Prognostic factors and clinical features of rhino-orbitalmucormycosis cases:an update for patient and visual survivals

AIM:To determine the frequency of patients’vision survival and prognostic factors and evaluate clinical features in rhino-orbital mucormycosis.METHODS:Forty-three eyes of 43 patients followed up with orbital mucormycosis infections were included in the study.Demographic characteristics of the patients,symptoms at admission,ophthalmologic and non-ophthalmologic examination findings,clinical findings during follow-up,medical and surgical procedures,and complications were recorded.Patient survival was determined by assessing the incidence of mortality,and vision survival was defined as achieving a final visual acuity of at least light perception.RESULTS:Twenty-seven(62.8%)patients were male,and 16(37.2%)were female.When the underlying disease status of the patients was examined,it was observed that all patients had an underlying disease and diabetes constituted the majority(65.2%).Periorbital swelling(69.8%)and ophthalmoplegia(53.5%)were the most common symptoms and findings at the admission of patients with mucormycosis infection.The disease resulted in death in 22(51.2%)patients.The presence of fever and shorter duration of antifungal therapy were associated with lower patient survival.Exenteration surgery was not found to be associated with the survival of the patients.Frozen eye,loss of pupillary light reflex,and development of central retinal artery occlusion were associated with lower vision survival.CONCLUSION:This study presents one of the most extensive patient series in the literature on rhino-orbital mucormycosis.Knowing the patients’symptoms at the time of admission and the clinical findings during the infection process will increase awareness about the disease.

Comprehensive analysis of glacier recession(2000-2020)in the Nun-Kun Group of Glaciers,Northwestern Himalaya

Himalayan glaciers are shrinking rapidly,especially after 2000.Glacier shrinkage,however,shows a differential pattern in space and time,emphasizing the need to monitor and assess glacier changes at a larger scale.In this study,changes of 48 glaciers situated around the twin peaks of the Nun and Kun mountains in the northwestern Himalaya,hereafter referred to as Nun-Kun Group of Glaciers(NKGG),were investigated using Landsat satellite data during 2000-2020.Changes in glacier area,snout position,Equilibrium Line Altitude(ELA),surface thickness and glacier velocity were assessed using remote sensing data supplemented by field observations.The study revealed that the NKGG glaciers have experienced a recession of 4.5%±3.4%and their snouts have retreated at the rate of 6.4±1.6 m·a^(-1).Additionally,there was a 41%increase observed in the debris cover area during the observation period.Using the geodetic approach,an average glacier elevation change of-1.4±0.4 m·a^(-1)was observed between 2000 and 2012.The observed mass loss of the NKGG has resulted in the deceleration of glacier velocity from 27.0±3.7 m·a^(-1)in 2000 to 21.2±2.2 m·a^(-1)in 2020.The ELA has shifted upwards by 83.0±22 m during the period.Glacier morphological and topographic factors showed a strong influence on glacier recession.Furthermore,a higher recession of 12.9%±3.2%was observed in small glaciers,compared to 2.7%±3.1%in larger glaciers.The debris-covered glaciers showed lower shrinkage(2.8%±1.1%)compared to the clean glaciers(9.3%±5%).The glacier depletion recorded in the NKGG during the last two decades,if continued,would severely diminish glacial volume and capacity to store water,thus jeopardizing the sustainability of water resources in the basin.

Boron carbide has unique properties for wide application possibilities;however,poor sinterability limits its applications.One approach to overcome this limitation is the addition of secondary phases into boron carbide.Boron carbide based composite ceramics are produced by the direct addition of secondary phases into the structure or via reactive sintering using a sintering additive.The present study investigated the effect of Ti_(3)SiC_(2) addition to boron carbide by reactive spark plasma sintering in the range of 1700-1900℃.Ti_(3)SiC_(2) phase decomposed at high temperatures and reacted with B4C to form secondary phases of TiB2 and SiC.The results demonstrated that the increase of Ti_(3)SiC_(2) addition(up to 15 vol%)effectively promoted the densification of B4C and yielded higher hardness.However,as the amount of Ti_(3)SiC_(2) increased further,the formation of microstructural inhomogeneity and agglomeration of secondary phases caused a decrease in hardness.

