Adsorptive removal of arsenic using adsorption gels prepared from orange and apple juice residues was reviewed by summarizing the authors’ previous papers. Orange and apple juice residues contain a large amount of pectin, partly methyl-esterified pectic acid, which exhibits high affinity for high-valent metal ions such as iron(III), rare earths(III) and zirconium(IV). Anionic species of arsenic(III, V) are effectively and selectively adsorbed on pectic acid gel via loading these high-valent metal ions. Raw orange juice residue was saponified using calcium hydroxide to improve the loading capacity for these metal ions. It was found that zirconium(IV) exhibits the most suitable adsorption behaviors for arsenic(III, V). Similar result was obtained also for apple juice residue. An actual sample of acid mine drainage from the Horobetsu mine which contained a high concentration of iron and low concentration of arsenic, was tested using the adsorption gel prepared from orange juice residue and the results were compared with those from the current treatment process based on coprecipitation with iron hydroxide. The new process using the above- mentioned adsorption gel was proposed for treatment of such acid mine drainage.

AIM: To analyze the risk factors of hepatocellular carcinoma (HCC) recurrence after radiofrequency ablation (RFA) treatment with HCV-associated hepatitis. METHODS: Twenty-six patients with HCV-associated HCC who were followed-up for more than 12 mo were selected for this study. Risk factors for distant intrahepatic recurrences of HCC were evaluated for patients in whom complete coagulation was achieved without recurrence in the same subsegment as the primary nodule. Twelve clinical and tumoral factors were examined: Age, gender, nodule diameter, number of primary HCC nodule, Child-Pugh classification, serum platelet, serum albumin, serum AST, post RFA AST, serum ALT, post RFA ALT, post RFA treatment.RESULTS: Distant recurrences of HCC in remnant liver after RFA were observed in 14 cases and in the number of primary HCC nodules (P = 0.047), and the serum platelets (P = 0.030), the clear difference came out by the recurrence group and the non-recurrence group. The cumulative recurrence rates after 1 and 2 years were30.8% and 86.8%, respectively for primary multinodular HCC, and 15.4% and 29.5% respectively, for primary uninodular HCC. In addition the 1-year recurrence rates for patients with serum albumin more than 3.4 g/dL and less than 3.4 g/dL were 23.1% for both, but the 2-years recurrence rates were 89.0% and 23.1%, respectively. The number of primary HCC nodules (relative risk, 6.970; P = 0.016) were found to be a statistically significant predictor for poor distant intrahepatic recurrence by univariate analysis.CONCLUSION: Patients who have multiple HCC nodules, low serum platelets and low serum albumin accompanied by HCV infection, should be carefully followed because of the high incidence of new HCC lesions in the remnant liver, even if coagulation RFA is complete.

Schwannoma of Hypoglossal Nerve Origin in the Upper Neck: Case Report and Review of the Literature

Schwannomas originating from the hypoglossal nerve are extremely rare neoplasms. Hypoglossal schwannomas usually occur between the third and fifth decades of life, with no sex predilection, commonly presenting as a painless, slow-growing, and lateral neck mass. The best treatment is complete surgical excision with preservation of the neural pathway, if possible. In fact, since these tumors are almost always benign, a conservative surgical approach is emphasized by most authors. We report a case of a large schwannoma of hypoglossal nerve origin in the upper neck in a 31-year-old male. Magnetic resonance imaging of the area demonstrated a 43 × 39 × 36 mm well-circumscribed mass with high and nonhomogeneous signal intensity on the right side of the upper neck. The lesion was successfully treated by extirpation, with no recurrence. The extracranial hypoglossal nerve sheath was as the origin of this tumor because the patient experienced remarkable disturbance of tongue motility after surgery.

