Introduction: Cervical cystic lymphangiomas are rare benign dysembryoplastic tumors of lymphatic origin. Its severity in the child is due on one hand to their fast evolution and the compression of the way aerodigestive and on the other hand, by the classical difficulty of their excision. The purpose of this work was to report a case of giant cervical cystic lymphangioma in a child to discuss the difficulties associated with its management in our context. Observation: This was a 3-year-old female child who was admitted to the EnT department of CHU “Luxembourg” for right lateral cervical swelling. The clinical examination had noted a large anterior-latero cervical swelling of soft, renitent, painless palpation, movable in relation to the superficial and deep plane, measuring about 20 cm × 17 cm, the skin was healthy. It wasn’t particularity to the rest of the physical examination. The diagnosis of giant cervical cystic lymphangioma was discussed. Thorough excision of the swelling by right lateral cervicotomy was performed. Anatomopathological examination of the operative specimen confirmed the diagnosis. The postoperative course was simple and the evolution was favorable. Conclusion: Cervical cystic lymphangiomas are rare. Their management involves surgery, with short and long-term post-operative outcomes are often excellent.

An Agent Based Model for Ransomware Detection and Mitigation in a Cloud System

The increasing trend toward dematerialization and digitalization has prompted a surge in the adoption of IT service providers, offering cost-effective alternatives to traditional local services. Consequently, cloud services have become prevalent across various industries. While these services offer undeniable benefits, they face significant threats, particularly concerning the sensitivity of the data they handle. Many existing mathematical models struggle to accurately depict the complex scenarios of cloud systems. In response to this challenge, this paper proposes a behavioral model for ransomware propagation within such environments. In this model, each component of the environment is defined as an agent responsible for monitoring the propagation of malware. Given the distinct characteristics and criticality of these agents, the impact of malware can vary significantly. Scenario attacks are constructed based on real-world vulnerabilities documented in the Common Vulnerabilities and Exposures (CVEs) through the national Vulnerability Database. Defender actions are guided by an Intrusion Detection System (IDS) guideline. This research aims to provide a comprehensive framework for understanding and addressing ransomware threats in cloud systems. By leveraging an agent- based approach and real-world vulnerability data, our model offers valuable insights into detection and mitigation strategies for safeguarding sensitive cloud-based assets.

ng>Introduction:ng> Tongue papillomas are benign epithelial tumors of the tongue lining. It is a precancerous lesion of viral origin caused by the Human papilloma virus (HPV). ng>Objective:ng> To describe the clinical and histological manifestations of a particular type of oral papilloma. ng>Observation:ng> A young woman aged 28 years resident in Siguiri (Guinea), received in consultation for a painless mass on the tongue slow evolving for 6 months duration. There was no notion of mouth bleeding. She had no particular medical and surgical history and negative for retroviral test/HIV test. Examination of the oral cavity found a fungating mass in at Right or Left posterior third of the tongue, painful on contact, sessile and pedunculated and not bleeding on contact and of soft consistency. The excision of the tongue mass was performed under general anesthesia. Biopsy was taken and sent for HPE. The consequences were mild, allowing her to be released 3 days after surgery. Histological examination showed an acanthotic stratified para-keratinized squamous epithelium containing small parts of connective tissue and koilocytes containing connective tissue nuclei reminiscent of papilloma. There was no malignancy cell. ng>Conclusion:ng> The papilloma of the tongue is a benign proliferative lesion characterized by slow and painless growth. I would say early diagnosis and surgical excision should be done to avoid recurrence, contact bleeding and morbidity secondary to mass effect.

近几年来，由于全球互通微波存取（WiMAX）技术的数据速率高、部署成本低以及覆盖范围广，使其被广泛地应用于提供无线最后一英里的连接服务。同时，WiMAX被认为是能在大城市区域实现宽带无线接入（BWA）的最有效的方法之一。但是，随着一些诸如VoIP以及视频流等对时延敏感的应用的出现，就需要网络能够提供连续的通信服务，此时就需要有一种高效的切换机制来满足服务质量（QoS）需求。目前，一些研究小组也正致力于研究如何提高切换机制的效率。在WiMAX中，切换被定义为过程和决策的集合，这个集合使得一个移动站（MS）能从一个基站（BS）的空中接口移动到另一个基站。但是，当MS在BS之间移动时，WiMAX系统的移动性仍会造成几个缺陷，尤其是与信道资源浪费、切换延时以及数据丢失有关的缺陷，这些缺陷可能会影响到应用的实时性。本文评估并实现了一种基于BS信号强度（RSSI）和MS方向信息的切换机制。在该机制中，假设通过使用全球定位系统（GPS）可以获得MS在任意时刻的位置信息。假设基站的位置是固定的，MS到网络中不同BS的距离已知。则MS可以计算出它邻近的BS接收到的信号强度。根据这些收集到的信息以及切换要求，MS就可以选择启动不同的进程来触发切换机制。这种触发切换机制的方法，同时利用到了RSS和移动节点与基站之间的距离信息，且由于RSSI是与距离有关的，这样MS就可以减少一些不必要的切换。仿真中分析了VoIP上影响WiMAX网络性能的主要因素的几个参数，如切换时延、吞吐量以及丢包率等。仿真结果表明，最终得到的参数值对于一些如VoIP、视频流等实时应用是容忍的。

