The adverse outcomes of a ventricular heart failure(left,right or biventricular)caused by cardiogenic shock are aggravated by lung oedema and organ mal *** advances in medical sciences,revascularisation and mechanical hemodynamic support have proved ineffective in reducing the mortality rate in such patients.A thorough study of the data available about cardio-vascular diseases reveals that the application of conventional methods of treatment are least helpful to practically restore normal functions of heart when it experiences end-stage systolic ventricular ***,to overcome the challenges and find alternatives to address this issue,percutaneous ventricular support devices/machines were designed and successfully *** devices have revolutionized the treatment of ventricular heart failures and are now in use all over the *** this review paper a newer mechanical circulatory support(MCS)device,Impella,has been discussed and compared with a few other devices like Intra-aortic Balloon Pump(IABP),Extracorporeal Circulation(ECLS)and Veno-arterial Extracorporeal Membrane Oxygenation(VA-ECMO).This article studies the challenges being faced during the treatment of cardiogenic shock,and thoroughly discusses the use and effectiveness of Impella Cardiac Axial Pump in each *** can be said that mechanical circulatory support(MCS)device use during percutaneous coronary intervention(PCI)should be individualized based on multiple factors with a recommended use in patients with the greatest potential benefit and a relatively low risk of device-related *** current literature suggests that the outcomes of use of Impella and other mechanical circulatory support devices like IABP and VA-ECMO are *** there seem to be a few advantages of Impella over the others,sufficiently powered,multi-centric,randomised control trials are needed to establish its superiority.

Locally Advanced Breast Cancer in Pakistani Women: What Is Different from Rest of the World and Why It Is Difficult to Manage

Locally Advance Breast Cancer refers to a heterogeneous group of breast cancer with locally extensive disease, which may or may not involve the nodes, without any distant metastases. The study was conducted at Faisalabad Medical University (FMU), Oncology, Allied Hospital Faisalabad (Pakistan). Data of 100 patients with LABC was collected. Demographics were recorded in the form of age, socio-economic status. In clinical data, time of presentation, family history of breast cancer, the presenting symptom in the form of lump, ulceration and other skin changes were noted. Histo-pathological variables including tumor size, histopathology, Bloom & Richardson grading, estrogen receptor (ER), progesterone receptor status (PR) and HER2 status. Results showed that after following a standard trimodality treatment approach in LABC patients, 30 percent died within two years. Disease free survival for more than two years was observed in only 25% of patients. Whereas, 70% patients had eventful (Recurrence/metastases) survival. This poor outcome was observed due to lack of health care facilities, awareness and poor socioeconomic status.

富有机质泥页岩蕴含丰富的油气资源,但成岩研究基础薄弱,已成为制约页岩油气勘探开发进程的重要因素。通过开展成岩热模拟实验,结合扫描电镜观察、流体成分测试及有机酸测试等实验测试手段,全面描述实验过程中所发生的水—岩化学反应,试图揭示泥页岩成岩过程及成岩演化规律。研究结果显示,有机质热演化过程中存在一个较宽的有机酸生成窗口,对孔隙流体性质具有重要影响;长石和方解石存在接力溶蚀现象,长石溶蚀高峰过后紧接着出现方解石溶蚀高峰,但方解石溶蚀窗较窄,此后出现方解石重新沉淀结晶;黏土矿物转化及长石的溶蚀过程中会产生大量自生微晶石英,成链状或簇状胶结泥页岩骨架。泥页岩地层作为一个相对封闭体系,各类成岩作用之间相互关联,相互影响,构成错综复杂的成岩体系,进一步增加其成岩作用研究难度。

Forecasting on success or failure of software has become an interesting and,in fact,an essential task in the software development *** order to explore the latest data on successes and failures,this research focused on certain questions such as is early phase of the software development life cycle better than later phases in predicting software success and avoiding high rework?What human factors contribute to success or failure of a software?What software practices are used by the industry practitioners to achieve high quality of software in their day-to-day work?In order to conduct this empirical analysis a total of 104 practitioners were recruited to determine how human factors,misinterpretation,and miscommunication of requirements and decision-making processes play their roles in software success *** discussed a potential relationship between forecasting of software success or failure and the development *** noticed that experienced participants had more confidence in their practices and responded to the questionnaire in this empirical study,and they were more likely to rate software success forecasting linking to the development *** analysis also shows that cognitive bias is the central human factor that negatively affects forecasting of software success *** results of this empirical study also validated that requirements’misinterpretation and miscommunication were themain causes behind software systems’*** has been seen that reliable,relevant,and trustworthy sources of information help in decision-making to predict software systems’success in the software *** empirical study highlights a need for other software practitioners to avoid such bias while working on software *** investigation can be performed to identify the other human factors that may impact software systems’success.

Phytochemical composition, biological potential and enzyme inhibition activity of Scandix pecten-veneris L.

