Ataxia telangiectasia in Turkey:multisystem involvement of 91 patients

Background:Ataxia telangiectasia (AT) is a genetically based multisystemic *** aimed to make a comprehensive evaluation of multisystem involvement in AT by describing clinical features and outcome of 91 ***:Medical records of the patients who were diagnosed and followed by a multidisciplinary approach during a 27-year period (1988-2015) were reviewed ***:Forty six female and 45 male patients with a mean follow-up period of 39.13±4.28 months were *** mean age at the time of symptom onset and diagnosis were 15.4±1.09 months and 73.6144.11 months,*** abnormalities were progressive truncal ataxia,nystagmus,dysarthria,oculomotor apraxia and *** one patients (34.1%) became dependent on wheelchair at a mean age of 12.1±2.8 *** patients (12.1%) became bedridden by a mean age of 14.7±1.8 *** magnetic resonance imaging revealed pathological findings in 47/66 *** immunological parameters were determined in 51/91 patients:immunoglobulin (Ig)A deficiency (n=38),lymphopenia (n=30),IgG (n=15) and IgG2 (n=11) *** of recurrent sinopulmonary infections (n=45) and bronchiectasis (n=22) were found to be more common in patients with impaired immunological parameters (P=0.029and P=0.023,respectively).Malignancy developed in 5 patients,being mostly lymphoreticular in origin and resulted in death of 4 ***:AT is a long lasting disease with muitisystem involvement necessitating multidisciplinary follow up,as described in our *** diagnosis of malignancy and supportive treatments regarding pulmonary and neurological health may prolong survival and increase the quality of life.

Research on the Contextual and Functional Differences Between New and Old Journalism

Mass Communication media were widespread in 19th and 20th centuries. Those technologies created by humans have had a big impact on both daily and business lives. Newspapers, Radio, TV and Cinema are obvious examples of this impact. Dinting the last two decades of 20th century, spread of personal computers transformed the way people live0 work and think. Some people evaluated the situation as revolution and others as threat to humans. Intemet, in this respect, is considered as mass media. People connect the Internet without time and space limitations. However, communication on the Internet is not mass commtmication because it requires every individual or organization to have a unique Intemet Protocol address （IP） to commtmicate. Because of its peer to peer communication nature just as in the telephone communication, this feature distinguishes Intemet from other commtmication media. On the other hand, it resembles mass communication media because of its mobility, spread and functionality both for sender and receiver. In many sectors, including communication and information technology sectors, mobility, spread and functionality features lead the way to increase interactivity between parties in communication. Businesses benefited the ＂interactivity tool＂ for many purposes： to increase market share for a product or service, to increase customer satisfaction, or to increase amount of information about their customers, competitors or suppliers in order to make strategic decisions. For journalism and generally, for mass media, interactivity is an important aspect of the business. Media corporations must know the needs of their audience before they express and take precautionary steps to satisfy them. This forces the mass media sector to increase interactivity in customer communication. Kefir treatment ameliorates dextran sulfate sodium-induced colitis in rats

AIM: To investigate the preventive effect of kefir on colitis induced with dextran sulfate sodium(DSS) in ***: Twenty-four male Wistar-albino rats were randomized into four groups: normal control,kefircontrol,colitis,and kefir-colitis groups. Rats in the normal and kefir-control groups were administered tap water as drinking water for 14 d. Rats in the colitis and kefir-colitis groups were administered a 3% DSS solution as drinking water for 8-14 d to induce colitis. Rats in the kefir-control and kefir-colitis groups were administered 5 m L kefir once a day for 14 d while rats in the normal control and colitis group were administered an identical volume of the placebo(skim milk) using an orogastric feeding tube. Clinical colitis was evaluated with reference to the disease activity index(DAI),based on daily weight loss,stool consistency,and presence of bleeding in feces. Rats were sacrificed on the 15 th day,blood specimens were collected,and colon tissues were rapidly removed. Levels of myeloperoxidase(MPO),tumor necrosisfactor(TNF)-α,interleukin(IL)-10,malondialdehyde,and inducible nitric oxide synthase(i NOS) were measured in colon ***: The DAI was lower in the kefir-colitis group than in the colitis group(on the 3rd and 5th days of colitis induction; P < 0.01). The DAI was also significantly higher in the colitis group between days 2 and 6 of colitis induction when compared to the normal control and kefir-control groups. The DAI was statistically higher only on the 6th day in the kefircolitis group when compared to that in the normal control groups. Increased colon weight and decreased colon length were observed in colitis-induced rats. Mean colon length in the colitis group was significantly shorter than that of the kefir-control group. Kefir treatment significantly decreased histologic colitis scores(P < 0.05). MPO activity in the colitis group was significantly higher than in the kefir-control group(P < 0.05). Digital Game as a New Media, and Digital Game Playing Habits of Youngsters in Turkey

