The key point in studying or teaching the history of Chinese medicine is on the doctrines underlying it and on its perception of the body,physiology,pathology,and its treatment.Namely,there is often a tendency to focus on reading and analysing the classical canons and therapy-related texts including formularies and materia medica collections.However,focusing on these sources provides us with a one-sided presentation of Chinese medicine.These primary sources lack the clinical down-to-earth know-how that encompasses medical treatment,which are represented,for instance,in the clinical rounds of modern medical schools.Our traditional focus on the medical canons and formularies provides almost no clinical knowledge,leaving us with a one-sided narrative that ignores how medicine and healing are actually practiced in the field.This paper focuses on the latter aspect of medicine from a historical perspective.Using written and visual sources dating to the Song dynasty,clinical encounters between doctors and patients including their families are depicted based on case records recorded by a physician,members of the patient’s family,and bystanders.This array of case records or case stories will enable us to narrate the interaction between physicians and patients both from the clinical perspective and from the social interaction.This paper will also discuss visual depictions of the medical encounter to provide another perspective for narrating medicine during the Song dynasty.Medical case records and paintings depicting medical encounters are exemplary of the potential of Chinese primary sources for narrative medicine.

BACKGROUND:Patients intubated in the prehospital setting require quick and definitive confirmation of endotracheal (ET) tube placement upon arrival to the emergency department (ED).Direct and adjunct strategies exist,but each has limitations and there is no definitive gold standard.The utility of bronchoscopy in ED intubation has been studied,but scant literature exists on its use for ET tube confirmation.This study aims to assess effectiveness,ease and speed with which ET tube placement can be confirmed with disposable fiberoptic bronchoscopy.METHODS:Emergency medicine residents recruited from a 3-year urban residency program received 5 minutes of active learning on a simulation mannequin using a disposable,flexible Ambu aScope interfaced with a monitor.With residents blinded,the researcher randomly placed the ET tube in the trachea,esophagus or right mainstem.Residents identified ET tube position by threading the bronchoscope through the tube and viewing distal anatomy.Each resident underwent 4 trials.Accuracy,speed and perceptions of difficulty were measured.RESULTS:Residents accurately identified the location of the ET tube in 88 out of 92 trials (95.7%).The median time-to-guess was 7.0 seconds,IQR (5.0-10.0).Average perceived difficulty was 1.6 on a scale from 1-5 (1 being very easy and 5 being very difficult).No tubes were damaged or dislodged.CONCLUSION:While simulation cannot completely replicate the live experience,fiberoptic bronchoscopy appears to be a quick and accurate method for ET tube confirmation.Further studies directly comparing this novel approach to established practices on actual patients are warranted.

Two types of cyanobacteria of the genus Arthrospira（commonly known as Spirulina） were tested for biosorption of cerium（III） ions from aqueous solutions. An endemic type（ES） found in the northern Negev desert, Israel, and a commercial powder（CS） were used in this study. Biosorption was evaluated as a function of p H, contact time, initial metal concentration, number of sorption-desorption cycles, and salt concentration. The optimum p H range for biosorption was found to be 5.0–5.5. The kinetic characteristics of both Spirulina types were found to be highly compatible with a pseudo-second order kinetic model. The adsorption isotherms of both types were found to be well-suited to Langmuir and Freundlich adsorption isotherms. Maximum biosorption uptakes, according to the Langmuir model, were 18.1 and 38.2 mg/g, for ES and CS, respectively. Sodium chloride concentrations of up to 5 g/L had a minor effect on cerium biosorption. Desorption efficiency was found to be greater than 97% with 0.1 mol/L HNO_3 after three sorption-desorption cycles, without significant loss in the biosorption capacity. The results indicated the feasibility of cerium recovery from industrial wastes using Spirulina biomass.

本文主要研究也门银行部门变革型领导行为(TLB)对组织创新(OI)的影响,以及公共和私营银行部门对变革型领导模式的使用情况。所要研究的课题是变革型领导行为对也门银行组织创新的影响,研究对象是也门的7家银行(也门复兴开发银行、农业合作贷款银行、也门国家银行-Al-Kuraimi伊斯兰小额信贷银行、也门和科威特银行、也门国际银行、阿拉伯银行有限公司),本文采用对比分析法和问卷调研法进行前期的工作情况分析整理,通过描述变革型领导的基本概念和问卷调查分析结果来研究该领导行为对也门银行组织创新能力的影响。本文使用现有资料和对也门银行部门雇员的问卷资料进行结合阐明也门银行部门中组织变革型领导行为和组织创新关系。大多数关于变革型领导行为的研究都是在发达国家进行的,而在包括阿拉伯地区在内的发展中国家的研究却很少。所以研究变革型领导行为如何激发也门的组织创新是十分必要的,因为它强调了变革型领导行为对更高的组织创新的作用。它可以指导也门银行部门领导在工作中引入变革型领导行为,是所需创新水平必不可少的步骤。该研究结果表明,变革型领导行为在其衡量标准(理想化影响、智力激励、鼓舞人心的动机、个性化思考)方面对组织创新产生了巨大影响(采用创新和创新能力)在也门。此外,实证研究结果表明,也门银行业的所有变革型领导行为(理想化影响、智力激励、鼓舞人心的动机、个性化思考)都处于正常水平,此外,还发现采用水平采用创新和创新能力也达到了也门银行业的正常水平。

