BACKGROUND Acute-on-chronic liver disease(AoCLD)accounts for the majority of patients hospitalized in the Department of Hepatology or Infectious *** To explore the characterization of AoCLD to provide theoretical guidance for the accurate diagnosis and prognosis of *** Patients with AoCLD from the Chinese Acute-on-Chronic Liver Failure(ACLF)study cohort were included in this *** clinical characteristics and outcomes,and the 90-d survival rate associated with each clinical type of AoCLD were analyzed,using the Kaplan-meier method and the log-rank *** A total of 3375 patients with AoCLD were enrolled,including 1679(49.7%)patients with liver cirrhosis acute decompensation(LC-AD),850(25.2%)patients with ACLF,577(17.1%)patients with chronic hepatitis acute exacer-bation(CHAE),and 269(8.0%)patients with liver cirrhosis active phase(LC-A).The most common cause of chronic liver disease(CLD)was HBV infection(71.4%).The most common precipitants of AoCLD was bacterial infection(22.8%).The 90-d mortality rates of each clinical subtype of AoCLD were 43.4%(232/535)for type-C ACLF,36.0%(36/100)for type-B ACLF,27.0%(58/215)for type-A ACLF,9.0%(151/1679)for LC-AD,3.0%(8/269)for LC-A,and 1.2%(7/577)for *** HBV infection is the main cause of CLD,and bacterial infection is the main precipitant of *** most common clinical type of AoCLD is *** diagnosis and timely intervention are needed to reduce the mortality of patients with LC-AD or ACLF.

目的探讨包头市昆都仑水库中微塑料的污染特征及生态风险。方法在昆都仑水库表层水、沉积物和周围土壤中各采集3个环境样本，经消解、密度浮选、二次消解和浓缩4个步骤提取出微塑料后，使用Clarity软件控制的LDIR8700反射模式检测微塑料的丰度、聚合物类型、粒径和形状等特征，使用风险指数（H）和污染负荷指数法（PLI）进行生态风险评估。结果表层水、沉积物和周围土壤中微塑料平均丰度分别为（210.83±50.1）n/L、（9 633.33±1 955.33）n/kg和（10 633.33±896.29）n/kg。在表层水中，微塑料聚合物类型以聚丙烯（PP）为主，粒径在0～30μm的微塑料占比最大，形状是以薄膜状为主。在沉积物中，聚合物类型以聚丁二酸丁二醇酯（PBS）为主，0～30μm大小和薄膜状微塑料占主要地位。在周边土壤中，聚合物类型同样以PBS为主，微塑料粒径与形状则是与沉积物中的相同。基于微塑料聚合物类型的H显示各采样点微塑料H指数范围在1.22～9 111.44，风险等级涵盖Ⅰ～Ⅳ级，表层水中风险等级普遍偏高，沉积物和周围土壤中风险等级略低；而基于微塑料污染丰度的污染负荷指数（PLI）显示各采样点微塑料PLI指数范围为1～1.273 3，其平均微塑料PLI指数为1.099 5，三种环境介质中总体呈现轻微污染状态。结论昆都仑水库3种环境介质中微塑料丰度较高、来源广泛且以小粒径为主，总体风险指数偏高，污染负荷较低。

Highly exposed NiFeO_(x) nanoclusters supported on boron doped carbon nanotubes for electrocatalytic oxygen evolution reaction

The development of efficient and cost-effective electrocatalysts for oxygen evolution reaction(OER) is crucial for the overall water splitting. Herein, we prepared a highly exposed NiFeO_(x) ultra-small nanoclusters supported on boron-doped carbon nonotubes catalyst, which achieves a 10 mA/cm^(2) anodic current density at a low overpotential of 213 mV and the Tafel slope of 52 mV/dec in 1.0 mol/L KOH, superior to the pristine NiFeO_(x)-CNTs and other state-of-the-art OER catalysts in alkaline media. A combination study(XPS, sXAS and XAFS) verifies that the local atomic structure of Ni and Fe atoms in the nanoclusters are similar to NiO and Fe_(2)O_(3), respectively, and the B atoms which are doped into the crystal lattice of CNTs leads to the optimization of Ni 3d egorbitals. Furthermore, in-situ X-ray absorption spectroscopies reveal that the high valence state of Ni atoms are served as the real active sites. This work highlights that the precise control of highly exposed multicomponent nanocluster catalysts paves a new way for designing highly efficient catalysts at the atomic scale.