Today, individuals can effectively use all kinds of communication tools with digital transformation. This transformation process is called ＂The Network Society＂ by Castells. According to Castells, people are living with long cables or networks that connect each other. With the progressing and spreading technological advances, entertainment sector is booming since 1950s even more dramatically than the computer sector. Digital games sector, in this perspective, has been the intersection area of the entertainment sector and the computer sector. We can include to these three sectors a fourth one： Communication （or Media） sector. With the convergence of these four sectors, there is an enormous sea of possibilities, opportunities and problemles faced by humans. Our information society is turning into games. Today, the traditional understanding of play has been replaced by digital games. Digital games as an independent communication tool differ from traditional play in that they include a new communication medium into action. There is a different communication process in these digital games, which especially is an interest to younger generations. Digital games provide an interactive environment with individuals as they allow electiveness, mobility and non-linearity. In an era of economic angst and where real and virtual identities are being discussed, individuals of the information society are searching for something different other than mass communication. They seek after individual experiences. The main focus in this study will be to analyze the habits of play in digital games. The relationship between digital games and their users will also be examined. The quantitative study of digital games will be a useful source for future studies and will bridge the gap in this area.

Anisotropy effect on strengths of metamorphic rocks

This paper aims to study the effect of anisotropy on strengths of several metamorphic rocks of southern(Cine) submassif of Menderes metamorphic massif in southwest Turkey. Four different metamorphic rocks including foliated phyllite, schist, gneiss and marble(calcschist) were selected and *** surveys were made along lines for each rock and evaluated with DIPS program. L-type Schmidt hammer was applied in the directions parallel and perpendicular to foliation during the field study. Several hand samples and rock blocks were collected during the field study for measurements of dry and saturated densities, dry and saturated unit weights and porosity, and for petrographic analysis and strength determination in laboratory. L-and N-type Schmidt hammers were applied in the directions perpendicular(anisotropy angle of 0°) and parallel(anisotropy angle of 90) to the foliation on selected blocks of phyllite, schist, gneiss and marble(calcschist). The phyllite and schist have higher porosity and lower density values than the other rocks. However, coarse crystalline gneiss and marble(calcschist) have higher rebound values and strengths, and they are classified as strong-very strong rocks. Generally, the rebound values in the direction perpendicular to the foliation are slightly higher than that in the direction parallel to foliation. Rebound values of N-type Schmidt hammer are higher than the L-type values except for phyllite. Sometimes, the rebound values of laboratory and field applications gave different results. This may result from variable local conditions such as minerals differentiation,discontinuities, water content, weathering degree and thickness of foliated structure.

Determination of fire-access zones along road networks in firesensitive forests

Wild forest fires are one of the greatest environmental disasters affecting forest resources. Along the coastal zone of the Mediterranean region in Turkey, forested areas are classified as first-degree, fire-sensitive areas. Every year, thousands of hectares of forests have been destroyed in Turkey. In this study, fire-access zones were determined in the Mediterranean forests of Turkey, by utilizing geographic information systems （GIS） technology. The effective reach distance of fire hoses from both sides of roads was considered in order to delineate fireaccess zones. The effective reach distance can vary based on the technical capabilities and hydraulic capacity of fire trucks （minimum and maximum pressures on water pump）; terrain structures （uphill, downhill and flat）; and ground slope. These factors and their influences were studied in fire sensitive forest areas located in the eastern Mediterranean city of Kahramanmaras in Turkey. First, terrain structures on both sides of the road network and groundslope classes were determined based on GIS data layers. Then, fire access zones were delineated according to water pressure data, terrain structures, and ground-slope classes. The results indicated that 69.30 % of the forested areas were within the fire-access zones, while the rest of the forest was out of reach the fire hoses. The accessible areas were also calculated for forested areas with different fire- sensitivity degrees. The accessible areas were 69.59, 69.96, and 67.16 % for the forested areas that are sensitive to fires at the first, second, and third degrees, respectively. This finding has implications for the monitoring and management of fire threats in areas outside of the reach distance. The outside areas should receive extra attention and monitoring during the fire season so that fires are detected ahead of time and management has sufficient time to react. Uncommon Tissue Defects of Upper Extremities: Bacillus Antracis

