Minimally Invasive Surgery for necrotizing Pancreatitis: A case Report

Introduction: necrotizing pancreatitis management is complex and varies significantly among clinicians. Minimally invasive approaches like transgastric necrosectomy via laparoscopy are emerging as effective treatment options. This case report underscores the technique’s efficacy, clinical outcomes, and role in reducing complications. clinical Observation: A 59-year-old male with a history of smoking and alcoholism presented with severe abdominal pain, nausea, and vomiting. Over the following weeks, he developed symptoms including asthenia, weight loss, and melena. Diagnostic workup revealed severe anemia and Balthazar E necrotizing pancreatitis, with significant intra-abdominal fluid collections and signs of infection. After initial conservative management, the patient underwent transgastric necrosectomy via laparoscopy due to deteriorating clinical status. The procedure involved removing necrotic tissue and performing a cystogastroanastomosis and jejunostomy. Postoperative care included fasting, parenteral nutrition, broad-spectrum antibiotics, and enzymatic replacement. The patient recovered well, with reduced necrotic tissue on follow-up imaging, and was discharged twelve days post-surgery [1]. conclusion: Transgastric necrosectomy by laparoscopy is a valuable first-line surgical option for patients with symptomatic necrotizing pancreatitis, particularly in cases without prior interventions. This minimally invasive technique helps reduce major complications and mortality, offering a less invasive alternative to traditional open necrosectomy. The multidisciplinary approach and careful postoperative management were crucial to the patient’s favorable outcome. The case highlights the potential of transgastric necrosectomy as an effective treatment strategy in managing complex pancreatitis cases, including those with associated duodenal perforation [2].

Different La2O3 contents（0, 1, 2, 3, and 5 wt%） were used to prepared La2O3-ZrO2 mixed oxides calcined at 600 ℃ by the sol-gel method. The catalytic activity was measured as biodiesel production from canola oil through a transesterification reaction. The characterization results indicate that the La2O3 monolayer formation and extent of basicity of m-ZrO2 have a large influence on biodiesel *** biodiesel conversion（56% at 4 h） was obtained with the 3% La2O3-ZrO2 catalyst in the presence of basic sites and the formation of a monolayer of La2O3. The decrease in the catalytic activity for 5% La2O3-ZrO2 resulted from the loss of active sites on the catalyst because of agglomeration, which was suggested by XPS and the isoelectric point. The kinetic data fit to a pseudo-first order constant, and the largest kinetic constant corresponds to 3% La2O3-ZrO2, currently the largest heterogeneous non-alkaline metal catalyst reported for a transesterification reaction.

Relict glacial landscape in the Sierra Baguales Mountain Range(50°-51°S):evidence of glaciation dynamics and types in the eastern foothills of the southern Patagonian Andes

The glacial morphology of southern South American presents invaluable evidence to reconstruct former glacier behaviour and its relation to climate and environmental changes. However, there are still spatial and temporal gaps in the reconstruction of the Holocene Patagonian glacial landscape. Here we present the first geomorphological record for the Sierra Baguales Mountain Range(SBMR), forming the eastern foothills of the Southern Patagonian Andes 200 km from the Pacific coast. This area is topographically isolated from the Southern Patagonian Ice Field(SPIF), and is affected by the Westerly Winds. The study area shows evidence of ice sheet and alpine glaciations related to Andean uplift,which caused a marked climatic contrast between its western and eastern flanks since the Last Glacial Maximum(LGM). The regional rock mass strength and precipitation gradient acted as a controlling factor in the glacial cirque distribution and sizes, as well as in the development of glaciation types. We report new radiocarbon dates associated with warm/dry to cold/wet climatic changes during the middle Holocene, when former small alpine glaciers were located in the uppermost section of the SBMR basins, and eventually converged to form a small ice field or a composite valley glacier at lower *** can be explained by an estimated regional temperature drop of 3.8°c±0.8°c, based on a 585±26m Equilibrium Line Altitude(ELA) descent, inferred by geomorphological evidence and the Accumulation Area Ratio(AAR), in addition to a free-air adiabatic lapse rate. Subsequently, the glaciers receded due to climatic factors including a rise in temperature, as well as non-climatic factors, mainly the glacier bedrock topography.