Silicon(Si)has emerged as a potent anode material for lithium-ion batteries(LIBs),but faces challenges like low electrical conductivity and significant volume changes during lithiation/delithiation,leading to material pulverization and capacity degradation.Recent research on nanostructured Si aims to mitigate volume expansion and enhance electrochemical performance,yet still grapples with issues like pulverization,unstable solid electrolyte interface(SEI)growth,and interparticle resistance.This review delves into innovative strategies for optimizing Si anodes’electrochemical performance via structural engineering,focusing on the synthesis of Si/C composites,engineering multidimensional nanostructures,and applying non-carbonaceous coatings.Forming a stable SEI is vital to prevent electrolyte decomposition and enhance Li^(+)transport,thereby stabilizing the Si anode interface and boosting cycling Coulombic efficiency.We also examine groundbreaking advancements such as self-healing polymers and advanced prelithiation methods to improve initial Coulombic efficiency and combat capacity loss.Our review uniquely provides a detailed examination of these strategies in real-world applications,moving beyond theoretical discussions.It offers a critical analysis of these approaches in terms of performance enhancement,scalability,and commercial feasibility.In conclusion,this review presents a comprehensive view and a forward-looking perspective on designing robust,high-performance Si-based anodes the next generation of LIBs.

Solute transport and geochemical modeling of the coastal quaternary aquifer, Delta Dahab Basin, South Sinai, Egypt

The wadi dahab delta is in a dry, arid coastal zone within Egypt’s south Sinai Peninsula’s eastern portion. The primary water source is the Quaternary coastal alluvial aquifer. The groundwater salinity varies from 890to 8213 mg/L, with a mean value of 3417 mg/L. The dissolved major ions have been used to calculate the seawater mixing index(SWMI) using a linear equation that discriminates the groundwater mostly affected by water–rock interaction(SWMI 1>) and other samples mixed with Seawater(SWMI < 1). The isotopic composition of groundwater for specifically chosen groundwater samples ranges from-0.645‰ to +5.212‰ for δ^(18)O and from-9.582‰ to + 22.778‰ for δ^(2)H, where the seawater represented by a Red Sea water sample(δ^(18)O + 1.64‰-δ^(2)H + 9.80‰) and reject brine water are considerably enriched the isotopic groundwater values. The geochemical NETPATH model constrained by the dissolved significant ions, isotopes, and the rock aquifer forming minerals as phases indicate the mixing percent with the seawater ranges from 9% to 97% of seawater from 91% to 3% of original recharge water. According to the SEAWAT 3-D flow models, seawater has penetrated the Northeastern Dahab delta aquifer, with the intrusion zone extending1500 m inland. The salt dissolution, upwelling of saline water, recharge from the upstream mountain block, and seawater encroachment are the primary aspects contributing to the deterioration of groundwater quality. These findings may have significance for effective groundwater withdrawal management in arid locations worldwide with similar hydrogeological systems.

A soliton is a packet of self-reinforcing waves that maintains its structure when moving at a constant speed.Solitons are caused by the cancellation of the medium’s nonlinear and dispersive effects.In plas-mas,the bilinear form of Hirota will be utilized to investigate the(2+1)-dimensional Korteweg-de Vries equation with electrostatic wave potential.Solutions for complexiton lump interaction have been devel-oped.To throw further light on the physical qualities of the recorded data,certain 3-dimensional and contour plots are presented to illustrate the interaction elements of these solutions.

Adaptive Segmentation for Unconstrained Iris Recognition

In standard iris recognition systems,a cooperative imaging framework is employed that includes a light source with a near-infrared wavelength to reveal iris texture,look-and-stare constraints,and a close distance requirement to the capture device.When these conditions are relaxed,the system’s performance significantly deteriorates due to segmentation and feature extraction problems.Herein,a novel segmentation algorithm is proposed to correctly detect the pupil and limbus boundaries of iris images captured in unconstrained environments.First,the algorithm scans the whole iris image in the Hue Saturation Value(HSV)color space for local maxima to detect the sclera region.The image quality is then assessed by computing global features in red,green and blue(RGB)space,as noisy images have heterogeneous characteristics.The iris images are accordingly classified into seven categories based on their global RGB intensities.After the classification process,the images are filtered,and adaptive thresholding is applied to enhance the global contrast and detect the outer iris ring.Finally,to characterize the pupil area,the algorithm scans the cropped outer ring region for local minima values to identify the darkest area in the iris ring.The experimental results show that our method outperforms existing segmentation techniques using the UBIRIS.v1 and v2 databases and achieved a segmentation accuracy of 99.32 on UBIRIS.v1 and an error rate of 1.59 on UBIRIS.v2.

