Acute effect of smoking on gallbladder emptying and refilling in chronic smokers and nonsmokers: A sonographic study

AIM: To ultrasonographicaly evaluate the acute effects of smoking on gallbladder contraction and refilling in chronic smokers and ***: Fifteen chronic smokers (21-30 years old) and fifteen nonsmokers (21-35 years old) participated in this study. Chronic smokers were selected among the volunteers who had been smoking for at least 5 years and 10 cigarettes per day (mean 17.5/d). Examinations were performed in two separate days. In the fi rst day, basal gallbladder (GB) volumes of volunteers were measured after 8-h fasting. After the examinations, participants had a meal containing at least 30-40 gram fat. Gallbladder volume was assessed at 5, 15, 30, 60, 120 and 180 min after the meal. In the second day, participants smoked 2 cigarettes after 8-h fasting. Then, they had the same meal, and gallbladder measurements were repeated at the same time points. Same procedures were applied to both groups. RESULTS: The mean starving GB volumes were 23.3 ± 3.3 mL in the fi rst day, 21.9 ± 3.0 mL in the second day in nonsmoker group and 18.3 ± 3.0 mL in the fi rst day, 19.5 ± 2.8 mL in second day in smoker group. There was no significant difference between starving GB volumes. We did not fi nd any signifi cant difference between the GB volumes measured at 5, 15, 30, 60, 120 and 180 min in the fi rst and second days in nonsmoker group. In smokers, post cigarette GB volume was found significantly higher at 5, 15 and 30 min which corresponded to GB contraction phase (P < 0.05). Control GB volume measurements were not signifi cantly different between the two groups. Post-smoking GB volumes were also not signifi cantly different between the two ***: Smoking prolongs the maximal GB emptying time both in smokers and in nonsmokers though it is not signifi cant. BACKGROUND Idiopathic omental infarction(IOI)is challenging to diagnose due to its low incidence and vague *** differential diagnosis also poses difficulties because it can mimic many intra-abdominal organ *** hypercoagulability and thrombosis are among the causes of omental infarction,venous thromboembolism scanning is rarely performed as an etiological *** SUMMARY The medical records of the 5 cases,who had the diagnosis of IOI by computed tomography,were *** majority of the patients were male(n=4,80%)and the mean age was 31 years(range:21-38).The patients had no previous abdominal surgery or a history of any chronic *** main complaint of all patients was persistent abdominal *** infarction was detected in all patients with contrast-enhanced computed *** treatment was initially preferred in all patients,but it failed in 1 patient(20%).After discharge,all patients were referred to the hematology department for thrombophilia *** 1 patient applied for thrombophilia screening and was homozygous for methylenetetrahydrofolate reductase(A1298C mutation)and heterozygous for a factor V Leiden *** IOI should be considered in the differential diagnosis in patients presenting with progressive and/or persistent right side abdominal *** risk factors such as hypercoagulability in patients with IOI is also important in preventing future conditions related to venous thromboembolism.

Additive manufacturing(AM)has revolutionized the design and manufacturing of patient-specific,three-dimensional(3D),complex porous structures known as scaffolds for tissue engineering *** use of advanced image acquisition techniques,image processing,and computer-aided design methods has enabled the precise design and additive manufacturing of anatomically correct and patient-specific implants and ***,these sophisticated techniques can be timeconsuming,labor-intensive,and ***,the necessary imaging and manufacturing equipment may not be readily available when urgent treatment is needed for trauma *** this study,a novel design and AM methods are proposed for the development of modular and customizable scaffold blocks that can be adapted to fit the bone defect area of a *** modular scaffold blocks can be combined to quickly form any patient-specific scaffold directly from two-dimensional(2D)medical images when the surgeon lacks access to a 3D printer or cannot wait for lengthy 3D imaging,modeling,and 3D printing during *** proposed method begins with developing a bone surface-modeling algorithm that reconstructs a model of the patient’s bone from 2D medical image measurements without the need for expensive 3D medical imaging or *** algorithm can generate both patient-specific and average bone ***,a biomimetic continuous path planning method is developed for the additive manufacturing of scaffolds,allowing porous scaffold blocks with the desired biomechanical properties to be manufactured directly from 2D data or *** algorithms are implemented,and the designed scaffold blocks are 3D printed using an extrusion-based AM *** and instructions are also provided to assist surgeons in assembling scaffold blocks for the self-repair of patient-specific large bone defects.

