Background:There are persistent controversies about the outcomes and benefits of drainage of malignant ureteral obstruction by percutaneous nephrostomy(PCN).This study aimed to assess the predictors of the time-to-nadir(TTN)of serum creatinine(SCr)levels after drainage of bilaterally obstructed kidneys(BOKs)due to bladder cancer(BC)by *** and methods:This prospective nonrandomized study included patients with BOKs due to BC treated by PCN between April 2019 and March *** primary outcome measure was ***:Of the 55 patients with a median age(range)of 66 years(47-86 years),32(58.2%)had a normal nadir SCr and 23(41.8%)had a high nadir SCr within 21 days after drainage of BOKs due to *** nadir SCr was associated with a higher mean age(p=0.011)and lower body mass index(BMI,p=0.043).However,patients with normal nadir SCr had a significantly shorter TTN(p=0.023)and an increased mean SCr trajectory(p<0.001)during *** multivariate analysis,low urine output at presentation(p=0.021)and high BMI(p=0.006)were associated with longer ***,the mean parenchymal thickness(p=0.428)and laterality of drainage(p=0.466)were not associated with the mean TTN and SCr normalization *** to the modified Clavien-Dindo classification,8 cases of hematuria were managed conservatively(grade 2),and 2 cases of PCN slippage were repositioned using local anesthesia(grade 3).Conclusions:Despite the safety of PCN for drainage of BOKs due to BC,more than 41%of the patients failed to have a normal nadir *** low urine output and high BMI were associated with longer *** of drainage had no significant effects on the TTN and SCr trajectory.

Background:Hospitals have reported that implementing rapid response system activation(RRS)activation has increased patient *** a result,there has been growing interest in identifying factors that lead to successful RRS *** introducing an automated RRS activation system has prompted nurses to be more vigilant about monitoring vital signs,it has not necessarily encouraged them to conduct thorough patient assessments to identify early signs of ***:The current study aimed to assess nurses’attitudes towards RRS activation for clinically deteriorated patients in the clinical units of King Abdul-Aziz ***:A descriptive cross-sectional research design was utilised in the study,and 144 nurses working in the medical and surgical units of King Abdul-Aziz Hospital were recruited to participate using a convenient non-probability sampling ***:The study’s findings reported that nurses have a positive attitude towards RRS benefits(Mean=3.70;SD=0.70).Their overall attitude towards RRS activation among clinically deteriorated patients is still low positive(Mean=2.71;SD=0.61).The nurses’attitudes towards RRS benefits significantly differ among nationalities and the clinical area/unit where they were assigned,with a P-value of 0.0194 and 0.000,*** towards RRS barriers significantly differ among nationality(P-value=0.0037),education level(P-value=0.0032),area of assignment(P-value=0.020),and whether they have a good understanding of abnormal observations(P-value=0.0122).Regarding the nurses’attitude towards management belief,the significant result is only with the clinical area/unit of assignment with a P-value of ***:The current study found a low positive attitude towards RRS activation among ward nurses,especially given that monitoring vital signs is critical to their *** may fear being perceived as clinically inept for redundant activations caused by poor quality,but their attitude t