Objective： Scandix pecten-veneris L. is a less studied wild edible herb and is considered an extinct plant species in many parts of the world. This study was designed to evaluate its phytochemical composition and biological potential of S. pecten-veneris L. Methods： Phytochemicals including alkaloids, fiavonoids, polyphenols, and tannins were determined in extracts of S. pecten-veneris. Antioxidant activity was determined using 2,2-diphenyl-1- picrylhydrazyl （DPPH）, while reducing power was tested by ferric reducing/antioxidant power （FRAP） assay. Antimi- crobial activity against seven bacterial and four fungal strains was evaluated using agar well diffusion assay. Enzymes inhibition study was performed for urease, phosphodiesterase-I, and catalase-II. Results： S. pecten-veneris showed moderate antiradical activity and reducing potential of hydroxyl radicals to about 20% of the initial value. The antiox- idant activity of various extracts of S. pecten-veneris showed a linear correlation with total phenolic contents in the order of water〉n-butanol〉chloroform〉ethyl acetate〉methanol extracts. S. pecten-veneris leaves showed the highest inhibitory activity against Staphylococcus aureus while the highest antifungal activity was observed against Candida albicans. The plant extract was most potent against urease enzymes but showed moderate activity against phosphodiestrase-I and carbonic anhydrase-II. Conclusions： Our data demonstrate that in addition to its culinary uses, S. pecten-veneris has good medicinal potential and hence could be used for treating some specific health ailments.

Growth, Physiological and Molecular Responses of Cotton (Gossypium arboreum L.) under NaCl Stress

Salinity is one of the most serious growth limiting factor, therefore, no longer being ignored. Although, cotton is fairly salt tolerant;its sensitivity at crop stand and yield is affected. This study is planned to identify the growth, physiological and molecular parameters in local cotton varieties FDH 171 and FDH 786 under NaCl stress. There was 100% seed germination but hypocotyl length was reduced at increasing level of NaCl. Plant height, fresh and dry biomass were reduced as the plants were subjected to increased stress of NaCl. Stomatal conductance, transpiration and photosynthetic rate and ionic imbalance were found to be reduced under the gradual increase in NaCl stress and affected the plant’s overall physiological processes. PCR product of AtNHX3 has been identified in stressed and non-stressed plants. Thus, the genotypes FDH 171 & FDH 786 were found tolerant to adoption of salt stress and could be used as a source in crop improvement.

BACKGROUND Musculoskeletal injuries(MSI)have plagued endoscopists and ancillary staff for decades without any innovative and strong ergonomic *** has placed a physical and mental strain on our endoscopists and ancillary *** have very have limited data supporting this claim in our region and most data is supported by western *** To document the prevalence of MSI,and awareness and practices of ergonomics by endoscopists and ancillary *** This is an observational cross-sectional study,conducted in Karachi,a city that boasts the maximum number of daily endoscopies in the *** eleven-point self-administered questionnaire was distributed and used to evaluate MSI and ergonomic adjustments amongst three tertiary care setups in *** onsite survey via a 13-point checklist for endoscopy suite facilities was used to assess the ergonomically friendly conveniences at five tertiary care setups in Karachi.A total of 56 participants replied with a filled *** There were 56 participants in total with 39(69.6%)*** and numbness were documented by 75%of the patients,with pain in the neck(41.1%),lower back(32.1%),shoulder(21.4%),thumb(12.5%),hand(23.2%),elbow(8.9%),and carpal tunnel syndrome(CTS)(7.1%).Of those,33.3%attributed their symptoms to endoscopy, 14.2% said that symptoms were not caused by endoscopy, and 52.4% were notcertain whether endoscopy had caused their symptoms. Twenty-one point four percent of patientshad to take time off their work, while 33.9% took medications for pain. Ergonomic modifications toprevent musculoskeletal injury, including placement of endoscopic monitor at eye level and thecardiac monitor in front, stopping the procedure to move patients, sitting while performingcolonoscopy, and navigating height-adjustable bed were used by 21.4%. These days, there is a lot of discussion about genetically modified plants. There are different schools of thoughts in public, and some people adjusted while others are reluctant to accept genetically modified organism foods. Many vegetables are transformed and are used in daily life. Chili is one of those which is genetically modified and used in our food. Race specific genes can be used more efficiently for disease resistance and improving metabolic pathways. Different genes and transcriptional factors are available in Capsicum for this purpose. We can optimize and use the better expressed genes while engineering the chili plants, Genetic modifications causing significant changes are related with metabolism, which cause disease resistance.

Evaluation of Different Radish (Raphanus sativus) Genotypes under Different Saline Regimes

An effective and simple screening technique for identification of salt tolerant and salt sensitive radish genotypes was observed. Sand is used as potting media. Six genotypes of radish were used for screening against four salinity levels (0, 1, 3, 5 and 7 dS/m-1). Twenty days old seedlings of radish were salinized with salt solution (NaCl). Morphological, physiological and ionic parameters were studied. Radish genotypes Laal-Pari and 40 Days executed the best performance in all the measured attributes and categorized as salt tolerant genotype while Green Neck was the poorest in retaining normal functioning at higher salinity levels thus grouped under salt sensitive cultivar.

Hypoglycemic myocardial stunning as cause of cardiogenic shock in a patient with ischemic cardiomyopathy: A case report and review of literature

Hypoglycemia is a common complication seen in patients with diabetes mellitus and has been proven to have adverse effects on cardiovascular mortality. Hypoglycemia can potentially lead to worsening of cardiac function in patients with ischemic heart disease. We present a case of cardiogenic shock in a patient with hypoglycemia secondary to insulin accumulation due to worsening renal function with dramatic recovery of shock once his sugars normalized.