The new requirements from the 5th and the 6th generation of wireless communication are ultra-high data rate, energy efficiency, wide coverage and connectivity, high reliability, and low latency. The current technologies cannot achieve all the mentioned requirements. New technologies and new approaches for deploying more active and passive nodes must be developed. Furthermore, the use of MMW band and THz band (30 - 300 GHz), in order to utilize their huge bandwidth, results in deploying more active node and more antennas due to high propagation losses and “LOS” behavior at this band. Development of innovative technologies is necessary to realize the above demand for growth of future wireless communication. The main task is to suggest solutions for the time varying characteristic of the wireless channel due to the user mobility and shadowing or blocking of communication channel. The current methods such as use of pilot channel to estimate the fading, various modulation or coding and beamforming, have overhead and limitations over random (large, unexpected changes) channels.

ConfDTree: A Statistical Method for Improving Decision Trees

Decision trees have three main disadvantages： reduced performance when the training set is small; rigid decision criteria; and the fact that a single ＂uncharacteristic＂ attribute might ＂derail＂ the classification process. In this paper we present ConfDTree （Confidence-Based Decision Tree） -- a post-processing method that enables decision trees to better classify outlier instances. This method, which can be applied to any decision tree algorithm, uses easy-to-implement statistical methods （confidence intervals and two-proportion tests） in order to identify hard-to-classify instances and to propose alternative routes. The experimental study indicates that the proposed post-processing method consistently and significantly improves the predictive performance of decision trees, particularly for small, imbalanced or multi-class datasets in which an average improvement of 5%-9% in the AUC performance is reported.

AIM:To study the management and outcome of children with extrahepatic portal vein obstruction(EHPVO) in a whole country population.METHODS:A nationwide multicenter retrospective case series of children with EHPVO was conducted.Data on demographics,radiographic studies,laboratory workup,endoscopic and surgical procedures,growth and development,were extracted from the patients' charts.Characteristics of clinical presentation,etiology of EHPVO,management and outcome were analyzed.RESULTS:Thirty patients,13 males and 17 females,19(63.3%) Israeli and 11(36.7%) Palestinians,were included in the analysis.Age at presentation was 4.8± 4.6 years,and mean follow-up was 4.9±4.3 years.Associated anomalies were found in 4 patients.The incidence of EHPVO in Israeli children aged 0-14 years was 0.72/million.Risk factors for EHPVO were detected in 13(43.3%)patients,including 9 patients(30%) with perinatal risk factors,and 4 patients(13.3%) with prothrombotic states:two had low levels of protein S and C,one had lupus anticoagulant,and one was homozygous for methyltetrahydrofolate reductase mutations.In 56.6% of patients,no predisposing factors were found.The most common presenting symptoms were an incidental finding of splenomegaly(43.3%),and upper gastrointestinal bleeding(40%).No differences were found between Israeli and Palestinian children with regard to age at presentation,etiology and clinical symptoms.Bleeding occurred in 18 patients(60%),at a median age of 3 years.Sclerotherapy or esophageal banding was performed in 20 patients.No sclerotherapy complications were reported.Portosystemic shunts were performed in 11 patients(36.6%),at a median age of 11(range 3-17)years:splenorenal in 9,mesocaval in 1,and a meso-Rex shunt in 1 patient.One patient underwent splenectomy due to severe pancytopenia.Patients were followed up for a median of 3(range 0.5-15)years.One patient died aged 3 years due to mucopolysaccharidase deficiency type Ⅲ.None of the patients died due to gastrointestinal bleeding.CONCLUSION:EHPVO is a rare disorder.

Monte Carlo forecasting of time series data using Polynomial-Fourier series model

The perishable nature of tourism products and services makes forecasting an important tool for tourism planning,especially in the current COVID-19 pandemic time.The forecast assists tourism organizations in decision-making regarding resource allocations to avoid shortcomings.This study is motivated by the need to model periodic time series with linear and nonlinear trends.A hybrid Polynomial-Fourier series model that uses the combination of polynomial and Fourier fittings to capture and forecast time series data was proposed.The proposed model is applied to monthly foreign visitors to Turkey from January 2014 to August 2020 dataset and diagnostic checks show that the proposed model produces a statistically good fit.To improve the model forecast,a Monte Carlo simulation scheme with 100 simulation paths is applied to the model residue.The mean of the 100 simulation paths within±2σbounds from the model curve was taken and found to give statistically acceptable results.
