Outcomes of endoscopic submucosal dissection in cirrhotic patients: First American cohort

BACKGROUnD Among patients with cirrhosis and pre-malignant or early malignant mucosal lesions,surgical intervention carries a much higher bleeding risk.When such lesions are discovered,endoscopic submucosal dissection(ESD)may offer curative therapy with lower risks than surgery and improved outcomes compared to traditional endoscopic resection.AIM To evaluate the outcomes of ESD in patients with cirrhosis.METHODS Patients with cirrhosis undergoing ESD between July 2015 and August 2022 were retrospectively matched in 1:2 fashion to controls based on lesion location,size,and anticoagulation use.Procedural outcomes were compared between groups.RESULTS A total of 64 Lesions from 59 patients were included(16 cirrhosis,43 control).There were no differences in patient or lesion characteristics between groups.En bloc and curative resection was achieved in 84.21%,78.94%of the cirrhosis group and 88.89%,68.89%of controls,respectively,with no significant differences.Cirrhotic patients had significantly higher rates of intra-procedural coagulation grasper use for control of bleeding(47.37%vs 20%;P=0.02).There were otherwise no significant differences in adverse event rates.In the 29 patients with follow up,we found higher rates of recurrence in the cirrhosis group compared to controls(40%vs 5.26%;P=0.019),however this effect did not persist on multivariable analysis controlling for known confounders.COnCLUSIOn ESD may be safe and effective in patients with cirrhosis.Most procedure related outcomes were not significantly different between groups.Intra-procedural bleeding requiring use of the coagulation grasper use was expectedly higher in the cirrhosis group given the known effects of liver disease on hemostasis.

Intelligent traffic control requires accurate estimation of the road states and incorporation of adaptive or dynamically adjusted intelligent algorithms for making the decision.In this article,these issues are handled by proposing a novel framework for traffic control using vehicular communications and Internet of Things data.The framework integrates Kalman filtering and Q-learning.Unlike smoothing Kalman filtering,our data fusion Kalman filter incorporates a process-aware model which makes it superior in terms of the prediction error.Unlike traditional Q-learning,our Q-learning algorithm enables adaptive state quantization by changing the threshold of separating low traffic from high traffic on the road according to the maximum number of vehicles in the junction roads.For evaluation,the model has been simulated on a single intersection consisting of four roads:east,west,north,and south.A comparison of the developed adaptive quantized Q-learning(AQQL)framework with state-of-the-art and greedy approaches shows the superiority of AQQL with an improvement percentage in terms of the released number of vehicles of AQQL is 5%over the greedy approach and 340%over the state-of-the-art approach.Hence,AQQL provides an effective traffic control that can be applied in today’s intelligent traffic system.

This article signals the use of Artificial Intelligence (AI) in information security where its merits, downsides as well as unanticipated negative outcomes are noted. It considers AI based models that can strengthen or undermine infrastructural functions and organize the networks. In addition, the essay delves into AI’s role in Cyber security software development and the need for AI-resilient strategies that could anticipate and thwart AI-created vulnerabilities. The document also touched on the socioeconomic ramifications of the emergence of AI in Cyber security as well. Looking into AI and security literature, the report outlines benefits including made threat detection precision, extended security ops efficiency, and preventive security tasks. At the same time, it emphasizes the positive side of AI, but it also shows potential limitations such as data bias, lack of interpretability, ethical concerns, and security flaws. The work similarly focuses on the characterized of misuse and sophisticated cyberattacks. The research suggests ways to diminish AI-generating maleficence which comprise ethical AI development, robust safety measures and constant audits and updates. With regard to the AI application in Cyber security, there are both pros and cons in terms of socio-economic issues, for example, job displacement, economic growth and the change in the required workforce skills.

The halopyrimidine 5-bromo-2′-deoxyuridine(BrdU)is an exogenous marker of DnA synthesis.Since the introduction of monoclonal antibodies against BrdU,an increasing number of methodologies have been used for the immunodetection of this synthesized bromine-tagged base analogue into replicating DnA.BrdU labeling is widely used for identifying neuron precursors and following their fate during the embryonic,perinatal,and adult neurogenesis in a variety of vertebrate species including birds,reptiles,and mammals.Due to BrdU toxicity,its incorporation into replicating DnA presents adverse consequences on the generation,survival,and settled patterns of cells.This may lead to false results and misinterpretation in the identification of proliferative neuroblasts.In this review,I will indicate the detrimental effects of this nucleoside during the development of the central nervous system,as well as the reliability of BrdU labeling to detect proliferating neuroblasts.Moreover,it will show factors influencing BrdU immunodetection and the contribution of this nucleoside to the study of prenatal,perinatal,and adult neurogenesis.Human adult neurogenesis will also be discussed.It is my hope that this review serves as a reference for those researchers who focused on detecting cells that are in the synthetic phase of the cell cycle.

