Background:Newly diagnosed breast cancer patients experience symptoms that may affect their quality of life,treatment outcomes,and *** and managing breast cancer-related symptoms soon after diagnosis is *** purpose of this study was to investigate the associations between health-related fitness(HRF)and patient-reported symptoms in newly diagnosed breast cancer ***:This study utilized baseline data from the Alberta Moving Beyond Breast Cancer Cohort Study that were collected within 90 days of *** measures included peak cardiopulmonary fitness(peak volume of oxygen consumption(VO_(2peak))),maximal muscular strength and endurance,flexibility,and body *** measures included depression,sleep quality,and *** multivariable logistic regression was performed for ***:Of 1458 participants,51.5%reported poor sleep quality,26.5%reported significant fatigue,and 10.4%reported moderate *** multivariable-adjusted models,lower relative VO_(2peak)was independently associated with a greater likelihood of all symptom measures,including moderate depression(p<0.001),poor sleep quality(p=0.009),significant fatigue(p=0.008),any symptom(p<0.001),and multiple symptoms(p<0.001).VO_(2peak)demonstrated threshold associations with all symptom measures such that all 3 lower quartiles exhibited similar elevated risk compared to the highest *** strength of the threshold associations varied by the symptom measure with odds ratios ranging from-1.5 for poor sleep quality to-3.0 for moderate depression and multiple ***,lower relative upper body muscular endurance was also independently associated with fatigue in a dose-response manner(p=0.001),and higher body weight was independently associated with poor sleep quality in an inverted U pattern(p=0.021).Conclusion:Relative VO_(2peak)appears to be a critical HRF component associated with multiple patient-reported symptoms in newly diag

Salinity effect on the compaction behaviour,matric suction,stiffness and microstructure of a silty soil

To better understand the salinity effect on the compaction behaviour of soil,standard Proctor compaction test was conducted on soil samples with different *** suction and small-strain shear modulus,G_(max),were determined and pore size distribution was also investigated on samples statically compacted at different water *** showed that with the decrease of soil salinity from initial value of 2.1‰(g of salt/kg of dry soil)to zero,the maximum dry density increased and the optimum water content decreased,whereas there was no significant change with the increase of soil salinity from 2.1‰ to 6.76‰.Interestingly,it was observed that G_(max) also decreased when the soil salinity decreased from initial value of 2.1‰ to zero and kept almost constant when the soil salinity increased from 2.1‰ to 6.76‰,for dry samples with similar matric suction and also for samples compacted at optimum state and on wet side whose matric suctions were slightly different due to the difference in remoulded water ***,the effect of salinity on compaction behaviour and G_(max) decreased for samples compacted from dry side to wet *** pore size distribution exhibited bi-modal characteristics with two populations of micro-and macro-pores not only for samples compacted on dry side and at optimum state,but also for those compacted on wet *** examination showed that the modal size of micro-pores shifted to lower values and that of macro-pores shifted to higher values for saline soil compared to the soil without salt.

Determining osmotic suction through electrical conductivity for unsaturated low-plasticity soils

Determining osmotic suction from the electrical conductivity(EC)of soil pore water was widely reported in the ***,while dealing with unsaturated soils,they do not have enough soil pore water to be extracted for a reliable measurement of *** this paper,the chilled-mirror dew-point hygrometer and contact filter paper method were used to determine the total and matric suctions for low-plasticity soils with different salinities(0.05‰,2.1‰,and 6.76‰).A new piecewise function was proposed to calculate the osmotic suction,with the piecewise point corresponding to the first occurrence of precipitated salt in mixed salt solutions(synthetic seawater).EC,ion and salt concentrations used for osmotic suction calculation were transformed from the established relationships of mixed salt solution instead of experimental *** calculated osmotic suction by the proposed equation and the equations in the literature was compared with the indirectly measured one(the difference between the measured total and matric suctions).Results showed that the calculated osmotic suction,especially the one calculated using the proposed function,was in fair agreement with the indirectly measured data(especially for specimens with higher salinity of 6.76‰),suggesting that the transformation of EC and concentrations from the established relationship is a good alternative to direct measurement for lowplasticity *** particular,the proposed method could be applied to unsaturated low-plasticity soils which do not have enough soil pore water for a proper EC measurement.