Background and objective: During routine follow up, there is no specific predictor to ascertain relapse after standard first line chemotherapy in diffuse large cell lymphoma. Therefore, this study was designed to assess the prognostic significance of the ratio between absolute lymphocyte and monocyte counts (LMR) in the peripheral blood to verify relapse in diffuse large B cell lymphoma. Patients and methods: A total of 139 patients with newly diagnosed diffuse large B cell lymphoma (DLBCL) were evaluated and treated with CHOP or R-CHOP between the years 2009 and 2016. Three months following completion of first line therapy, Lymphocyte/monocyte ratio (LMR) was calculated from the routine automated complete blood cell count (CBC) attained a plateau after the bone marrow recovery after first line chemotherapy. The absolute lymphocyte count/absolute monocyte count ratio (LMR) was calculated by dividing the ALC by the AMC. Results: ROC curve analysis of 139 patients established 2.8 as cutoff point of LMR for relapse with AUC of 0.97 (95% CI 0.93 - 0.99, P ≤ 0.001). Cox regression analysis was performed to identify factors predicting relapse. In univariate regression analysis, ALC (95% CI 0.003 - 0.03, p ≤ 0.001), AMC (95% CI 15.4 - 128.8, p ≤ 0.001), LMR (95% CI 0.001 - 0.01, p ≤ 0.001), and LDH (95% CI 0.1 - 0.5, p ≤ 0.001) following completion of therapy are significant factors for relapse. Other significant factors for relapse are Ann Arbor stage (95% CI 1.1 - 6.9, P = 0.03), extranodal sites (95% CI 1.2 - 6.1, P = 0.01), age (95% CI 1.3 - 6.5, P = 0.01) and treatment of CHOP protocol (95% CI 0.05 - 0.6, P = 0.007). In a multivariate analysis LMR following completion of therapy was predictive for relapse (95% CI 0.001 - 0.2, P = 0.005). ALC was also significant in multivariate analysis (95% CI 0.01 - 0.8, P = 0.03). This study aims to improve a way for reducing backscattered radiation in an X-ray room. An iron steel grid, capable to absorb a significant portion of the backscattered radiation, was used. X-ray machine as a source for radiation was directed normally on the phantom, and the backscattered radiation was measured by using ion chamber. The measurements were recorded at various applied voltages (60 kvp to 120 kvp) and the fabricated grid was designed from iron steel constructed of perpendicular parallel strips mounted on a base. The results indicated that the use of iron steel grid was very effective in the reduction of backscattered radiation in an X-ray room up to about 46% by using fabricated iron steel grid.

开发特殊的多相材料用于提高过渡金属催化剂的原子效率是有机合成中的一个重要问题,由于他们简单分离而又易回收的特性,并且它的反应催化活性在反应中不受影响,在科研和生产应用中一直为科研工作者及其工程师所探究。本文研究了两种类型非均相催化剂的构建:(1)受独特的中空壳结构和多孔有机硅烷辅助材料拥有高稳定性的启发,我们通过以TEOS(正硅酸四乙酯)为硅源,CTAB(十六烷基三甲基溴化铵)为模板剂,经过刻蚀、去除模板剂等过程,通过用2-(二苯基膦基)乙基三乙氧基硅烷改性内壁,并将衍生物二苯基膦与Pd(OAc)交联,构建了中空介孔结构的介孔二氧化硅基非均相催化剂HS@Pd PPh@MSN。通过结构分析和材料表征,表明内壳内有明确定义的单点Pd活性物质,并且提供了HS@Pd PPh@与相关的均相对照,通过与其他核/中空壳固体催化剂相比,MSN表现出优异的催化能力和活性。催化剂HS@Pd PPh@MSN在1-碘-4-甲氧基苯和苯基硼酸的有效Suzuki-Miyaura交叉偶联反应中,得到一系列二芳基,产率高达95%。此外,催化剂HS@Pd PPh@MSN还可以在过滤和洗涤后回收,并在Suzuki-Miyaura偶联反应中可以循环使用10次,反应中保持其活性并未发生变化。这种简单而经济高效的构造HS@Pd PPh@MSN纳米材料使其在精细化学工程中具有广阔的应用前景,符合绿色化学工艺的理念。(2)受到经典易得的手性有机分子笼CC3的启发,并对其进行改造。我们首先合成了1,3,5-三(对甲酰基苯基)苯,再利用其与手性环己二胺进行可逆的缩合反应,创新性地合成了CC3-4Ph手性有机分子笼。将CC3-4Ph经过简单的还原操作合成RCC3-4Ph后,利用其独特的空间构型,并通过环己二胺的螯合能力配位镍离子后,在自制手性配体的辅助下,探究了其对迈克尔加成反应的应用。结果表明该分子笼独特的手性结构以及双氮中心的辅助,能够以较高的反应效率以及不对称选择性催化迈克尔加成反应,从而构建高对映选择性的迈克尔加成产物,底物适用范围广,有较广的应用前景。

In this study, the feasibility of using chitosan, a multifunctional environmentally friendly biopolymer, at different molecular weights to simultaneously adsorb various metal ions from aqueous solutions was evaluated. The experiments were carried out to estimate adsorptivity and selectivity of three molecular weights chitosan, 3.60 × 105 （Chl）, 5.97 × 105 （Ch2） and 9.47 × 105 （Ch3） g/mol, towards Cu（II）, Ni（II）, and Pb（II） ions in aqueous solutions. Experimental results indicated that chitosan of high molecular weight （Ch3） was the best at adsorption capacity than the others.
