Validation of the chronic liver disease questionnaire in Serbian patients

AIM:To translate into Serbian and to investigate the validity of the cross-culturally adapted the chronic liver disease questionnaire(CLDQ).METHODS:The questionnaire was validated in 103 consecutive CLD patients treated between October 2009 and October 2010 at the Clinic for Gastroenterology,Clinical Centre of Serbia,Belgrade(Serbia).Exclusion criteria were:age 2)and liver *** of the CLDQ was done based on the following parameters:(1)acceptance is shown by the proportion of missing items;(2)internal reliabilities were assessed for multiple item scales by using Cronbach alpha coefficient;and(3)in order to assess whether the allocation of items in the domain corresponds to their distribution in the original questionnaire(construction validity),an exploratory factor analysis was *** validity was determined by comparing the corresponding CLDQ score/sub-score in patients with different severity of the ***:The Serbian version of CLDQ questionnaire completed 98% *** of missing items was 0.06%.The total time needed to fill the questionnaire was ranged from 8 to 15 *** in completing the questionnaire required 4.8% patients,while 2.9% needed help in reading,and 1.9% involved writing *** mean age of the selected patients was 53.8 ± 12.9 years and 54.4% were *** CLDQ score was 4.62 ± ***'s alpha for the whole scale was *** for all domains was above 0.70,except for the domain "Activity"(0.49).The exploratory factor analysis model revealed 6 factors with eigenvalue of greater than 1,explaining 69.7% of cumulative *** majority of the items(66%)in the Serbian version of the CLDQ presented the highest loading weight in the domain assigned by the CLDQ developers:"Fatigue"(5/5),"Emotiona

Right liver lobe/albumin ratio:Contribution to non-invasive assessment of portal hypertension

AIM： To study the value of biochemical and ultrasonographic parameters in prediction of presence and size of esophageal varices.METHODS： The study includes selected cirrhotic patients who underwent a complete biochemical workup, upper digestive endoscopic and ultrasonographic examinations. Albumin/right liver lobe diameter and platelet count/spleen diameter ratios were calculated. The correlation between calculated ratio and the presence and degree of esophageal varices was evaluated.RESULTS： Ninety-four subjects （62 males, 32 females）, with a mean age of 52.32 ± 13.60 years, were studied. Child-Pugh class A accounted for 42.6%, class 13 37.2%, whereas class C 20.2%. Esophageal varices （OE） were not demonstrated by upper digestive endoscopy in 24.5%, while OE grade Iwas found in 22.3% patients, grade Ⅱ in 33.0%, grade m in 16.0%, and grade iV in 4.3%. The mean value of right liver lobe diameter/ albumin ratio was 5.51± 1.82 （range from 2.76 to 11.44）, while the mean platelet count/spleen diameter ratio was 1017.75 ± 729.36 （range from 117.39 to 3362.50）, respectively. Statistically significant correlation was proved by Spearman＇s test between OE grade and calculated ratios. The P values were 0.481 and -0.686, respectively.CONCLUSION： The right liver lobe diameter/albumin and platelet count/spleen diameter ratios are noninvasive parameters providing accurate information pertinent to determination of presence of esophageal varices, and their grading in patients with liver cirrhosis.

AIM:This paper describes the procedure of detection of Helicobacter pylori(***)in bile specimens in patients suffering from benign diseases of biliary ducts(lithiasis with/without nonspecific cholangitis). METHODS:The group of 72 patients entering the study consisted of 32 male and 40 female(45 % and 55 %, respectively).Bile was obtained during ERCP in 68 patients, and during cholecystectomy in 4 patients.A fast urease test (FUT)to determine the existence of *** in gastric mucosa was carried out for all the patients during the endoscopic *** existence of genetic material of *** was determined by detection of ure A gene by the method of nested *** results of this reaction were shown by electrophoresis on 10 g·L^(-1)agarose gel in a band of 256 bp. RESULTS:The majority of the patients included in our study had biliary lithiasis without signs of cholangitis(48 patients, 67 %),whereas other patients were complicated by cholangitis(17 patients,24 %).Seven patients(9 %)had normal ERCP,forming thus the control *** the group of patients with lithiasis 26 patients(24.2 %)had positive PCR of *** in bile and among the patients with associated cholangitis positive PCR was detected in 9 patients(52.9 %).Among the seven patients with normal ERCP only one(14 %)had positive PCR of ***.A high percentage of *** infection of gastric mucosa was observed(57 patients,79 %).It was also observed that its slightly higher positivity was in the patients with distinct bile pathology:81% FLIT positive patients in the group with choledocholithiasis alone and 76 % in the group with choledocholithiasis associated with ***-one percent of the patients with regular findings had positive ***: The prevalence of H. pylori infection both in bile and in gastric mucosa in patients with benign diseases of biliary ducts does not show a statistically significant difference in relation to the prevalence of the same with the patients

