Endoscopic ultrasonography-guided drainage for patients with symptomatic obstruction and enlargement of the pancreatic duct

AIM: To evaluate the use of translumenal pancreatography with placement of endoscopic ultrasonography(EUS)-guided drainage of the pancreatic ***: This study enrolled all consecutive patients between June 2002 and April 2014 who underwent EUSguided pancreatography and subsequent placement of a drain and had symptomatic retention of fluid in the pancreatic duct after one or more previous unsuccessful attempts at endoscopic retrograde cannulation of the pancreatic duct. In all,94 patients underwent 111 interventions with one of three different approaches:(1) EUS-endoscopic retrograde drainage with a rendezvous technique;(2) EUS-guided drainage of the pancreatic duct; and(3) EUS-guided,internal,antegrade drainage of the pancreatic ***: The mean duration of the interventions was 21 min(range,15-69 min). Mean patient age was 54 years(range,28-87 years); the M:F sex ratio was 60:34. The technical success rate was 100%,achieving puncture of the pancreatic duct including pancreatography in 94/94 patients. In patients requiring drainage,initial placement of a drain wassuccessful in 47/83 patients(56.6%). Of these,26 patients underwent transgastric/transbulbar positioning of a stent for retrograde drainage; plastic prostheses were used in 11 and metal stents in 12. A ring drain(antegrade internal drainage) was placed in three of these 26 patients because of anastomotic stenosis after a previous surgical intervention. The remaining 21 patients with successful drain placement had transpapillary drains using the rendezvous technique; the majority(n = 19) received plastic prostheses,and only two received metal stents(covered self-expanding metal stents). The median follow-up time in the 21 patients with transpapillary drainage was 28 mo(range,1-79 mo),while that of the 26 patients with successful transgastric/transduodenal drainage was 9.5 mo(range,1-82 mo). Dietary nano chromium picolinate can ameliorate some of the impacts of heat stress in cross-bred sheep

Two studies were conducted to evaluate the effect of nano chromium picolinate(nCrPic)during heat stress(HS)in *** the initial study,36 Merino×Poll cross-bred sheep were individually penned and allocated to 3 dietary treatments(0,400 and 800μg/kg nCrPic)for 8 *** composition was determined at the beginning and end of the experiment using dual energy X-ray *** sheep remained in their dietary groups but were then placed in metabolic cages and randomly allocated within the dietary group to differing ambient temperature regimes,i.e.,thermo-neutral(Tn)(n=18)and HS(n=18),for 3 *** nCrPic had no effect on growth performance and body composition during the initial study conducted under Tn *** stress decreased average daily feed intake(ADFI)(P=0.002)whereas sheep under HS had reduced average daily gain(ADG)and indeed lost weight(PnCrPic increased both ADFI(P=0.041)and ADG(P=0.049)under both TH and HS conditions such that the performance of sheep receiving supplemental nCrPic and exposed to HS was similar to that of control sheep maintained under Tn *** stress increased rectal temperature(Pnd respiration rate(Png the hottest parts of the day as indicated by interactions(Pn time of day and thermal *** temperature was lower in sheep fed nCrPic(P=0.050),particularly under peak HS conditions during the afternoon as indicated by the interactions between dietary nCrPic and time of day(Pnd dietary nCrPic,thermal treat-ment and time of day(P=0.010).Similarly,respiration rate was lower in sheep fed nCrPic under peak HS conditions during the afternoon as indicated by the interactions between dietary nCrPic and thermal treatment(Pnd dietary nCrPic and time of day(P=0.030).In conclusion,dietary nCrPic can partially ameliorate the negative effects of HS as indicated by the maintenance of ADFI and decreased physiological responses,such as elevatio

A Possible Solution to the Disagreement about the Hubble Constant

The current disagreement about the Hubble constant H0 was described as a “Crisis in Cosmology”, at the April (2018) Meeting of the American Physical Society, and hence its resolution is of utmost importance. This work proposes that the solution to the disagreement between the Planck Collaboration cosmic microwave background (CMB) value of H0, together with the very close BOSS Collaboration baryon acoustic oscillation (BAO) value, and the significantly higher value of H0 found by the SHOES Collaboration cosmic distance ladder (CDL) work, is due to the fact that the CMB and BAO values of H0 are not for an accelerating universe, as generally believed, but are actually the values for a decelerating universe. In contrast, the CDL value of H0 is indeed that for an accelerating universe. It is shown that by replacing the negative deceleration parameter in the expression for logH0 in the CDL work by a positive deceleration parameter, the value of H0 can be brought down to agree with the CMB and BAO lower values. There is a brief review of the author’s decelerating model based on the Einstein de Sitter universe, augmented by a model of dark energy that does not have a negative pressure, but instead has a non-dispersive index of refraction n, causing the speed of light through the dark energy of intergalactic space to be reduced to c/n. As reported earlier, this assumption is sufficient to accommodate the increase in apparent magnitude of the Type Ia supernovae (Sne Ia). Additional support for the model is presented, together with a proposal for astronomical falsification.

