An Evaluation of Prevention of the Mother-to-Child Transmission Program in Murewa District, 2021

Background: Prevention of mother-to-child transmission (PMTCT) program includes a cascade of services given to prevent mother-to-child transmission (MTCT) of HIV in-utero, during delivery and during breastfeeding. Zimbabwe has made tremendous progress in increasing coverage of PMTCT services and reducing MTCT rate. COVID-19 has negatively impacted HIV programs. Murewa was among the districts with the most COVID-19 cases. We evaluated the PMTCT program in Murewa District in the face of COVID-19 challenges. Methods: We assessed inputs, processes, outputs and outcomes of the PMTCT program using the logic model approach. We collected data using interviewer-administered questionnaire, records review, and a checklist to assess availability of inputs, processes carried out and outputs realized from the program. We randomly selected health workers and program recipients. We used Epi info 7 to compute frequencies, means and proportions. Results: Murewa District had 58 health workers in post against an establishment of 92. Seven out of 16 facilities reported having HIV test-kit stockout. Only 4/16 were conducting postnatal clinics. No PMTCT training were conducted. Only 1872/5693 (33%) of HIV consumables including HIV test kits and CD4 count reagents were procured. The reasons reported for low PMTCT coverage in Murewa were fear of contracting COVID-19 at health facilities 33/43 (77%), lack of bus fare 28/43 (65%) and unavailability of medical consumables 26/43 (60%). Conclusion: The PMTCT program in Murewa District failed to meet targets for inputs, processes, and outputs worsened by the impact of the COVID-19 pandemic. The reasons for low PMTCT coverage in Murewa District were reported to be due to fear of contracting COVID-19, unavailability of medical consumables and COVID-19 travelling restrictions. Cervical Conglutination: A Case Report

Conglutination is one of the rarest disorders encountered during labor. Reported by few authors over the centuries, its diagnosis and management are fraught with pitfalls in terms of the experience of the health-care team and the likely complications of vaginal delivery. We report on a case received at the university clinics in Kinshasa, Democratic Republic of the Congo, where we successfully delivered vaginally. Discussions are not clear-cut as to the preferred route of delivery in cases of external cervical conglutination, but in our context where society is pronatalist and women are unwilling to undergo caesarean section, every intervention [to be] performed should consider the impact of such intervention on the woman’s obstetrical future and the vaginal route should be preferred whenever reasonably possible.

Study on Food Import in D. R. Congo

Democratic Republic of Congo is a developing country located in Central Africa with a population of over 75 million. Generally, it relies on import in international market for food as it has constantly been experiencing domestic crisis which hinders production. Given the importance of shores price policy and income policy, the government pays meticulous attention to these factors to meet the present and future food security. In order to understand the import behavior related to GDP and international trade prices, the study employs Almost Idea Demand System (AIDS) model using FAO data. The result explains that price effect is more useful to the food security of the country than the income effect, thus this study suggest the government should give priority to policy on price.

Determinants and Knowledge on Geo-Helminthiasis in Prison Inmates: Case of Kassapa Central Prison, Lubumbashi, Democratic Republic of Congo

Introduction: Geohelminths are infections due to various species of parasitic worms transmitted by eggs present in human excreta which contaminate the soil where sanitation is poor. The objective of this study was to analyse the determinants and knowledge on geo-helminthiasis among inmates and care providers in the Kassapa prison environment in Lubumbashi in the Demographic Republic of Congo. Methods: This is a cross-sectional descriptive study which was conducted from September 21 to October 15, 2020 in Kassapa prison in Demographic Republic of Congo. Estimates of the prevalence and general characteristics of geo-helminthiasis in prisoners were calculated by descriptive analyses. The analysis of determinants was performed based on a binary logistic regression. Results: With regard to the quantitative section, the study used a sample size of 1083 while a sample size of six was considered for the qualitative section. The annual prevalence of geo-helminthiasis was 10.2%, 4.8% and 2.9% respectively for 2017, 2018 and 2019. It was found that the general prevalence of geo-helminthiases was 17.9%. The age group above 39 years constitutes the modal class with 29.4%, the least affected age group being that of minors with 6.3%. The association is statistically significant between sex, treatment, prison residence and year. Substandard/Falsification Antibacterial Agents: A Systematic Review of Liquid Chromatographic and Spectrophotometric Methods for Their Detection

Falsified antibacterial agents continue to pose serious public health problems around the world. They are notably responsible for resistance emergence in the treatment of infectious diseases that can lead to death. The aim of this study was to summarize the literature on the methods developed and validated to detect poor quality antibacterial agents in order to assess the impact of these available methods in the fighting against Counterfeit/substandard medicines. Thus, studies published in the time from January 2000 to July 2017, were accessible via Google Scholar and Pubmed and allowed to analyze 41 papers. The majority (30) of developed and validated methods concerned LC methods, 9 regarded UV/V is spectrophotometry and two studies simultaneously developed both methods. Antibacterial agents belonging to beta-lactams group were the most concerned by the methods developed and validated (39%), followed by quinolones (18%) and macrolides (12%). Regarding active ingredients, amoxicillin (12%) was the most concerned by the development and validation methods. In spite of several available methods to detect substandard drugs, antibacterial agents for which methods were developed and validated were the most concerned by worldwide detected non-conformities. The fight against this scourge should not be only limited by their detection, but it also requires an effective involvement of different actors notably: health authorities, international organizations, pharmaceutical industries etc.

A Cross-Sectional Study According to Risk Factors Associated with Erectile Dysfunction in Men

Background: Erectile dysfunction (ED) is a common condition in general population. It has a negative impact on the couple’s quality of life. In 1995, it was estimated that it affected 152 million people worldwide, and that it would affect more than 322 million people in 2025 with a large increase in developing countries. Several studies have suggested a causal link between ED and cardiovascular disease. In the Democratic Republic of Congo, no studies exist on this subject. Nonetheless, we are seeing increased promotion on the management of sexual disorders in our media, which could testify to the extent of this problem. Objective: To assess the extent of cardiovascular risk factors for ED in Kinshasa. Material and Methods: This is a cross-sectional study for analytical purposes. It collected several categories of individuals from May 1 to October 31, 2018. The severity of ED was assessed by the IIEF-5 score and the associated risk factors sought via logistic regression. Results: The mean age of the respondents was 59.9 ± 19.2 years, more than half of the respondents were over the age of 60 years old (60%). The ED proportion was 78.8%, and 75% of respondents had at least moderate ED. This anomaly was significantly elevated among respondents aged over 60 years (94.8%), diabetics (87.5%), hypertensive patients (92.3%) and stroke victims (86%). The independently associated risk factors were: age > 60 years [ORa: 9.87;95% CI: 6.42 - 10.48, p p = 0.013] and the stroke [ORa: 2.1;95% CI: 1.36 - 3.39;p = 0.012%]. The age threshold predicting ED was 67 years. Conclusion: ED is a frequent disorder in our environment and likely to affect the quality of life of men who suffer from it.