The Influence of climate Variability on Holoplanktonic Polychaetes Populations of Ocean Sector of the colombian Pacific Basin

Studies of holoplanktonic polychaetes in the Tropical Eastern Pacific have focused on their systematics and distribution however, population responses related to climate variability are non-existent. Therefore, the present work described for the first time the influence of the climatic variability 2004-2012 on the holoplanktonic polychaetes populations, as well as their relationship to environmental variables. Species of the Alciopidae, Lopadorrhynchidae and Iospilidae families were identified from 85 samples collected in the Pacific Basin of colombia, contrasting them with the presence of EnSO, and correlating them with the environmental variables of temperature, salinity and chlorophyll-α concentration. Of the 14 species identified, only Lopadorrhynchus cf. henseni and L. cf. brevis increased their densities under El niñn>o and correlated positively with temperature. Rhynchonereella cf. petersii correlated negatively with temperature;while R. cf. gracilis and Pelagobia cf. longicirrata were positively correlated with chlorophyll-α concentration and salinity, respectively. Additionally, with the first description of the environmental ranges of the 14 species identified for the colombian Pacific basin, it was found that the most commons species presented broader environmental ranges. The results from the current research are consistent with previous studies in other regions of the Pacific and latitudes. The responses of the organisms suggest trophic and mobility trends that were not known for tropical holoplanktonic polychaete species.

Knowledge to Ecological Preferences in a Tropical Epiphytic Algae to Use with Eutrophication Indicators

In Ayapel floodplain periphyton associated to macrophyte roots on the basis of material was collected in fourteen sampling sites in nine moments of compilation. From the analysis of the samples were recorded 550 algal species, of which 56% were diatoms. Furthermore it was valued relative abundances of taxa and the average physicochemical variables to identify the optimal values and tolerance ranges of species. In the case of nitrates optimal value was below 2 mg/L for all algae except for Actinella sp. and Euastrum sinuosum which optimal values were close to zero with low variability. Regarding all species the algal nitrite concentration showed optimum values below 2 mg/L except Encyonema minutum and E. sinuosum whose optimum were 0.4 and 0.5 mg/L respectively. About nitrogen as ammonia was repeated the above situation, i.e. optimum values below 2 mg/L except for E. minutum and E. sinuosum. Respect to phosphorus the species with optimum greater than 0.6 mg/L are the diatoms, in contrast, the lowest values were optimal for E. sinuosum and Actinella sp. From the viewpoint of bioindication high abundance of Synedra goulardii indicates high transparency, Ulnaria ulna and Planothidium lanceolatum indicate lower concentration of dissolved oxygen and water temperature between 29°c - 30°c, Actinella aff. robusta, E. minutum, cylindrospermopsis raciborski, Lyngbya cf. limnetica, Eunotia naegueli, navicula cryptocephala, Diadesmis confervaceae, Frustulia rhomboides and Aulacoseira granulata.(For more information,please refer to the PDF.)

AIM: To establish the diagnostic performance of sev-eral serological tests, individually and in combination, for diagnosing celiac disease (cD) in patients with different pretest probabilities, and to explore potential se- rological algorithms to reduce the necessity for biopsy. METHODS: We prospectively performed duodenal biopsy and serology in 679 adults who had either high risk (n = 161) or low risk (n = 518) for cD. Blood samples were tested using six assays (enzyme-linked immunosorbent assay) that detected antibodies to tissue transglutaminase (tTG) and deamidated gliadin peptide (DGP). RESULTS: cD prevalence was 39.1% in the high-risk population and 3.3% in the low-risk group. In high-risk patients, all individual assays had a high diagnostic efficacy [area under receiving operator characteristic curves (AU ROc): 0.968 to 0.999]. In contrast, assays had a lower diagnostic efficacy (AU ROc: 0.835 to 0.972) in the low-risk group. Using assay combinations, it would be possible to reach or rule out diagnosis of cD without biopsy in 92% of cases in both pretest populations. We observed that the new DGP/tTG Screen assay resulted in a surplus compared to more conventional assays in any clinical situation. cOncLUSIOn: The DGP/tTG Screen assay could be considered as the best initial test for cD. combinations of two tests, including a DGP/tTG Screen, might be able to diagnose cD accurately in different clinical scenarios making biopsy avoidable in a high proportion of subjects.

The lithium ionic conductingLa0.45ce0.1Li0.27TiO3 has been synthesized by sol-gel method. This solid is the result of substitutional doping with ce(IV) in La0.45ce0.1Li0.27TiO3 compound. The aim of the replacement of La(III) by ce(IV) is increase the number of vacancies in the structure and favors the ionic mobility. Structural characterization shows that the obtained material have the expected tetragonal P4/mmm perovskite structure. chemical analysis shows that composition was homogeneus in all the sample. The bulk conductivity measured at room temperature is about the same as previously reported for its related lanthanum lithium titanate. However, the lower activation energy for ionic conduction encourages further searching for better conductors in this system.
