Backgroun.: South Asian.population. are vuln.rable to poor health outcomes associated with climate due to the region.s complex topography, poverty, lack of education. an. population.den.ity. Physician., bein. highly trusted members of society, can.help mitigate climate chan.e effects in.their coun.ries. Objectives: The presen. study assessed perception. of physician. from four South Asian.coun.ries (Ban.ladesh, Bhutan. In.ia, an. Pakistan.. We hypothesized that physician. from these coun.ries will be aware an. kn.wledgeable about health effects of climate chan.e. Methods: We con.ucted a cross-section.l on.in. survey of physician. between.March an. July 2022. Data of 201 physician. was an.lyzed usin. the Kruskal-Wallis test an. Dun.’s post hoc method. Results: Physician. from four South Asian.coun.ries were attun.d to the health risks associated with climate chan.e. A majority of physician. (86.6%) believed that climate chan.e would make health con.ition. in.their coun.ries more severe or frequen. (Ban.ladesh 83.3%;Bhutan.70%;In.ia 91% an. Pakistan.90.8%). The most common.health effect related to climate chan.e was illn.sses related to reduced outdoor air quality (54.3%). Common.barriers to discuss climate chan.e with patien.s from all four coun.ries were physician.’ lack of kn.wledge (stron.ly agree, 13.8%;agree 47.7%) in.how to approach the issue with patien.s, lack of time (stron.ly agree, 12%;agree, 48%) an. their perception.that patien.s were n.t in.erested in.kn.win. about climate chan.e (stron.ly agree, 8.6%;agree, 42.6%). More than.half (56.2%) of the physician. reported n. train.n. (Ban.ladesh, 51.7%;Bhutan. 80%;In.ia, 44.6%;Pakistan. 63.1%) an. approximately 30% reported less than.20 hours of train.n. (Ban.ladesh, 28.3%;Bhutan. 15.0%;In.ia, 39.3%;Pakistan. 27.7%) in.climate-chan.e-related health effects. Con.lusion. Objective: To develop an. in.ern.lly validate a n.mogram to predict recurren.e-free survival (RFS) in.ludin. the time to radical cystectomy (RC) an. perioperative blood tran.fusion.(PBT) as poten.ial ***: Patien.s who un.erwen. open.RC an. ileal con.uit between.Jan.ary 1996 to December 2016 were split in.o developin. (n.948) an. validatin. (n.237) cohorts. The time to radical cystectomy (TTC) was defin.d as the in.erval between.the on.et of symptoms an. RC. The regression.coefficien.s of the in.epen.en. predictors obtain.d by Cox regression.were used to con.truct the n.mogram. Discrimin.tion. validation. an. clin.cal usefuln.ss in.the validation.cohort were assessed by the area un.er the curve, the calibration.plot, an. decision.curve ***: In.the developin. dataset, the 1-, 5-, an. 10-year RFS were 83.0%, 47.2%, an. 44.4%, respectively. On.multivariate an.lysis, in.epen.en. predictors were TTC (hazards ratio [HR] 1.07, 95% con.iden.e in.erval [CI] 1.05-1.08, pn. un.t: HR 1.40, 95% CI 1.03-1.90, p=0.03;two or more un.ts: HR 1.72, 95% CI 1.29-2.29, pn.phrosis (HR 1.54, 95% CI 1.21-1.97, pn.ma (HR 0.60, 95% CI 0.45-0.81, p=0.001), pT3-T4 (HR 1.77, 95% CI 1.41-2.22, pn.de status (HR 1.53, 95% CI 1.21-1.95, pn. lymphovascular in.asion.(HR 1.28, 95% CI 1.01-1.62, p=0.044). The areas un.er the curve in.the validation.dataset were 79.3%, 69.6%, an. 76.2%, for 1-, 5-, an. 10-year RFS, respectively. Medical diagn.stic tests to detect Severe Acute Respiratory Syn.rome Coron.virus-2 (SARS-CoV-2) for in.ividuals in.the Un.ted States were in.tially limited to people who were travelin. or symptomatic to track disease in.iden.e due to the cost of providin. testin. for all people in.a commun.ty on.a routin. basis. As an.altern.tive to ran.omly samplin. large groups of people to track disease in.iden.e at sign.fican. cost, wastewater-based epidemiology (WBE) is a well-established an. cost-effective techn.que to passively measure the prevalen.e of disease in.commun.ties without requirin. in.asive testin.. WBE can.also be used as a forecastin. tool sin.e the virus is shed in.in.ividuals prior to developin. symptoms that might otherwise prompt testin.. This study applied the WBE approach to un.erstan. its effectiven.ss as a possible forecastin. tool by mon.torin. the SARS-CoV-2 levels in.raw wastewater sampled from sewer lift station. at a large public un.versity campus settin. in.ludin. dormitories, academic buildin.s, an. athletic facilities. The WBE an.lysis was con.ucted by samplin. from buildin.-specific lift station. an. en.meratin. target viral copies usin. RT-qPCR an.lysis. The WBE results were compared with the 7-day rollin. averages of con.irmed in.ected in.ividuals for the followin. week after the wastewater sample an.lysis. In.most cases, chan.es in.the WBE outcomes were followed by similar tren.s in.the clin.cal data. The positive predictive value of the applied WBE approach was 86% for the followin. week of the sample collection. In.con.rast, positive correlation. between.the two data with Spearmen.correlation.(rs) ran.ed from 0.16 to 0.36. A stron.er correlation.(rs = 0.18 to 0.51) was observed when.WBE results were compared with COVID-19 cases iden.ified on.the n.xt day of the samplin. even.s.
