Prevalence and Risk Factors for Albuminuria among HIV Ivorian Children, Using the Dipstick Method

Context: Human immunodeficiency virus (HIV) infected children are at risk of developing chronic kidney disease defined by decreased glomerular filtration rate and or proteinuria. Studies on the prevalence of and risk factors for proteinuria among HIV-infected children in sub-Saharan Africa are scarce and in Cote d’Ivoire, there is paucity of reported data. We determined the prevalence of and the risk factors for proteinuria among HIV-infected Ivorian children in both ART (antiretroviral therapy)-na?ve and ART-exposed children. Method: It was a cross-sectional bi-centric study, conducted from July 2013 to April 2014. Each child’s urine was tested for proteinuria and microalbuminuria using multi test strips and mitral test, respectively. Proteinuria was defined as of significant of 1+ and above on the dipstick or the presence of microalbuminuria ≥ 20 mg/l. Results: Of the 155 HIV-infected children enrolled, there were 78 boys. Sixty seven point seven percent were treated with antiretroviral and 74.1% had treatment duration of less than 5 years. In the first dipstick test, the prevalence of proteinuria was 29% which persisted in 9.7% after 24 hour urinary protein examination. It was significantly different between the 2 groups treated and na?ve (2.8% versus 24%). The advanced CDC clinical stage emerged as the main independent determinant of proteinuria. Conclusion: Proteinuria is common in HIV-infected Ivorian children. Screening for proteinuria in HIV-infected children will help in early and treatment, and thus prevention and progression to chronic kidney disease to end-stage kidney disease.

The objective of this multicentric study was to assess the prevalence of Chlamydia trachomatis, neisseria gonorrhoeae, Mycoplasma génitalium and Trichomonas vaginalis infections in Brazzaville, in the Republic of Congo, using molecular methods. From January to December 2021, the sexually transmitted disease risk participants were recruited from six centers: The Association of Young HIV-Positive People of Congo, The Congolese Association for Family Welfare, The Association for Support to Vulnerable Groups, Talangaï hospital, Brazzaville university hospital (outpatient service) and the private clinic COGEMO (outpatient service). The real-time multiplex PCR was carried out to detect these pathogens. Each patient had at least one specimen (urine, urethral, anal and/or vaginal samples). The patients were considered infected when one of their samples was positive. 287 participants made of 227 women and 60 men were tested. The general prevalence of these infections was: Chlamydia trachomatis 2.79%, neisseria gonorrhoeae 3.14%, Mycoplasma génitalium 3.45% and Trichomonas vaginalis 2.97. The prevalence rates according to sex were: C. trachomatis, M. génitalium, n. gonorrhea and T. vaginalis were 1.32%, 2.05%, 1.32% and 3.42% in women and 8.33%, 7.02%, 10% and 1.75% in men, respectively. Most infected patients were asymptomatic. Prevalence rates were higher in bisexual individuals, with the exception of T. vaginalis which showed higher prevalence in heterosexual patients. The bisexual and homosexual individuals represent a major public health problem in sexually active young adults, particularly among men having sex with men. These sexually transmitted infections are mainly asymptomatic, their diagnosis and management remain difficult in developing countries.

Comparison of conventional and supported liquid extraction methods for the determination of sitagliptin and simvastatin in rat plasma by LC-ESI-MS/MS

Three extraction methods were compared for their efficiency to analyze sitagliptin and simvastatin in rat plasma by LC-MS/MS, including （1） liquid-liquid extraction （LLE）, （2） solid phase extraction （SPE） and （3） supported liquid extraction （SLE）. Comparison of recoveries of analytes with different extraction methods revealed that SLE was the best extraction method. The detection was facilitated with ion trap-mass spectrometer by multiple reactions monitoring （MRM） in a positive ion mode with ESI. The transitions monitored were m./z 441.1→325.2 for simvastatin, 408.2→235.1 for sitagliptin and 278.1→260.1 for the IS. The lower limit of quantification （LLOQ） was 0.2 ng/mL for sitagliptin and 0.1 ng/mL for simvastatin. The effective SLE offers enhanced chromatographic selectivity, thus facilitating the potential utility of the method for routine analysis of biological samples along with pharmacokinetic studies.

Allometric models are necessary for estimating biomass in terrestrial ecosystems. Generalized allometric relationship exists for many tropical trees, but species- and region-specific models are often lacking. We developed species-specific allometric models to predict aboveground biomass for 11 native tree species of the Sudanian savanna- woodlands. Diameters at the base and at breast height, with species means ranging respectively from 11 to 28 cm and 9 to 19 cm, and the height of the trees were used as predictor variables. Sampled trees spanned a wide range of sizes including the largest sizes these species can reach. As a response variable, the biomass of the trees was obtained through destructive sampling of 4 754 trees during wood harvesting. We used a stepwise multiple regression analysis with backward elimination procedure to develop models separately predicting, total biomass of the trees, stem biomass, and biomass of branches and twigs. All species- specific regression models relating biomass with measured tree dimen- sions were highly significant （p 〈 0.001）. The biomass of branches and twigs was less predictable compared to stem biomass and total biomass, although their models required fewer predictors and predictor interac- tions. The best-fit equations for total above-ground biomass and stem biomass bad R2 〉 0.70, except for the Acacia species; for branches including twig biomass, R2-values varied from 0.749 for Anogeissus leiocarpa to 0.183 for Acacia macrostachya. The use of these equations in estimating available biomass will avoid destructive sampling, and aid in planning for sustainable use of these species.

