Anterior sacral meningocele is characterized by the herniation of the thecal sac into the retroperitoneal space. A 33-year-old man was referred to our hospital with the complaints of a longlasting constipation and nonspecific lower abdominal pain. At ultrasound, a cystic mass was detected at the presacral space. Computed Tomography revealed a defect at the anterior aspect of the sacrum, together with a presacral cystic mass which showed a connection with the dural sac. At Magnetic Resonance Imaging, it was demonstrated that the cystic mass made an impression on the rectum, and did not possess any solid components. An anteroposterior pelvis radiogram demonstrated a sacral deformity and the scimitar sign. Upon these imaging findings, the patient got the diagnosis of an anterior sacral meningocele. The possibility of an anterior sacral meningocele must always be kept in mind in the differential diagnosis of a pelvic cystic mass.

Evaluation of the Early Age Married Girls Applying to Our Department

Underage marriages are going on to keep their commonness in countries such as Turkey although frequencies of them are decreasing in the world. According to the law, marriages before eighteen years old are also defined as early marriages. Being married of girls at early ages makes these children deprived from education life and their vicinity. Therefore severe psychological difficulties occur in these children. In this study, 19 girls, who were being married at early ages and psychologically examined between January 1st, 2013-April 1st, 2015 in physical and mental health committee of the Dicle University, Medical Faculty, were taken to evaluate. These 19 cases were retrospectively assessed in sociodemographic data (age, education level, and relative degrees), type of marriage, state of mind, forensic reports and files. Of 410 cases, who admitted to our mental and physical committee, 19 (4.6%) were at the category of early age marriage. These 19 cases range between 13 and 17 years of age. Three of them (15.8%) were thinking of suicide and one of them (5.3%) attempted suicide. At least, one mental disorder was diagnosed in 8 (42.1%) of the cases and the most common diagnose was acute stress disorder in four (21.1%) cases. It has been stated that marriages at early ages are caused by economic and cultural reasons. As conclusion, being married of children at early ages is an important risk factor in social and mental health.

Vaccinations Rates in the Elderly with Diabetes Mellitus

Infections are a major cause of morbidity and mortality in the elderly. Improvements in health care prevention including vaccinations and treatment have led to an increase in the birth life expectancy. The vaccines that are now recommended for people over 60 are influenza, herpes zoster and a vaccine combining tetanus toxoid, reduced diphtheria toxoid, and pneumococcal vaccine. Our aim was to estimate the prevelance of vaccination among diabetic patients attending in Tepecik Training Hospital and in Ege University Faculty of Medicine. Patients and Methods: Individuals at or over the age of 60 who were attending to Tepecik Training Hospital and at Ege University Faculty of Medicine. Department of Internal Medicine was targeted from February to May 2014. Our data are collected from the patients face to face by questionnaire. Results: A total of 274 elderly patients with a mean ± SD (range) age of 72 ± 6 (62 - 93) years were questioned. The vaccination ratios were determined as 34% (93) for influenza, 9.5% (26) for pneumococcal, 10.6% (27) for tetanus vaccine. The patients were divided in two groups as diabetics and non-diabetics. The influenza vaccination rate is 38.1% in diabetic patients, rate of 31.8% in non-diabetic patients, the pneumococcal vaccine rate is 13.4% in diabetic patients, rate of 7.39% in non-diabetic patients. The tetanus vaccination rate is 9.28% in diabetic patients, rate of 11.36% in non-diabetic patients. No statistically significant difference between two groups was determined.

BACKGROUND: In recent years, thrombolytic therapy has become the main treatment of ischemic stroke. But the increasing use of alteplase in ischemic stroke has made some complications more evident. Angioedema is a rare but potentially life-threatening complication of alteplase treatment. Only a few studies have examined the incidence of angioedema after treatment with alteplase for ***: A 75-year-old man complaining of right hemiparesis was admitted to our emergency department. He was diagnosed as having acute ischemic stroke, and alteplase infusion was given two hours after the onset of stroke symptoms. Immediately after the completion of infusion he was noted to have a large swollen ***: His neurological symptoms resolved gradually within 4 hours, whereas his upper extremity strength improved to 4/5 and lower extremity 5/5. Lingual edema resolved within 16 hours without any complication. He died from presumed nosocomial infection 5 days ***: Lingual angioedema may appear as a possible complication in patients who were treated with alteplase. The management of these patients should be very careful.

