Head and neck Cancer: Closer Look at Patients Quality of Life

Head and neck cancer (HnC) as a disease and its treatment have a disproportionate impact on all aspects of patients’ quality of life (QOL). QOL is a multidimensional construct of an individual’s subjective assessment of the impact of an illness or treatment on his/her physical, psychological, social and somatic functioning and general well-being [1]. Thus, QOL has become an important treatment outcome in HnC [2]. Objective: It is to evaluate the quality of life of patients dealing with squamous cell carcinoma of the head and neck treated in the radiotherapy department, national Cancer Institute, Cairo University. Material and methods: Starting from March 2012 to December 2013, 87 patients with histopathologically proven Head and neck Squamous Cell Carcinoma (HnSCC) were taking part in our prospective study. The patients’ QOL answers to the questionnaire were collected for both the EORTC QLQ-C30 and the EORTC Head and neck questionnaires (QLQ-H&n35) and then all points were transformed to a 0 - 100 scale according to the guidelines of the EORTC. Certain clinical factors were chosen to evaluate their effect on the QOL;site of primary tumor, clinical stage, and mode of therapy applied: radiotherapy versus chemo-radiotherapy (CRT). Moreover, age, gender and smoking were studied. Results: Majority of the items assessed in the QLQ-C30 and QLQ-H&n35 questionnaire showed that they were negatively impacted at the end of treatment. As for the factors that had a negative effect on the QOL, they comprised;female gender (p ng (p nical stage III and IV (p nx and hypopharynx as a site of primary (p nd CRT (p nclusions: Results of our study showed that treatment options had a significant negative effect on the QOL by the end of the treatment course. Moreover, our results showed that age, gender, smoking habit, tumor site, and clinical stage of disease had a significant impact on the QOL of HnSCC

Structure Mechanism of Ordinary Matter Mass Formation

The famous paradoxes of quantum mechanics are created by the fact that elementary particles exist as the alternation between two structural states with different properties. This leads to probabilistic behavior, uncertainty principle, quantum tunneling, etc. The alternation of states plays the role of the frequency generator or clock. But for the objective character of quantum interactions the length standard also should exist in nature. Such analog of the rule must be physically real and in direct sense have to participate in the of particles interactions. Just this is the main role of physical vacuum. For such role vacuum should have quasi-crystalline geometry structure. Its symmetry requires in standard views only one fundamental change. In the quasicrystalline structure of the vacuum, the virtual shells of the real particles and atomic nuclei are not diffuse “clouds”, as is assumed today. Virtual environments are clearly structured and rigidly quantised shells with the geometric structure similar to fullerenes. Such shells are forming for greater than 99% of the known substance mass. Virtual particles forming such shells belong to the group of bosons and probably are just Higgs bosons existing in the ordinary matter. Chemical fullerenes form a series of discrete geometric structures. In a similar manner virtual analogues of fullerenes form a series of discrete masses, which really exist in the nature as a set of elementary particles and atomic nuclei masses.

Life Origin in the Milky Way Galaxy: I. The Stellar nucleogenesis of Elements necessary for the Life Origin

Chemical elements in space can be synthesized by stellar nuclear reactors. Studying the dynamics of processes occurring in the stars introduces a concept of the ensemble-averaged stellar reactor. For future interstellar missions, the terrestrial and solar abundances were compared with considerable number of stars allocated in the ~200 pc solar neighborhood. According to the value of the effective temperature, four stellar classes are distinguished, for which the correlation coefficients and standard deviation are calculated. The statement about the possibility of transferring heavy elements synthesized by stars over long distances in space has been completely refuted. There is no immutability of the distribution of elements on neighboring stars and in the Solar System. It is shown that chemical elements are mainly synthesized inside each stellar reactor. The theory of the buoyancy of elements is generalized to stars. It has been suggested that stars overheat due to a shift in the parameters of nuclear processes occurring inside stars, which leads to the synthesis of heavy elements.

Is Dosage Adjustment for Olanzapine Really necessary?

Schizophrenia is a prevalent and disabling disorder, commonly treated with medications such as clozapine and olanzapine. However, long-term side effects and limitations of these drugs, coupled with treatment resistance in a significant proportion of patients, necessitate alternative strategies. Furthermore, individuals with schizophrenia are at an increased risk of developing kidney and liver diseases, which may be influenced by cardiovascular comorbidities and shared genetic markers. Considering the use of olanzapine in patients with severe liver or kidney diseases requires careful evaluation. Although these organs play crucial roles in olanzapine excretion and metabolism, current pharmacological research suggests that dosage adjustment may not be necessary even in the presence of severe organ disease. Olanzapine acts on D2 and 5HT2A receptors, alleviating both positive and negative symptoms of schizophrenia. However, the metabolism and clearance of olanzapine exhibit substantial inter-individual variability influenced by factors such as gender, age, ethnicity, smoking habits, and co-medication. Additionally, olanzapine may induce unwanted side effects, including prolactin release, metabolic dysregulation, and liver-related complications. The present study aims to investigate whether dosage adjustment of olanzapine is necessary for individuals with comorbid moderate liver and severe kidney disease. While the study remains ongoing, preliminary findings using a pharmacokinetic model predict that dosage adjustment may not be required in these patients. The expected olanzapine plasma concentration in individuals with both conditions is estimated to be 18.14ng/ml, which is considerably below the identified toxic dosage threshold of 100ng/ml. However, further investigations are warranted to validate the findings and establish definitive guidelines and personalize treatment strategies for individuals with both liver and kidney disease.

