A chemo-mechanical model is developed to investigate the effects on the stress development of the coating of polycrystalline ni-rich LinixMnyco_(z)O_(2)(x≥0.8)(nMc)particles with poly(3,4-ethylenedioxythiophene)(PEDOT).The simulation results show that the coating of primary nMc particles significantly reduces the stress generation by efficiently accommodating the volume change associated with the lithium diffusion,and the coating layer plays roles both as a cushion against the volume change and a channel for the lithium transport,promoting the lithium distribution across the secondary particles more ***,the lower stiffness,higher ionic conductivity,and larger thickness of the coating layer improve the stress *** paper provides a mathematical framework for calculating the chemo-mechanical responses of anisotropic electrode materials and fundamental insights into how the coating of nMc active particles mitigates stress levels.

目的探讨血清成纤维细胞生长因子21(FGF21)、分泌型卷曲蛋白5(Sfrp5)、微小RnA-21(miRnA-21)对冠心病(cHD)经皮冠状动脉介入治疗术(PcI)后支架内再狭窄(ISR)的预测价值。方法选择商水县人民医院2021年1月至2022年9月间行PcI手术治疗的cHD患者218例为cHD组,同期非心血管疾病的住院患者97例为对照组,比较两组FGF21、Sfrp5及miRnA-21;cHD组术后随访6个月,以是否发生ISR分为ISR组(n=56)及无ISR组(n=162);分析cHD患者行PcI手术后发生ISR的影响因素,分析FGF21、Sfrp5及miRnA-21对ISR的预测价值。结果cHD组FGF21、miRnA-21均高于对照组,Sfrp5水平低于对照组,差异有统计学意义(t=11.196、15.030、13.685,Pc)、FGF21、miRnA-21均高于无ISR组,Sfrp5水平低于无ISR组,差异有统计学意义(t=7.093、7.216、6.388,Pc回归分析显示,LDL-c、合并糖尿病、FGF21、Sfrp5、miRnA-21是cHD患者行PcI术后发生ISR的影响因素(Pc曲线分析显示,FGF21、Sfrp5、miRnA-21三者单独及联合检测预测PcI术后发生ISR的AUc分别为0.772、0.758、0.763、0.905(PnA-21在行PcI手术后发生ISR的cHD患者中表达异常,联合检测三者水平对于预测cHD患者PcI术后发生ISR具有较好的应用价值。

目的通过RP-HPLc法对瑞格列奈原料药的含量和其有关物质进行分析。方法采用j&K WpHc18色谱柱(4.6 mm×250 mm,5μm),使用0.1%甲酸溶液-甲醇(32∶68)作为流动相,流速为1.0 mL/min,检测波长为243 nm,柱温为20℃,进样量为20μL。结果瑞格列奈浓度在10.06~80.72μg/mL范围内线性相关性良好(Y=0.0121X+0.0107,r=0.9997),重复性试验RSD值为1.01%(n=6),平均回收率为99.2%,检测限为4.0 ng,定量限为11.5 ng。外标法测定瑞格列奈原料药平均含量为99.6%。主峰和其他杂质峰明显区别,三个批次的瑞格列奈样品有关物质的含量分别为0.17%、0.14%和0.15%。结论RP-HPLc法操作便捷、专属性好、精密度高、结果可靠,可用于瑞格列奈原料药的检测。

Objective:To evaluate the polio laboratory surveillance carried out from january,2019 to May,2023 by the Polio Regional Reference Laboratory,Sri ***:This retrospective study analyzed all stool samples received under the acute flaccid paralysis(AFP)and immunodeficient vaccine-derived poliovirus(VDPV)surveillance at Polio Regional Reference Laboratory,Sri Lanka from january,2019 to May,*** results of the testing methodologies were extracted from the laboratory data system,i.e.,poliovirus virus isolation,intra-typic differentiation/VDPV real time reverse transcriptase polymerase chain reaction(ITD/VDPV rRTPcR)and sequencing,along with the data on timing of reporting results,stool adequacy and *** was analyzed using descriptive ***:A total of 2141 stool samples from 1644 cases were received for AFP surveillance from Sri Lanka(93.61%),Maldives(1.52%),and immunodeficient VDPV(4.86%)*** polioviruses(19/1644,1.15%)and non-polio enteroviruses(73/1644,4.44%)were isolated,while Sabin-like 3 virus was detected in majority(12/19,63.15%)among the poliovirus *** polioviruses or circulating VDPVs were not detected among the *** all years of the study,the non-polio AFP detection rate was>1/100000 in children aged less than 15 years,whereas stool adequacy rate was>80%.All results were reported within 14 days of receipt,ensuring timely reporting as per global ***:The Polio Regional Reference Laboratory,Sri Lanka plays a vital role in maintaining the polio-free status in the country through its robust laboratory surveillance,while adhering to the surveillance ***-detection of wild polioviruses and circulating VDPV during the study period reinforces the polio-free status in the country.

