Prevalence of Cerebral Venous Thrombosis in COVID-19 Patients: A Systematic Review

Background: COVID-19 is a novel coronavirus that has been rapidly transmitted between individuals globally. COVID-19 has been associated with thrombotic events. A cerebral venous thrombosis is a rare form of stroke with a most common site of origin believed to be the junction of larger sinuses and cerebral veins. So, it is important to identify the prevalence and incidence of cerebral venous thrombosis among COVID-19 patients. Aim: To assess the prevalence of cerebral venous thrombosis in COVID-19 patients by reviewing the previous studies reported on this subject. Method: We searched for articles focused on our subject through PubMed and Google scholar databases. The included searching terms included “CVT, COVID-19, Prevalence, Complications, and Effect.” The inclusion criteria were studies conducted on COVID-19 patients, English articles, available full-text articles, and original articles. Result: We got 1178 articles;only six articles were eligible for the inclusion criteria. The included articles involved a total number of 551,727 participants with a mean age ranging between 34 years and 53.3 years and covered seven countries. The incidence of CVT among COVID-19 was reported as well as risk factors and symptoms. Conclusion: The incidence of CVT is high among COVID-19 patients compared to the general population, patients with influenza, and those who received the mRNA vaccine, whereas the prevalence requires further studies for precise data. A management strategy or treatment regimen should be established based on the severity of CVT patients, as the mortality rate among those patients was high.

Aconite Poisoning with Good Outcome, Case Study and Literature Review

Aconite poisoning has been reported in previously published literature and case reports. It may occur due to accidental consumption of aconite tubers or when it is used in medicinal preparations. In this case report we describe a case of aconite poisoning in a primary case of anxiety disorder, after use of herbal medicine. He gave history of consumption of herbal remedy with honey from folk medicine with unknown content. Our patient presented with symptoms of vomiting, diarrhea, numbness and dizziness that are common symptoms of aconite poisoning. The management was mainly supportive care depending on the symptoms. Our patient recovered after 30 hours, probably due to slow elimination of the metabolites overtime. Our case represents one case where poisoning was not due to wrong consumption but due to drug-drug interaction. It is important that physicians should take drug interactions into consideration while prescribing drugs and also educate patients regarding the same.

N-Acetyl Cysteine to Treat <i>Polygonum multiflorum</i>Hepatotoxicity: A Case Report

Background: Polygonum multiflorum (brand name Fo-ti) is a Chinese herb used as an antioxidant, antiaging agent, and liver and kidney health. It can cause acute hepatotoxicity and even death. There is no published data on the effectiveness of N-Acetyl cysteine (NAC) as a sole effective therapy. Case: A 23-year-old, healthy male, presented with jaundice and malaise of one-month duration. He had been taking a Chinese herb named Fo-Ti for 2 months. Physical examination revealed jaundice, with no stigmata of chronic liver disease. His laboratory results showed transaminitis with markedly elevated transaminases. Testings for infectious and autoimmune causes were unremarkable. The patient was admitted to the hospital. Fo-Ti was stopped and he was started on oral NAC as peroral protocol. He was discharged home on esomeprazole and oral NAC. He returned on the second day with worsening jaundice. He was readmitted and esomeprazole was discontinued. NAC was continued and his liver enzymes eventually improved. The supplement was tested by gas-chromatography mass spectrometry and measured the presence of emodin. Discussion: This case highlights the potential benefit of NAC as treatment of Polygonum multiflorum induced hepatotoxicity. The proposed mechanism of emodin induced liver injury is through disturbing glutathione metabolism in liver cells. This could explain the favorable response as NAC has the ability to replenish intracellular glutathione. Conclusion: Polygonum multiflorum use has been associated with severe and prolonged hepatotoxicity. Oral NAC should be considered in the treatment of these patients due to the favorable risk-benefit ratio.