Extended-spectrum β-lactamase (ESBL) appeared some years after the introduction in hospital environment of unhydrolysable or extended-spectrum cephalosporins. Several studies have been reported on the blaTEM, blaCTX-M and blaSHV genes in ESBL producing Enterobacteria, however, very few studies reported in the literature were related to blaCTX-M subgroup blaTOHO. TOHO enzymes were responsible for healthcare-associated infections in hospitals and in the community. In Burkina Faso, data related to these types of enzymes were scarce. The purpose of this study was to detect TOHO enzymes in Escherichia coli and Klebsiella pneumoniae in order to know the prevalence of infections related to bacterial resistance due to TOHO enzymes at Saint Camille Hospital of Ouagadougou (Burkina Faso). The study was conducted firstly by microbiological identification of ESBLs-producing by Escherichia coli and Klebsiella pneumoniae using API 20 E gallery;secondly the antibiogram was performed by the diffusion method and finally the molecular characterization was made by conventional PCR to search for the blaTOHO gene. The visualization of the specific bands was made using the ultraviolet lamp (Gene Flash) for the photography of the gels. Data were entered and analyzed using Excel 2013 and EPI Info version 6.0 software. A p-value nsidered as significant. We obtained at all 39 strains constituted by 21 (53.8%) Escherichia coli and 18 (46.2%) Klebsiella pneumoniae. Molecular characterization showed the presence of the blaTOHO gene in 25 bacterial strains (64.1%). It was therefore established in this study the existence of blaTOHO gene at Saint Camille Hospital in Ouagadougou in Burkina Faso. Our study made it possible to know the distribution of the blaTOHO gene in Escherichia coli and Klebsiella pneumoniae.

neuropeptide Y Increases Both Ingestion of a Self-Selection Macronutrient Diet and Fos Expression in the Medial Amygdala of Rats

The rat posterodorsal medial amygdala (MePD) is responsive to the orexigenic neuropeptide Y (nPY) and is a putative candidate to participate in neural circuits that modulate feeding behavior. Here, we studied the effects of intracerebroventricular (icv) microinjection of nPY on the appetitive and food intake behaviors of rats under the paradigm of the self-selection macronutrient isolated diets [high-carbohydrate (high-CHO), high-protein and high-lipid food pellets]. At the same time, Fos expression was also evaluated in the MePD as a marker of local cellular activation. Adult male rats received icv microinjections of nPY (1mg and10mg/5mL, n = 10 and 8, respectively) whereas the control groups either received icv microinjection of artificial cerebrospinal fluid (5mL, n = 8) or underwent sham procedure (n = 8). The data were obtained after a fasting protocol. Feeding behavior was evaluated during a 2 h test period of free access to the selective diets. Rats in all groups preferred the high-CHO diet. Compared to controls, both doses of nPY increased the appetitive behaviors (searching for food and the frequency of attempts to eat any diet) and the percentage of animals eating high-CHO diet. However, only nPY at a dose of 1 μg led to a significant increase in food intake and showed a strong positive correlation with Fos expression in the MePD (p 0.05 in all cases). These new data reveal a biphasic effect of nPY on the appetite and food intake behaviors and suggest that the MePD participates in the nPY-induced feeding behavior in rats.

PURPOSE. To investigate the test- retest variability of multifocal visual evoked potential (mfVEP) and threshold perimetry in glaucoma, and to examine the relationship between the two techniques. METHODS. Data were recorded using the AccuMap mfVEP and sita standard program of the Humphrey Field Analyzer. Data were obtained twice within a 4- week period from both eyes of 74 patients with varying amounts of glaucomatous visual field loss. The number of defective test locations (those falling beyond a given probability value of being normal) were calculated for mfVEP and sita, using databases incorporated within the instruments software. Reliability measures and test times were recorded along with patient test preference. RESULTS. Both tests showed a large degree of test- retest variability in the number of defective test locations (95% limits of agreement for mfVEP and sita being 13.39 and 9.88, respectively). A “ fair to moderate” degree of spatial agreement was found between mfVEP and sita. The number of mfVEP defective locations was dependent on the signal amplitude. no relationship was found between test- retest variability and the reliability indices for either test. The mean time taken to perform mfVEP and sita standard was 33 and 20 minutes, respectively, and 73 of the 74 patients preferred the mfVEP test. COnCLUSIOnS. Test- retest variability was found to be slightly greater for mfVEP. The processing of mfVEP signals needs to be changed to remove the relationship between the number of defective locations and signal amplitude. The majority of patients preferred mfVEP to conventional perimetiy although mfVEP takes longer to perform.
