Description of Post-Stroke Cognitive Disorders at Abidjan University Hospitals, Côte d’Ivoire

Stroke patients often experience motor deficits and cognitive problems after a stroke. Objective: To improve our understanding of the cognitive consequences of stroke. Method: a descriptive and analytical cross-sectional study was conducted over 12-month period in the neurology departments of the Cocody and Treichville Hospitals in Côte d’Ivoire. Results: Out of 724 patients admitted to the neurology department, 415 (57.32%) were stroke patients, of which 145 (34.94%) were screened. The frequency of global cognitive functioning impairment was 86.21%, significantly higher than the frequency of patients without impairment, which was 13.79%. The study focused on detailing the cognitive status of stroke patients in neurology departments, assessing several cognitive functions during the subacute phase of stroke. These functions included global cognitive functioning, executive functions, language and memory. The frequency of post-stroke cognitive impairment is high among stroke patients in Abidjan. This frequency is comparable to figures found in Subsaharian stroke populations. Demographic and clinical characteristics studied included age, gender, education level, employment status, vascular diseases and cerebral affected area. Among these characteristics, only the education level and the cerebral affected area have been found significant. Conclusion: The incidence of cognitive impairment after a stroke is significantly high among stroke patients in Abidjan.

The aim of this study is to produce large quantities of yam tubers in the field from seed tubers derived from vitroplants regenerated from aerial stem cuttings and farmers’ tuber seeds. Seed tubers from ten yam cultivars, including five from Dioscorea alata (cDa083, cDa053, cDa115, cDa150 and cDa266) and five from Dioscorea cayenensis-rotundata (cDr015, cDr027, cDr150, cDr206 and cDr148) with distinct agro-morphological characteristics, were used as material. A completely randomized block design with 3 replications was used. In each block, all cultivars and seed types were represented. When put into cultivation, the tubers produced by the vitroplants all germinated and gave 100% healthy plants, compared with 86% to 100% healthy plants for the seed tubers produced by the farmers. Yields per hectare ranged from 10 to 25 tonnes for seed tubers produced by in vitro plants, and from 4 to 9 tons for seed tubers produced by farmers. Seed tubers from vitroplants can therefore be recommended to farmers as a solution to the problem of seed tuber unavailability.

With the full growth of energy needs in the world, several studies are now focused on finding renewable sources. The aim of this work is to optimise biofuel formulation from a mixture design by studying physical properties, such as specific gravity and kinematic viscosity of various formulated mixtures. Optimization from the mixture plan revealed that in the chosen experimental domain, the optimal conditions are: 40% for used frying oil (UFO), 50% for bioethanol and 10% for diesel. These experimental conditions lead to a biofuel with a density of 0.84 and a kinematic viscosity of 2.97 cSt. These parameters are compliant with the diesel quality certificate in tropical areas. These density and viscosity values were determined according to respective desirability values of 0.68 and 0.75.

Background: Surgical site infections are serious healthcare problems. The aim was to describe the epidemiological, diagnostic, therapeutic, and prognostic aspects of surgical site infections in Trauma and Orthopaedics at Bouaké Teaching Hospital. Method: This retrospective, descriptive, and analytical study was conducted from January 2019 to December 31, 2021. The data studied included prevalence, initial lesions, type of surgical intervention, type of SSI, bacteria involved, treatment, and outcomes. Results: Forty-four (11%) of the 399 patients included in the study developed a surgical site infection. The mean age was 27 years, with 36 male and 8 female. Initial lesions were predominantly open fractures (n = 31;70%), with a mean delay of 48 hours for surgical management. Emergency interventions accounted for 70% (n = 31) of cases. The nnISS infection risk score was 1 in 80% (n = 35) of cases. Superficial infections (n = 34;77%) appeared early, on a mean 6 days postoperatively. Bacteriological analysis primarily identified Pseudomonas aeruginosa (n = 10;23%), sensitive to Imipenem and Chloramphenicol but resistant to Amoxicillin-clavulanic acid, Ceftriaxone, Gentamicin, and Ciprofloxacin. Multidrug-resistant bacteria were found in 89% (n = 8) of cases, with all bacteria resistant to Ceftriaxone. Surgical revision was performed in 10 patients (23%), primarily involving debridement with hardware retention (n = 7;70%). Chloramphenicol was the most commonly used antibiotic post-antibiogram (61%). Outcomes were favourable in 98% of cases. Identified risk factors included the type of lesion according to nRC classification, the delay in managing open fractures, and the nnISS score. Conclusion: The prevalence of surgical site infection was 11%, favoured by the delayed operation of open fractures.

