Introduction: Autosomal dominant polycystic kidney disease (ADPKD) is a common, multisystemic, and progressive hereditary disease. It accounts for 6 to 8% of incident cases of end-stage chronic renal disease (ESRD) in developed countries. The aim of this study is to describe the predictive factors for the development of end-stage chronic kidney disease (CKD) in the course of this disease. Material and Methods: This is a retrospective, descriptive, and analytical study including 300 cases of ADPKD collected at the Nephrology Department of Ibn-Sina Hospital in Rabat over a period of 30 years (1993 to 2023). Included in the study are all patients with ADPKD meeting the ultrasound diagnostic criteria. The analysis focused on demographic, clinical, paraclinical, evolutionary data, as well as prognostic factors associated with renal function deterioration. Results: The mean age of patients at diagnosis is 51.53 +/− 17 years [16 - 93] with a male predominance. The median serum creatinine at diagnosis is 15.5 mg/l [10 - 34]. 21% of patients had ESRD (eGFR 300 mg/24h (21%). The most common cystic complication is hemorrhage (12.3%). 21.3% of patients had hepatorenal polycystic disease. In adjusted analysis, the predictive risk factors for the occurrence of ESRD were smoking (p = 0.019), anemia (p Conclusion: ADPKD can progress insidiously to ESRD. Identification and early treatment of predictive factors for poor renal prognosis could contribute to a better outcome for this disease.

Hypoxic hepatitis, also known as ischemic hepatitis, is characterized by acute hepatocellular injury due to inadequate oxygen delivery to the liver. Celiac trunk stenosis can lead to hepatic ischemia and subsequent liver damage. We present the case of an 81-year-old patient with a history of hypertension, ischemic heart disease, hypothyroidism, and biliary lithiasis, who developed hypoxic hepatitis secondary to Dunbar syndrome and a stenosis of the superior mesenteric artery. The patient improved symptoms and liver function tests with conservative management, including intravenous fluids and supportive care. Long-term management involved continued antiplatelet therapy and statins, with consideration of further interventions for celiac trunk stenosis. This case underscores the importance of recognizing Dunbar syndrome as well as superior mesentery trunk stenosis as a potential cause of hypoxic hepatitis. It highlights the need for multidisciplinary management in such cases.

Unraveling the Impact of Direct-Acting Antivirals on Hepatitis-Linked Cirrhosis: A Comprehensive Analysis of Fibrosis, Child Score, and Disease Progression

The treatment of hepatitis C has undergone a significant boom since the advent of direct acting antivirals (DAA). Indeed, the interferon-ribavirin combination that has been used to treat hepatitis C has a virological response in only 45% of cases with significant side effects. The advent of direct-acting antivirals has changed the prognosis of cirrhotic patients with hepatitis C. DAAs have ensured a sustained viral response in the majority of patients. Our work aims to see the evolution of hepatitis C patients at the cirrhosis stage under DAA. We conducted a retrospective study over 15 years (January 2009, January 2024) including all patients with post-viral cirrhosis C, whom we divided into two groups: group A, cirrhotic patients who received ribavirin and interferon, and group B, patients on DAA. From January 2009 to January 2024, we conducted a study of 182 patients with viral hepatitis C, including 102 cirrhotic patients. The mean age was 55 years. 66% of patients were initially treated with the ribavirin interferon combination, while 34% received direct-acting antivirals (DAAs). Since the introduction of DAAs, the most commonly used regimens have been sofosbuvir/daclatasvir with or without ribavirin and sofosbuvir/ledipasvir with or without ribavirin. Group A achieved sustained virological response (SVR) in 60% of cases, with notable side effects. In Group B, SVR was 98.18%, with improved tolerability and fewer side effects than previous treatments. Fifteen patients developed hepatocellular carcinoma (HCC), with a significantly lower mortality rate in those treated with DAAs compared with pegylated dual therapy (p: 0.001).