Early Prosthetic Valve Endocarditis with Mycobacterium Tuberculosis after Mitral Valve Replacement: A Case Report

Background: Tuberculous endocarditis is a rare but serious complication of heart valve replacement surgery. We report the case of a 24-year-old patient, who presented with tuberculous endocarditis after mechanical mitral valve replacement, with a favorable clinical course following anti-tuberculosis treatment. Case Presentation: We report a 24-year-old male patient, admitted to the cardiac surgery department of the Fann Hospital (Dakar, Senegal), for the management of severe mixed (rheumatic and endocarditic) mitral insufficiency with associated tricuspid insufficiency. He had a history of recurrent angina and polyarthralgia in childhood, was hospitalized several times for refractory global cardiac decompensation, and for a suspected infective endocarditis a month before his admission. On admission, the clinical examination revealed signs suggestive of mitral and tricuspid insufficiency. Transthoracic echocardiography revealed severe post-endocarditic mitral insufficiency with A3 amputation, highly mobile 15 mm vegetations on the free edge of the large valve, moderate tricuspid insufficiency, and severe pulmonary artery hypertension. Mechanical mitral valve replacement and tricuspid valve annuloplasty using autologous pericardial strip were performed via median sternotomy. After ten days, the patient presented with global cardiac decompensation associated with a clinico-biological infectious syndrome, and tans-oesophageal echography revealed an abscess at the sinotubular junction, communicating with the aorta. A thoraco-abdomino-pelvic CT scan was done, which revealed a bilateral alveolar-interstitial syndrome with mediastinal lymphadenopathy. Anti-tuberculosis treatment with RHZE was initiated for 06 months. The clinical course was favorable. Conclusion: Tuberculous endocarditis in prostheses is a serious complication of heart valve replacement surgery, which may evolve favorably under medical treatment.

Replacement of alkali silicate solution with silica fume in metakaolin-based geopolymers

A metakaolin(Mk)-based geopolymer cement from Tunisian Mk mixed with different amounts of silica fume(SiO_2/Al_2O_3 molar ratio varying between 3.61 and 4.09) and sodium hydroxide(10M) and without any alkali silicate solution, is developed in this work. After the samples were cured at room temperature under air for 28 d, they were analyzed by X-ray diffraction(XRD), Fourier transform infrared(FTIR) spectroscopy, environmental scanning electron microscopy, mercury intrusion porosimetry, ^(27)Al and ^(29)Si nuclear magnetic resonance(NMR) spectroscopy, and compression testing to establish the relationship between microstructure and compressive strength. The XRD, FTIR, and ^(27)Al and ^(29)Si NMR analyses showed that the use of silica fume instead of alkali silicate solutions was feasible for manufacturing geopolymer cement. The Mk-based geopolymer with a silica fume content of 6 wt%(compared with those with 2% and 10%), corresponding to an SiO_2/Al_2O_3 molar ratio of 3.84, resulted in the highest compressive strength, which was explained on the basis of its high compactness with the smallest porosity. Silica fume improved the compressive strength by filling interstitial voids of the microstructure because of its fine particle size. In addition, an increase in the SiO_2/Al_2O_3 molar ratio, which is controlled by the addition of silica fume, to 4.09 led to a geopolymer with low compressive strength, accompanied by microstructures with high porosity. This high porosity, which is responsible for weaknesses in the specimen, is related to the amount of unreacted silica fume.