The emergence of antiretroviral resistance mutations represents a major threat to the achievement of national and global goals for the elimination of HIV-1 infection. The global strategy in 2019 in Cte d'Ivoire is a new national policy for the management of people living with HIV with the administration of dolutegravir (DTG)-based fixed-dose combination. The aim of our study was to evaluate HIV-1 resistance to antiretrovirals (ARVs) in infected adult subjects in Cte d’Ivoire in the context of a systematic switch to a DTG-based combination. Between February 2022 and October 2023, a cross-sectional survey with random sampling was conducted in 06 services caring for people living with HIV. A total of 139 participants were included in the study. Adults with a viral load ≥ 1000 copies/mL were tested for HIV-1 ARV resistance mutations. Molecular analyses were performed using protocol of AnRS-MIE (national Agency for Research on AIDS and emerging infectious diseases). The interpretation is performed by HIVGRAD (https://***/cms/grade/). The frequencies of HIV-1 resistance to non-nucleotide reverse transcriptase inhibitors (nnRTIs), nucleotide reverse transcriptase inhibitors (nRTIs), integrase inhibitors (IInTs) and protease inhibitors (PIs) were 82%, 73%, 19% and 11% respectively. The main mutations observed in the different classes were K103n (45%), M184V (64%), E157Q (19%) and L10V/M46I/A71V/I54V (6%) respectively. This study reveals the emergence of resistance to DTG-based fixed-dose combinations, favored by high rates of resistance to nRTIs and nnRTIs. This finding underlines the need for enhanced viral load monitoring and HIV-1 genotyping tests to guide the choice of nRTIs for combination therapy. In addition, monitoring for mutations to second-generation nRTIs is essential, given the scale-up of DTG-based regimens currently underway in Cte d’Ivoire.

Introduction: A dental abscess is a collection of pus located in the tissues surrounding the tooth roots. The aim of this work is to describe the epidemiological, clinical and therapeutic aspects of dental abscess in order to contribute to the management of patients suffering from this pathology at the dental office of the Siguiri Prefectural Hospital. Methods: This was a retrospective descriptive study lasting 2 months carried out on the registers of patients received between October 2017 and October 2020 at the dental office of the Siguiri prefectural hospital. Results: 4690 patient files were identified during the study, of which 546 met our selection criteria, i.e. a frequency of 11.64%. During our study, we found 51.83% female compared to 48.17% male, i.e. a sex ratio of 1.07. 54.21% of our patients resided in rural communes compared to 45.76% for the urban commune. The age group most affected was that of 16 - 25 years old or 33.69%. Swelling, pain and functional discomfort were the reasons for consultation in all patients followed by other associated reasons in 41.21%. The premolar-molar group was the most represented, i.e. 86.70%. 80.04% of our patients presented an acute abscess compared to 19.96% of chronic cases. Antibiotic, analgesic, and mouthwash were prescribed in all patients during the study followed by 96.37% of cases prescription of steroidal anti-inflammatory drugs. 93.04% of causal teeth were extracted followed by 6.78% of cases of conservative treatment in our study. In our study, 80.95% of our patients had a good post-therapeutic outcome. Conclusion: This study highlights the need for prevention and early management of dental abscesses.

