Interest of SAAG in Etiological Diagnosis of Ascites in African Black

Purpose: To show the interest of the serum-ascites concentration gradient in albumin (SAAG) in the etiological diagnosis of ascites in African black. Materiel and Methods: This was a monocentric observational and analytical study performed on patients over 15 years old and hospitalized in the Hepato-Gastroenterology department of Cocody from January 1st, 2010 to March 31st, 2017 for ascites. The cytology and chemistry of the ascites fluid and the protidogram were performed. The main outcome measure was the prediction of portal hypertension (PHT) or non-portal hypertension related causes of ascites from SAAG. Results: The hospital prevalence of ascites was 11%. There were 82 women (46.9%) and 93 men (53.1%) with a sex ratio (M/F) of 1.1. SAAG was low in 68.6% of cases and high in 31.4% of cases. The most common etiologies were post-viral cirrhosis (74%), HCC (16.60%) and peritoneal tuberculosis (13.7%) respectively. In the case of high SAAG, 91% of ascites related to PHT were observed, but also 77.5% of a low SAAG corresponded to ascites related to PHT (p = 0.024) with Se = 35% Sp = 84% and accuracy = 44%. All patients with peritoneal tuberculosis had a SAAG low of less than 1.1 g/dl significantly (p = 0.002). SAAG had a diagnostic performance of 89% in peritoneal tuberculosis at the cutoff value of ?1.24 g/dl (AUROC = 0.91, Se = 96%, Sp = 86%) while in post viral hepatitis cirrhosis at a threshold of 0.58 g/dl the diagnostic performance was 60% (AUROC = 0.69, Se = 53%, Sp = 79%). Conclusion: The simple technical SAAG, allowing to distinguish the ascites linked to the PHT and the ascites not bound to the PHT, can be used like a method of diagnosis of the etiologies of the ascites especially in the peritoneal tuberculosis in the African countries low income.

石油族组分的分离和制备过程是后续分子组成、同位素分析的基础。由于同分异构体多,传统技术制备的芳烃化合物组分仍然无法满足单体化合物的同位素分析要求。采用两种不同的分离方案对原油中的多环芳烃进行精细的分离和制备,其中"一步法"是直接将脱沥青质的原油样品经过一次氧化铝/硅胶层析柱,用不同体积比的石油醚/二氯甲烷混合溶剂进行洗脱;"两步法"先将原油采用常规的族组分分离方法得到芳烃馏分,再将芳烃馏分经氧化铝层析柱,用不同比例的石油醚/二氯甲烷混合溶剂进行洗脱,可得到不同环数芳烃的组分。对洗脱出来的不同组分进行色谱—质谱分析(GC-MS),与"一步法"相比,"两步法"可以更好地实现单环、双环与三环芳烃化合物的分离制备,其中,石油醚/二氯甲烷(99/1,V/V)的混合溶剂可以制备单环芳烃化合物;石油醚/二氯甲烷(9/1,V/V)混合溶剂可制备得到以双环芳烃化合物占绝对优势的组分;石油醚/二氯甲烷(8/2,V/V)混合溶剂则可制备得到三环芳烃化合物组分。

Molecular Detection of Resistance to Rifampin and Isoniazid among Patients Eligible for Retreatment Regimen in Côte d’Ivoire in 2012

Drug-resistant tuberculosis is an important health problem in Cote d’Ivoire. Patients of category I treatment are based on 2RHZE/4RH regimen. For the patients of category II, the 2RHZES/1RHZE/5HRE regimen is systematically initiated. Study objective was to describe the susceptibility profile to Rifampin and Isoniazid among previously treated patients who are eligible for retreatment and who had received the 2RHZES/1RHZE/5HRE regimen in Cote d’Ivoire with a molecular method in 2012. Two sputum samples were collected to each patient recruited in the reference regional centres for tuberculosis. Sputum samples were decontaminated by nALC method. The DnA extraction was realized with 500 μl of decontaminated sputum sample with smear-positive. MTBDRplus assay version 1.0 was performed according to the manufacturer’s instruction. An internal quality control program with positive and negative controls was implemented for interpretation of results. A total of 278 patients were enrolled, 148 (53.2%) were recurrent TB cases, 118 (42.5%) failure cases, and 12 (4.3%) defaulters. From sputum of previously treated patients, mutli-drug resistant tuberculosis was diagnosed for 60 (69.8%, 95% IC, 60% - 80%) treated with the 2RHZE/4RH regimen, 24 (75%, 95% IC, 60% - 90%) with the 2RHZES/1RHZE/5HRE regimen, 60 (41%, 95% IC, 33% - 49%) recurrent TB cases, and 4 (33.3%) defaulters. The comparison of resistance rates to Rifampin estimated from sputum samples in the categories of treatment failures and the recurrence TB cases showed a statistically significant difference. In Cote d’Ivoire, genotype? MTBDRplus assay has permitted to estimate the prevalence of MDR-TB in categories of previously treated patients for tuberculosis.

