Peripheral nerve injuries induce a severe motor and sensory deficit. Since the availability of autologous nerve transplants for nerve repair is very limited, alternative treatment strategies are sought, including the use of tubular nerve guidance conduits(tnGCs). however, the use of tnGCs results in poor functional recovery and central necrosis of the regenerating tissue, which limits their application to short nerve lesion defects(typically shorter than 3 cm). Given the importance of vascularization in nerve regeneration, we hypothesized that enabling the growth of blood vessels from the surrounding tissue into the regenerating nerve within the tnGC would help eliminate necrotic processes and lead to improved regeneration. In this study, we reported the application of macroscopic holes into the tubular walls of silk-based tnGCs and compared the various features of these improved silk^(+) tnGCs with the tubes without holes(silk^(–) tnGCs) and autologous nerve transplants in an 8-mm sciatic nerve defect in rats. Using a combination of micro-computed tomography and histological analyses, we were able to prove that the use of silk^(+) tnGCs induced the growth of blood vessels from the adjacent tissue to the intraluminal neovascular formation. A significantly higher number of blood vessels in the silk^(+) group was found compared with autologous nerve transplants and silk^(–), accompanied by improved axon regeneration at the distal coaptation point compared with the silk^(–) tnGCs at 7 weeks postoperatively. In the 15-mm(critical size) sciatic nerve defect model, we again observed a distinct ingrowth of blood vessels through the tubular walls of silk^(+) tnGCs, but without improved functional recovery at 12 weeks postoperatively. Our data proves that macroporous tnGCs increase the vascular supply of regenerating nerves and facilitate improved axonal regeneration in a short-defect model but not in a critical-size defect model. AIM: To investigate the outcome of patients with symptoms of gastroesophageal reflux disease (GERD) referred for endoscopy at 2 and 6 mo post endoscopy. METhODS: Consecutive patients referred for upper endoscopy for assessment of GERD symptoms at two large metropolitan hospitals were invited to participate in a 6-mo non-interventional (observational) *** two institutions are situated in geographically and socially disparate areas. Data collection was by selfcompletion of questionnaires including the patient assessment of upper gastrointestinal disorders symptoms severity and from hospital records. Endoscopic finding using the Los-Angeles classification, symptom severity and it's clinically relevant improvement as change of at least 25%, therapy and socio-demographic factors were assessed. RESULTS: Baseline data were available for 266 patients and 2-mo and 6-mo follow-up data for 128 and 108 patients respectively. At baseline, 128 patients had erosive and 138 non-erosive reflux disease. Allmost all patient had proton pump inhibitor (PPI) therapy in the past. Overall, patients with non-erosive GERD at the index endoscopy had significantly more severe symptoms as compared to patients with erosive or even complicated GERD while there was no difference with regard to medication. After 2 and 6 mo there was a small, but statistically significant improvement in symptom severity (7.02 ± 5.5 vs 5.9 ± 5.4 and 5.5 ± 5.4 respectively); however, the majority of patients continued to have symptoms (i.e. , after 6 mo 81% with GERD symptoms). Advantaged socioeconomic status as well as being unemployed was associated with greater improvement. COnCLUSIOn: The majority of GORD patients receive PPI therapy before being referred for endoscopy even though many have symptoms that do not sufficiently respond to PPI therapy.

AIM: To evaluate the practice of nutritional assessment and management of hospitalised patients with cirrhosis and the impact of malnutrition on their clinical ***: This was a retrospective cohort study on patients with liver cirrhosis consecutively admitted to the Department of Gastroenterology and hepatology at the Royal Adelaide hospital over 24 mo. Details were gathered related to the patients' demographics, disease severity, nutritional status and assessment, biochemistry and clinical outcomes. nutritional status was assessed by a dietician and determined by subjective global assessment. Estimated energy and protein requirements were calculated by Simple Ratio Method. Intake was estimated from dietary history and/or food charts, and represented as a percentage of estimated daily requirements. Median duration of follow up was 14.9(0-41.4) mo. RESULTS: Of the 231 cirrhotic patients(167 male, age: 56.3 ± 0.9 years, 9% Child-Pugh A, 42% ChildPugh B and 49% Child-Pugh C), 131(57%) had formal nutritional assessment during their admission and 74(56%) were judged to have malnutrition. In-hospitalcaloric(15.6 ± 1.2 kcal/kg vs 23.7 ± 2.3 kcal/kg, P = 0.0003) and protein intake(0.65 ± 0.06 g/kg vs 1.01 ± 0.07 g/kg, P = 0.0003) was significantly reduced in patients with malnutrition. Of the malnourished cohort, 12(16%) received enteral nutrition during hospitalisation and only 6(8%) received ongoing dietetic review and assessment following discharge from hospital. The overall mortality was 51%, and was higher in patients with malnutrition compared to those without(hR = 5.29, 95%CI: 2.31-12.1; P nCLUSIOn: Malnutrition is common in hospitalised patients with cirrhosis and is associated with higher mortality. Formal nutritional assessment, however, is inadequate. This highlights the need for meticulous nutritional evaluation and management in these patients.

Watershed-based Image Segmentation with Region Merging and Edge Detection

The clustering technique is used to examine each pixel in the image which assigned to one of the clusters depending on the minimum distance to obtain primary classified image into different intensity regions. A watershed transformation technique is then employes. This includes: gradient of the classified image, dividing the image into markers, checking the Marker Image to see if it has zero points (watershed lines). The watershed lines are then deleted in the Marker Image created by watershed algorithm. A Region Adjacency Graph (RAG) and Region Adjacency Boundary (RAB) are created between two regions from Marker Image. Finally region merging is done according to region average intensity and two edge strengths (T1, T2). The approach of the authors is tested on remote sensing and brain MR medical images. The final segmentation result is one closed boundary per actual region in the image.

Collaborative work on increasingly complex hydroclimatic investigations often crosses disciplinary boundaries. Elements of scientific inquiry, such as data or the results of analyses can become objectified, or capable of being adopted and/or adapted by users from multiple disciplinary realms. These objects often provide a bridge for collaborative endeavors, or are used as tools by individuals pursuing multi-disciplinary work. Boundary object terminology was first formalized and applied by social scientists. however, few examples of the application of this useful framework are found in the hydrologic literature. The construct is applied here to identify and discuss how common researcb products and processes are used both internally and externally through providing examples from a project examining the historical and paleo proxy-based hydroclimatology of a headwaters region of Mongolia. The boundary object concept is valuable to consider when conducting and critiquing basic research, collaborating across multiple disciplinary teams as when studying climate change issues, as an individual researcher working in a cross boundary sense using methods from differing disciplines to answer questions, and/or when one group adapts the work of another to their own research problems or interpretive needs, as occurred with selected products of this project.
