Is convenience really king? Comparative evaluation of catastrophic costs due to tuberculosis in the public and private healthcare sectors of Viet Nam: a longitudinal patient cost study

background In Viet Nam, tuberculosis (TB) represents a devastating life-event with an exorbitant price tag, partly due to lost income from daily directly observed therapy in public sector care. Thus, persons with TB may seek care in the private sector for its flexibility, convenience, and privacy. Our study aimed to measure income changes, costs and catastrophic cost incurrence among TB-affected households in the public and private sector.Methods Between October 2020 and March 2022, we conducted 110 longitudinal patient cost interviews, among 50 patients privately treated for TB and 60 TB patients treated by the National TB Program (NTP) in Ha Noi, Hai Phong and Ho Chi Minh City, Viet Nam. Using a local adaptation of the WHO TB patient cost survey tool, participants were interviewed during the intensive phase, continuation phase and post-treatment. We compared income levels, direct and indirect treatment costs, catastrophic costs using Wilcoxon rank-sum and chi-squared tests and associated risk factors between the two cohorts using multivariate regression.Results The pre-treatment median monthly household income was significantly higher in the private sector versus NTP cohort (USD 868 vs USD 578;P=0.010). However, private sector treatment was also significantly costlier (USD 2075 vs USD 1313;P=0.005), driven by direct medical costs which were 4.6 times higher than costs reported by NTP participants (USD 754 vs USD 164;P<0.001). This resulted in no significant difference in catastrophic costs between the two cohorts (Private: 55% vs NTP: 52%;P=0.675). Factors associated with catastrophic cost included being a single-person household [adjusted odds ratio [(aOR=13.71; 95% confidence interval (CI): 1.36–138.14;P=0.026], unemployment during treatment (aOR=10.86; 95%CI: 2.64–44.60;P<0.001) and experiencing TB-related stigma (aOR=37.90; 95%CI: 1.72–831.73;P=0.021).Conclusions Persons with TB in Viet Nam face similarly high risk of catastrophic costs whether treated in the public or private sector. Land use type affects the physicochemical properties of soil.The value of OM and TN is high in acacia soils.Strong acidity of soil is considered a serious constraint for agriculture.Suitable cash crops are acacia,cassava,banana,rice and maize.Soil fertility is affected by land-use types and land management,which exacerbates soil erosion and various other forms of soil degradation in the mountainous regions of Viet-nam.This study was conducted in A Luoi District,Thua Thien Hue,Vietnam to identify the effects of land-use types on specific soil physicochemical characteristics related to soil fertility.Soil physicochemical properties,such as organic matter(OM),total nitrogen(TN),total phosphorous(TP),and K+were significantly affected by land-use type.The results showed that the soils were sandy in rice but clay loam for acacia and cassava.The mean bulk density value of acacia soil was significantly greater than that of other soils.TN were higher in the acacia soils than those in the rice,maize,and banana soils.The OM content was significantly higher in the acacia,cassava,and banana soils than those in the rice and maize soils.The mean of exchangeable K+in the rice soil was higher compared to those in other soils and was affected by land-use type.The high exchangeable acidity content in the soils was probably due to intensive precipitation.However,both land use type and management did not affect the CEC value.Overall,the inappropriate land use caused the disturbance of soil physicochemical properties,indicating that the conditions of rice and maize soils are becoming worse than acacia soils.Therefore,lowering the intensity of cultivation,adopting incorporated soil fertility management,and applying organic fertilizer should preserve the existing conditions and enhance soil properties.

Heterotopic ossification(HO)is a consequence of traumatic bone and tissue damage,which occurs in 65%of military casualties with blast-associated amputations.However,the mechanisms behind blast-induced HO remain unclear.Animal models are used to study blast-induced HO,but developing such models is challenging,particularly in how to use a pure blast wave(primary blast)to induce limb fracture that then requires an amputation.Several studies,including our recent study,have developed platforms to induce limb fractures in rats with blast loading or a mixture of blast and impact loading.However,these models are limited by the survivability of the animal and repeatability of the model.In this study,we developed an improved platform,aiming to improve the animal's survivability and injury repeatability as well as focusing on primary blast only.The platform exposed only one limb of the rat to a blast wave while providing proper protection to the rest of the rat's body.We obtained very consistent fracture outcome in the tibia(location and pattern)in cadaveric rats with a large range of size and weight.Importantly,the rats did not obviously move during the test,where movement is a potential cause of uncontrolled injury.We further conducted parametric studies by varying the features of the design of the platform.These factors,such as how the limb is fixed and how the cavity through which the limb is placed is sealed,significantly affect the resulting injury.This platform and test setups enable well-controlled limb fracture induced directly by pure blast wave,which is the fundamental step towards a complete in vivo animal model for blast-induced HO induced by primary blast alone,excluding secondary and tertiary blast injury.In addition,the platform design and the findings presented here,particularly regarding the proper protection of the animal,have implications for future studies investigating localized blast injuries,such as blast induced brain and lung injuries.

