Role of urine studies in asymptomatic febrile neutropenic patients presenting to the emergency department

BACKGROUND: The role of urine studies in the detection of urinary tract infection(UTI) in febrile neutropenic patients with urinary symptoms(having a urinary catheter or having a positive urine analysis) is inarguable. However, the evidence is scarce regarding the indication for urine studies in asymptomatic(i.e., without urinary symptoms) patients with febrile neutropenia(FN) presenting to the emergency department(ED). The aim of this study is to evaluate the need for obtaining urine studies in asymptomatic febrile neutropenic ***: This was a retrospective cohort study conducted on adult cancer patients who presented to the ED with FN and had no urinary symptoms. We included all ED presentations of eligible patients between January 2013 and September 2018. Student's t-test and Wilcoxon rank-sum test were used for continuous data, while Chi-square and Fisher's exact tests were used for categorical data. Participants were divided into two groups based on their urine culture(UC) results: negative and positive UCs. Two cut-offs were used for positive UC results: ≥10~5 cfu/m L and ≥10~4 cfu/m ***: We included 284 patients in our study. The age of our patient population was 48.5±18.5 years. More than two-thirds(68.7%) of patients had severe neutropenia, while only 3.9% and 9.9% of the patients had positive UCs at ≥10~5 cfu/m L and ≥10~4 cfu/m L, respectively. UCs were expectedly positive in most patients with urinalysis(UA) abnormalities. However, 27.3% and 32.1% of patients with positive UCs at ≥10~5 cfu/m L and ≥10~4 cfu/m L respectively had a normal UA. CONCLUSIONS: In our study, the incidence of UTI in adult febrile neutropenic cancer patients who present to the ED without urinary symptoms is low. Consequently, routine urine testing may not be warranted in this population, as it adds unnecessary financial burdens on the patients and delays timely management.

A polytyramine-copper oxalate nanocomposite modified copper(PTCOxNMC) electrode prepared by electropolymerization was examined for electrocatalytic activity towards the oxidation of methanol in alkaline solution using cyclic voltammetry and impedance spectroscopy. The prepared PTCOxNMC electrode showed a significantly high response for adsorbed methanol oxidation. The effects of various parameters such as potential scan rate and methanol concentration on the electrocatalytic oxidation at the surface of the PTCOxNMC electrode were investigated. Spectrometry techniques such as Fourier transform infrared spectroscopy and scanning electron microscopy were used to determine the surface physical characteristics of the modified electrode and revealed that the polytyramine-copper oxalate nanocomposite particles were highly dispersed on the surface of the copper electrode with a narrow size up to 40 nm. The very high current density obtained for the catalytic oxidation may have resulted from the high electrode surface area caused by modification with the poly-tyramine-copper oxalate nanocomposite.

Variability and Predictors of Mothers and Fathers’ Socialization Behaviors and Bidirectional Links with Their Preschoolers Socio-Emotional Competences

