Factors Associated with Antiretroviral Therapy Defaulting among Adult Patients Receiving Care at Chikankata Mission Hospital, Chikankata District, Zambia

Background: Defaulting on antiretroviral therapy has been identified as the most important factor contributing to the antiretroviral therapy failure rate. This study aimed to investigate factors associated with defaulting on antiretroviral therapy among adult patients receiving care at Chikankata Mission Hospital antiretroviral therapy clinic. Method: Cross-sectional analytical study on 385 participants selected by a computer generated random numbers technique of simple random sampling from among the patients receiving antiretroviral therapy at Chikankata Mission Hospital. Data collected were processed and analysed using Statistical Package for Social Science version 27. Univariate and backward multivariable logistic regression analysis was performed to identify factors associated with antiretroviral therapy defaulting. The level of significance was set at 5% with a confidence level of 95%. Results: Over half (58.4%) of the study participants defaulted on antiretroviral therapy. About 65.8% of study participants indicated improved health as the reason they defaulted on antiretroviral therapy. Most participants indicated that it was important to always go for antiretroviral therapy services (Adjusted Odds Ratio 1.95;95% Confidence Interval: [1.14 - 3.33], p = 0.015). Very few participants indicated poor family support for antiretroviral therapy services (Adjusted Odds Ratio 4.08;95% Confidence Interval: [2.02 - 8.23], p Conclusion: Defaulting on antiretroviral therapy continues to be a significant problem and needs to be addressed as a matter of priority. More counselling and awareness-raising programmes are required to improve knowledge and understanding on the importance of attending scheduled antiretroviral therapy clinics and services as well as the consequences of defaulting on antiretroviral therapy.

Factors Contributing to the Acceptability of Second Dose of Measles Vaccine among Children in Livingstone District, Zambia

Introduction: Measles is a viral disease that is a major public health problem in both developed and developing countries. It is a contagious disease resulting from infection with the measles virus and is still responsible for more than 100,000 deaths per year. Although vaccination is one of the most successful and cost-effective public health interventions to reduce mortality and morbidity, approximately 1.4 million children worldwide die from vaccine-preventable diseases each year. In Zambia, MCV2 was 66% below WHO recommended target of 95%. This study therefore assessed the acceptability of second dose measles vaccination services among caretakers/mothers with children less than two years of age in Livingstone, Zambia. Methods: The study used analytical cross-sectional study design. A logistic regression analysis was performed to identify factors associated with acceptability of the second dose of measles vaccine. A semi structured questionnaire was administered to collect data on socio demographic characteristics and factors related to acceptability of the second dose of measles vaccine. SPSS version 26.0 was used for descriptive and inferential analysis at 5% level of significance. Results: There was no statistically significant difference in acceptability of the second dose of measles vaccine by gender, marital status, type of employment, income level, socio-cultural and traditional beliefs, and education level. However, there was a statistically significant difference in acceptability of the second dose of measles vaccine by knowledge level of second dose of measles vaccine. Determinants of Breastfeeding Practices among Working Mothers in Solwezi District of Zambia

Background: As one of the strategies to reduce infant morbidity and mortality, the World Health Organization recommends early initiation of exclusive breastfeeding and timely introduction of complementary feeding with continued breastfeeding for up to two years or beyond. Although breastfeeding is a natural process, it is influenced by different socio-cultural factors, habits, standards, and behaviours. Rapid industrialization and changes in lifestyle have seen increasing numbers of women in the labour force, and associated poor Exclusive Breastfeeding practices among working mothers. The aim of this study was to identify the determinants of breastfeeding practices among working mothers in Solwezi District of Zambia. Methods: This was a cross-sectional comparative survey comparing determinants of breastfeeding practices among working mothers aged 15 - 49 years in urban and rural areas of Solwezi District. The study was carried out in the Under-five Clinics at Solwezi, Kimasala and Mushitala Urban Clinics, as well as St. Francis, Kapijimpanga and Kisalala Rural Health Centers. Eighty-seven participants were successfully recruited using convenience sampling. A researcher-assisted questionnaire was used to collect data. Data analysis was done using SPSS version 27.0. Descriptive statistical analysis was done using numerical measures such as measures of central tendency and dispersion. The Pearson Chi Square test, correlation and linear regression analysis were performed to test association between the variables at the 0.05 level of significance. Quality of Life in Chronic Kidney Disease Patients on Dialysis at the University Teaching Hospital-Adult Hospital, Lusaka, Zambia

