). There is statistically significant difference in perifollicular resistance index (RI) and pulsatility index (PI) between pregnant and non pregnant cases (P value = 0.016 and 0.047 respectively). study objective： to verify if the PAA （angle of the pubic arch） is a predictive data for the mode of delivery, the duration of labour and the expulsion period. Methods： we chose to measure the PAA in 100 women recruited, chosen from the 38th to the 42th week of gestation. The method involves the use of two-dimensional transperineal ultrasound. Data collection took place over a period of time from May to september 2015. The ante-partum data taken into consideration were the patient’s name and surname, PAA, parity, age, height; those acquired in the post-partum provide for the calculation in minutes of the active phase of labor, the duration of the expulsion period, the execution of the amniorrhexis and the use of the synthetic oxytocin, the fetal cranial circumference, the fetal weight and the position of the presented part. Results： results discovered ultrasoundly 100 PAA, with an average value of 115.5° and we proceeded by evaluating 90 vaginal births. Infants at birth had a CC with an average value of 34.5 mm. By relating the two variables PAA and CC for the dependent variable “period expulsion in minutes”, confirming that the expulsive period depends on the relationship between PAA and CC. The average of the minutes of the expulsion period is 50＇ in women with a PAA 〉 111.5° and a CC 〉 34.5 mm while in women with an PAA 〉 111.5° the media time is 30＇ regardless of the CC. It has been shown that oxytocin and amniorrhexis reduce the minutes of the expulsion period only in a case of PAA 〉 111.5° and a CC 〉 34.5 mm bringing them from 60＇ to 23＇. Conclusion： the duration of the active phase of labor is not influenced by the PAA but it depends on external factors. In the case of the expulsion period, a predictability of the PAA on its duration is shown if the CC is taken into consideration; as regards the influence of the PAA on the method of delivery, it was not possible to carry out any analysis since all the useful cases had vaginal delivery.

Collagenous gastritis (CG) is characterized by patchy subepithelial collagen bands. Effective treatment and the clinical and histological outcome of CG in children are poorly defined. The aim of this study is to summarize the published literature on the clinical outcome and response to therapy of pediatric CG including two new cases. We performed a search in Pubmed, OVID for related terms; articles including management and clinical and/or endo-histologic follow up information were included and abstracted. Reported findings were pooled in a dedicated database including the corresponding data extracted from chart review in our patients with CG. Twenty-four patients were included (17 females) with a mean age of 11.7 years. The clinical presentation included iron deficiency anemia and dyspepsia. The reported duration of follow up (in 18 patients) ranged between 0.2-14 years. Despite most subjects presenting with anemia including one requiring blood transfusion, oral iron therapy was only documented in 12 patients. Other treatment modalities were antisecretory measures in 13 patients; proton pump inhibitors (12), or histamine-2 blockers (3), sucralfate (5), prednisolone (6), oral budesonide in 3 patients where one received it in fish oil and triple therapy (3). Three (13%) patients showed no clinical improvement despite therapy; conversely 19 out of 22 were reported with improved symptoms including 8 with complete symptom resolution. spontaneous clinical resolution without antisecretory, anti-inflammatory or gastroprotective agents was noted in 5 patients (4 received only supplemental iron). Follow up endohistopathologic data (17 patients) included persistent collagen band and stable Mononuclear cell infiltrate in 12 patients with histopathologic improvement in 5 patients. Neither collagen band thickness nor mononuclear cell infiltrate correlated with clinical course. Langerhans cell histiocytosis (LCH) is a proliferation of Langerhans cells with inflammatory cells, characterized by the activation of histiocytic cells or their proliferation that affects structures of the human body, including the temporal bone which localization is rare in children. The objective of this study is to illustrate the clinical presentation, management and prognosis of this disease, and to compare our case with previously reported series. We report the case of a young 4-year-old patient with a history of otorrhea and recurrent otalgia, with left axillary non-blowing exophthalmia progressive since 5 months associated with indurated left temporal swelling and also an indurated swelling of posterior face of the left shoulder on pathological fracture. A cerebro-orbital MRI performed at home showed an extra-conical process with extension at the temporal fossa. The patient, then, underwent an open-air biopsy of the temporal mass and the pathological analysis returned to favor Langerhans cell histiocytosis. The diagnosis was made by immunohistochemical findings of the s-100 protein and/or the CD1 antigen. The child was treated by chemotherapy with a good outcome. Our results concord with literature. The prognosis is different and better in children with unifocal bone disease than those with a multifocal disease with a survival rate of 65% - 100%.

It is theorized that enhanced thermal heating may result from exposing single-walled carbon nanotubes （sWNTs） embedded in a conductive host to radiofrequency （RF） electric fields. We examine the RF-induced （13.56 MHz） heating behaviors of 95% metallic- and semiconducting-enriched sWNTs （m-/s-sWNTs） suspended in aqueous solutions with varying NaC1 molarity （0.001 mM-1 M）. The heating effects were only evident for host molarities below 1 mM （equivalent to 0.1 s/m） at which the s-sWNT heating rates dominated those of the m-sWNTs. The heating effects were localized to aligned and aggregated ＂sWNT ropes＂ -1 cm in length that formed in suspension, parallel to the electric-field vector, during the RF exposure. For molarities above 1 mM, no enhancements were evident, owing to the large heating effects of the bulk ionic NaC1 suspensions, which were observed in previous studies. Although larger enhancement effects proportional to the host conductivity have been theoretically predicted for m-/s-sWNT suspensions, this was not observed most likely because of the aggregation and screening effects, which diminished the scattered electric field near the m-/s-sWNTs. Our research may further the development of better nanoparticle heating agents for applications such as non-invasive RF-induced cancer hyperthermia.