The importance of this study is to identify the newly reordered and recognized basaltic intrusion for the first time in Maasser el Chouf in Lebanon. The recorded basaltic intrusion cut the Jurassic-Lower Cretaceous rock in this area. Necessary field inspection, geology, mineralogy and chemical tests were carried out on 8 basalt samples to determine their mineralogy, petrography and chemical composition. Representative samples have been tested with polarizing microscope, X-ray diffraction (XRD) and X-ray fluorescence (XRF). Petrographic and mineralogical studies show that the basalt is characterized by presence mainly of calcic-plagioclase feldspar, pyroxene-augite and olivine minerals. Secondary minerals of iron oxides also present (ilmenite and magnetite). The most appeared property is the alteration of olivine mineral to iddingsite that indicated highly weathered process. The composition of the basaltic samples reflects ultrabasic-basic type (Basanite-Tholeiitic basalt). The existence of volcanic activity occurred mostly with Pliocene age (elong to the alkaline olivine basalt, suite generally associated with tension, rifting and block faulting movements of the continental crust. Most of the volcanisms in Lebanon and in Harrat Ash Shaam Basalt from Syria and Palestine through Jordan to Saudi Arabia are related and connected to the opening of the Red Sea Rift System, making the area with tremendous volcanic tectonic activities.

2-Thioxo-1,2-dihydropyridine derivatives 2a, 2b were reacted with methyl iodide to give 2-methylthiopyridines 3a, 3b, which were reacted with hydrazine hydrate to produce 3-aminopyrazolo[5,4-b]pyridines 4a, 4b. Compounds 4a, 4b were diazotized to afford the corresponding diazonium salts 5a, 5b, which were reacted with some active methylene compounds 6a-6h to give the corresponding pyrido[2′,3′ ： 3,4]pyrazole[5,1-c][1,2,4]triazines 7-14.

Drought stress has become more common in recent years as a result of climate change impacts on the production of banana crops and other fruit *** growth and productivity of Musa spp are severely impacted by the gradual degradation of water resources and the erratic distribution pattern of annual precipitation *** aim of the work includes increased drought tolerance in light of water scarcity in the world as a result of the bananas’being gluttonous for water *** investigation was carried out from 2019 to 2020 to study the effect of potassium silicate on morphological growth and biochemical parameters of Musa acuminata L under drought stress by *** a result,drought stress reduced the morphological characteristics such as shoots number,shoot length,roots number,and survival percentage and biochemical characteristics such as chlorophyll a,b,carotenoids,stomatal status,and *** proline content increased in the leaf of *** *** complemented with K2SiO3(2 to 6 mM)either individually or in combination with PEG led to an improvement in all morphological and biochemical *** activities of CAT,POD,and PPO enzymes increased significantly compared to ***,the lowest PPO,CAT,and POD activity were ***,K2SiO3 treatments under drought stress successfully enhanced the leaf stomatal *** results suggest that K2SiO3 can help to maintain plant integrity in the tested cultivar under drought stress.

Social Media such as Facebook plays a substantial role in virtual com-munities by sharing ideas and ideologies among different populations over *** interaction analysis aids in deﬁning people’s emotions and aids in assessing public attitudes,towards different issues such as violence against women and *** this paper,we proposed an Arabic language prediction model to identify the issue of Violence-Induced Stress in social *** searched for Arabic posts of many countries through Facebook application programming interface(API).We discovered that the stress state of a battered woman is usually related to her friend’s stress states on *** applied a large real database from Facebook platforms to analytically investigate the correlation of violence-induced stress states and the victim interactions on social *** extracted a set of tex-tual,spatial,and interaction attributes from various ***,we are proposing a hybrid model–an interaction graph model incorporated in a deep learning neural model to leverage post content and interaction data for vio-lence-induced stress *** depict that our proposed hybrid mod-el can enhance the prediction performance by 10%in ***,considering the user interaction information can learn an interesting phenomenon,where,the sparse social interactions of violence-induced stress stressed victims is higher by around 15%percent non-battered users,signifying that the structure of the friends of such victims is less connected than non-stressed users.