Introduction: The importance of determining health related quality of life in Chronic Kidney Disease patients on dialysis is well established. However, research was limited in establishing the health related quality of life for chronic kidney disease patients on dialysis at University Teaching Hospital-Adult Hospital. Further the effects of haemoglobin levels and adequacy of dialysis on their health related quality of life were unknown. Therefore, the study sought to answer a research question: what is the health-related Quality of Life for Chronic Kidney Disease patients on dialysis at University Teaching Hospital? Method: The study was an analytical cross-sectional study that used a census sampling method. The study participants comprised of 104 patients who sought dialysis services (2020-2021) from the University Teaching Hospital-Renal Unit in Lusaka, Zambia. A structured Kidney Disease Quality of Life Short form (KDQOL-SF 36) was used to collect data. The Data was analyzed using the Statistics and Data software version 13, Chi-square tests, and logistic regression analysis was employed to analyse the data. A confidence interval of 95% was set with a significant level of 0.05. Results: The study included 104 Chronic Kidney Disease patients from the Dialysis Unit at University Teaching Hospital in Lusaka, Zambia. Two-thirds (68%) of the patients had a good overall health-related quality of life with the biological wellbeing having exceptionally high scores. The male gender (66.7%), unemployment (69.4%), and low haemoglobin levels (77.8%) were identified as factors associated with poor health-related quality of life. Conclusion: The health related quality of life of Chronic Kidney Disease patients at University Teaching Hospital was good. Low haemoglobin levels, age, male sex and unemployment were found to be factors associated with poor health related quality of life. Women’s Experience with Socio-Economic Factors Associated with Perinatal Morbidity and Mortality in Lusaka and Mumbwa Districts of Zambia

Background: More than half a million women and four million infants are reported to die every year due to complications related to pregnancy and child bearing. The efforts to improve quality maternity care have been on the World Health Organization member countries’ agenda. Zambia has been striving to reduce maternal mortality by ensuring universal access to maternal and child health care services. Our study aimed to explore women’s experience with socio-economic factors associated with perinatal morbidity and mortality in Lusaka and Mumbwa districts, Zambia. Methodology: This hermeneutic phenomenological study was conducted at four health facilities in Mumbwa and Lusaka Districts of Zambia. A purposeful sample of 45 consenting women organized in four groups was selected. Each group comprised of 11 to 12 women. The focus group discussion guide was used to direct the discussion and the Olympus Digital Voice Recorder WS-852 (Olympus Corporation, Shinjuku, Tokyo, Japan) was used to record the discussions. The audio data was manually transcribed and verbatim transcript analyzed using *** 8.0 qualitative data software (*** Scientific Software Development GmbH, Berlin, Germany) to ascertain patterns of relationships between themes and quotations. Results: Money, husbands and family support, adequate health care resources, and good nurses attitude simplify the burden of pregnancy, delivery and child rearing for the health care providers, mothers and society. Conclusion: Having money and all the necessary resources during pregnancy simplifies the burden of pregnancy, delivery and child rearing for the health care providers, mothers and society. Enhancing Evidence-Based Practice and Critical Thinking Skills of Nursing Students: Lessons from the Review of the Bachelor of Science in Nursing Curriculum of the University of Zambia