Saturation behavior of a one-pump fiber optical parametric amplifier in the presence of the fourth-order dispersion coefficient and dispersion fluctuation

The influence of the fourth-order dispersion coefficient on the behavior of parametric gain and saturation power of a one-pump fiber optical parametric amplifier over a signal wavelength span in the presence of fiber random dispersion fluctuations was investigated. The output signal power for the parametric gain calculation was obtained by numerically solving the three-coupled amplitude equations. Based on the calculations of the parametric gain over a variation of the signal wavelength, it is found that the saturation power behavior is dependent on the behavior of parametric gain. The manipulations of signal wavelength and the fourth-order dispersion coefficient changed the phase-matching condition, thereby affecting the resulting parametric gain and saturation power.

This study presents the Chebyshev polynomials-based Ritz method to examine the thermal buckling and free vibration characteristics of metal foam beams.The analyses include three models for porosity distribution and two scenarios for thermal distribution.The material properties are assessed under two conditions,i.e.,temperature dependence and temperature independence.The theoretical framework for the beams is based on the higher-order shear deformation theory,which incorporates shear deformations with higher-order polynomials.The governing equations are established from the Lagrange equations,and the beam displacement fields are approximated by the Chebyshev polynomials.numerical simulations are performed to evaluate the effects of thermal load,slenderness,boundary condition(BC),and porosity distribution on the buckling and vibration behaviors of metal foam beams.The findings highlight the significant influence of temperature-dependent(TD)material properties on metal foam beams'buckling and vibration responses.

Influence of biomass burning on ozone levels in the Megalopolis of Central Mexico during the COVID-19 lockdown

The massive reductions in anthropogenic emissions resulting from the COVID-19 lockdown provided a unique opportunity to evaluate the effect of mitigation measures aiming to abate air pollution.In Mexico,the total lockdown period took place during the dry-hot season when biomass burning activity is enhanced.Here,we investigate the role of biomass burning emissions on regional ozone levels in the Megalopolis of Central Mexico.The studied period covers the lockdown phases 2 and 3,and the first month of the new normal.We applied a factor separation technique and process analysis to estimate the pure and synergistic contributions of emission reductions under lockdown and that from biomass burning to daily ozone maximum concentrations in 7 metropolitan areas of different states in the Megalopolis.The results revealed that biomass burning plumes likely masked the effect of massive reductions from mobile emissions,impacted the PBL development during phase 3 and favored transition and mixed nO_(x)-limited and VOC-limited regional regimes.This contributed to increased ozone production in the middle to lower PBL by changing the regional background levels which potentially could bias high ozone production efficiency estimations.Given the Megalopolis contribution to economic and societal development at national scale,our study suggests that ozone mitigation measures during the dry-hot season targeting mainly mobile emissions will likely be offset by biomass burning plumes.A regional and synergic policy aiming to control biomass burning would help to reduce the occurrence of high ozone levels in Central Mexico with the co-benefit of tackling short-lived climate pollutants.

This study has isolated, characterized, and identified potential plant growthpromoting rhizobacteria（PGPR） with multiple PGP characteristics（n2-fixation,P-and K-solubilization, IAA, and siderophores production） from the rhizosphere BRIS soil of Acacia mangium. A total of 24 pure colonies were isolated and only 8 colonies were selected for further evaluation of the growth rate in 5% organic molasses medium supplemented with 2% KnO3. Based on the biochemical, potential PGP characteristics and growth performance, 3 superior PGPR strains were selected and identified as Paraburkholderia unamae（UA1）, Bacillus amyloliquefaciens（UA6）, and Enterobacter asburiae（UAA2） by partial sequencing of the 16S rRnA gene. The selected bacterial strains either in single or mixed（UA1 + UA6 + UAA2） cultures have shown a significant biochemical estimation of the PGP characteristics. Each strain has its own PGPR traits superiority with UA1 showing the best PGP characteristic followed by UA6 and UAA2. The use of mixed bacterial strains was beneficial as it showed the best performance in n2-fixation, siderophores production, and significant effect on corn phenology, growth and yield compared to using a single strain. These types of microbes showed potential to be used as biofertilizer and should be exploited more.

In today’s manufacturing industries,hard competition between rival firms makes it compulsory for researchers to design lighter and cheaper machine components due to the megatrends of cost-effectiveness and anti-pollution.At this point,aluminum syntactic foams(ASFs)are new-generation engineering composites and come into the upfront as a problem-solver.Owing to their features like low density,sufficient elongation,and perfect energy absorption ability,these advanced foams have been considerably seductive for many industrial sectors nowadays.In this study,an industrial-oriented automatic die casting technology was used for the first time to manufacture the combination of AA7075/porous expanded clay(PEC).Micro evaluations(optical and FESEM)reveal that there is a homogenous particle distribution in the foam samples,and inspections are compatible with the other ASF studies.Additionally,T6 aging heat treatment was operated on one half of the produced foams to explore the probable impact of aging on the compressive responses.Attained results show that PEC particles can be an alternative to expensive hollow spheres used in the previous works.Besides,a favorable relationship is ascertained between the aging treatment and mechanical properties such as compression strength and plateau strength.
