Insulin,a small protein with 51 amino acids synthesized by pancreatic β-cells,is crucial to sustain glucose homeostasis at biochemical and molecular *** metabolic dysfunctions are related to insulin-mediated altered glucose *** of the significant pathophysiological conditions linked to the insulin associated disorder is diabetes mellitus(DM)(type 1,type 2,and gestational).Insulin resistance(IR)is one of the major underlying causes of metabolic disorders despite its association with several physiological *** syndrome(MS)is another pathophysiological condition that is associated with IR,hypertension,and ***,several other pathophysiological disorders/diseases are associated with the insulin malfunctioning,which include polycystic ovary syndrome,neuronal disorders,and *** are an uncommon type of pancreatic β-cell-derived neuroendocrine tumor that makes up 2% of all pancreatic *** revealed that different biochemical events,molecular signaling pathways,microRNAs,and microbiota act as connecting links between insulin disorder and associated pathophysiology such as DM,insuloma,neurological disorder,MS,and *** this review,we focus on the insulin-related disorders and the underlying mechanisms associated with the pathophysiology.

Assessment of Wind Power Potential at Eastern-Jerusalem, Palestine

Due to the energy demand and lack of supplied energy of Palestinian cities, wind resource assessment is important and necessary. The objective of the work is to analyze the wind speed data characteristics and wind power potential at eastern Jerusalem that are collected at 10 m above ground level from 2008 to 2018. The variations of monthly, seasonal, and annual wind speed are analyzed, and the measured maximum, minimum, and mean values are presented in this study. Wind speed characteristics have been analyzed by the well-known Weibull distribution function, and used to evaluate the wind power of the proposed site. Moreover, the relationship between wind power and mean wind speed is fitted by a second-order polynomial. The shape parameter moderate values showed that wind speed was relatively steady at the site. The highest average maximum value was found to be 5.7 m/s in June-2008, whereas the mean maximum values ranged from 5.4 m/s in June to 3.8 m/s in November. The highest mean power value was found to be 31.66 w/m2 in July with a maximum value of 23.18 w/m2 in 2013. R2 of the polynomial fit provides values of 95% for monthly mean and 96% for annual mean.

On Second-Order Duality in Nondifferentiable Continuous Programming

A Mond-Weir type second-order dual continuous programming problem associated with a class of nondifferentiable continuous programming problems is formulated. Under second-order pseudo-invexity and second-order quasi-invexity various duality theorems are established for this pair of dual continuous programming problems. A pair of dual continuous programming problems with natural boundary values is constructed and the proofs of its various duality results are briefly outlined. Further, it is shown that our results can be regarded as dynamic generalizations of corresponding (static) second-order duality theorems for a class of nondifferentiable nonlinear programming problems already studied in the literature.