Comparative Study of the Physicochemical Quality of the Waters of the Méné River (Sassandra Watershed, Ivory Coast) in the Dry Season and in the Rainy Season

The study carried out on the waters of the Méné River led to an overall assessment of its water quality during the dry season and the rainy season. The analysis focused on eight (8) water samples taken from the river during a period of dry season (January-February) and a period of rainy season (June and September). The various physicochemical parameters were measured according to Afnor standardized methods. The readings of temperature, turbidity, pH and conductivity made it possible to account for the disturbances occurring in water quality. A temporal variation correlated with the seasons (dry or rainy) is noted. Turbidity depends on the concentration of suspended solids (SS) in the water and drained particles and therefore on the seasons. Just like the temperature, the conductivity changes with the season. The waters of the Méné River are generally acidic. The results obtained show that there is a low level of pollution by chlorides, phosphates, nitrites and nitrates. A slight pollution of the waters of Méné in organic matter (chemical oxygen demand values are less than 25 mg∙L−1 during dry season and 32.33 ± 4.73 mg∙L−1 during rainy season) was observed. The concentrations of metallic trace elements such as iron, manganese and aluminum indicate significant pollution of these waters by these elements. Overall, the waters of the Méné River are of satisfactory quality because all the physicochemical parameters analyzed have values below standards during the dry season as well as during the rainy season with the exception of COD and a few metallic trace elements.

Context: The gut microbiota represents a complex ecosystem encompassing all unicellular microorganisms residing in the digestive tract, primarily bacteria, fungi, archaea, and even viruses. The relationship between the host and the microbiota is symbiotic: bacteria benefit from a stable environment, while the host gains numerous capabilities in terms of digestion, metabolism, nutrition, and immunity. However, numerous studies suggest that the gut microbiota plays a crucial role in various non-communicable diseases, including obesity, chronic inflammatory bowel diseases, allergic and immune disorders, behavioral disorders, and even certain cancers. The objective of our study was to characterize the gut microbiota of a group of breast cancer patients by comparing it to that of control subjects in Côte d’Ivoire, using a metagenomic approach. Method: A case-control study was conducted from May 2020 to September 2023. A total of 85 women (39 cases and 46 controls) were recruited, and stool samples were collected from both breast cancer patients and healthy women. Among these, ten (10) samples from patients and ten (10) samples from healthy women were randomly selected for the study of the gut microbiota. The gut microbiota was characterized by sequencing the V4 region of 16S rRnA using metagenomic nGS technology, and bioinformatic analysis was performed using the mothur pipeline. Results: In women with breast cancer, we observed a reduction in the relative abundance of the phyla Firmicutes and Bacteroidetes, as well as an increase in the phyla Actinobacteria and Verrucomicrobia. Additionally, their microbiota exhibited lower Chao1 and Sobs diversities compared to the control women (p nce analysis (AMOVA) revealed a significant difference between the case and control groups (p nificant difference in the relative abundance of major phyla within the gut microbiota of cases compared to healthy controls. Antibiotic resistance has become a major threat to human health worldwide. Environment, particularly the water environment, has long been overlooked as a player in the antibiotic resistance cycle, although its role remains unclear. These can provide an ideal setting for the acquisition and dissemination of antibiotic resistance, as they are frequently affected by anthropogenic activities. The objective of this study was to establish a diffusion map of resistance integrons used as genetic markers of resistance associated with antibiotic resistance conferring genes (ARGs). Total DnA extracts from non-cultivable bacterial communities were used for the analyses. These communities were obtained from wastewater samples from 14 sites upstream and downstream of drainage channels or effluents in the cities of Abidjan, Bouaké, and Yamoussoukro. The results obtained correspond to the number of positives among the treated samples (n = 39). Among the genetic markers of dissemination, class 1 integrons were the most evident in 94.8% of samples in Abidjan (93.3%), Bouaké (100%) and Yamoussoukro (91.6%). Class 2 integrons and class 3 integrons were found respectively in 41% and 51% of all samples. Genes coding for β-lactamases and blaTEM was identified in almost all samples at a rate of 97.4%. A co-presence of the three genes blaTEM, blaSHV and blaCTX-M is also remarkable in the sites of the city of Yamoussoukro. Among the genes coding for carbapenemases, only blaKPC 17.94%, blanDM 30.76% and blaOXA48 38.46% were detected in the samples.