Biochemical markers for non-invasive assessment of disease stage in patients with primary biliary cirrhosis

AIM: To evaluate different biochemical markers and their ratios in the assessment of primary biliary cirrhosis (PBC) stages. METHODS: This study included 112 patients with PBC who underwent a complete clinical investigation. We analyzed the correlation (Spearman's test) between ten biochemical markers and their ratios with different stages of PBC. The discriminative values were compared using areas under receiver operating characteristic (ROC) curves. RESULTS: The mean age of patients included in the study was 53.88 ± 10.59 years, including 104 females and 8 males. We found a statistically significant correlation between PBC stage and Aspartate aminotransferase (AST), Alanine aminotransferase (ALT) to platelet ratio (APRI), ALT/platelet count, AST/ALT, ALT/AST and ALT/Cholesterol ratios, with the values of Spearman's rho of 0.338, 0.476, 0.404, 0.356, 0.351 and 0.325, respectively. The best sensitivity and specificity was shown for AST/ALT, with an area under ROC of 0.660. CONCLUSION: Biochemical markers and their ratios do correlate with different sensitivity to and specificity of PBC disease stage. The use of biochemical markers and their ratios in clinical evaluation of PBC patients may reduce, but not eliminate, the need for liver biopsy.

Mucinous cystadenoma of the appendix associated with adenocarcinoma of the sigmoid colon and hepatocellular carcinoma of the liver:Report of a case

Mucinous cystadenoma of the appendix is a rare condition and represents one of the three entities with the common name mucocele of the appendix. It is characterized by a cystic dilatation of the lumen with stasis of mucus inside it. Histopathologically mucocele is divided into three groups： focal or diffuse mucosal hyperplasia, mucinous cystadenoma and mucinous cystadenocarcinoma. This condition is often associated with other neoplasia, especially adenocarcinoma of the colon and ovaries. We here describe a 57 year old male patient who presented with abdominal discomfort, constipation, fresh blood in stool and frequent urination. He had a big cystadenoma of the appendix associated with adenocarcinoma of the colon and hepatocellular carcinoma of the liver. The patient underwent right haemicolectomy, sigmoid colon resection and segmental resection of the liven Now 3 years later he has no evidence of disease relapse. According to this, we stress the need of accurate preoperative diagnosis and intraoperative exploration of the whole abdomen in these patients.

Double common bile duct: A case report

Double common bile duct （DCBD） is a rare congenital anomaly in which two common bile ducts exist. One usually has normal drainage into the papilla duodeni major and the other usually named accessory common bile duct （ACBD） opens in different parts of upper gastrointestinal tract （stomach, duodenum, ductus pancreaticus or septum）. This anomaly is of great importance since it is often associated with biliary lithiasis, choledochal cyst, anomalous pancreaticobiliary junction （APBJ） and upper gastrointestinal tract malignancies. We recently recognized a rare case of DCBD associated with APB3 with lithiasis in better developed common bile duct. The opening site of ACBD was in the pancreatic duct. The anomaly was suspected by transabdominal ultrasonography and finally confirmed by endoscopic retrograde cholangiopancreatography （ERCP） followed by endoscopic sphincterotomy and stone extraction. According to the literature, the existence of DCBD with the opening of ACBD in the pancreatic duct is most frequently associated with APB3 and gallbladder carcinoma. In case of DCBD, the opening site of ACBD is of greatest clinical importance because of its close implications with concomitant pathology. The adequate diagnosis of this rare anomaly is significant since the operative complications may occur in cases with DCBD which is not recognized prior to surgical treatment.