Confounding response of macrofauna from a confluence of impacts: brine and sewage pollution

Throughout the shores of the world, a confluence of different sources of pollution is common, e.g., through outlets releasing brine, industrial, and domestic sewage. In this study, we assessed whether a combined disposal of domestic sewage and brine altered the patterns of abundance and assemblage structure of sub- tidal macrofauna inhabiting sandy seabeds on the south coast of Gran Canaria （Canary Islands, NE Atlantic Ocean）. Samples were collected in two surveys （May 2008 and January 2009） at three distances （0, 15, and 30 m） away from the discharge point. Macrofaunal abundances showed different patterns with varying proximity from the disposal point in the two surveys. In May 2008, lower abundances were observed at 0 m （53.89±46.82 ind.） than at 15 m （120±91.7 ind.） and 30 m （283.89±189.33 ind.） away from the dis- posal point. In January 2009, however, higher abundances were observed at 0 m （131.33±58.69 ind.） than at 30 m （100±24.44 ind.） and 15 m （84.78±58.39 ind.） away from the disposal point. Significant differ- ences in macrofaunal assemblage structure were found with varying proximity from the discharge point in both surveys. The effect of proximity from the disposal point was confounded by changes in sedimen- tary composition between surveys, which contributed to explain differences in macrofaunal abundances and assemblage structure with varying proximity from the discharge point. As a result, confounding driver- s of macrofaunal assemblage structure, here, changes in sediment composition, accounted for changes in macrofaunal abundances and assemblage structure with varying proximity from a combined disposal point of brine and sewage.

Soft power is a useful concept of political theory and sociology developed by J.S. Nye ***1990. The purpose of this study is to identify the relationship between soft and hard power attwo levels: national level and international one. The relation between the hard power and the softpower gives the fact that the growth of the hard power can’t automatically promote the growth ofthe soft power, while the strength of the soft power relies on the government’s political wish andnational will, along with the long-term molding, operation and construction. So molding softpower should be taken into account from the strategic perspective and the molding systemshould be formed gradually so as to create circumstances for China’s rise. Beside, the research indicates the main ways of this relationship;emphasize majoradvantages and disadvantages of soft power. With the globalization and information-centeredbackground of the day, the position and function of the soft power are increasingly becoming sonoticeable that it is considered as the important criterion and significant instrument on theinternational relation system. The influence of soft power is remarkably increased in nowadaysworld politics. The research outlines domestic and foreign cultural policy analyzes the fundamentaltrends in the study of China foreign cultural policy by Chinese and European researchcommunities, identifies the basic principles, courses and benefits of this policy.

Review:DNA-damage response network at the crossroads of cell-cycle checkpoints, cellular senescence and apoptosis

Tissue homeostasis requires a carefully-orchestrated balance between cell proliferation, cellular senescence and cell death. Cells proliferate through a cell cycle that is tightly regulated by cyclin-dependent kinase activities. Cellular senescence is a safeguard program limiting the proliferative competence of cells in living organisms. Apoptosis eliminates unwanted cells by the coordinated activity of gene products that regulate and effect cell death. The intimate link between the cell cycle, cellular senes- cence, apoptosis regulation, cancer development and tumor responses to cancer treatment has become eminently apparent. Extensive research on tumor suppressor genes, oncogenes, the cell cycle and apoptosis regulatory genes has revealed how the DNA damage-sensing and -signaling pathways, referred to as the DNA-damage response network, are tied to cell proliferation, cell-cycle arrest, cellular senescence and apoptosis. DNA-damage responses are complex, involving “sensor” proteins that sense the damage, and transmit signals to “transducer” proteins, which, in turn, convey the signals to numerous “effector” proteins implicated in specific cellular pathways, including DNA repair mechanisms, cell-cycle checkpoints, cellular senescence and apoptosis. The Bcl-2 family of proteins stands among the most crucial regulators of apoptosis and performs vital functions in deciding whether a cell will live or die after cancer chemotherapy and irradiation. In addition, several studies have now revealed that members of the Bcl-2 family also interface with the cell cycle, DNA repair/recombination and cellular senescence, effects that are generally distinct from their function in apoptosis. In this review, we report progress in understanding the molecular networks that regulate cell-cycle checkpoints, cellular senescence and apoptosis after DNA damage, and discuss the influence of some Bcl-2 family members on cell-cycle checkpoint regulation.