Short term effects of fire intensity and fire regime on vegetation dynamic in a tropical humid savanna(Lamto,central Cote d’Ivoire)

We tested the effects of different fire regimes (with different fire date and fuel load) on grass growth, tree debarking and topkill in a Guinea savanna of West Africa. Different fire intensities were simulated on two plots of 3.72 ha each, delimited in two shrubby savanna of the Lamto reserve (C?te d’Ivoire). Two fire regimes were applied, the mid-season fire (January) and the late fire (April). Two fuel levels (single: C1 and double: C2) have been used. For each fire regime, fire intensity was determined. It increases with the fuel quantity (1259 ± 356 kW·m–1 and 3380 ± 1472 kW·m–1 respectively for C1 and C2). The regrowth speed of grasses is also dependent on the fuel quantity (grasses). It increases with it and is higher after the mid-season fire than the late fire. The average intensity of the mid-season fire (2966 ± 2233 kW·m–1) is not significantly different from that of the late fire (1673 ± 1124 kW·m–1). Damages or debarking caused by fire on adult trees were recorded and are linked to fire intensity. Those damages appear to initiate the external cavity observed on trees, known to be detrimental to tree trunk mechanical resistance. They generally affect adult trees of Crossopteryx febrifuga species which is also the most commonly hollowed species. Mid-season fire remains the recommanded fire regime because it insures a faster regrowth of grasses which leads to the maintenance of equilibrium between grasses and trees.

Background: African animal trypanosomiasis (AAT) is caused mainly by Trypanosoma congolense, T. vivax, and T. brucei brucei and is the major constraint for livestock productivity in Sub-Saharan African countries. Information about animal trypanosomiasis status in Ivory Coast is missing, especially regarding molecular epidemiology. Therefore, this study intended to apply molecular tools to identify and characterize trypanosomes in Ivory Coast for sustainable control. Methods: 363 cattle blood samples were collected from Ferkessedougou Region in northern Ivory Coast in 2012. Buffy coat technique (BCT) and species-specific PCR assays were used to detect trypanosome species. Results: Out of 363 cattle examined with BCT, 33 were found positive with all trypanosomes species accounting for an average of 9.09% prevalence whereas polymerase chain reaction (PCR) using species-specific primers showed that 81 out of 363 cattle were infected with trypanosomes with an overall prevalence of 22.31%. Trypanosoma congolense savanah type, T. Vivax and T. brucei sl. accounted for 28.39%, 49.38% and 23.45% of the infection rate respectively. no infection with T. congo forest?type was detected. T. vivax infection was the most prevalence in the area investigated compared to the two other trypanosome species. Mixed infections with different trypanosomes species were detected accounting for 7.32% of prevalence. Regarding sexrelated prevalence, male cattles were slightly more infected than female but the difference was not significant. Conclusion: Our results showed that there was a high prevalence of AAT in livestock in Ferkessedougou Area. There is therefore a need to strengthen control policies and institute measures that help prevent the spread of the parasites for sustainable control of animal trypanosome in this area.

