Adjuvant Rectal Diclofenac for Post Operative Analgesia after caesarean Section—A Randomized controlled Study

BAcKGROUnD: Pain management following caesarean section still remains a challenge in our environment. Most potent analgesics are either not readily available or expensive. Diclofenac suppository is an nSAID that can be used for postoperative analgesia. It is available and affordable. OBJEcTIVE: To compare the efficacy and safety of combined rectal diclofenac and intramuscular pentazocine with intramuscular pentazocine alone for post operative pain control following lower segment caesarean section. METHODOLOGY: A total of 120 women who met the selection criteria scheduled for caesarean section under spinal anaesthesia with bupivacaine were randomized into two equal groups to receive either 75 mg diclofenac suppository 12 hourly for 24 hours or one anusol suppository (the placebo) 12 hourly for 24 hours. Both groups received pentazocine as primary analgesia. RESULT: The primary outcome measure is the proportion of patients with severe pain at 24 hours using the visual analogue rating scale. Secondary outcome measures are the time from surgery to ambulation, Passage of flatus, maternal satisfaction and presence of complications. Statistical analysis was done using spss version 22 and graph pad statistical package. Student T-test was used for continuous variables whereas chi square was used for categorical variables P cOncLUSIOn: Adjuvant rectal diclofenac is superior to pentazocine alone in the management of pain after caesarean section. Less number of patients had moderate to severe pain at 24 hours post operation. Maternal satisfaction in relation to pain management is better with diclofenac suppository. The levels of complications were comparable in both groups.

Ross’ epidemic model describing the transmission of malaria uses two classes of infection, one for humans and one for mosquitoes. This paper presents a stochastic extension of a deterministic vector-borne epidemic model based only on the class of human infectious. The consistency of the model is established by proving that the stochastic delay differential equation describing the model has a unique positive global solution. The extinction of the disease is studied through the analysis of the stability of the disease-free equilibrium state and the persistence of the model. Finally, we introduce some numerical simulations to illustrate the obtained results.

Description of Post-Stroke cognitive Disorders at Abidjan University Hospitals, côte d’Ivoire

Stroke patients often experience motor deficits and cognitive problems after a stroke. Objective: To improve our understanding of the cognitive consequences of stroke. Method: a descriptive and analytical cross-sectional study was conducted over 12-month period in the neurology departments of the cocody and Treichville Hospitals in côte d’Ivoire. Results: Out of 724 patients admitted to the neurology department, 415 (57.32%) were stroke patients, of which 145 (34.94%) were screened. The frequency of global cognitive functioning impairment was 86.21%, significantly higher than the frequency of patients without impairment, which was 13.79%. The study focused on detailing the cognitive status of stroke patients in neurology departments, assessing several cognitive functions during the subacute phase of stroke. These functions included global cognitive functioning, executive functions, language and memory. The frequency of post-stroke cognitive impairment is high among stroke patients in Abidjan. This frequency is comparable to figures found in Subsaharian stroke populations. Demographic and clinical characteristics studied included age, gender, education level, employment status, vascular diseases and cerebral affected area. Among these characteristics, only the education level and the cerebral affected area have been found significant. conclusion: The incidence of cognitive impairment after a stroke is significantly high among stroke patients in Abidjan.

In this work,several ceria-zirconia based catalysts with very lo w（and equimolar） metal contents were prepared,characterised and tested for the cO oxidatio n reaction（under lean and stoichiometric conditions）,trying to emulate those conditions found in a diesel oxidation catalyst（DOc） system from a diesel engine and those encountered under gasoline exhaust（λ=1）.The metals chosen are cu,co,Ag and Pt（as a reliable benchmark）.The results reveal enormous differences among reducibility and catalytic activity despite quite similar structural and textural properties of the catalysts,showing differences among dispersion（Ag-catalyst seems to present a low level of dispersion）.This catalyst seems to be characterised,as well,by a strong electronic interaction between ce and Ag centres which is suggested to yield an improved reducibility under H2-TPR ***,the order in catalytic activity（cu>Ag>co≈Pt>>support） seems not to follow the order found in reducibility and the cu-catalyst seems to be the most active independently on the reaction conditions,yielding nearly overlapped cO oxidation catalytic *** restingly,a stro ng correlation between the catalytic activity under the two conditions tested and the OSc values of the ce0.8Zr0.2O2-supported metal catalysts is ***,OSc parameter measured at 150℃ can be used as a relevant descriptor to evaluate the cO oxidation activity at low and medium conversions for the investigated catalysts,much better than the H2-TPR measurements.