Terminalia mantaly H. Perrier is one of the plants of the Combretaceae family that is widely used in traditional African medicine for its antibacterial, antifungal, and especially in the treatment of malaria. In this study, we evaluated the effect of the hydroalcoholic extract of Terminalia mantaly (HAETM) on the tissue and kidney biochemical markers of rats. Forty (40) rats were randomly divided into 4 groups, assigning 10 animals within each group (5 males and 5 females per group). Animals in group 1 received distilled water and were used as the control group;on the other hand, groups 2, 3, 4 received by gavage a volume of the extract corresponding to 1 ml/100g body weight at 150 mg/kg, 300 mg/kg, 600 mg/kg, respectively. The hydroalcoholic extract was administered at the same time daily for 28 days, and blood was collected once a week to evaluate renal biochemical parameters;the kidney tissues were used to perform the histopathological study. The creatinine rate increased significantly (p Terminalia mantaly should be globally well tolerated by the body when used in doses ranging from 150 to 600 mg/kg of the body weight in animals (for this study, rats). However, further in-depth studies would be needed to get a more thorough and complete picture of the safety profile of the extract.

Petrogenetic Characterization of the Geological Formations of the Localities of Goumere-Iguela in the South West of the Bui Belt (north-East of Cote d’Ivoire)

Located in the north-east of Côte d’Ivoire, the Gouméré-Iguéla sectors were the subject of a geological mapping. These geological formations are located southwest of the Bui trench. In order to improve the petrogenetic knowledge of the study area, a multidisciplinary methodology integrating microscopic observations and geochemical analyses of major and trace elements was carried out on 15 samples considered as representative of the studied outcrops. The macroscopic and microscopic petrographic study allowed to highlight three major lithological units: 1) a volcano-plutonic unit, formed by gabbros and amphibole-pyroxenite, basalt and rhyodacite;2) a volcano-sedimentary unit containing pyroclastites (of basaltic and andesitic composition);3) a sedimentary unit (meta-sedimentary). These rocks are affected by a general metamorphism of green schist facies and a hydrothermal alteration with pervasive and vein alteration marked by the presence of quartz, calcite, epidote, chlorite and sericite. From a geochemical point of view, the results obtained indicate that the plutonites are gabbros while the volcanites and volcanosedimentary rocks have compositions of basalts, basaltic andesites, rhyolites and dacites. They have mainly a tholeitic character related to subduction zones with crustal contamination. The metasediments are arenites emplaced in an active continental margin environment.

A descriptive study allowed to estimate the risks of formation of PAHs （polycyclic aromatic hydrocarbons） in the traditionally smoked fish consumed in the area of Abidjan, Cbte d＇Ivoire. The study was carried out from surveys on processing methods of fresh fish into smoked fish in the production areas. It took place from September 2009 to March 2012 on three smoking sites Port-Bou^t in the South of Abidjan, lie Boulay, Abobo-Doum6 and Koweit in the South-West of Abidjan and Abobo-Macaci in the north of Abidjan. The main risk factors which were identified in the formation of PAH, are the stakeholders, the smoking sites, the technologies, the handling and the storing. The black color and bitter taste of the fish were identified as sensory indicators of PAH formation in smoked fish.

Petro-Structural Study of Paragneiss in the SASCA Domain(GbowéSector at Grand-Bereby),Southwestern Cote d’Ivoire

The petrographic and structural study of Gbowé (Grand-Béréby) formations located in the SASCA domain (South-West of Côte d’Ivoire) revealed migmatitic paragneisses. For an in-depth understanding of the petrographic, structural and metamorphic characteristics, six (6) thin sections were made from these paragneisses. These gneisses are characterized by paleosomes and neosomes (leucosome and melanosome), consisting of quartz, garnet, plagioclase, biotite, cordierite, sillimanite, myrmekite and microcline. The mineralogical assemblage thus described indicates a retrograde metamorphism (transition from granulitic facies to amphibolitic facies). The structural and microstructural study identified two types of deformation (ductile and brittle). The ductile deformation is characterized by phases D1 (nE-SW flattening) and D2 (nW-SE flattening), materialized by foliations (n140˚, n050˚), folds (asymmetrical folds, similar folds, concentric folds, ptymatic folds) and boudins. Fracture schistosity and fold fracture schistosity are characteristic of the brittle deformation (phase D3). The microstructural study coupled with the metamorphic study shows that the deformations had an impact on the texture of the minerals (recrystallization and mineral reactions). It also allowed giving the paragenesis of phases D1 and D2. The D1 phase is characterized by garnet1, biotite1, quartz1, sillimanite1 and cordierite1 and the D2 phase is characterized by garnet1, quartz2, sillimanite2, biotite2, garnet2 and orthose2. These parageneses thus highlighted bear witness to a polydeformation and polymorphism that affects the study area.

