Cropland agroforest is an important production system in the southwest region of Bangladesh. This study focused on the floristic composition and management of existing cropland agroforests. A total of 313 cropland agroforests were surveyed and 83%respondents practiced pure agroforestry while the remaining 17% practiced agroforestry with fisheries. A total of 18 forest trees and 2 shrubs were recorded from 11 families and 59 species of agricultural crops were from 28 families. A higher proportion （79%） of cropland agroforests were occupied small land areas （0.12-0.80 ha）. About 63% of respondents planted trees for fruit production and 47%for timber production, and 35%of respondents engaged in commercial production （35%）. Swietenia macrophylla was the most prevalent species （relative prevalence 20.83） followed by man-gifera indica （relative prevalence 15.57） and Cocos nucifera （relative prevalence 7.08）. Shorter spacing was used for timber and fuel wood species and wider spacing for fruit trees. A wide range of rotation periods, from 5 to 25 years, was observed for both cases. The use of chemical fertilizer was highest followed by cow dung and compost in cropland agroforests. Overall management practices of cropland agroforest in southwest Bangladesh were determined by the end product and local demand.

Two cases of acute pancreatitis with leptospirosis are reported in this article. Case 1: A 68-year-old woman,presented initially with abdominal pain, nausea, vomiting,and jaundice. She was in poor general condition, and had acute abdominal signs and symptoms on physical examination. Emergency laparotomy was performed, acute pancreatitis and leptospirosis were diagnosed on the basis of surgical findings and serological tests. The patient died on postoperative d 6. Case 2: A 62-year-old man, presented with fever, jaundice, nausea, vomiting, and malaise. Acute pancreatitis associated with leptospirosis was diagnosed,according to abdominal CT scanning and serological *** patient recovered fully with antibiotic treatment and nutritional support within 19 d.

We report a case of right lower abdominal wall and groin abscess resulting from acute appendicitis. The patient was an 27-year-old man who had no apparent abdominal signs and was brought to the hospital due to progressive painful swelling of right lower abdomen and the groin for 10 d. Significant inflammatory changes of soft tissue involving the right lower trunk were noted without any apparent signs of peritonitis. Laboratory results revealed leukocytosis. Abdominal ultrasonography described the presence of abscess at right inguinal site also communicating with the intraabdominal region. Right inguinal exploration and laparotomy were performed and about 250 mL of pus was drained from the subcutaneous tissue and preperitoneal space. No collection of pus was found intraabdominally and subserous acute appendicitis was the cause of the abscess. The patient fully recovered at the end of the second post-operation week. This case reminds us that acute appendicitis may have an atypical clinical presentation and should be treated carefully on an emergency basis to avoid serious complications.

Coexistence of Mirizzi syndrome with adenomyomatosis in the gallbladder:report of a case

BACKGROUND: Mirizzi syndrome is a rare complication of cholelithiasis. Adenomyomatosis is a common tumor-like lesion of the gallbladder. METHODS: A 52-year-old man was admitted to our hospital complaining of right hypochondriac pain and jaundice. Ultrasonography and computed tomography revealed stones in the gallbladder and dilation of the intrahepatic bile ducts. Magnetic resonance cholangiopancreatography revealed narrowing of the common bile duct caused by compression of the gallbladder. Laparotomy revealed type II Mirizzi syndrome. RESULTS: Partial cholecystectomy with a Roux-en-Y hepaticojejunostomy reconstruction was performed. Histologically, Rokitansky-Aschoff sinus proliferation, hypertrophy of smooth muscles, and fibrosis were seen in the gallbladder. A segmental type of adenomyornatosis of the gallbladder was diagnosed. CONCLUSIONS: The pathogenic link between the two peculiar entities is unclear. A possible explanation is considered that the pathogenesis of Mirizzi syndrome is resulted from chronic inflammation due to adenomyomatosis.

Combined duplication of the colon and vermiform appendix in an adult patient

Combined duplication of the colon and vermiform appendix is one of the rare congenital anomalities of the alimentary tract. Only a few cases have been reported in the adult population. A 28-year-old man presented to the clinic with a mass in the right flank. Imaging showed only a hydronephrotic atrophic kidney. The f inal diagnosis was only available at exploration. Combined duplication of the tubular colon and vermiform appendix was conf irmed histopathologically. The patient was treated with nephrectomy and complete resection of the duplicated colon and vermiform appendix. The patient recovered uneventfully,and has done well for the past year. This is believed to be one of the first reports of combined duplication of the tubular colon and vermiform appendix as a cause of hydronephrotic atrophic kidney in an adult patient.

Due to their unique characteristics, such as the dynamic changing topology, the absence of central management, the cooperative routing mechanisms, and the resources constraints, Mobile ad hoc networks (manETs) are relatively vulnerable to both active and passive attacks. In manET, routing attacks try to disrupt the functions of routing protocol by intentionally or unintentionally dropping packets or propagating faked routing messages. However, due to their computation requirements, the prevention mechanisms are not powerful enough to secure manET. In this paper, we propose a distributed and cooperative scheme using statistical methods to detect routing attacks in manETs. Our scheme uses both direct and indirect observations to characterize the behaviors of both neighboring and remote nodes. Simple threshold and Grubb’s Test are utilized to propose our new detection methods. The scheme includes innovative methods to compute our proposed measures, Maximum Accusation Number (man) and Accusation Number (AN), which are used to make decision about node’s behavior. Experimental results show that our scheme performs well in detecting anomalous events in routing functions.

52-year-old man presented with a history of angina .pectoris over 3 months. He had a history ofhypertension and hyperlipidemia. His physical examination, blood chemistry, ECG, chest X-ray were normal. A treadmill exercise ECG test was performed using the Bruce protocol. Owing to 1 mm ST segment depressions at derivations DⅡ, DⅢ, aVE V4-V6, and typical chest pain at third stage the test was evaluated as ischemia-positive test. Therefore, he was scheduled fox diagnostic coronary angiography.

Arterial Ischemic Stroke as a First manifestation of Trousseau’s Syndrome

Malignancy related hypercoagulobility can manifest as acute arterial infarction, non-bacterial thrombotic endocarditis and thrombophlebitis. Rarely, ischemic stroke may be first symptom for a systemic occult cancer. We presented 62-year-old man who had recurrent stroke as the initial manifestation of an occult neoplastic process. Subsequently, the patient had venous thrombotic disease that led to the diagnosis of the malignancy. Conclusions: Acute or sub-acute multiple ischemic stroke may be the first manifestation of in an occult cancer.
