Cytogenetic and molecular aberrations of multiple myeloma patients： a single-center study in Singapore

Background Much is known about the cytogenetic lesions that characterize multiple myeloma （MM） patients from the USA, Europe, and East Asia. However, little has been published about the disease among Southeast Asians. The aim of this study was to determine the chromosomal abnormalities of MM patients in our Singapore population. Methods Forty-five newly-diagnosed, morphologically confirmed patients comprising 18 males and 27 females, aged 46-84 years （median 65 years） were investigated by karyotyping and fluorescence in situ hybridization （FISH）. FISH employing standard panel probes and lp36/lq21 and 6q21/15q22 probes was performed on diagnostic bone marrow samples. Results Thirty-four cases （75.6%） had karyotypic abnormalities. Including FISH, a total detection rate of 91.1% was at- tained. Numerical and complex structural aberrations were common to both hyperdiploid and non-hyperdiploid patients. Numerical gains of several recurring chromosomes were frequent among hyperdiploid patients while structural rear- rangements of several chromosomes including 8q24.1 and 14q32 characterized non-hyperdiploid patients. With FISH, immunoglobulin heavy chain （IGH） gene rearrangements, especially fibroblast growth factor receptor 3 （FGFR3）/IGH and RB1 deletion/monosomy 13 were the most common abnormalities （43.4%）. Amplification lq21 was 10 times more frequent （42.5%） than del（1p36） and del（6q21）. Conclusions We have successfully reported the comprehensive cytogenetic profiling of a cohort of newly-diagnosed myeloma patients in our population. This study indicates that the genetic and cytogenetic abnormalities, and their frequencies, in our study group are generally similar to other populations.

Prospective validation of %p2PSA and the Prostate Health Index, in prostate cancer detection in initial prostate biopsies of Asian men, with total PSA 4-10 ng ml-1

Despite its widespread use for prostate cancer screening, low specificity makes PSA a suboptimal biomarker, especially in the diagnostic ＂gray zone＂ of 4-10 ng ml-1, False-positives lead to unnecessary biopsies with attendant morbidities. This is the first prospective validation study of %p2PSA and the Prostate Health Index （PHI） in Asian men presenting with a total PSA between 4.0 and 10 ng m1-1. We studied 157 Asian men between 50 and 75 years old, with normal per rectal prostate examinations, undergoing their first prostate biopsy, using a standardized biopsy protocol, for PSA levels of 4-10 ng ml-1, Thirty （19.1%） were found to have prostate cancer on biopsy. Statistically significant differences between patients with and without prostate cancer were found for total PSA, p2PSA, %p2PSA, and PHh The areas under the curve of the receiver operating characteristic curve for total PSA, %fPSA, %p2PSA, and PHI were 0.479, 0.420, 0.695, and 0.794, respectively. PHI predicts prostatic biopsies results best. At a sensitivity of 90%, the specificity （95% CI） of PHI was 58.3%, more than triple the specificity of total PSA at 17.3%, potentially avoiding 77 （49%） unnecessary biopsies. Similar to studies in mainly Caucasian populations, we have prospectively shown that %p2PSA and PHI greatly outperform total and free to total PSA ratio, in the detection of prostate cancer at first biopsy. Higher PHi levels also correspond to increasing the risk of detecting GS ≥7 cancers. We have validated the use of PHI to aid decision-making regarding prostate biopsies in Asian men with serum PSA between 4 and 10 ng *** its widespread use for prostate cancer screening, low specificity makes PSA a suboptimal biomarker, especially in the diagnostic ＂gray zone＂ of 4-10 ng m1-1, False-positives lead to unnecessary biopsies with attendant morbidities. 对于热带气旋的路径与强度预测，数值天气预报模式提供了相当有用的参考或指导预报。为进一步了解数值模式对掌握热带气旋生成的特性，基于近年国内外对热带气旋生成预报的研究结果，构造了一套热带气旋生成的多阈值组合判据系统，并利用该判据和ECMWF模式资料对2013-2015年于西太平洋海域生成的热带气旋进行了预报试验。结果表明，对于热带气旋生成的判据，使用不同百分位的历史统计阈值会对预报效果造成显著影响。在特定区间对某个物理量阈值进行合理的调整可以有效改善预报效果。在西太平洋海区，850hPa相对涡度阈值的微幅调整对结果的影响最为敏感。最优阈值组合方案的预报命中率检验结果表明，在数值模式起报时间为低压生成时间的前12—72小时时效曲线上，24和48小时时效是拐点，24小时时效之前，命中率基本维持在0．7以上的高位，随预报时间的降幅很小；之后预报质量陡降，48小时时效后预报效果趋向稳定。此外，不同年份的预报效果存在明显差异。在西太平洋不同海域，ECMWF模式对热带气旋的生成预报质量差异明显。菲律宾以东的洋面在不同起报时间的预报能力明显高于南海海域。西太平洋海区存在两个明显的高错报率区域：一个位于南海中、北部至菲律宾群岛近海，一个位于西太平洋中部。在不同大洋之间，ECMWF模式的热带气旋生成预报性能也存在显著分别。起报时间在热带气旋生成前24小时内，西太平洋海区对热带气旋生成的预报要明显优于大西洋海区。