Role of pre-transplant 18F-FDG PET/CT in predicting hepatocellular carcinoma recurrence after liver transplantation

The last two decades have seen a paradigm shift in the selection of patients with hepatocellular carcinoma(HCC)for liver *** invasion and differentiation have been the most significant factors affecting post-transplant recurrence;however,because of inherent disadvantages of pre-transplant biopsy,histological criteria never gained ***,the selection criteria evolved from morphological to biological criteria,such as biomarkers and response to loco-regional *** the introduction of multimodality imaging,combination of computed tomography with nuclear medicine imaging,particularly,18F-fluorodeoxyglucose positron emission tomography fulfilled an unmet need and rapidly became a critical component of HCC *** review article will focus on the use of 18F-fluorodeoxyglucose positron emission tomography combined with computed tomography in the pre-transplant evaluation of HCC patients with special discussion on its ability to predict HCC recurrence after liver transplantation.

Intraocular pressure-lowering effects of commonly used fixed combination drugs with timolol in the management of primary open angle glaucoma

AIM:To evaluate intraocular pressure(IOP)-lowering effect and ocular tolerability of brimonidine/timolol,dorzolamide/timolol and latanoprost/timolol fixed combination therapies in the management of primary open angle glaucoma.· METHODS:Each drug was administered for two months, after which a circadian tonometric curve was recorded using a Goldmann applanation *** discomfort(conjunctival hyperemia, burning or stinging, foreign body sensation, itching, ocular pain) of each eye was assessed by the subject on a standardized ocular discomfort ***:Among the three study groups, there were no significant differences in the mean baseline IOP measurements, mean 2ndmo IOP measurements, and mean(%) change of IOPs from baseline. Among the three study groups, there were no significant differences in the mean IOP measurements obtained at circadian tonometric curves at baseline and at two months controls. In sum brimonidine/timolol, dorzolamide/timolol and latanoprost/timolol fixed combination therapies showed similar effects on IOP levels.· CONCLUSION:Brimonidine/timolol, dorzolamide/timolol and latanoprost/timolol fixed combination therapies showed similar lowering efficaties on IOP levels whereas there was no any difference between each other.

Ataxia telangiectasia in Turkey:multisystem involvement of 91 patients

Background:Ataxia telangiectasia (AT) is a genetically based multisystemic *** aimed to make a comprehensive evaluation of multisystem involvement in AT by describing clinical features and outcome of 91 ***:Medical records of the patients who were diagnosed and followed by a multidisciplinary approach during a 27-year period (1988-2015) were reviewed ***:Forty six female and 45 male patients with a mean follow-up period of 39.13±4.28 months were *** mean age at the time of symptom onset and diagnosis were 15.4±1.09 months and 73.6144.11 months,*** abnormalities were progressive truncal ataxia,nystagmus,dysarthria,oculomotor apraxia and *** one patients (34.1%) became dependent on wheelchair at a mean age of 12.1±2.8 *** patients (12.1%) became bedridden by a mean age of 14.7±1.8 *** magnetic resonance imaging revealed pathological findings in 47/66 *** immunological parameters were determined in 51/91 patients:immunoglobulin (Ig)A deficiency (n=38),lymphopenia (n=30),IgG (n=15) and IgG2 (n=11) *** of recurrent sinopulmonary infections (n=45) and bronchiectasis (n=22) were found to be more common in patients with impaired immunological parameters (P=0.029and P=0.023,respectively).Malignancy developed in 5 patients,being mostly lymphoreticular in origin and resulted in death of 4 ***:AT is a long lasting disease with muitisystem involvement necessitating multidisciplinary follow up,as described in our *** diagnosis of malignancy and supportive treatments regarding pulmonary and neurological health may prolong survival and increase the quality of life.