ng>Introduction:ng> Bladder tumours are frequent, have a poor prognosis and are the second most common genitourinary tumour in men. In Mali, recent statistical data on the prevalence of bladder tumours show a progression with 6.71% in 2006. The euro scanner is used in the diagnostic workup. The work was initiated in order to specify the role of the scanner in the diagnosis of these tumors. ng>Patients and Methods:ng> Prospective descriptive study was carried out in the medical imaging department of Mali Hospital from February to november 2018. Were included any patient with a bladder tumour on euro scanner. The patients had benefited from a preliminary preparation and a renal biological assessment in order to eliminate renal insufficiency. Our examinations were performed with a Siemens 16-slice scanner. These were axial slices performed without, with iodine injection from the pulmonary base to the pubic bones. ng>Results: ng>We had collected 17 patients out of 541 scans i.e. a frequency of 3.76%. The age range of 21 to 40 years was 58.8% with an average age of 25 years. The antecedents (ATCD) found were urinary bilharzia (47%) and smoking (17%). The clinic was dominated by haematuria. It was associated with a bladder mass (52%) and dysuria (23.52%). The CT scan showed a budding appearance, a size greater than 3 cm (82%), infiltration of peri-vesical fat (47.05%), and adenopathy (94%). Associated lesions were: utero-hydronephrosis (58.82%) and metastases (bone and liver in 11.76% of cases each). ng>Conclusion: ng>They are frequent and the history of urinary bilharziasis remains the main risk factor in endemic countries. CT scans are essential in the management of this disease. The diagnosis of certainty remains histology.

ng>Summary:ng> A malignant tumour that develops from the bronchial epithelium and then invades the lungs. It ranks third after colorectal and breast cancer. In Mali, computed tomography (CT) is the main imaging tool used at all stages of treatment. We had initiated this study with the objective of specifying the place of CT in the diagnosis of primary bronchopulmonary cancer at the hospital of Mali. ng>Method: ng>This is a prospective and descriptive study conducted by the Medical Imaging and Thoracic Surgery departments at Mali Hospital from March 2014 to February 2015. All the patients who have benefited from a CT scan and presenting at least one histologically confirmed bronchopulmonary tumor were included. ng>Results: ng>We were recruiting 60 patients with bronchopulmonary cancers out of 500 referred for a thoracic CT scan performed, a frequency of 12%. There was one male patient with a sex ratio of 4H/1F. The average age was 60 years. The history of smoking was 85%. The symptoms were dominated by general condition (63.33%), cough (25%) and haemoptysis (58.33%). CT scans were performed in all patients. The lesions were located more in the lower lobes (71%). The tumours were: heterogeneous tissue density (70%), spiculated contours (80%), and the histology was more non-small cell carcinoma. ng>Conclusion: ng>The incidence of bronchopulmonary cancer is very high and its prognosis is poor at an advanced stage. The evaluative role of CT and biopsy guidance has given it a place of choice in its management. The diagnosis of certainty remains histology.

Ultrasound Biometry of the Liver at the Point “G” University Hospital Center in Bamako

Introduction: Biometrics therefore corresponds to the measurement of the morphological elements of humans. One of the most common ultrasound requests by clinicians is the assessment of liver size. The aim of our study was to study liver biometry using ultrasound in healthy adult subjects. Material and Methods: This was a prospective, descriptive and analytical study, carried out at CHU Point-G over a period of 7 months. A liver ultrasound was performed on 100 individuals without lesions, by a doctor in his final year of specialization in radiology and medical imaging. For each subject, we determined the height and anteroposterior diameter of the right liver and the left liver. Results: The mean age was 39.05 ± 16.86 years. The body mass index (BMI) 18.5 - 24.9 group was the most represented with 58%. The mean height of the right liver was 138.40 ± 14.85 mm. It was 136.81 ± 14.70 mm in men and 139.92 ± 14.99 mm in women (P = 0.306). That of the left liver was 95.55 ± 14.34 mm, in men, it was 91.79 ± 13.51 mm and 99.16 ± 14.31 mm in women (P = 0.019). We found a significant correlation between right liver height and BMI (P = 0.013). Conclusion: The mean values of liver biometry were established in our series. There was a significant correlation between right liver height and BMI. Liver ultrasound remains a reliable technique for liver biometry.

The physicochemical parameters of the soils made it possible to assess their fertility and pollution around the gold district of *** 3-fraction granulometry indicates a dominance of sandy soils with a silty-sandy *** OM(Organic Matter)content is less than 1%,like most soils in Burkina *** characterization of the fertility of the 26 composite soil samples indicates that 96.15%are of average *** pollution assessment indicates that the measured pH values are between 5.5 and 8,therefore in accordance with the local reference standards in force,while the determination of the ratio of Al on the exchangeable bases and the low phosphorus contents express a very high aluminium *** electrical conductivities measured vary from 17.5 to 252μS/cm,with a threshold limit of 2μS/cm,expressing significant soil *** the Pearson correlation matrix and the PC(Principal Component)analysis,the causes of pollution are attributable to the high contents of Al and exchangeable bases,linked to agricultural and mining *** analysis of the geological and pedological context indicates that the origin of Al and the exchangeable bases is natural,therefore pedo-geogenic,attributable to the destabilization of feldspars,ferromagnesian minerals and carbonates under supergene alteration.
