Pancreaticoduodenectomy in the elderly over 80 years:a case report

BACKGROUnD: An increasing number of elderly patients have been considered for major surgical procedures, such as pancreaticoduodenectomy. The decision to recommend this operation for localized pancreatic cancer or other periampullary process in a very elderly patient is complicated by the frailty of the patient and the poor prognosis of the disease. Moreover, increased surgical experience associated with better patient selection may reduce the mortality rate, even in very elderly patients (over 80 years of age), after pancreaticoduodenectomy. METHODS: An 84-year-old woman underwent pancreatico- duodenectomy for ampullary adenocarcinoma. The tumor was classified pT3n0M0. RESULT: A good postoperative outcome was obtained. The patient is still alive, 18 months after operation. COnCLUSIOnS: Radical resection of periampullary tumors is safe in selected patients of advanced age, with morbidity and mortality rates approaching those observed in younger patients. Age alone should not be a contraindication for pancreatic resection.

A rigorous full wave technique based on the Transverse Wave Concept Iterative Procedure (WCIP) is used to design a complex Frequency Selective Surface (FSS). These surfaces include a periodically arrangement of identical circuit. There are used as filters and reflector antenna as well as deep-space exploration for multi-frequencies operations. A simple FSS structure is studied in the first stage to validate our approach. In the second stage two different complex structures are studied. The good agreement between simulated and published data justifies the design procedure.

Objectives: To evaluate the prevalence and the risk factors of new onset atrial fibrillation (AF) in a single medical ICU. Methods: A prospective observational study was conducted in a 10 bed single medical ICU over a period of 18 months. All patients with sinus rhythm admitted in the medical ICU were included. Those presenting with any arrhythmia on admission, having a PACEMAKER or having undergone a recent cardiothoracic surgery were excluded. Results: Of the 377 patients included on the study, atrial fibrillation occurred in 26 patients (7%). Patients who developed AF were older, had higher severity scores on admission, and required significantly more fluids, catecholamine, and mechanical ventilation. They had a longer ICU and hospital LOS. ICU mortality was significantly higher in AF patients. Five inde- pendent risk factors of AF were identified by multivariate analysis: advanced age, the presence of COPD, sepsis, car- diogenic shock, and hypoxemia. Conclusion: AF occurs in approximately 7% of medical critically ill patients and is associated with age, a history of COPD, the presence of sepsis, cardiogenic shock and hypoxemia. AF occurs in more critically ill patients and is associated with increased morbidity and mortality.

Introduction: The high resolution computed tomography (HRCT) is an important part in the diagnostic approach of interstitial lung disease (ILD) associated with connective tissue diseases (CTD) by providing detailed information on the elementary lesion and the radiological pattern of ILD. Aim: to point out the role of HRCT in the diagnosis of ILD associated with CTD (ILD-CTD). Methods: A Retrospective descriptive study was conducted between 2008 and 2017. Data of 24 patients presenting ILD-CTD were collected. A review of HRCT was performed by a radiologist without knowledge of the CTD. Results: Predominant elementary lesion of ILD associated with dermatomyositis (9 cases) was ground glass opacity (n = 9) followed by consolidation (n = 6). non Specific Interstitial Pneumonia (nSIP) was the most reported pattern (5 cases). Ground glass opacity was also the predominant elementary lesion for the 2 cases of scleroderma and in Sjögren’s syndrome (4 cases/5). nSIP was the predominant radiological presentation in these two CTD. Lymphoid interstitial pneumonia revealed Sjögren’s syndrome in one case. In rheumatoid arthritis (6 cases), the elementary HRCT lesions were irregular interlobular septal thickening (n = 4) and honeycombing (n = 4) consistent with Usual Interstitial Pneumonia (UIP) in 2 cases. Similarly UIP has been described for the 2 patients with lupus and mixed connective tissue disease. Conclusion: HRCT plays an important role in the management of ILD-CTD. Description of the HRCT elementary lesions and the radiological pattern of ILD can be helpful for CTD’s diagnosis.

Introduction: Apart from its increasingly important frequency, occupational asthma (OA) has become the most important cause of occupational respiratory disorders in developed countries. The number of etiological agents continues to grow following the constant introduction of new harmful substances in industry. Objectives: Determine the prevalence and incidence of OA in our country, the socio-occupational characteristics of asthmatics, and study the forensic and social impact of such pathology. Material and Methods: An exhaustive retrospective epidemiological study was conducted in Tunisia about charts of occupational asthma cases recognized by the competent commissions of the national Medical Care Fund (CnAM) during the period ranging from January 1st 2000 to December 31st 2008. Results: 361 occupational asthma cases, accounting for a prevalence of 7.17% of all occupational diseases, were recognized during the period of the study. Occupational asthma annual incidence was estimated to be 24.42 cases per 1000000 workers. Our study population was mainly women with a sex ratio of 0.86. More than a half (57.6%) of recognized OA cases were working in the textile industry sector. Concerning the asthmatics’ professional career, most of recognized OA cases (n = 225 i.e. 70.6% of cases) kept their jobs, whereas 58 patients did not. The professional career was linked to gender with a job loss that is 1.79 time more important in asthmatic women. Conclusion: The incidence of occupational asthma in Tunisia is comparable with that found in other Mediterranean countries. A rigorous and adequate prevention is necessary and allows reducing the importance of this occupational pathology and its serious consequences.

The effect of ethylene diamine tetraacetic acid(EDTA) modification on the physico-chemical properties and catalytic performance of silica nanosprings(nS) supported cobalt(Co) catalyst was investigated in the conversion of syngas(H^(2+) CO) to hydrocarbons by Fischer-Tropsch synthesis(FTS). The unmodified Co/nS and modified Co/nS-EDTA catalysts were synthesized via an impregnation method. The prepared Co/nS and Co/nS-EDTA catalysts were characterized before the FTS reaction by BET surface area,X-ray diffraction(XRD),transmission electron microscopy(TEM),temperature programmed reduction(TPR),X-ray photoelectron spectroscopy(XPS),differential thermal analysis(DTA) and thermogravimetric analysis(TGA) in order to find correlations between physico-chemical properties of catalysts and catalytic performance. FTS was carried out in a quartz fixedbed microreactor(H_2/CO of 2 ∶1,230 ℃ and atmospheric pressure) and the products trapped and analyzed by GC-TCD and GC-MS to determine CO conversion and reaction selectivity. The experimental results indicated that the modified Co/nS-EDTA catalyst displayed a more-dispersed phase of Co_3O_4 nanoparticles(10.9%) and the Co_3O_4 average crystallite size was about 12.4 nm. The EDTA modified catalyst showed relatively higher CO conversion(70.3%) and selectivity toward C_(6-18)(JP-8,Jet A and diesel) than the Co/nS catalyst(C_(6-14))(JP-4).