背景:多囊卵巢综合征(PCOS)是最常见的妇科内分泌疾病之一。这是育龄妇女常见的一种荷尔蒙失调。它目前是无排卵性不孕症的主要解释。育龄妇女的发病率为5%～10%。约70%的患者不孕。患有PCOS的患者可能会出现月经不调或经期延长或雄性激素(雄激素)水平过高的情况。卵巢可能会形成许多小体积的液体(卵泡)并且无法定期排出卵子。PCOS患者内分泌水平紊乱导致体内雌激素高、雄激素高、黄体酮低,最终导致子宫内膜容受性降低。腹部肥胖和胰岛素抵抗会刺激卵巢和肾上腺雄激素的产生,而SHBG水平会降低。在正常体重和肥胖的PCOS患者中都可以看到腹部肥胖和炎症系统的激活增加,导致血脂异常、糖尿病的风险增加。雄激素过多在PCOS相关代谢紊乱的发展中起着重要作用,对主要的外周代谢组织,尤其是脂肪组织,以及肝脏、胰腺和肌肉,尤其是大脑,会导致一系列代谢并发症,从PCOS,肥胖对胰岛素抵抗。然而,目前对高雄激素血症在PCOS代谢功能障碍中的致病作用及其潜在机制的认识尚不完全。此外,为PCOS患者的代谢开发更有效甚至个性化的治疗策略仍然是一个未满足的需求,更好地了解这种异质性综合征的分子基础当然是有益的。大量研究表明,对这一结果的解释也可能是PCOS患者复杂的内分泌和代谢变化。该事件取决于黄体酮和雌激素,限制其发展。适当水平的雌激素和孕激素可以促进胚胎和内膜发育同步并提高种植率。因此,诊断为PCOS的患者应筛查代谢综合征中的元素,包括体重、腰围、血压、HbA1c和脂质状态。此外,还需要更多数据来确定不同PCOS患者亚组的内分泌和代谢风险。目标:多囊卵巢综合征患者内分泌代谢紊乱的临床分析。材料和方法:对新乡医学院第三附属医院就诊的患者进行了回顾性队列研究。本研究对2018年1月至2020年11月在我院妇产科收治的156例符合鹿特丹标准诊断的多囊卵巢综合征患者进行回顾性分析,患者年龄为16-35岁(平均,24.78±3.69)岁。已婚患者占60.89%(95/156)和35.69%(34/95)有妊娠史。未婚患者占39.10%(61/156)。青少年和成年患者分别占8.97%(14/156)和91.03%(112/156)。所有受试者在过去3个月内均无类固醇摄入史。根据鹿特丹标准,将156名患者分为以下4个表型组:Ⅰ型(N=44);Ⅱ型(N=34):和2);Ⅲ型(N=29);和类型Ⅳ(N=49)。我的研究设计是针对这组患者,通过临床结果评估来观察生活质量的改善,这也将有助于进一步的研究和开发。结果:156例多囊卵巢综合征患者中,100例(64.10%)有血脂异常。分析多囊卵巢综合征患者内分泌代谢紊乱。表Ⅰ-Ⅲ中Ⅰ-Ⅳ组间BMI和WHR无显着差异;NOB组和NHI组的BMI和WHR分别显着低于OB和HI组。脂质指标TG、TC、LDL-C和HDL-C在Ⅰ-Ⅳ组之间无显着差异;而OB和HI组TG、TC、LDL-C分别显着高于NOB和NHI组,而OB和HI组HDL-C水平分别显着低于NOB和NHI 组。结论:多囊卵巢综合征是一种多因素激素相关疾病。我发现大约64.10%的患者有血脂异常,而Ⅰ-Ⅳ组的体重指数和腰臀比之间没有显着变化。多囊卵巢综合征患者内分泌代谢紊乱表现为血脂异常、肥胖和胰岛素抵抗,导致雄激素分泌增加,SHBG水平降低。

Liquid chromatography tandem mass chromatography （LC-MS/MS） is an important hyphenated technique for quantitative analysis of drugs in biological fluids. Because of high sensitivity and selectivity, LC-MS/MS has been used for pharmacokinetic studies, metabolites identification in the plasma and urine. This manuscript gives comprehensive analytical review, focusing on chromatographic separation approaches （column packing materials, column length and mobile phase） as well as different acquisition modes （SIM, MRM） for quantitative analysis of glucocorticoids and stimulants. This review is not meant to be exhaustive but rather to provide a general overview for detection and confirmation of target drugs using LC-MS/MS and thus useful in the doping analysis, toxicological studies as well as in pharmaceutical analysis.

Optimization of Air Quality Monitoring Network Using GIS Based Interpolation Techniques

This paper proposes a simple method of optimizing Air Quality Monitoring Network (AQMN) using Geographical Information System (GIS), interpolation techniques and historical data. Existing air quality stations are systematically eliminated and the missing data are filled in using the most appropriate interpolation technique. The interpolated data are then compared with the observed data. Pre-defined performance measures root mean square error (RMSE), mean absolute percentage error (MAPE) and correlation coefficient (r) were used to check the accuracy of the interpolated data. An algorithm was developed in GIS environment and the process was simulated for several sets of measurements conducted in different locations in Riyadh, Saudi Arabia. This methodology proves to be useful to the decision makers to find optimal numbers of stations that are needed without compromising the coverage of the concentrations across the study area.