Background: Making changes in the existing curriculum aims, objectives, course contents learning outcomes and assessment strategies have become a fact of life for nurse educators. Purpose: To enhance the existing Bachelor of Science (BSc) in nursing curriculum through integration of evidence-based practice (EBP) and teaching of critical thinking skills. Materials and Methods: A needs analysis was conducted using a five-phased approach to review the BSc in nursing Curriculum. Kern’s six-step model was adapted and introduced through a series of workshop exercises that highlighted the application of each step: 1) Desk review of the BSc curriculum offered globally;2) Administration of the needs assessment questionnaire to key informants;3) Strengths, weakness, opportunities and threat analysis;4) Consultative meeting with major stakeholders;5) Curriculum review. Results: The five-phased approach established some gaps in existing curricula, and identified critical core competences and best practices in integrating EPB and critical thinking in the BSc undergraduate curriculum and some “A” level content that was not in tandem with the practice of nurses. New courses were developed to support students in academic writing and enhance professionalism and duration of training was reduced from 5 to 4 years. Conclusion: The process demonstrated that BSc curriculum review, in fact, should be thoroughly scrutinized to encourage positive changes to the curriculum, provide opportunities for team building and the development of leadership skills and a whole-of course perspective on the curriculum.

Contextualisation of Early and Enhanced Clinical Exposure Model through Development of Curricula for Advanced Practice Nursing and Midwifery

Background: An Advanced Practice Nurse is a generalist or specialized nurse who has acquired thorough graduate education a minimum of a master’s degree. The need for Advanced Practice Nurses is increasingly recognized globally. This paper describes the process, which was undertaken by School of Nursing Sciences, University of Zambia in reviewing and developing advanced practice nursing and midwifery curricula which will be implemented using the Early and Enhanced Clinical Exposure model (EECE). Materials and Methods: The curricula development/review process utilized a modified Taba’s Model which followed a step-by-step approach including: 1) desk review, 2) diagnosis of needs (needs assessment), 3) stakeholder consultations, 4) content development, 5) validations and approval from which several lessons were learnt and recommendations made. Findings and recommendations from different stages were used as a basis for reviewing and developing advanced practice nursing and midwifery curricula. Results: Desk review needs assessment and stakeholder consultations identified both strengths and weaknesses in the existing curricula. Major strengths were duration and core courses which met the minimum requirement for postgraduate nursing and midwifery training. Major weaknesses/gaps included some content that was too basic for the master’s level and the delayed exposure to practicum sites which limited the development of advanced practice skills. Others were inadequate competence for advanced practice, inadequate research methodology course, lack of content to foster development of personal soft skills and predominant use of traditional teaching methods. Stakeholders recommended implementing advanced, clinical and hands-on Masters of Nursing and Midwifery programmes which resulted in the review of four existing and development of five demand-driven curricula. Background and Objectives: Early and Enhanced Clinical Exposure immediately places postgraduate students in a clinical setting and incorporates continual hands-on instruction throughout their studies. It aims to motivate students by strengthening their academics, improving clinical and communication skills, and increasing their confidence. The underlying principles are to provide a clinical context and to ensure that the patient remains the centre of learning. The School of Nursing Sciences implemented this model in 2021 to produce hands-on Masters-level neonatal practitioners who can work in multidisciplinary clinical contexts. Therefore, this study explored the experiences of postgraduate nursing students on the Early and Enhanced Clinical Exposure model and draw implications for the future. Methods: A phenomenological study design was utilized at the University of Zambia, School of Nursing Sciences and comprised of eight Master of Science Neonatal Nursing students in their second year. Convenience sampling was used to select the study site and participants. Data was collected between 15th January 2023 and 31st January 2023 using an in-depth interview guide. Audio recording and notes were transcribed immediately after data collection. Data analysis was conducted using thematic analysis and codes and themes were constructed from the coded data. Ethical clearance and permission were sought before conducting the study. Results: Four major themes emerged from the study: identity and role confusion, challenging and hectic experiences, positive educational experiences, and personal and professional growth. These themes contributed to the promotion of evidence-based practice by helping students to assess, diagnose, and treat various conditions, as well as gain interest, experience, knowledge, and exposure.