Prediction of Pretest Probability Scoring Systems in Pulmonary Embolism: Wells, Kline and Geneva

Pulmonary embolism (PE) is a clinical entity with high mortality rate and therefore rapid diagnosis is necessary. For this purpose many diagnostic strategies have been developed for avoiding or necessitating further investigations. The hallmark of these strategies is assessing the pretest clinic probability of PE. In this study, the effectiveness of Wells, Geneva and Kline methods were investigated in a university hospital emergency department. 74 patients were enrolled in this study. The inter-group differences in scoring systems were significant for Wells and Kline but non-significant for Geneva method. The diagnosis of PE was correlated with Wells and Kline system but Geneva system lacked it. The Receiver Operating Characteristic analyses was performed for comparing the pretest clinical probability scoring systems and the greatest area under the curve was found in Wells system. Wells method seems more useful compared to Geneva and Kline methods particularly in emergency department.

Aims: In this study, Doppler parameters were studied in patients with advanced stage renal disease, and the relationship between Doppler parameters and renal disease stage was investigated. Doppler values were evaluated for a cut-off value between normal and end-stage kidney disease group. Materials and methods: 50 patients with chronic kidney disease and 15 patients belonging to the same age group and with normal serum creatinine levels were comprised of the study and control groups. Resistivity and pulsatality indices were measured at the main renal arteries and interlobular arteries at both sides. Results: Statistically significant differences were found between the PI and RI values obtained from the main renal and interlobular arteries. PI sums were compared with those from the control group, and a cut-off value of 2.15 was found, with a sensitivity of 90% and a specificity of 86.7%. Conclusion: Any increase in the RI and PI values obtained from the main renal and interlobular arteries must bring to mind the possibility of advancing renal damage and interstitial fibrosis. Patients with and without renal parenchymal damage can be differentiated by means of comparing the total PI values obtained from the right and left main renal and interlobular arteries.

Purpose: The purpose was to evaluate the diurnal variation (DV) of intraocular pressure (IOP) in patients with exfoliation syndrome (XS), to measure retinal nerve fiber layer (RNFL)-thickness by using scanning laser polarimetry, and to compare these measurements with those of normal subjects. Methods: Forty-five subjects with XS and 40 healthy, age/sex matched subjects were recruited into the study. A detailed ophthalmologic examination was performed. IOP measurements were obtained at 08:00 am, 12:00 pm, 03:00 pm, and 06:00 pm. The XS group was further divided into DV ≥ 5 mmHg and DV<5 mmHg groups and also according to the existence of IOP fluctuation. The IOP measurements and RNFL thickness measurements were compared between the groups. Results: The mean IOP value was found to be highest in the morning both in the XS and control groups. IOP showed a gradual decrease from 8.00 am to 6.00 pm in the control group, whereas a second peak at 03:00 pm was observed in the XS group. There was a fluctuation in 53.3% of the XS group, while none of the healthy subjects showed fluctuation. Superior and inferior ratios were statistically lower in XS patients than those in control subjects (p<0.05). Moreover, in patients with XS showing a DV ≥ 5 mmHg and/or a fluctuation, the superior ratio, inferior ratio, the number, superior average and superior integral were significantly different (all p values<0.05) from those of control subjects. Conclusions: As the XS patients with high diurnal IOP variation and fluctuating pattern of IOP had lower RNFL thickness measurements, it is crucial to follow up these patients by performing scanning laser polarimetry in order to discover any possible glaucomatous damage at an earlier stage than with the use of conventional visual field analysis.
