A Surgical Perspective on Ischemic Mitral Regurgitation: Tethering or Prolapse? Going Back to Papillary Muscles Anatomy. What the Surgeons Really Need to Know

More than one third of patients with ischemic mitral regurgitation (IMR) present a valve prolapse whose mechanism is subtended by a papillary injury. The recent literature is pointing at a regional ventricular injury or wall motion abnormality rather than a global LV dysfunction as responsible for IMR and the presence of localized valve prolapse related to papillary dysfunction is additionally supporting this idea. Leaflet tethering or prolapse in these patients is subtended by lesion of the papillary muscle (PM) per se rather than its dysfunction secondary to regional and global ventricle enlargement. Identification of this type of lesion is difficult and can be overlooked. Morphological characteristics and anatomical variability of the papillary muscles determine their different susceptibility ischemic damage and dysfunction. Pioneering work in mitral anatomy shows a range of morphological diversity of PM anatomy and leads to an anatomical classification with important implications in IMR surgery. New methods of investigation, as multidetector computed tomography or magnetic resonance provide a very accurate and proper identification of the morphological pattern of the subvalvular apparatus, which is crucial for a long-lasting and successful surgical correction. The involvement of PM in the pathophysiology of IMR not only in terms of their functional anomaly, but also of their effective anatomical aspects and characteristics is increasingly emerging. The modern advancements of imaging techniques can guide the preoperative surgical planning and the surgeon needs to be aware of morphological features of the subvalvular apparatus and combine these findings with echographic functional parameters before embarking in complex mitral repairs.

AIM: To investigate the frequency of occult hepatitis B, the clinical course of hepatitis B virus (HBV) reactivation and reverse seroconversion and associated risk factors in autologous hematopoietic stem cell transplantation (HSCT) recipients. METHODS: This study was conducted in 90 patients undergoing autologous HSCT. Occult HBV infection was investigated by HBV-DNA analysis prior to transplantation, while HBV serology and liver function tests were screened prior to and serially after transplantation. HBV-related events including reverse seroconversion and reactivation were recorded in all patients. RESULTS: None of the patients had occult HBV prior to transplantation. Six (6.7%) patients were positivefor HBV surface antigen (HBsAg) prior to transplantation and received lamivudine prophylaxis; they did not develop HBV reactivation after transplantation. Clinical HBV infection emerged in three patients after transplantation who had negative HBV-DNA prior to HSCT. Two of these three patients had HBV reactivation while one patient developed acute hepatitis B. Three patients had anti-HBc as the sole hepatitis B-related antibody prior to transplantation, two of whom developed hepatitis B reactivation while none of the patients with antibody to HBV surface antigen (anti-HBs) did so. The 14 anti-HBs-and/or anti-HBc-positive patients among the 90 HSCT recipients experienced either persistent (8 patients) or transient (6 patients) disappearance of anti-HBs and/or anti-HBc. HBsAg seroconversion and clinical hepatitis did not develop in these patients. Female gender and multiple myeloma emerged as risk factors for loss of antibody in regression analysis (P < 0.05). CONCLUSION: Anti-HBc as the sole HBV marker seems to be a risk factor for reactivation after autologous HSCT. Lamivudine prophylaxis in HbsAg-positive patients continues to be effective.

A New Probe: AFM Measurements for Random Disorder Systems

We study the quenched random disorder(QRD) effects created by aerosil dispersion in the octylcyanobiphenyl(8CB) liquid crystal(LC) using atomic force microscopy technique. Gelation process in the 8CB+aerosil gels yields a QRD network which also changes the surface topography. By increasing the aerosil concentration, the original smooth pattern of LC sample surfaces is suppressed by the emergence of a fractal aerosil surface effect and these surfaces become more porous, rougher and they have more and larger crevices. The dispersed aerosil also serves as pinning centers for the liquid crystal molecules. It is observed that via the diffusion-limitedaggregation process, aerosil nano-particles yield a fractal-like surface pattern for the less disordered samples. As the aerosil dispersion increases, the surface can be described by more aggregated regions, which also introduces more roughness. Using this fact, we show that there is a net correlation between the short-range ordered x-ray peak widths(the results of previous x-ray diffraction experiments) and the calculated surface roughness. In other words, we show that these QRD gels can also be characterized by their surface roughness values.