Epiretinal membrane related vascular changes in diabetic eyes evaluated with optical coherence tomography angiography

AIM:To evaluate the retinochoroidal microvascular circulation and anatomical structure of diabetic and nondiabetic patients with epiretinal membrane(ERM)with the help of optical coherence tomography angiography(OCT-A)and compare them with healthy control ***:In this prospective,cross-sectional study,a total of 165 eyes were evaluated,including 50 eyes of patients with diabetic ERM,54 eyes of idiopathic ERM(iERM)patients,and 61 eyes of healthy *** and disc angiography was performed by *** vessel density(VD)ratio was evaluated by dividing the VD of the foveal region by the VD of the parafoveal *** calculations were evaluated at the 95%confidence ***:Macula superficial VD values of ERM cases were lower than that in the control group,while foveal VD was higher in ERM *** deep VD values of ERM cases were lower in all quadrants,except the *** width of the foveal avascular zone(FAZ)area was significantly lower in the ERM groups,and the FAZ width was lowest in iERM *** superficial VD ratio was significantly higher in the ERM groups,but there was no significant difference between ERM *** deep VD ratio was significantly higher in the iERM group than in the control ***:Diabetic and idiopathic ERMs differ in their mechanism of formation and clinical presentation,as well as their effect on retinal vascular *** the relationship of increase of retinal thickness with vascular integrity can be demonstrated with OCT-A,then,OCT-A can be used as a guide for ERM prognosis.

Visual acuity and endothelial cell density with respect to the graft thickness in Descemet's stripping automated endothelial keratoplasty: one year results

AIM: To evaluate the visual acuity and endothelial cell density according to the thickness in Descemet’s stripping automated endothelial keratoplasty(DSAEK)one year after ***: DSAEK patients’ data were reviewed. Thirty seven eyes of 37 patients who underwent DSAEK for pseudophakic bullous keratopathy(PBK) were included in this study. Graft thickness was measured with optical coherence tomography(OCT) 12 mo after DSAEK. Eyes were divided into 3 groups based on the graft thickness:thick(>200 μm), medium-thick(150-200 μm) and thin(<150 μm). Best corrected visual acuity(BCVA),endothelial cells density(ECD) and complications were assessed and comparisons were done between ***: Median thickness of postoperative grafts was 188(range 73-317 μm). There was no significant difference in age, sex, preoperative BCVA, or follow-up period between DSAEK groups. At postoperative 12 mo,mean BCVA was 0.28±0.10 in thick graft group, 0.52±0.08 in medium-thick graft group, and 0.72 ±0.06 in thin graft group. Thin grafts showed better postoperative BCVA as compared with the medium-thick and thick grafts(P =0.001). Thick graft group had 1637.44 ±88.19-mm2,medium thick graft had 1764.50±34.28-mm2 and thin graft group had 1845.30 ±65.62-mm2. Thin graft group had better ECD at 12 mo after surgery(P =0.001).CONCLUSION: Thin grafts after DSAEK ensure better visual rehabilitation. Eyes with thin grafts had significantly lesser loss of ECD compared to eyes withmedium-thick and thick grafts one year after surgery.

Changes in endothelial cell density following penetrating keratoplasty and deep anterior lamellar keratoplasty

· AIM: To compare the corneal endothelial cell density (ECD) of clear grafts after penetrating keratoplasty (PK) and deep anterior lamellar keratoplasty (DALK). · METHODS: The study included 44 and 54 patients treated with PK and DALK, respectively, between March 2006 and April 2010. Corneal ECD was examined using specular microscopy at postoperative 1, 3, 6, 12, and 18 months, and the values were compared. · RESULTS: Corneal ECD reduction in the PK group was 7.4%, 15.2%, 23.5%, and 28.9% at 3, 6, 12 and 18 months respectively after surgery, compared with 4.2 % in the first month (P 0.05). · CONCLUSION: Compared with DALK,PK significantly reduced ECD of the clear grafts. These results suggest that survival of endothelial cells in grafts is better after DALK than after PK. ·