Introduction: Anthrax is a zoonotic disease caused by Bacillus anthracis spores. Cutaneous form is the most common and the least fatal form of the disease, however the disease is uncommon in Europe and U.S. and the reports in the literature are mostly case reports for upper extremity. Case Series: 7 patients, who were treated between 1999 and 2015 in our clinic with the diagnosis of cutaneous anthrax in their upper extremity, were included to our study. 3 patients applied directly to plastic surgery, others were consulted from the infectious disease clinics. Reconstructions were done with skin grafts or secondarily. Discussion: Anthrax causes cutaneous, gastrointestinal and inhalation forms of the disease. Despite being very rare in Europe and US, in Turkey there are many reported anthrax cases. However, cutaneous anthrax of upper extremity is reported rarely in the literature. Painless papule and vesicles are well defined as clinical findings. In anthrax, the excision of skin lesions or biopsies is contraindicated because of the risk of systemic spread. Conclusion: Despite cutaneous anthrax of the hand and upper extremity is very rare, the diagnosis is easy because of the typical presentation and history. Once the definitive diagnosis is set, the treatment is effortless with the antibiotherapy and first steps of the reconstruction ladder.

Iloprost inhibits fracture repair in rats

Background Previous studies have shown that prostaglandins （PGs） dramatically stimulate healing processes in ***,the effect of prostaglandin l2 （PGI2） on fracture healing remains *** investigate the effect of PGI2,a study on fracture healing process in closed tibia fractures was *** Thirty-six Sprague-Dawley male rats were randomized into two *** the first day,their right tibias were fractured by three-point bending *** study group （n=18） received a single injection of 10 μg/kg iloprost for 5 days,while the control group （n=18） received saline solution in the same *** the 7th,14th and 28th days following the fracture,six rats were sacrificed and their right legs were harvested in each *** progression of fracture healing was assessed for each specimen by the scores of radiography （by Lane-Sandhu） and histology （by Huo et al）.Results On the 7th day,the radiographic and histologic scores were *** the 14th day radiographic total score was 6 and histologic total score was 23 in the iloprost group,whereas radiographic total score was 11 and histologic total score was 33 in the control *** the 14th day radiographic and histologic scores were significantly decreased in the iloprost group compared to the control group （P 〈0.05）.On the 28th day radiographic total score was 12 and histologic total score was 37 in the iloprost group,whereas radiographic total score was 15 and histologic total score was 40 in the control *** the 28th day although there was a decrease in radiographic and histologic scores of the iloprost group acording to control group,it was not statistically significant （P 〉0.05）.Conclusion Iloprost delays fracture healing in early stage in rats.

Does the intrathecal propofol have a neuroprotective effect on spinal cord ischemia?

The neuroprotective effects of propofol have been confirmed. However, it remains unclear whether intrathecal administration of propofol exhibits neuroprotective effects on spinal cord ischemia. At 1 hour prior to spinal cord ischemia, propofol（100 and 300 μg） was intrathecally administered in rats with spinal cord ischemia. Propofol pre-treatment greatly improved rat pathological changes and neurological function deficits at 24 hours after spinal cord ischemia. These results suggest that intrathecal administration of propofol exhibits neuroprotective effects on spinal cord structural and functional damage caused by ischemia.

A Double-Sided View to Adult Vaccination:The Opinions and Attitudes of Patients and Health Workers

Objective: Although there are adult vaccination schemes in our country, there are serious deficiencies in the way that doctors direct the patients to this vaccination, but also patients have to make and demand these vaccinations. The aim of our study is to identify the shortcomings in this area and draw a roadmap for what arrangements should be made in terms of physicians and patients in order to increase adult immunization rates in primary care. Method: We conducted a two-phase, multicentered, descriptive clinical trial between October and December 2017. The first phase of the trial was carried out with patients from 3 Family Health Centers in Antalya, Istanbul and Osmaniye. Patients to be interviewed were selected voluntarily among Family Health Center’s applicants. The second phase of the trial was carried out with health workers, who were participated to trial from 26 different provinces of Turkey. In the process, a questionnaire of 19 questions was applied to primary health care workers by the internet. Results: 490 patients were included in the study. There was a significant difference between gender, age, education level and guideline follow-up and vaccination status of patients [p Conclusion: As a result of the studies including our study about adult vaccination, only 10% - 20% of the targeted groups in adults can be vaccinated. However, just like in childhood during adulthood, vaccinations protect individuals from diseases and provide economic benefits. Firstly, the knowledge level of physicians on adult immunization should be updated with in-service trainings, they should first be convinced to vaccinate because they are in risk group and then they should be recommended vaccination to patient to increase the vaccination rates.