A Possible Alternative to the Accelerating Universe II

This work revises and extends the author’s previous work (2015), Journal of Modern Physics, 6, 78- 87, by proposing that the index of refraction n of intergalactic space (IGS) is of electromagnetic origin. This leads to a theoretical expression for n that agrees very well with the least squares value obtained previously. A table comparing the fractional distance increase predicted by the two differently obtained indices is given. This revised view requires that the high energy charged particles found in cosmic rays originate from high energy neutral particles, presumably high energy gamma rays, that were able to travel through the IGS without energy loss due to Cherenkov radiation. An alternative explanation for the counter indication from the IceCube findings of Abassi, R., et al. (2012) nature, 484. 351-353 is proposed, which might also explain the findings of Aartsen et al. (2013) Physical Review Letters, 111, 021103. Since the model predicts galaxies act as divergent lenses, a geometrical analysis and corresponding figure describing this effect is given, as well as a table for a range of angles to the image galaxy relative to the direction to a target galaxy that is divergently lensed. The reduction of the speed of light in the IGS leads to a revision of the Planck (2015) value of the Hubble constant of ~68 km·s-1·Mpc-1 to ~47 km·s-1·Mpc-1, and hence an age for the Einstein-de Sitter universe greater than that of the oldest white dwarfs in the Galaxy, thereby resolving a long-standing problem with this model of the universe.

Structural Analysis of Hydroxamate Reagents by X-Ray Diffraction

Axis House manufactures and supplies alkyl hydroxamate reagent of various carbon chain lengths for application in flotation of oxide copper and other minerals. As a part of reagent performance improvement, the structural factors of hydroxamate are examined by various analytical tools in particular with X-ray diffraction of single crystals. In a recent publication, the structures of n-octyl hydroxamic acid and its corresponding potassium salt are presented with findings of hydroxamic functional group geometric preference, H-bonding, keto-enol tautomerisation, acid-salt aggregation and alignment of carbon chain. A Possible Alternative to the Accelerating Universe IV

This work briefly reviews and extends the author’s three previous works (2015), Journal of Modern Physics, 6, 78-87, and 1360-1370;(2016), 7, 1829-1844 that propose as an alternative to the accelerating ΛCDM universe, the decelerating Einstein de Sitter (EdS) universe, in which dark energy is a different phase of dark matter located only in intergalactic space (IGS), and that instead of a negative pressure, it has an index of refraction n≈1.50, and hence a reduced speed of light c / n through it. This allows the EdS universe to expand the extra distance necessary to obtain the diminished brightness of the Type Ia supernovae. In view of the recent suggestion that the universe is not accelerating, but possibly expanding uniformly, a table is given comparing both the accelerating and uniformly expanding universes with the EdS universe supplemented by the reduced speed of light. It is shown that fitting the uniformly expanding universe leads to a smaller value of n, and hence too short an age for the EdS universe, unlike the case with fitting the accelerating universe. The main result is that the proposed reduced speed of light in the IGS predicts discordant redshifts. It is shown that the current explanation of “accidental superposition,” is most likely insufficient to explain the number of observations, and that the present proposal could make up the difference. It can be tested astronomically, as illustrated in a figure.

Resolving impact volatilization and condensation from target rock mixing and hydrothermal overprinting within the Chicxulub impact structure

This work presents isotopic data for the non-traditional isotope systems Fe,Cu,and Zn on a set of Chicxulub impactites and target lithologies with the aim of better documenting the dynamic processes taking place during hypervelocity impact events,as well as those affecting impact structures during the post-impact *** focus lies on material from the recent IODP-ICDP Expedition 364 Hole M0077A drill core obtained from the offshore Chicxulub peak *** ejecta blanket samples from the UnAM 5 and 7 cores were used to compare the crater lithologies with those outside of the impact *** datasets of bulk Fe,Cu,and Zn isotope ratios are coupled with petrographic observations and bulk major and trace element compositions to disentangle equilibrium isotope fractionation effects from kinetic *** observed Fe and Cu isotopic signatures,with δ^(56/54)Fe ranging from0.95‰to 0.58‰and δ^(65/63)Cu from0.73‰to 0.14‰,mostly reflect felsic,mafic,and carbonate target lithology mixing and secondary sulfide mineral formation,the latter associated to the extensive and long-lived(>105 years)hydrothermal system within Chicxulub *** the other hand,the stable Zn isotope ratios provide evidence for volatility-governed isotopic *** heavier Zn isotopic compositions observed for the uppermost part of the impactite sequence and a metamorphic clast(δ^(66/64)Zn of up to 0.80‰and 0.87‰,respectively)relative to most basement lithologies and impact melt rock units indicate partial vaporization of Zn,comparable to what has been observed for Cretaceous-Paleogene boundary layer sediments around the world,as well as for tektites from various strewn *** contrast to previous work,our data indicate that an isotopically light Zn reservoir(δ^(66/64)Zn down to0.49‰),of which the existence has previously been suggested based on mass balance considerations,may reside within the upper impact melt rock(UIM)*** observation is restricted to a few UIM sampl

Speed of Gravitation and Electromagnetic Waves through the Dark Energy of Intergalactic Space and Its Implications of a Unified Theory

The recent finding that gravitational waves (GW170817) traveled at the same speed as electromagnetic (EM) waves (GRB 170817A) from a binary neutron star merger does not necessarily mean that they traveled throughout their journey at speed c. Some recent works by the author (2015) Journal of Modern Physics, 6, 78-87, and 1360-1370;(2016), 7, 1829-1844;(2017), 8, 622-635 show that the diminished brightness of Type Ia supernovae (Sne Ia) can be explained by assuming that dark energy, instead of having a negative pressure, has an index of refraction n, causing the speed of light through intergalactic space (IGS) to be reduced to c/n, with? n≈1.5. It follows that GWs (not considered in the previous works) would also travel with speed c/n through IGS. However, speed of GWs and EMWs within galaxies themselves is c. A brief review of the model is given, together with other predictions, e.g., new values for the Hubble constant and age of the universe, and necessary absence of correlation of neutrinos with gamma ray bursts (GRBs), in agreement with numerous searches. In the previous works, there were implications of a unified theory. If the model holds, since GWs would experience the same speed reduction as EMWs, this would further support unification. An improved falsification methodology for a previously proposed astronomical test based on discordant redshifts is given.