A comparison of short-term postoperative outcomes including nutritional status between gastrectomy with simultaneous cholecystectomy and gastrectomy only in patients with gastric cancer

Objective: We aimed to evaluate the effect of simultaneous cholecystectomy on the short-term postoperative outcomes and nutritional status in patients with gastric ***: We retrospectively reviewed data from 4,820 patients with gastric cancer who underwent gastrectomy from january 2011 to December 2016. Patients who underwent only gastrectomy(N=4,578) were matched to those who underwent simultaneous cholecystectomy during gastrectomy(N=242) at a 1:1 ratio using propensity score matching analysis. The nutritional status and inflammatory responses preoperatively and postoperatively and postoperative outcomes were compared between the ***: The simultaneous cholecystectomy group showed more intraoperative blood loss and a longer operative time than the gastrectomy only group [150.0(100.0, 200.0) m L vs. 100.0(100.0, 200.0) m L, P=0.006;176.0(150.0,210.0) min vs. 155.0(128.0, 188.0) min, Pmplication rate, and postoperative recovery did not differ between the groups. All parameters including body weight, the hemoglobin level, absolute lymphocyte count, total protein level, albumin level, fasting glucose level,and prognostic nutritional index excluding the cholesterol level were not significantly different between the groups,and their changing patterns were similar. Although the cholesterol level was significantly lower in the simultaneous cholecystectomy group than in the gastrectomy only group at all follow-up points, the mean value of the decreased cholesterol level was within normal ***: In gastric cancer patients with gallbladder disease, simultaneous cholecystectomy is safe and not associated with additional nutritional loss.

Sustained-release genistein from nanostructured lipid carrier suppresses human lens epithelial cell growth

AIm： To design and investigate the efficacy of a modified nanostructured lipid carrier loaded with genistein（Gen-NLC） to inhibit human lens epithelial cells（HLECs） proliferation.·mETHODS： Gen-NLC was made by melt emulsification method. The morphology, particle size（PS）, zeta potentials（ZP）, encapsulation efficiency（EE） and in vitro release were characterized. The inhibition effect of nanostructured lipid carrier（NLC）, genistein（Gen） and Gen-NLC on HLECs proliferation was evaluated by cell counting kit-8（CCK-8） assay, gene and protein expression of the proliferation marker Ki67 were evaluated with real-time quantitative polymerase chain reaction（RT-q PCR） and immunofluorescence analyses.·RESULTS： The mean PS of Gen-NLC was 80.12±1.55 nm with a mean polydispersity index of 0.11±0.02. The mean ZP was-7.14 ±0.38 m V and the EE of Gen in the nanoparticles was 92.3% ±0.73%. Transmission electron microscopy showed that Gen-NLC displayed spherical-shaped particles covered by an outer-layer structure. In vitro release experiments demonstrated a prolonged drug release for 72 h. The CCK-8 assay results showed the NLC had no inhibitory effect on HLECs and Gen-NLC displayed a much more prominent inhibitory effect on cellular growth compared to Gen of the same concentration. The m Rna and protein expression of Ki67 in LECs decreased significantly in Gen-NLC group.·CONCLUSION： Sustained drug release by Gen-NLCs may impede HLEC growth.

marine natural products play critical roles in the chemical defense of many marine organisms and are essential,reputable sources of successful drug ***-seven 14-membered resorcylic acid lactone derivatives 3–27 and 30–71 of the natural product zeaenol(1)isolated from the marine-derived fungus Cochliobolus lunatus were semisynthesized by chlorination,acylation,esterification,and acetalization in one to three *** structures of these new derivatives were established by HRESImS and NmR *** the compounds(1–71)were evaluated for their antialgal and antiplasmodial *** them,14 compounds displayed antifouling activities against adhesion of the fouling *** particular,9 and 34 exhibited strong and selective inhibitory effects against the diatoms navicula laevissima and navicula exigua(EC50=6.67 and 8.55μmol/L),respectively,which were similar in efficacy to those of the positive control SeaNine 211(EC50=2.90 and 9.74μmol/L).more importantly,38,39,and 69–71 showed potent antiplasmodial activities against Plasmodium falciparum with IC50 values ranging from 3.54 to 9.72μmol/*** interestingly,the five antiplasmodial derivatives displayed non-toxicity in the cytotoxicity assays and the zebrafish embryos model,thus,representing potential promising antiplasmodial drug *** preliminary structure–activity relationships indicated that biphenyl substituent at C-2,acetonide at positions C-5′and C-6′,and tri-or tetra-substituted of acyl groups increased the antiplasmodial ***,combining evaluation of chemical ecology with pharmacological models will be implemented as a systematic strategy,not only for environmentally friendly antifoulants but also for structurally novel drugs.