Theoretical Investigation of Ru(II) Complexes as Photosensitizer for Photodynamic Therapy

This work was undertaken to see how Ru II complexes can be suitable for photodynamic therapy through theoretical prediction. For that, four Ru II complexes, α-RuCl2(Azpy)2, , and were used in unrestricted state by providing with no more energy than 2.68 eV. The unrestricted state allows the complex molecule to display each of its electrons in one orbital. All the calculations such as optimization, frequency and TD-DFT calculations were performed at WB97XD/Lanl2dz level. It resulted from this investigation that Ru II complexes are active for both mechanisms suitable for photodynamic therapy in presence or absence of 3O2. Moreover, this reaction was assumed to take place only with Guanine DnA base as demonstrated in literature. Therefore, Guanine is admitted as the base most reacting with ruthenium complexes for photodynamic therapy. This work confirms our prediction regarding metallic complexes that are assumed to be photosensitized in condition that an electron must be isolated to favor the excitation. nevertheless, Ru II complexes are found suitable for superficial therapy while Ru III must be active for deep therapy.

Assessment of Anxiety among Caregivers at the Covid-19 Treatment Centers in Grand-Bassam and Cocody-Abidjan from July to September 2020

The rapid spread of coronavirus disease (COVID-19) has led to general psychosis among the population in Ivory Coast. The medical profession, on the front line of the treatment, was in distress, given the risk of exposure to the patient. It is in this context that this study has set itself the objective of studying anxiety among caregivers involved in the management of people with COVID-19. This is a cross-sectional, descriptive and analytical study that included a sample of 50 stakeholders. It took place from July to September 2020 at the Treatment Center of the Village of Information Technologies and Biotechnology in Grand-Bassam (VITIB) and the Infectious Diseases Treatment Center (CTMI) of the University Hospital (CHU) of Cocody. The results indicate that caregivers were male in 52% of cases with an average age of 36 years. These are: doctors, nurses and caregivers respectively in the proportions of 18%, 42% and 40% with more than half (58%) who had more than 5 years of seniority. A proportion of 34% of caregivers felt they were insufficiently protected. The Hamilton Scale assessment of anxiety showed that 14% had mild anxiety, 6% moderate anxiety and 2% severe anxiety. There was no link between the degree of anxiety and professional seniority but also between the degree of anxiety and the marital situation. This work has therefore made it possible to see that anxiety is a real problem for caregivers dedicated to the management of COVID-19 patients.

Analysis and Forecasting of the Impact of Climatic Parameters on the Yield of Rain-Fed Rice Cultivation in the Office Riz Mopti in Mali

During the period spanning the 1970s and1980s, countries in the West African Sahel experienced severe drought. Its impact on agriculture and ecosystems has highlighted the importance of monitoring the Sahelian rainy season. In Sahelian countries such as Mali, rainfall is the major determinant of crop production. Unfortunately, rainfall is highly variable in time and space. Therefore, this study is conducted to analyze and forecast the impact of climatic parameters on the rain-fed rice yield cultivation in the Office Riz Mopti region. The data were collected from satellite imagery, archived meteorology data, yield and rice characteristics. The study employed Hanning filter to highlight interannual fluctuation, a test of Pettitt and the standardized precipitation index (SPI) to analyze the rainfall variability. Climate change scenarios under the RCP 8.5 scenario (HadGEM-2 ES) and agroclimatic (Cropwat) model are carried out to simulate the future climate and its impact on rice yields. The results of satellite image classifications of 1986 and 2016 show an increase of rice fields with a noticeable decrease of bare soil. The analysis of the SPI reveals that over the 30 years considered, 56.67% of the rainy seasons were dry (1986-2006) and 43.33% were wet (2007-2015). The modelling approach is applied over 1986-2006 and 2007-2015 periods—considered as typical dry and rainy years—and applied over the future, with forecasts of climate change scenarios in 2034. The results show a decrease in potential yield during dry and slightly wet years. The yields of rain-fed rice will be generally low between 2016 and 2027. Deficits are observed over the entire study area, in comparison with the potential yield. Thus, this situation could expose the population to food insecurity.
