Long-term outcomes after endoscopic removal of malignant colorectal polyps:Results from a 10-year cohort

BACKGROUnD Choosing an optimal post-polypectomy management strategy of malignant colorectal polyps is challenging,and evidence regarding a surveillance-only strategy is *** to evaluate long-term outcomes after endoscopic removal of malignant colorectal *** A single-center retrospective cohort study was conducted to evaluate outcomes after endoscopic removal of malignant colorectal polyps between 2010 and *** disease rate and nodal metastases after secondary surgery and local and distant recurrence rate for those with at least 1 year of follow-up were *** rates for categorical variables and means for continuous variables with 95%confidence intervals were calculated,and Fisher’s exact test and Mann-Whitney test were *** risk factors of adverse outcomes were RESULtS In total,135 lesions(mean size:22.1 mm;location:42%rectal)from 129 patients(mean age:67.7 years;56%male)were *** proportion of pedunculated and non-pedunculated lesions was similar,with en bloc resection in 82%and 47%of lesions,*** differentiation,distance from resection margins,depth of submucosal invasion,lymphovascular invasion,and budding were reported at 89.6%,45.2%,58.5%,31.9%,and 25.2%,*** tumor was found in 10 patients,and nodal metastasis was found in 4 of 41 patients who underwent secondary surgical *** analysis identified piecemeal resection as a risk factor for residual malignancy(odds ratio:1.74;P=0.042).At least 1 year of follow-up was available for 117 lesions from 111 patients(mean follow-up period:5.59 years).Overall,54%,30%,30%,11%,and 16%of patients presented at the 1-year,3-year,5-year,7-year,and 9-10-year surveillance *** outcomes occurred in 9.0%(local recurrence and dissemination in 4 patients and 9 patients,respectively),with no difference between patients undergoing secondary surgery and surveillance *** Reporting of histological featur

Phlogopite solid-solutions have a wide pressure-temperature(P-t)stability field and are ubiquitous in a wide variety of geological settings,from deep lithosphere magmatic environments to upper crust metamorphic *** composition represents therefore a valuable physical-chemical archive and may provide important information regarding its crystallization and the petrogenesis of the *** this paper we examine the phlogopite phenocrysts from the well-known Fort Regent mica-bearing lamprophyre minette from ***(Island of Jersey,UK).Phlogopite phenocrystals from lamprophyres generally show normal-step and continuous compositional zoning,however those from the Fort Regent minette show a peculiar texture characterized by dark brown high-ti(average tiO_(2)≈8.5 wt.%)cores enveloped by euhedral low-to mid-amplitude zonation due to oscillatory contents in ti,Fe and ***-barometry modelling based on biotite-only composition yields relatively high P-t estimates(t≈970±54℃at P≈0.73±0.13 GPa)for cores whereas lower values(t≈790±54℃at P≈0.29±0.13 GPa)are obtained for the outer *** temperatures(t≈1075±54℃)but extremely high and anomalous pressure values(P≈1.82±0.13 GPa)are obtained for the yellowish inner *** combination of electron micro probe(EMP)analysis and single-crystal infra-red(FtIR)imaging in the OH-stretching region shows that the exceptional and oscillatory ti contents are due to the ti-vacancy substitution,typical of crystallization and growth processes of HP/Ht *** imaging provides additional insight for this process,confirming the dominant dioctahedral nature for the ti-Fe-rich cores and outer *** of thermobaric estimates obtained from the phlogopite compositiononly model,based on the fine-scale compositional evolution,shows that pressure-temperature values from low-ti high-Mg domains should be carefully evaluated because the substitution mechanisms during the dark mica growth are not univocal

Chronic hepatitis B causes a liver disease characterized by inflammation of the liver parenchyma. the aim of this study was to investigate the evolution of biological parameters in patients treated with tenofovir for chronic B infection at the Commune V referral health center in Bamako. We obtained a prevalence of 14.15%. the most represented age group was 31 - 40 years, with 36.8%. the sex ratio was 1.44 in favour of men. Viral load was undetectable after 18 months of treatment in 25 patients (42.37%). tenofovir, the 1st-line drug in Mali, is effective on the biological parameters monitored in patients.

the reduction of nitrate to nitrite by the oral microbiota has been proposed to be important for oral health and results in nitric oxide formation that can improve cardiometabolic conditions. Studies of bacterial composition in subgingival plaque suggest that nitrate-reducing bacteria are associated with periodontal health, but the impact of periodontitis on nitrate-reducing capacity(nRC)and, therefore, nitric oxide availability has not been evaluated. the current study aimed to evaluate how periodontitis affects the nRC of the oral microbiota. First, 16S rRnA sequencing data from five different countries were analyzed, revealing that nitratereducing bacteria were significantly lower in subgingival plaque of periodontitis patients compared with healthy individuals(P n all five datasets with n = 20–82 samples per dataset). Secondly, subgingival plaque, saliva, and plasma samples were obtained from 42 periodontitis patients before and after periodontal treatment. the oral nRC was determined in vitro by incubating saliva with 8 mmol/L nitrate(a concentration found in saliva after nitrate-rich vegetable intake) and compared with the nRC of 15healthy individuals. Salivary nRC was found to be diminished in periodontal patients before treatment(P t recovered to healthy levels 90 days post-treatment. Additionally, the subgingival levels of nitrate-reducing bacteria increased after treatment and correlated negatively with periodontitis-associated bacteria(P no significant effect of periodontal treatment on the baseline saliva and plasma nitrate and nitrite levels was found, indicating that differences in the nRC may only be revealed after nitrate intake. Our results suggest that an impaired nRC in periodontitis could limit dietary nitrate-derived nitric oxide levels, and the effect on systemic health should be explored in future studies.

numerical assessment of spacing–burden ratio to effective utilization of explosive energy

the spacing–burden(S/B) ratio plays significant role on rock fragmentation and proper utilization of explosive energy to minimize the undesirable *** S/B ratio generates fine fragments due to pressure rings coalescence of two blast holes,whereas boulder generations were observed above optimum S/B *** conditions are not acceptable because of wastage of explosive ***,to resolve this issue,a numerical model study was conducted to optimize the S/B ratio and to envisage its effect on rock fragmentation based on utilization of explosive *** element simulation tool was used to see the extent of two blast hole influence area variation with varying S/B *** better results were obtained at S/B ratio of 1:2 with optimum utilization of peak explosive *** performance was observed based on peak kinetic energy,peak pressure,radial and hoop stresses on centre of the two blast holes,where pressure rings coalescence.

Applying Earth Observation technologies to Economic Consequence Modeling:A Case Study of COVID‑19 in Los Angeles County,California

Earth observation(EO) technologies,such as very high-resolution optical satellite data available from Maxar,can enhance economic consequence modeling of disasters by capturing the fine-grained and real-time behavioral responses of businesses and the *** investigated this unique approach to economic consequence modeling to determine whether crowd-sourced interpretations of EO data can be used to illuminate key economic behavioral responses that could be used for computable general equilibrium modeling of supply chain repercussions and resilience *** applied our methodology to the COVID-19 pandemic experience in Los Angeles County,California as a case *** also proposed a dynamic adjustment approach to account for the changing character of EO through longer-term disasters in the economic modeling *** found that despite limitations,EO data can increase sectoral and temporal resolution,which leads to significant differences from other data sources in terms of direct and total impact *** findings from this analytical approach have important implications for economic consequence modeling of disasters,as well as providing useful information to policymakers and emergency managers,whose goal is to reduce disaster costs and to improve economic resilience.