Evaluation of Mandibular Deprogramming Using TEnS (Transcutaneous Electrical nerve Stimulation) in Patients at the Specialty clinic of the Juarez Autonomous University of Tabasco from 2022 to 2024

Temporomandibular disorders (TMD) negatively affect quality of life, causing pain and restricted jaw movements. This study evaluates the effectiveness of Transcutaneous Electrical nerve Stimulation (TEnS) on TMD patients before orthodontic treatment at the Juarez Autonomous University of Tabasco. A quantitative, quasi-experimental study was conducted with 30 patients who met the inclusion criteria: aged 18 to 65 years, with TMJ pain symptoms, diagnosed with temporomandibular disorders using the Helkimo index, and who had provided informed consent for the study. The range of mandibular movement and the Visual Analog Scale (VAS) were measured to record values before therapy. The electrodes of the TEnS therapy were positioned on both sides of the jaw in the preauricular area and the masseter muscle, the therapy was administered for 40 minutes, utilizing a frequency of 2 - 4 Hz and a pulse duration of 300 microseconds. Following this, the range of mandibular movement and the VAS were reassessed to evaluate the outcomes., significantly improved maximum mouth opening (37.7 ± 6.4 mm to 44.6 ± 5.88 mm), right laterality (3.86 ± 1.57 mm to 5.43 ± 1.21 mm), left laterality (3.13 ± 1.63 mm to 4.53 ± 1.61 mm), protrusion (4.01 ± 1.7 mm to 5.36 ± 1.32 mm), and pain perception (4.56 ± 2.045 to 2.03 ± 1.5) (P nS therapy enhances mandibular movement and reduces pain, making it a well-tolerated, non-invasive complementary treatment for TMD.

Efficacy of 1% Permethrin and 2% Permethrin combined with 1% Piperonyl Butoxide against Rhipicephalus sanguineus Ticks in naturally Infested Dogs

The aim of the present study was to compare the ixodicide efficacy of 1% permethrin applied as a shampoo-bath in single treatment and a combined formulation of 2% permethrin + 1% piperonyl butoxide applied as powder in a single treatment against Rhipicephalus sanguineus ticks, in naturally infested dogs. Eighteen adult cross-mixed dogs, harboring to at least 20 ticks/animal, were selected on pre-treatment live tick counts. On day 0, the dogs were then divided in 3 groups of 6 animals each, for treatment: Group 1 (G1) received 1% permethrin applied as shampoo-bath (approximately 10 ml/kg bw), according to label instructions. G2 was treated with 2% permethrin + 1% piperonyl butoxide applied as powder, and rubbed as massage on the body, as recommended by the producer. G3 served as a non-treated control. After the treatment, the dogs were checked out for live ticks counting on days 0, 1, 7, and 14. The ixodicide efficacy was assessed based on the percentage reductions of ticks on the days analyzed post-treatment, with respect to the untreated control. Results showed that permethrin alone removed ticks on 81%, 93.1% and 89.6% for days 1, 7 and 14, respectively. The combined formulation of 2% permethrin + 1% piperonyl butoxide exerted a better efficacy of 98.8, 97.7 and 97.0%, respectively. It is concluded that this combined formulation was highly effective at rapidly repelling and killing Riphicephalus sanguineus ticks on naturally infected dogs, showing a significant effect on days 1, 7 and 14 after tick exposure. The sustained high and quick level of efficacy of this combination may well interfere with the transmission of Riphicephalus sanguineus ticks. It is concluded that the sustained high level of efficacy of the permethrin + piperonyl butoxide combination may well exert an additive effect and could interfere with the transmission of Riphicephalus sanguineus ticks.

目的在重症监护病房(Intensive care Unit,IcU),护士难免会遇到需要限制治疗努力(limiting therapeutic effort,LTE)的患者,尽管多数护十还缺乏为这些临终患者及其家庭提供支持的准备。本综述旨在描述IcU护士在LTE中需要具备的能力。方法对2010年—2023年发表的文献进行综述。系统检索了5个数据库:Medline、Wiley Online Library SciELO、ScienceDirect和Web of Science。使用关键评估项目工具西班牙语版(critical Appraisal Skills Programme in Spanish,cASPe)作为评判性阅读工具。使用牛津循证医学中心(center for Evidence-Based Medicine,cEBM)评估工具确定证据级别和推荐强度。结果共纳入25篇文献。IcU护士LTE护理的能力包括对患者的直接护理、以家庭为中心的护理以及护士在团队中的作用。该综述确定了3个类别的能力:作为患者、家庭与跨学科团队之间维护者的能力,在LTE方面的决策能力,在患者生命末期治疗和护理干预的能力。结论 IcU护十在LTE方面需要具备相应的能力,这对患者的生活质量、死亡尊严以及患者家庭应对悲伤的全面支持至关重要。