Nomograms as Predictive Tools for Prostate Cancer Patients Who Had Radical Prostatectomy

Prostate cancer is the most common solid cancer for men in the developed countries. Radical prostatectomy is the most preferred treatment modality for localized prostate cancer. Individual decision making is necessary for each patient because of the diversities in the biological characteristics of the prostate cancer. The prediction of pathologic stage, prognosis and cancer specific mortality after curative therapy and quality of life issues are essential for counseling and tailoring treatment in possible candidates of radical prostatectomy. Several studies demonstrated that nomograms are the best predictive tools regarding the other prediction models. For better understanding the nomograms in radical prostatectomy patients, they should be classified according to categories for their use. PSA, Gleason grade and clinical stage are seemed to be the most important prognostic factors in patients who are candidates for radical prostatectomy. Additionally, the pathological parameters are remarkable prognostic criteria. The Partin tables for predicting the radical prostatectomy pathology and Kattan nomograms for predicting the biochemical recurrences free survival rates are the most frequently used nomograms. Today, these nomograms should not replace the clinical decisions but they give significant information for the patients’ prognosis, treatment selection and follow up.

Long-Term Structural Valve Degeneration in Cryopreserved Mitral and Aortic Homograft: Is Pregnancy a Factor?

Objective: This study reports the outcomes of the cryopreserved mitral homograft in 119 patients prospectively followed with clinical, echocardiographic and structural valve deterioration assessments. Methods: 119 patients undergoing mitral and aortic homograft implantation. Patient’s causes of mitral disease were: rheumatic disease (n = 75), endocarditis (n = 37) and others (n = 7). There were 40 partial homografts and 88 total homografts. Mitro-aortic homograft valve replacement was performed in 29 cases. Results: Mean follow-up was 9.8 ± 6.3 years (up to 19.2 yrs). Ureterolithotripsy under Spinal Anesthesia in Pompe Disease (Glycogen Storage Disease Type 2): A Case Report

Pompe disease is an autosomal recessive inherited glycogen storage disease, usually respiratory muscles, cardiac muscle and skeletal muscles are affected in childhood and mortality is inevitable but has a slower progression in the adult. The patient was a 35-year-old male patient who was scheduled to undergo ureterorenoscopy due to right ureteral stone. He was diagnosed with pompe disease after detection of myopathy on EMG due to muscle weakness in the legs and hips. He was started on myozyme vials once every 15 days. Spinal anesthesia with 3 ml 0.5% hyperbaric bupivacaine was performed and adequate sensorial and motor block was achieves at T10 level. The surgery was completed with no complications and he was discharged two days later. The selection of appropriate anesthetic technique and a careful perioperative and postoperative monitoring are of utmost importance due to progressive muscle weakness, hypotonia, respiratory failure and cardiomyopathy in pompe disease.

Effect of Surface Treatment on Elemental Composition of Recast NiCr Alloy

Objective: The aim of this study was to compare the effects of different surface treatments on the elemental composition stability of as-received and recast types of NiCr casting alloys. Material and Methods: Six commercial NiCr dental casting alloys (Kera N, Nodelco, Bellabond, Wiron 99, Metaplus VK, and Tritech D) were used in this study. Seventy-two specimens (12 × 12 × 1.2 mm) were produced using the lost-wax casting process. Three casting protocols were established in relation to the proportion of as-received and recast alloys: Group A (100% as-received metal), Group B (50 wt% new metal, 50 wt% once-recast metal), and Group C (100% once-recast metal). Two specimens in every group received two different surface treatments: sandblasting with 110 μmm Al2O3 or oxidation firing. An elemental analysis was performed with x-ray energy-dispersive spectroscopy. Results were subjected to one-way analysis of variance and Tukey HSD tests. Results: The elemental composition was significantly different in the casting groups and treatment groups (p < 0.05). The lowest mean weight percentage Ni value was recorded for Group C and the highest for Group A. Aluminum-oxide sandblasting of the alloy surface reduced the mean weight percentage for Cr. Conclusion: Recasting metal alloys may adversely affect surface quality.