In their Parental Socialization of Emotions model, Eisenberg, Cumberland and Spinrad (1998) differentiated parents’ Emotion-Related Socialization Behaviours (ERSBs) that support their child’s socio-emotional development: their reactions to their child’s emotions, their discussions about emotions with the child and the expressions of their own emotions in the family. The cross- sectional study focused on the variability of parents’ ERSBs according to children’s and parents’ characteristics (Study 1) and included 167 mothers and 152 fathers of preschoolers. The short-term longitudinal study examined the interaction between parents’ ERSBs and children’s socio-emotional abilities (Theory of Mind, emotional regulation and social adjustment) (Study 2) in 53 two-parent families and their children, with a 6 months interval. In Study 1, parents’ ERSBs, their emotional competences and children’s personality were assessed by means of questionnaires. In Study 2, we combined direct and indirect measures to assess children’s socio-emotional competences. Results of Study 1 indicated that parental ERSBs were mainly predicted by children’s personality, such as emotional stability and parents’ emotional competences, such as communication about their own emotions. Moreover, we observed a differential sensitivity between mothers and fathers toward children’s personality. Results of Study 2 revealed that both mothers and fathers socialized their children’s socio-emotional competences, particularly by the way of emotion-related conversations with their children. Mothers’ emotion-related conversations predicted children’s Theory of mind abilities and social adjustment, while fathers’ emotion-related conversations predicted children’s emotional regulation. In addition, fathers’ reactions to their children’s emotions were predicted by children’s socio-emotional competences. Convection-dispersion of fluids flowing through porous media is an important phenomenon in immiscible and miscible displacement in hydrocarbon reservoirs. Exact calculation of this problem leads to perform more robust reservoir simulation and reliable prediction. There are various techniques that have been proposed to solve convection-dispersion equation. To check the validity of these techniques, the convection-dispersion equation was solved numerically using a series of well known numerical techniques. Such techniques that employed in this study include method of line, explicit, implicit, Crank-Nicolson and Barakat-Clark. Several cases were considered as input, and convection-dispersion equation was solved using the aforementioned techniques. Moreover the error analysis was also carried out based on the comparison of numerical and analytical results. Finally it was observed that method of line and explicit methods are not capable of simulating the convection-dispersion equation for wide range of input parameters. The Barakat-Clark method was also failed to predict accurate results and in some cases it had large deviation from analytical solution. On the other hand, the simulation results of implicit and Crank-Nicolson have more qualitative and quantitative agreement with those obtained by the analytical solutions.

Recent results for side-on ignition of uncompressed proton-boron （HB 11） fusion that use the Chu-Bobin side-on ignition with petawatt-picosecond laser pulses is extended to the reaction of helium 3-helium 3 （He3）. The HBll reaction resulted in radioactivity is lower values than from burning coal per generated energy. This was based on the very rare experiments with extreme suppression ofpre-pulses in order to suppress relativistic self-focusing. Subsequently, acceleration of highly directed plasma blocks of modest temperature and ultra-high ion current densities above 10H Amps/cm2 were measured in agreement with earlier derived theory. This permits the conditions of the Chu-Bobin for side-on ignition of solid density fusion. Results for similar neutron lean He3 are reported. A detailed comparison with the usual spherical laser compression and ignition of fusion is given for clarifying the basic differences of the ignition process.

The main goal of this research was the scrutiny of relationship between students’ personality characteristics and mental health at the University of Guilan. The population was the students of the Faculty of Humanities in 2009 with 3000 which 196 were selected using Kerjeci & Morgan table and stratified sampling method. They answered to the NEO and GHQ questionnaires. Results showed that there was relationship between personality characteristic of neurotic, openness, extraversion and being conscionable with mental health. Meanwhile, openness had the highest and extraversion with the lowest correlation scale and most of students (65%) had low mental health. Also, there were no significant differences among students with respect to different courses but significant difference with respect to gender and age.

In this study the mechanical version of the three-disk Tower of London task with changes in the movements was conducted by fifteen elderly participants with concurrent articulatory suppression. Also, this executive task was conducted without verbal secondary task and the results of these two states were com- pared with each other. From this comparison, got evidences based on inner speech role in more complicated Tower of London tasks, although in general, the results suggest a more outstanding role of inner scribe in spatial planning in this executive task. Then inner speech and inner scribe roles have been described in Tower of London task applying “Baddeley and Logie” working memory model.

Women with both type 1 diabetes and polycystic ovarian syndrome (PCOS) represent a unique population, with long term implications on fertility and diabetic complications, which need to be addressed. The studies reviewed herein address the increased incidence of PCOS in women with type 1 diabetes. The leading theories suggest that the underlying pathophysiology is related to intensive insulin therapy and resulting ovarian hyperandrogenism. We have searched the literature to determine what studies have been done to address the role of insulin resistance and insulin sensitizers for reduction of hyperandrogenism in these patients. According to the available evidence, we conclude that metformin may have a role in reducing the androgenic picture in type 1 patients with PCOS and insulin resistance.