A series of cyclometalated iridium（Ⅲ） complexes has been designed and synthesized, and their photophysical, photochromic and electrochemical properties have been studied. The X-ray crystal structure of complex 2 has been determined. The emission properties of the complexes have been shown to be readily perturbed through the modification of the electronic properties of the phenylpyridine ligand, leading to their tunable optical properties[1] for photochromic and material applications[2].

＜正＞Objectives:We have recently demonstrated that endothelium-dependent contractions can be observed in the hamster aorta treated with ng-nitro-L-arginine methyl ester（L-NAME）which inhibits nitric oxide（NO）production and found a likely endothelium-derived contracting factor（EDCF）,***,it remains unclear whether the reduction in endothelium-dependent contractions by endothelium-derived NO is due to the inhibition of PGF2alpha release,or whether NO plays a predominant role on vascular smooth muscle *** present study investigated the interaction between NO and ***:Aortic rings from young golden hamsters（aged～3 months） were suspended between two stainless steel wires in myograph chambers for the measurement of changes in isometric *** of prostaglandin F2alpha（PGF2alpha） was measured by enzyme ***: Endothelium-dependent contractions were elicited by acetylcholine（ACh） only in the presence of L-NAME. Incubation of 1H-[1,2,4]oxadiazolo[4,3-a]quinoxalin-1-one（ODQ,3μmol/L） also unmasked ACh-induced contractions with similar *** nitroprusside（SNP） inhibited endothelium-dependent contractions. Release of PGF2alpha in response to ACh was similar regardless the presence of ***:Our results showed that the release of PGF2alpha,which has been proposed as the EDCF in hamster aorta,was unaffected by the presence of *** on endothelium-dependent contractions by NO is possibly a downstream event occurring in vascular smooth muscle cells.

目的研究视紫红质基因(RHO)和视网膜色素变性1(RP1)基因在香港地区汉族视网膜色素变性(RP)患者中的突变频率及特征,并进一步探讨它们在RP发病机理中潜在的相互作用。方法运用高通量构象敏感凝胶电泳和直接测序方法,对151例香港地区汉族RP先证者和150名对照者进行了RHO和RP1基因全编码区和邻近剪切位点的内含子区域序列突变的检测。对携带基因突变的12例RP先证者的46名亲属进一步作分离分析。运用单因素分析、多因素分析和基因型家系不平衡分析研究RHO和RP1基因对RP的作用。结果RHO基因和RP1基因的突变检出率各为1.3%。RHO基因中-26G>A可增高RP危险性,而RP1基因中R872H则可降低RP危险性。多因素Logistic回归分析揭示了RHO基因IVS423G>A分别与RP1基因N985Y和C2033Y之间存在相互作用。结论在香港地区汉族RP患者中,RHO和RP1基因的突变率比其他人群中RP患者的突变率低。除了致病性基因突变,非编码区的序列改变也可改变RP的易感性。部分RP患者由双基因突变引起,当然多基因共同作用也完全可能存在。

以新疆交河故城遗址土做试样,首先用不同浓度的PS加固,经过一段时间干燥后得到不同初始含水率的试样,然后进行渗透系数测试,之后再进行不固结不排水试验,测量试样的抗剪强度。同时,进行PS加固试样的水稳定变化测试(即水中崩解试验),耐风蚀的风洞模拟试验,冻融破坏对加固强度的影响、温差破坏对力学强度影响及干湿破坏对力学强度影响等试验,研究古丝绸之路特殊环境因素对PS加固土遗址保护效果的影响。结果显示,PS加固对土的饱和渗透系数影响很小,而被3%和7%PS加固后的试样峰值抗剪强度比加固前的分别提高了90%和360%,残余抗剪强度也分别提高了50%和250%。结果说明,PS加固后土遗址仍具有很好的渗透性,但被雨水渗透饱和后,仍具有很明显的加固强度。这就证明PS适用于丝绸之路干旱区古代土遗址的加固保护。