Functional expression of a proliferation-related ligand in hepatocellular carcinoma and its implications for neovascularization

AIM: To detect the expression of a proliferation-related ligand on human hepatocellular carcinoma (HCC) cell lines (SK-Hep1, HLE and HepG2) and in culture medium.METHODS: APRIL expression was analyzed by Western blotting in HCC cell lines. Effects of APRIL to cell count and angiogenesis were analyzed, too.RESULTS: Recombinant human APRIL (rhAPRIL) increased cell viability of HepG2 cells and, in HUVEC, rhAPRIL provided slight tolerance to cell death from serum starvation. Soluble APRIL (sAPRIL) from HLE cells increased after serum starvation, but did not change in SK-Hep1 or HepG2 cells. These cells showed down-regulation of VEGF after incubation with anti-APRIL antibody.Furthermore, culture medium from the HCC cells treated with anti-APRIL antibody treatment inhibited tube formation of HUVECs.CONCLUSION: Functional expression of APRIL might contribute to neovascularization via an upregulation of VEGF in HCC.

Acupuncture therapy had established in China and it distributed among Asian countries and developed in each country as traditional medicine. In this project we compared the procedures and effects of traditional acupuncture therapies in China, Korea and Japan by using 10 healthy young male volunteers with written informed consent. Total white cell counts, leukocyte subsets and noradrenalin contents were used as outcomes. Three well trained traditional acupuncture therapists were chosen and they treated the same subjects at random order with intervals of at least two months. Each acupuncturist made his diagnosis and points selection for potentiate vital conditions. Clearly different procedures and point selections were used among three traditional acupuncture therapies on the same subject and the obtained changes of outcome measures also differed. Although variations of data were quite large, these results clearly demonstrated that acupuncture therapies have developed among three countries in unique manner.

Two primary defense systems develop in vertebrates, innate and adaptive. Despite those defense systems, the overwhelming problems of possessing this dual system, the innate or adoptive do not seem to guard or even prevent the development of one internal threat to survival, moreover sometimes direct to autoimmunity accompanying hypersensitivity. Further, every individual exposes to the risk of immunodeficiency in daily life with both internal and external stimuli. In this report, we tried to report the suitable menu for regulating the immune system modulate and discuss how we can compare and select each menu by evidence-based manner more than VAS. In a series of investigation, we have assessed the various health promoting menus such as hot spring hydrotherapy, acupuncture & moxibustion, light walking, independent to the western medicine. In this report, we try to summarize the regulatory effect of hot spring hydrotherapy as our representative menu under the influence of hypothalamus system. Their effects of elements were evaluated by the total number of peripheral leukocyte, its subsets, granulocyte and lymphocyte ratio, emotional hormones, receptor positive lymphoid cells. The regulative effects were confirmed also by other menu than hydrotherapy, acupuncture, walking etc. However, just by the simple comparison between the group before and after the hydrotherapy, we cannot exhibit the real regulatory effect by each menu. So we tried to show the effect by trying to indicate the constitution/condition dependent manner. Because the effect was not uniformly to each individual but it was dependent on each condition/constitution before the start of the menu. This is the one of the main proposes of this report for summaries that it is important to regulate the number and levels of each factor as ideal value. In other words, the regulation of each factor affected under the hypothalamus system is regulated with each constitution, under the influence of daily circumstances. The polymodai receptor hypothesis of acupuncture and moxibustion,and its rational explanation of acupuncture points

The participation of polymodal-type receptor (PMR) in the action of acupuncture and moxibustion (A&M) was briefly reviewed and a working hypothesis was proposed. The PMR is known as a kind of nociceptor, and is responsive to mechanical (acupuncture), thermal (moxibustion) and chemical stimuli, and its sensory terminals are free nerve endings and exist in various tissues of the entire body. Selective activation of the PMR produced the de-qi sensation (a kind of deep pain) and neurogenic inflammation (flare and wheal), and considered as an input of the endogenous pain inhibition, autonomic, immune and endocrine functions. These characteristics of PMR are in agreement with a possible candidate of A&M. One of the functional characteristics of acupuncture points is its tenderness (ah-shi points) and its close relation to the trigger points. The sensitization of PMR is suggested to be the major cause of formation of trigger points, so if one choose to acupuncture (trigger) points as the treatment loci, the sensitized PMR are activated more easily by gentle A&M procedures and activate various bio-regulatory systems more effectively. Thus, the present PMR hypothesis offers a rational explanation of functional meanings of the acupuncture (tigger) points.