Multiobjective Duality in Variational Problems with Higher Order Derivatives

A multiobjective variational problem involving higher order derivatives is considered and optimality condi-tions for this problem are derived. A Mond-Weir type dual to this problem is constructed and various duality results are validated under generalized invexity. Some special cases are mentioned and it is also pointed out that our results can be considered as a dynamic generalization of the already existing results in nonlinear programming.

Partial Protection by Lipoic Acid Against Carboplantin-induced Ototoxicity in Rats

Objective To investigate the alterations in auditory brainstem evoked responses (ABRs) and the changes of carboplatin-induced ototoxicity in the cochlear oxidant/antioxidant systems and otoprotection by an antioxidant lipoate. Methods Male wistar rats were divided into four groups and treated as follows: 1) vehicle (saline) control, 2) carboplatin (256 mg/kg, i.p.), 3) lipoate (100 mg/kg, i.p.), 4) lipoate + carboplatin. Post-treatment ABRs were performed after four days and rats were sacrificed with their cochleae harvested and analyzed. Results Carboplatin significantly elevated ABR threshold above the pretreatment thresholds. Lipoate+carboplatin treated rats showed decreased elevation of hearing threshold. Carboplatin significantly depleted cochlear reduced to oxizized glutathione (GSH/GSSG) ratio, whereas lipoate+carboplatin treatment increased GSH/GSSG ratio. Carboplatin significantly decreased cochlear copper zinc-superoxide dismutase (CuZn-SOD), catalase (CAT), glutathione peroxidase (GSH-Px), glutathione reductase (GR) and glutathione-S-transferase (GST) activities and enzyme protein expressions and a significant increase in Mn-SOD activity, protein expression and malondialdehyde (MDA) level. Cochlear antioxidant enzyme activities, enzyme protein expressions and MDA level were partially restored in lipoate+carboplatin treated rats, compared to carboplatin alone. Conclusion Carboplatin-induced ototoxicity is related to impairment of cochlear antioxidant system and otoprotection conferred by lipoate is associated with partial sparing of the cochlear antioxidant defense system.

Inflammation, oxidative stress and renin angiotensin system in atherosclerosis

Atherosclerosis is a chronic inflammatory disease associated with cardiovascular dysfunction including myocardial infarction, unstable angina, sudden cardiac death, stroke and peripheral thromboses. It has been predicted that atherosclerosis will be the primary cause of death in the world by 2020. Atherogenesis is initiated by endothelial injury due to oxidative stress associated with cardiovascular risk factors including diabetes mellitus, hypertension, cigarette smoking, dyslipidemia, obesity, and metabolic syndrome. The impairment of the endothelium associated with cardiovascular risk factors creates an imbalance between vasodilating and vasoconstricting factors, in particular, an increase in angiotensin Ⅱ(Ang Ⅱ) and a decrease in nitric oxide. The renin-angiotensin system(RAS), and its primary mediator Ang Ⅱ, also have a direct influence on the progression of the atherosclerotic process via effects on endothelial function, inflammation, fibrinolytic balance, and plaque stability. Anti-inflammatory agents [statins, secretory phospholipase A2 inhibitor, lipoprotein-associated phospholipase A2 inhibitor, 5-lipoxygenase activating protein, chemokine motif ligand-2, C-C chemokine motif receptor 2 pathway inhibitors, methotrexate, IL-1 pathway inhibitor and RAS inhibitors(angiotensin-converting enzyme inhibitors)], Ang Ⅱ receptor blockers and ranin inhibitors may slow inflammatory processes and disease progression. Several studies in human using anti-inflammatory agents and RAS inhibitors revealed vascular benefits and reduced progression of coronary atherosclerosis in patients with stable angina pectoris; decreased vascular inflammatory markers, improved common carotid intima-media thickness and plaque volume in patients with diagnosed atherosclerosis. Recent preclinical studies have demonstrated therapeutic efficacy of vitamin D analogs paricalcitol in Apo E-deficient atherosclerotic mice.