通过对各向同性固结不排水试样进行循环三轴试验,研究了饱和重塑砂土的液化行为。试验采用恒幅正弦载荷,通过测量剪切模量、阻尼比、轴向应变和超孔隙水压力比的变化,评估了加载频率(分别为0.1、0.5、1.0 Hz)和压实度(分别为0.95、0.90和0.80)对砂土液化行为的影响。试验结果表明,对于压实度为0.95的砂样,在5000次加载循环中,0.1 Hz的加载频率对液化过程影响甚微;而对于压实度为0.90和0.80的砂样,此频率的影响显著。对于压实度为0.80的砂样,当频率增加到0.5 Hz时几乎一加载就直接发生液化。这表明增加加载频率会加快液化速度,而增加压实度则会减慢液化速度。因此,通过一系列的试验可以模拟现场砂土在不同频率下的动荷载条件。试验结果表明,饱和砂土的液化行为由以下因素决定:(1)试样的压实度越低,液化越容易发生;(2)加载频率越高,液化越容易发生;(3)循环轴向应变越大,发生液化所需的循环次数越少;(4)超孔隙水压力随加载循环次数增加,直至试样完全液化。

Optimizing Flow Path Design(FPD)is a popular research area in transportation system design,but its application to Overhead Transportation Systems(OTSs)has been limited.This study focuses on optimizing a double-spine flow path design for OTSs with 10 stations by minimizing the total travel distance for both loaded and empty flows.We employ transportation methods,specifically the North-West Corner and Stepping-Stone methods,to determine empty vehicle travel flows.Additionally,the Tabu Search(TS)algorithm is applied to branch the 10 stations into two main layout branches.The results obtained from our proposed method demonstrate a reduction in the objective function value compared to the initial feasible solution.Furthermore,we explore howchanges in the parameters of the TS algorithm affect the optimal result.We validate the feasibility of our approach by comparing it with relevant literature and conducting additional tests on layouts with 20 and 30 stations.

This study examined the feasibility of using the grey wolf optimizer(GWO)and artificial neural network(ANN)to predict the compressive strength(CS)of self-compacting concrete(SCC).The ANN-GWO model was created using 115 samples from different sources,taking into account nine key SCC factors.The validation of the proposed model was evaluated via six indices,including correlation coefficient(R),mean squared error,mean absolute error(MAE),IA,Slope,and mean absolute percentage error.In addition,the importance of the parameters affecting the CS of SCC was investigated utilizing partial dependence plots.The results proved that the proposed ANN-GWO algorithm is a reliable predictor for SCC’s CS.Following that,an examination of the parameters impacting the CS of SCC was provided.

Shear failure of slender reinforced concrete beams without stirrups has surely been a complicated occurrence that has proven challenging to adequately understand.The primary purpose of this work is to develop machine learning models capable of reliably predicting the shear strength of non-shear-reinforced slender beams(SB).A database encompassing 1118 experimental findings from the relevant literature was compiled,containing eight distinct factors.Gradient Boosting(GB)technique was developed and evaluated in combination with three different optimization algorithms,namely Particle Swarm Optimization(PSO),Random Annealing Optimization(RA),and Simulated Annealing Optimization(SA).The findings suggested that GB-SA could deliver strong prediction results and effectively generalizes the connection between the input and output variables.Shap values and two-dimensional PDP analysis were then carried out.Engineers may use the findings in this work to define beam’s geometrical components and material used to achieve the desired shear strength of SB without reinforcement.

Attribute reduction,also known as feature selection,for decision information systems is one of the most pivotal issues in machine learning and data mining.Approaches based on the rough set theory and some extensions were proved to be efficient for dealing with the problemof attribute reduction.Unfortunately,the intuitionistic fuzzy sets based methods have not received much interest,while these methods are well-known as a very powerful approach to noisy decision tables,i.e.,data tables with the low initial classification accuracy.Therefore,this paper provides a novel incremental attribute reductionmethod to dealmore effectivelywith noisy decision tables,especially for highdimensional ones.In particular,we define a new reduct and then design an original attribute reduction method based on the distance measure between two intuitionistic fuzzy partitions.It should be noted that the intuitionistic fuzzypartitiondistance iswell-knownas aneffectivemeasure todetermine important attributes.More interestingly,an incremental formula is also developed to quickly compute the intuitionistic fuzzy partition distance in case when the decision table increases in the number of objects.This formula is then applied to construct an incremental attribute reduction algorithm for handling such dynamic tables.Besides,some experiments are conducted on real datasets to show that our method is far superior to the fuzzy rough set based methods in terms of the size of reduct and the classification accuracy.

3-dimension(3-D)printing technology is growing strongly with many applications,one of which is the garment industry.The application of human body models to the garment industry is necessary to respond to the increasing personalization demand and still guarantee aesthetics.This paper proposes amethod to construct 3-D human models by applying deep learning.We calculate the location of the main slices of the human body,including the neck,chest,belly,buttocks,and the rings of the extremities,using pre-existing information.Then,on the positioning frame,we find the key points(fixed and unaltered)of these key slices and update these points tomatch the current parameters.To add points to a star slice,we use a deep learning model tomimic the form of the human body at that slice position.We use interpolation to produce sub-slices of different body sections based on the main slices to create complete body parts morphologically.We combine all slices to construct a full 3-D representation of the human body.