With the Hausdorff-metric, they are topological spaces. In this paper, we prove that, if X is an infinite compact metric space with a dense set of isolated points, then (↓USCC(X), ↓CC(X)) ≈ (Q, c 0 ∪ (Q Σ)), i.e., there is a homeomorphism h:↓USCC(X) → Q such that h(↓CC(X)) = c 0 ∪ (Q Σ), where Q = [?1,1]ω, Σ = {(x n ) n∈? ∈ Q: sup|x n | < 1} and c 0 = {(x n ) n∈? ∈ Σ: lim n→+∞ x n = 0}. Multi-criteria decision analysis for sub-watersheds ranking via the PROMETHEE method

Soil and water resources are important elements of the environment that is managed to reduce the erosion rate and the destructive effects of torrential *** of the measures to reach this goal requires the ranking of sub-watersheds and areas within the sub-watersheds,from most to least vulnerable,which can be achieved using Multi-criteria decision analysis *** this paper,using the Preference Ranking Organization Method for Enrichment Evaluations(PROMETHEE)Ⅱ method ranking of nine sub-watersheds delineated in the Topciderska river watershed,Serbia,was performed by using ArcGIS *** criteria used for determining the order of the most vulnerable sub-watersheds are land cover,rainfall,soil erodibility as well as *** importance of criteria was determined by using the Analytic Hierarchy Process(AHP)method,and the influence of the criteria weights on ranking results was *** research identified that the most vulnerable sub-watershed is located in the upper part of the study area,where 15% of the territory is at high risk of ***,this sub-watershed should have priority for protection through the implementation of appropriate measures and *** results of the PROMETHEE Ⅱ method and the ArcGIS application represent the valuable information for watershed management planning and implementation of soil erosion and torrent control measures.

Performance characterization of different configurations of gas turbine engines

This paper investigates the performance of different configurations of gas turbine engines.A full numerical model for the engine is *** model takes into account the variations in specific heat and the effects of turbine cooling flow.A lso,the model considers the efficiencies of all component,effectiveness of heat exchangers and the pressure drop in relevant *** model is employed to compare the engine performances in cases of employing intercooler,recuperation and reheat on a single spool gas turbine engine.A comparison is made between single-spool engine and two-spool engine with free power ***,the performance of the eng ine with inter-stage turbine burner is investigated and compared with engine employing the nominal reheat *** engine employing inter-stage turbine bumers produces superior improvements in both net work and efficiency over all other *** effects of ignoring the variations on specific heat of gases and turbine cooling flow on engine performance are *** the variation in specific heat can cause up to 30%difference in net specific *** optimum locations of the intercooler and the reheat combustor are detemined using the numerical model of the *** maximum net specific work is obtained if the reheat combustor is placed at 40%of the expansion *** the other hand,to get maximum efficiency the reheat combustor has to be placed at nearly 10%-20%of the expansion *** optimum location of the intercooler is almost at 50%of the compression section for both maximum net specific work and efficiency.

Multi-sensor Data Fusion for Wheelchair Position Estimation with Unscented Kalman Filter

This paper investigates the problem of estimation of the wheelchair position in indoor environments with noisy mea- surements. The measuring system is based on two odometers placed on the axis of the wheels combined with a magnetic compass to determine the position and orientation. Determination of displacements is implemented by an accelerometer. Data coming from sensors are combined and used as inputs to unscented Kalman filter （UKF）. Two data fusion architectures： measurement fusion （MF） and state vector fusion （SVF） are proposed to merge the available measurements. Comparative studies of these two architectures show that the MF architecture provides states estimation with relatively less uncertainty compared to SVF. However, odometers measurements determine the position with relatively high uncertainty followed by the accelerometer measurements. Therefore, fusion in the navigation system is needed. The obtained simulation results show the effectiveness of proposed architectures.