1. In 1867 Sylvester published a paper in Philos. Mag. with perhaps the funniesttitle ever written in the mathematical literature. The various questions treated hereall led him to the problem to determine for which n-values one can construct a determinantof order n consisting exclusively from±1 and which is orthogonal in the sense that thecomposition of any two different rows is 0. Temporal no-Linearity: An Alternative to Dark Energy

Mathematical compatibilities and constraints of a hypothetical 5D space-time with time referenced by two coordinates (3-2) have been revisited in detail in several recent papers. It has been prescribed from the GR the compatibility constraints of the FLRW metric in each temporal brane, to be restricted to Closed Universes, Smooth Initial Singularities, and “Open CTC”. In a first view, this leads to leaving these works considering mathematical games discarded by the Standard Candles data. However, if time would be referred by two coordinates, they would not be linearly related, and it will be mathematically stated that space-time may not be flat in any case because time-like branes geometry will never be. If so, the time scale “lived” over a two-time dimension geodesic necessarily is not constant over its linear projection on one of both coordinates. Consequently, the correlations between Redshift and Distance Modulus—Distance Ladder—may be corrected by a synchronization function (if a no-linear two-time geodesic trajectory over a “warped temporal geometry” is linearly divided into constant segments, then their projections are not linear in any case). We apply an example of time-trajectory over the time slices, matching the Standard Candle data for a Closed Universe dominated by matter (>90%) in a bulk 3-2 configuration, with open temporal branes and smooth singularity. If time can be referred by two coordinates, then there is no need of Darkness to explain astrophysical data and Universe can be closed.

Giant Comet C/2014 Un271 (Bernardinelli-Bernstein) Provides new Evidence for Cometary Panspermia

The discovery of a giant comet (C/2014 Un271) at a distance of 29 AU in October 2014 and the later discovery in September 2021 of a dramatic brightening episode offers an ideal opportunity for verifying the predictions of a “biological” comet. The eruptions of the comet at a heliocentric distance of 20 AU are plausibly explained as due to high pressure venting of the products of microbial metabolism in radioactively heated subsurface lakes. The standard non-biological model of comets is woefully inadequate to account for eruptions at such large distances from the sun where surface temperatures are as low as 60 K.

The Ultimate Anti-Seismic Design Method

The design mechanisms and methods of the invention are intended to minimize problems related to the safety of structures in the event of natural phenomena such as earthquakes, tornadoes, and strong winds. It is achieved by controlling the deformations of the structure. Damage and deformation are closely related concepts since the control of deformations also controls the damage. The design method of applying artificial compression to the ends of all longitudinal reinforced concrete walls and, at the same time, connecting the ends of the walls to the ground using ground anchors placed at the depths of the boreholes, transfers the inertial stresses of the structure in the ground, which reacts as an external force in the structure’s response to seismic displacements. The wall with the artificial compression acquires dynamic, larger active cross-section and high axial and torsional stiffness, preventing all failures caused by inelastic deformation. By connecting the ends of all walls to the ground, we control the eigenfrequency of the structure and the ground during each seismic loading cycle, preventing inelastic displacements. At the same time, we ensure the strong bearing capacity of the foundation soil and the structure. By designing the walls correctly and placing them in proper locations, we prevent the torsional flexural buckling that occurs in asymmetrical floor plans, and metal and tall structures. Compression of the wall sections at the ends and their anchoring to the ground mitigates the transfer of deformations to the connection nodes, strengthens the wall section in terms of base shear force and shear stress of the sections, and increases the strength of the cross-sections to the tensile at the ends of the walls by introducing counteractive forces. The use of tendons within the ducts prevents longitudinal shear in the overlay concrete, while anchoring the walls to the foundation not only dissipates inertial forces to the ground but also prevents rotation of

Life Origin in the Milky Way Galaxy: II. Scanning for Habitable Stellar Systems on Behalf of Future Space Missions

The possibility of the life origin in the stellar systems, located at a distance of ~200 pc from the solar system, was investigated. The stars, in the spectrums of which C (carbon), O (oxygen), n (nitrogen), and P (phosphorus) are found, are called DnA-stars. Based on stellar abundances a new method for searching for habitable exoplanets has been developed and a list of 48 DnA-stars in the solar neighborhood, on which life is possible, has been defined. The quota of DnA-stars is equal 1.3% of the total amount of Hypatia Stellar Catalog. Only three DnA-stars out of selected 48 stars belong to the spectral class as our Sun (G2V). The closest to the solar system is the DnA-star with the number HIP 15510, which belongs to the G8V class and is 6 pc away from the solar system. nine DnA-stars, which have the highest chemical similarity with solar spectrum, were identified. It is identified that one of these nine stars, HIP 24681, has six planets.

The Possible Involvement of Apoptotic Decay of Terminal Deoxynucleotidyl Transferase-Positive Lymphocytes in the Reutilization of the Extracellular DnA Fragments by Surrounding Living Cells

The migrating TdT+ thymocytes can die in other tissues, promoting the surrounding cells’ renewing likes holocrine secretion does. To clarify the role of TdT-enzyme for this function of progenitor lymphocytes, their extracellular media with its components included by living cells analyzed in vitro before and after in vivo irradiation of donor rats. The nucleoid with Dnase-sensitive (free) DnA and TdT activity discovered in extracellular media conditioned preliminary by spontaneous apoptotic death of a minor part of the thymocyte’s suspension in vitro. The penetration of labeled products of non-template synthesis with free DnA’ primers from media into cells by pinocytosis confirmed by exogenous polymeric DnA marked artificially. The DnA penetration into cells follows an increase of the cell’s viability and acceleration of spontaneous intracellular DnA-synthesis controlled with labeled thymidine uptake. Both phenomena are typical for either the lowest initial concentration of intact cells or their preliminary irradiation in vivo. The data point to possible involvement of apoptotic decay of TdT+ cells in the reutilization of the extracellular DnA fragments for reparation/regeneration of surrounding living cells.