We achieve the robust nonadiabatic holonomic two-qubit controlled gate in one step based on the groundstate blockade mechanism between two Rydberg atoms. By using the Rydberg-blockade effect and the Raman transition mechanism, we can produce the blockade effect of double occupation of the corresponding ground state,i.e., ground-state blockade, to encode the computational subspace into the ground state, thus effectively avoiding the spontaneous emission of the excited Rydberg state. On the other hand, the feature of geometric quantum computation independent of the evolutionary details makes the scheme robust to control errors. In this way,the controlled quantum gate constructed by our scheme not only greatly reduces the gate infidelity caused by spontaneous emission but is also robust to control errors.

Altered gut microbiome in FUT2 loss-of-function mutants in support of personalized medicine for inflammatory bowel diseases

The FUT2 loss-of-function mutations are highly prevalent and are associated with inflammatory bowel disease(IBD).To investigate the impact of FUT2 loss-of-function mutation on the gut microbiota in patients with IBD,81 endoscopically confirmed IBD patients were genotyped and divided into 3 groups:homozygous for functional FUT2 genes(SeSe),with one copy of non-functional FUT2 gene(Sese),or homozygous for non-functional FUT2 genes(sese).Escherichia,which attaches to fucosylated glycoconjugates,was the only abundant genus exhibiting decreased abundance in sese *** with SeSe or Sese patients,sese patients exhibited higher abundance in CD8+inducing Alistipe and Phascolarctobacterium and Th17 inducing Erysipelotrichaceae ***-intuitively,butyrate-producing bacteria were more abundant in sese ***,metabolomics analysis found higher levels of butyrate in sese *** data support the hypothesis that FUT2 loss-of-function mutation participates in the IBD pathogenesis by decreasing binding sites for adherent bacteria and thus altering the gut *** abundances of adherent bacteria may allow the overgrowth of bacteria that induce inflammatory T cells,leading to intestinal *** FUT2 loss-of-function mutations are highly prevalent,the identification of T cell inducing bacteria in sese patients could be valuable for the development of personalized microbial intervention for IBD.

维奈克拉方案治疗复发/难治性急性髓系白血病的临床研究

目的:评价维奈克拉(venetoclax,VEN)快速剂量递增、最长治疗时间为14天,联合低剂量阿糖胞苷(low-dose cytarabine,LDAC)方案挽救治疗复发/难治性急性髓系白血病(relapsed/refractory acute myeloid leukemia,R/RAmL)的安全性和有效性。方法:回顾性分析2018年10月至2023年11月于江苏大学附属医院接受VEN+LDAC方案挽救治疗的16例R/R AmL患者,所有患者既往均未接受过含VEN方案治疗。该方案VEN的剂量第1天为200 mg,其后均为400 mg固定剂量;LDAC 20 mg/m^(2)/d皮下注射。患者在治疗第8天复查骨髓,根据骨髓增生情况决定总疗程为10天还是14天。所有患者均不给予VEN单药治疗。有治疗反应的患者采用相同方案维持直到疾病进展或移植。结果:本研究纳入的R/R AmL患者,中位随诊时间为27.5个月。治疗期间未发生有临床表现的肿瘤溶解综合症(tumor lysis syndrome,TLS)。治疗后总反应率(overall response rate,ORR)为68.75%,其中4例达完全缓解(complete response,CR),1例达血液学未恢复的完全缓解(CR with incomplete hematologic recovery,CRi),6例达部分缓解(partial response,PR)。达最佳疗效的治疗周期中位数为1个周期。中位总生存期(overall survival,OS)为5.8(0.5~47.2)个月,中位无进展生存期(progression-free survival,PFS)为22.2(7.3~42.9)个月。发生的不良反应主要为3~4级的血液学不良事件和感染。结论:本研究根据治疗第8天骨髓复查结果调整用药天数的VEN+LDAC方案,对于既往没有接受过含VEN方案治疗的R/R AmL患者有较好的安全性和有效率。即使14天的VEN+LDAC治疗也是安全的。
