Accelerated Hyper Fractionated Radiotherapy in Localized Ewing’s Sarcoma with or without Surgery: What’s New? A Phase II Study

Background and aim of work: We aimed from this study to determine the response and local relapse free survival (RFS) of pediatric patients with localized Ewing’s sarcoma treated with accelerated hyper-fractionated RT. Patient and methods: This study was a nonrandomized uncontrolled phase II study and was conducted at clinical oncology department and south Egypt cancer institute;it involved 28 patients with histologically confirmed Ewing’s sarcoma;all of them were subjected to PET/CT whenever possible or MRI with contrast of the primary site, MSCT chest, bone scan, and LDH to ensure absence of metastasis followed by the protocol of accelerated hyper fractionated RT. Results: The overall response rate (ORR) was 92.9% by MRI with significant effect of the type of response on local RFS (P 0.002). The median local RFS of 28 patients with localized Ewing’s sarcoma was 30 ± 8.599 months with 95% CI = 13.147 - 46.853;the 3-year local control was 35%. Conclusion: Accelerated hyper fractionated RT didn’t achieve better results than standard fractionation RT, but it is recommended to be done on a large sample size, and multiple centers, and continued follow up is also recommended to evaluate 5-year LRFS, 5-year OS.

Radio-labelled MIBG therapy has been tried in the treatment of advanced stage III and IV neuroblastoma in an attempt to improve patients outcome. The use of radiolabelled MIBG to treat neuroblastoma has arisen from the high sensitivity and specificity of in vivo MIBG imaging for detection of primary and metastatic tumours. Although, clinical trials have been performed to determine the role of MIBG therapy, the overall therapeutic strategy of MIBG therapy is not precisely defined. Aim of the work: to determine the impact of MIBG therapy on patients’ outcome especially their quality of life. Patients & methods: 42 paediatri neuroblastoma patients are included in this study, 12 (28.5%) stage III disease and 30 (71.5%) stage IV. I-131 MIBG doses ranged from 100 to 150 mCi with number of courses ranged from 1-8 according to response and toxicity. Results: Group I (stage III): 1/12 patients showed complete response, 5/12 patients had partial response and the other 6 patients remained stable. The 5 years event free survival (EFS) of stage III patients was 46.6% and the overall survival was 75%. Group II (stage IV): 2/30 patients achieved CR, 9/30 patients showed PR to I-131 MIBG and the other 18 remained stable. The 5 years event free survival (EFS) of stage IV patients was 48.2% and the overall survival was 69%. Conclusion: We conclude that 131I-MIBG therapy has favourable therapeutic effects that are translated into an overall improved outcome with good quality of life.

Bipartite Laparoscopic Cholecystectomy: New Technique for Avoiding Bile Duct Injury in Difficult Cases

The incidence of bile duct injury in laparoscopic cholecystectomy (LC) is still two times greater compared to classic open surgery. This study offers new procedure to avoid this complication during LC. The gall bladder was divided into two parts above the Hartmann pouch and all contents were aspirated. Then, the distal part was dissected for short distance. The proximal part was dissected dome down until reaching to cystic duct which was tied or clipped and cut. J-vac drain was put in peritoneal cavity. Between September 2012 and October 2013, overall 77 patients (53 females and 24 males) with mean age of 49 years (between 23 and 67 years) underwent bipartite laparoscopic cholecystectomy. The mean operative time was 60 minutes (between 40 and 90 minutes). The dissection of both parts of gall bladder was safe and easy as close as possible from its wall. No biliary tract injuries were recorded during or after procedure and also at follow-up period (20 months). Bipartite laparoscopic cholecystectomy is safe, easy to do, and can avoid all complications especially bile duct injuries in difficult cases.

AIM To evaluate the safety and efficacy of sedating pediatric patients for outpatient flexible *** A retrospective chart review was conducted for all children, age 17 years or under who underwent flexible bronchoscopy under deep sedation in an outpatient hospital-based setting. Two sedation regimens were used; propofol only or ketamine prior to propofol. Patients were divided into three age groups; infants(less than 12 mo), toddlers(1-3 years) and children(4-17 years). Demographics, indication for bronchoscopy, sedative dosing, sedation and recovery time and adverse events were *** Of the total 458 bronchoscopies performed, propofol only regimen was used in 337(74%) while propofol and ketamine was used in 121(26%). About 99% of the procedures were successfully completed. Children in the propofol + ketamine group tend to be youngerand have lower weight compared to the propofol only group. Adverse events including transient hypoxemia and hypotension occurred in 8% and 24% respectively. Median procedure time was 10 min while the median discharge time was 35 min. There were no differences in the indication of the procedure, propofol dose, procedure or recovery time in either sedative regimen. When compared to other age groups, infants had a higher incidence of *** Children can be effectively sedated for outpatient flexible bronchoscopy with high rate of success. This procedure should be performed under vigilance of highly trained providers.

Background: Acute lymphoblastic leukemia (ALL), a common pediatric malignant neoplasia, showed high relapse rate after induction therapy. Some miRNAs have been shown to regulate normal hematopoiesis and their disruption could contribute to leukemogenesis: Recently, specific miRNA, including miR-181a, was shown to be involved in the pathogenesis of ALL, serving as a biomarker for diagnosis and relapse of ALL. Aim of the Study: To evaluate miR-181a expression level as a predictive marker for children with acute lymphoblastic leukemia. Patients and Methods: 40 pediatric ALL patients were included in this study. miR-181a expression was assessed at diagnosis before start of treatment. Samples were either peripheral blood or bone marrow aspirate sample. Patients were evaluated clinically and laboratory after the induction therapy.

A new composite of silicone rubber and boric acid was developed to be used as a personal wearing for protection from nuclear radiation. The capability of this material for absorbing thermal neutrons of different intensities followed by a detonation of nuclear weapon has been investigated. This investigation was performed by using californium-252 neutron source of like fission spectrum. The thermal neutron flux was measured behind different thicknesses of the developed material using a BF3 detector. Two positions of measurements were performed;at position near the detonation where the intensity of thermal neutron flux is low and at position far from the detonation where the intensity of thermal neutron is high. For both cases, the contribution of total thermal, initial incident and new produced thermal neutron fluxes are measured. The obtained results indicated that, addition of boron with concentration of about 17% to the silicon rubber tends to decrease the flux by more than 70%.

The purpose of this study was to determine the effects of road transportation under heat conditions on some haematological [Ht （haematocrit）, blood cells count and EOF （erythrocytes osmotic fragility）] and physiological [Tr （rectal temperature）, HR （heart） and RR （respiratory rates）, and circulating levels of Cor （cortisol）, Glu （glucose） and minerals] parameters in Moroccan dromedary camels. The animals were subjected to road transportation stressor for 2 h by truck during the hot-dry season. Blood samples were collected before loading and transport, and at the end of transport. Transportation induced a significant increase （P 〈 0.05） of erythrocytes count, Ht, EOF, Tr, HR and RR by comparison to values observed before transportation. The same stress conditions induced a significant increase （P 〈 0.05） of plasma Cor （ng/mL） and blood Glu （mM）（220 ± 30 vs. 137 ± 20, 9.7 ± 1.2 vs. 6.4 ± 1. 1 respectively） and a significant decrease （P 〈 0.05） of plasma magnesium （mM）（0.5 ± 0.1 vs. 0.9 ± 0.1） comparatively to pre-transportation values. These results indicate that road transportation associated to heat may be considered as a potent stressor which is able to induce several cellular alterations in camels. Further studies of an eventual protective role of vitamin C against haemolysis induced by transportation stress in camel are needed.