目的探讨重复经颅磁刺激(rTMs)联合多奈哌齐治疗卒中后轻度认知障碍的临床疗效,以及对患者脑神经代谢物和炎性因子水平的影响。方法选取医院2020年1月至2023年10月收治的卒中后轻度认知障碍患者145例,根据治疗方案的不同分为对照组(70例)和观察组(75例)。两组患者均予多奈哌齐治疗,观察组患者加用rTMS,均治疗4周。结果观察组总有效率为84.00%,显著高于对照组的64.29%(Pch)、血清去甲肾上腺素(nE)、脑组织n-乙酰天门冬氨酸(nAA)与对侧nAA的比值]水平,认知功能[简易智力状态检查量表(MMSE)、蒙特利尔认知评估量表(MocA)、Rivermead行为记忆量表(RBMT)]评分,日常生活能力[改良Barthel指数(MBI)、功能独立性评定量表(FIM)]评分均显著升高(Pc/cr)、炎性因子[血清肿瘤坏死因子-α(TnF-α)、白细胞介素6(IL-6)、同型半胱氨酸(Hcy)、c反应蛋白(cRP)]水平均显著降低(P<0.05),且观察组上述指标改善均更显著(P<0.05)。观察组的不良反应发生率为8.00%,显著低于对照组的20.00%(P<0.05)。结论rTMS刺激联合多奈哌齐治疗卒中后轻度认知障碍的临床疗效良好,可改善患者的认知功能,提高脑神经代谢物水平和日常生活能力,降低炎性因子水平,促进预后,且安全性较好。

目的探讨经颅超声多普勒系统(transcranial doppler ultrasound, TcD)联合神经电生理监测技术(neurophysiological monitoring, SEP)对颈动脉内膜剥脱术(carotid endarterectomy, cEA)患者术中个体化血压管理、心肌耗氧及术后认知功能的影响。方法纳入择期行cEA治疗的老年患者120例作为研究对象,根据随机数字表法分为研究组和对照组,每组60例。对照组通过血管活性药物升高血压,研究组术中根据TcD和SEP联合监测结果指导血压调节工作。收集2组麻醉前(T00),麻醉后10 min(T0)、颈动脉阻断前5 min(T1)、颈动脉阻断前后即刻(T2)、解除颈动脉阻断即刻(T3)及解除颈动脉阻断后1 h(T4)时大脑中动脉血流速度(Vm)值和外周动脉收缩压,比较2组术中一般情况(心肌耗氧量、血管活性药物用量等)、术后认知功能障碍(postoperative cognitive dysfunction, POcD)发生率和半年后超声复查结果。结果研究组术中麻黄碱用量、尼卡地平用量、去甲肾上腺素用量、心肌耗氧量及术中发生心动过缓患者比例均低于对照组(PcD总发生率差异无统计学意义(P>0.05)。2组c反应蛋白、丙二醛水平呈先升高后降低趋势,超氧化物歧化酶水平呈先降低后升高趋势,研究组c反应蛋白、丙二醛水平明显高于对照组,超氧化物歧化酶水平明显低于对照组,组间、时点间、组间·时点间交互作用差异有统计学意义(PcD监测提示达到流转标准,其中7例患者同时达SEP波幅下降流转标准,给予转流术操作。TcD监测阳性率高于SEP监测(PcD联合SEP对cEA患者术中个体化血压管理具有较高的指导价值,可显著减少术中心肌耗氧量,降低术后不良事件的发生风险,其中TcD对cEA术中脑血流高灌注现象监测效果较好,而SEP对术中转流的实施指导意义更高。

Based on the use of the coupling coordination model to calculate the coupling coordination degree of carbon emission and pollutant control，the national，regional，and provincial spatiotemporal characteristics of the synergistic effect of pollution control and carbon emissions reduction in china were further analyzed，facilitating the crucial to identification of key areas. The fixed effects regression models and provincial panel data from 2006 to 2020 were used to explore factors contributing to better synergizing the reduction of pollution and carbon emissions in china. On this basis，the adjustment variable of R&D investment intensity was introduced，and the regulation effect model was constructed to further explore the influence mechanism of the synergistic effect of pollution reduction and carbon reduction. The results showed that：synergy exists between carbon emission reduction and the air pollution control system，the evolution of the synergistic effect of pollution reduction and carbon reduction in china presented an inverted "U"-shaped trend from 2006 to 2020，and there was spatial aggregation and a spatial spillover effect in pollution reduction and carbon reduction. The synergistic governance of carbon emission and pollutant control was still at a relatively low level. The carbon emission and air pollutant emission systems were still in an unstable and uncoordinated state. The results showed that：The degree of coordination of eastern china，central china，and western china decreased in turn. At the national level，energy consumption structure，per capita GDP，and the proportion of green investment were the main factors affecting the synergistic effect of pollution reduction and carbon. 目的在重症监护病房(IntensivecareUnit,IcU),护土难免会遇到需要限制治疗努力(limiting therapeutie efort,LTE)的患者,尽管多数护士还缺乏为这些临终患者及其家庭提供支持的准备。本综述旨在描述IcU护士在LTE中需要具备的能力。方法对2010年--2023年发表的文献进行综述。系统检索了5个数据库:Medline、Wiley Online Library、SciELO、ScienceDirect和Web of Science。使用关键评估项目工具西班牙语版(critical Appraisal Skills Programme in Spanish,cASPe)作为评判性阅读工具。使用牛津循证医学中心(center for Evidence-Based Medicine.,cEBM)评估工具确定证据级别和推荐强度。结果共纳入25篇文献。IcU护士LTE护理的能力包括对患者的直接护理、以家庭为中心的护理以及护士在团队中的作用。该综述确定了3个类别的能力:作为患者、家庭与跨学科团队之间维护者的能力,在LTE方面的决策能力,在患者生命末期治疗和护理干预的能力。结论IcU护士在LTE方面需要具备相应的能力,这对患者的生活质量死亡尊严以及患者家庭应对悲伤的全面支持至关重要。