Platform Evolution in an Oligo-Miocene Back-arc Basin:An Example from the Central Iran Basin

The Qom Formation is the most important hydrocarbon reservoir target in Central Iran.The Qom platform developed in a back-arc basin during the Oligo-Miocene due to the closing of the Tethyan Seaway.This formation consists of a variety of carbonate and non-carbonate facies deposited on a platform ranging from supratidal to basin.A combination of tectonic and eustatic events led to some lateral and vertical facies variations in the study area.Six third-order depositional sequences and related surfaces were identified regarding vertical facies changes in the studied sections of this Oligo-Miocene succession.According to all results and data,this succession was initially deposited during the Chattian upon a distally steepened ramp of siliciclastic-carbonate composition,including the Bouma sequence.Then,from the late Chattian to the Aquitanian,the platform changed into a homoclinal carbonate ramp with a gentle profile.With respect to tectonic activity,this phase was a calm period during the deposition of the Qom Formation.Finally,a drowned carbonate platform and a rimmed shelf emerged during the Burdigalian,terminated by the continental deposits of the Upper Red Formation.Regarding all geological characteristics,three main tectono-eustatic evolutionary phases have been recognized in the Qom back-arcbasin.

Reduction of epinephrine in the lumbar spinal cord following repetitive blast-induced traumatic brain injury in rats

Traumatic brain inju ry-induced unfavorable outcomes in human patients have independently been associated with dysregulated levels of monoamines,especially epinephrine,although few preclinical studies have examined the epinephrine level in the central nervous system after traumatic brain injury.Epinephrine has been shown to regulate the activities of spinal motoneurons as well as increase the heart rate,blood pressure,and blood flow to the hindlimb muscles.Therefore,the purpose of the present study was to determine the impact of repeated blast-induced traumatic brain injury on the epinephrine levels in seve ral function-s pecific central nervous system regions in rats.Following three repeated blast injuries at 3-day intervals,the hippocampus,motor cortex,locus coeruleus,vestibular nuclei,and lumbar spinal cord were harvested at post-injury day eight and processed for epinephrine assays using a high-sensitive electrochemical detector cou pled with high-performance liquid chromatography.Our results showed that the epinephrine levels were significantly decreased in the lumbar spinal cord tissues of blast-induced traumatic brain injury animals compared to the levels detected in age-and sex-matched sham controls.In other function-specific central nervous system regions,although the epinephrine levels were slightly altered following blast-induced tra u matic brain injury,they were not statistically significant.These results suggest that blast injury-induced significant downregulation of epinephrine in the lumbar spinal cord could negatively impact the motor and cardiovascular function.This is the first repo rt to show altered epinephrine levels in the spinal cord following repetitive mild blast-induced traumatic brain injury.

Epidemiological and clinical features of paediatric open globe injuries in southwestern Turkey

AIM:To evaluate the epidemiologic,anatomic,and clinical features of open globe injuries in children.METHODS:The medical files of patients under the age of 16 who had been operated for an open globe injury at Akdeniz University Hospital’s Department of Ophthalmology were retrospectively evaluated.RESULTS:A total of 90 patients were evaluated in this study.Among these patients,26(28.9%)were female and64(71.1%)were male.The mean age of the patients was7.7±4.2 years.The male/female ratio was observed to increase with increasing age(P=0.006,P=7.48).Injuries were most likely to occur in spring and autumn(P=0.028).The time interval between the injury and the surgical repair was 9.36±27.4h.Forty(44.4%)of the injuries occurred in the home,27(30%)occurred in the yard,and 21(23.3%)happened while playing in the street.The most common causes of injury were sharp metal objects(P<0.001).Injury to the cornea occurred in47(52.2%)of the patients(P<0.001).The most common complication to occur was cataract formation.Additional operations were necessary for 37(41.1%)of the patients.The final visual acuity was correlated with both the initial visual acuity of the wounded eye prior to surgery and the length of the wound(P<0.001,r=0.502 and P<0.001,r=-0.442,respectively).CONCLUSION:Open globe injuries that are suffered in childhood generally occur either at home,in the yard,or on the street,with sharp metal objects being the most common cause of injury.The initial visual acuity and the length of the wound are the most important determinants of the final visual acuity.