Diabetic retinopathy(DR) is the most feared ocular manifestation of diabetes. DR is characterized by progressive retinal damage that may eventually result in blindness. Clinically, this blindness is caused by progressive damage to the retinal microvasculature, which leads to ischemia, retinal swelling, and neovascularization. Retinopathy is associated with both type 1 and type 2 diabetes, with DR being the leading cause of new onset blindness in United States adults. Despite this strong association with diabetes, it must be noted that the development of retinopathy lesions is multifactorial and may occur in individuals without an established history of diabetes. Metabolic syndrome is a multifactorial condition of central obesity, hypertriglyceridemia, dyslipidemia, hypertension, fasting hyperglycemia, and insulin resistance. Although several studies examined the individual components observed in the metabolic syndrome in relation to the development of DR, there is conflicting data as to the association of the metabolic syndrome with the development of retinopathy lesions in nondiabetic subjects. This review will summarize the current literature on the evidence of the metabolic syndrome on retinopathy in subjects with and without an established history of diabetes. This review will also discuss some of the mechanisms through which metabolic syndrome can contribute to the development of retinopathy.

Coronaviruses caused an outbreak pandemic disease characterized by a severe acute respiratory distress syndrome leading to the infection of more than 200 million patients and the death of more than 4 million *** primary treatment is either supportive or *** products have an important role in the development of various ***,screening of natural compounds with reported antiviral activities can lead to the discovery of potential inhibitory entities against *** the current study,an in-silico molecular docking experiment was conducted on the effects of some of these natural antiviral phytoconstituents,(e.g.,procyanidin B2,theaflavin,quercetin,ellagic acid,caffeoylquinic acid derivatives,berginin,eudesm-1β,6α,11-triol and arbutin),on the crystal structure of SARS-CoV-2 main protease(PDB ID:6w63)using AutoDock-Vina *** of the docked compounds revealed good binding affinity,with procyanidin B2(–8.6 Kcal/mol)and theaflavin(–8.5 Kcal/mol)showing a better or similar binding score as the ligand(–8.5 Kcal/mol).Molecular dynamics simulations were carried out at 100 ns and revealed that procyanidin B2 forms a more stable complex with SARS-CoV-2 main protease than *** B2,theaflavin,and 4,5-dicaffeoylquinic acid were evaluated for toxicity by ProTox-II webserver and were non-toxic according to the predicted LD50 values and safe on different organs and ***,these phytoconstituents showed good ADME properties and acceptable lipophilicity,as evaluated using *** the tested compounds,procyanidin B2 showed the highest lipophilic *** is worth mentioning that these natural inhibitiors of SARS-CoV-2 main protease are components of green and black tea that can be used as a supporting supplement for COVID patients or as potential nuclei for further drug design and development campaigns.

Al-Nakheel beach is located northwest of Alexandria city,Egypt,along the Mediterranean *** the period from 1998 to 2003,seven detached breakwaters were constructed along Al-Nakheel beach to create a sheltered area for ***,the structures amplify rip currents,shoreline accretions,and *** aim of this research is to track the variations of the rip currents within the study area and show the effects of the breakwaters on the *** research is based on the hydrodynamic and morphological data of the study area and uses the Delft3D hydrodynamical model combined with other data analysis tools to serve the model *** data include measured sea-level observations in 2013,the ERA-interim wave datasets from 2015 to 2018 and wind data in 2018,bed morphologies,and Google Earth satellite images from 2010 to *** model is calibrated on the basis of the available current measurements within the nearshore *** show that the shoreline eroded at an average rate of about 0.9 m/***,pairs of vortices are formed behind the breakwaters with an average current velocity of 0.6 m/*** predominant northwest waves induce rip currents on the leeside of the structures with velocities reaching 1.2 m/s,associated with the rip pulsation that extends *** problem solution decision recommends the removal of the sand deposition on the leeside of the breakwaters by an average amount of 100000 m3/yr and the fencing of the safe area for swimming by a floating fence of 1000 m length and 65 m average width.
