Development and validation of a nomogram for predicting in-hospital mortality of intensive care unit patients with liver cirrhosis

BACKGROUND Liver cirrhosis patients admitted to intensive care unit(ICU)have a high mortality *** To establish and validate a nomogram for predicting in-hospital mortality of ICU patients with liver *** We extracted demographic,etiological,vital sign,laboratory test,comorbidity,complication,treatment,and severity score data of liver cirrhosis patients from the Medical Information Mart for Intensive Care IV(MIMIC-IV)and electronic ICU(eICU)collaborative research database(eICU-CRD).Predictor selection and model building were based on the MIMIC-IV *** variables selected through least absolute shrinkage and selection operator analysis were further screened through multivariate regression analysis to obtain final *** final predictors were included in the multivariate logistic regression model,which was used to construct a ***,we conducted external validation using the *** area under the receiver operating characteristic curve(AUC),decision curve,and calibration curve were used to assess the efficacy of the *** Risk factors,including the mean respiratory rate,mean systolic blood pressure,mean heart rate,white blood cells,international normalized ratio,total bilirubin,age,invasive ventilation,vasopressor use,maximum stage of acute kidney injury,and sequential organ failure assessment score,were included in the multivariate logistic *** model achieved AUCs of 0.864 and 0.808 in the MIMIC-IV and eICU-CRD databases,*** calibration curve also confirmed the predictive ability of the model,while the decision curve confirmed its clinical *** The nomogram has high accuracy in predicting in-hospital *** the included predictors may help improve the prognosis of patients.

Ultra-high nickel layered oxide cathode material with high energy density is the most promising material to improve the electrochemical performance of lithium-ion batteries(LIBs).However,the poor structural stability and severe surface/interface side reactions of the material lead to poor rate performance and cyclic stability,which limits its application in *** this paper,the dual-modification strategy of Co doping and La_(2)O_(3) coating is used to meet the above *** doping can effectively widen layer spacing and reduce Li^(+)/Ni^(2+) mixing,and La_(2)O_(3) coating can effectively eliminate the residual alkali on the surface of active material,inhibit the thickening of cathode electrolyte interphase(CEI)film and reduce surface/interface side ***,the modified material(NM90-CL)with excellent electrochemical properties is achieved through the synergistic enhancement of Co doping and La_(2)O_(3) *** capacity retention rate can reach 77.9%after 200 cycles at 1.0℃ and 75.7%after 200 cycles at 5.0℃.Its reversible capacity can up to 153.5 mAh·g^(–1) at 10.0℃.This dual-modification strategy will provide theoretical guidance and technical support for the synthesis of other high-performance electrode materials.

Background:Magnesium cantharidate(MC)is a protein phosphatase 2A(PP2A)inhibitor antitumor ***,its antitumor mechanism in hepatocellular carcinoma cell(HCC)remains ***:PP2A lentiviral vector over expression strategy was utilized both in vivo and in vitro to explore the antitumor effect in MC and okadaic acid(OA).Tumor weight was detected in mice after MC and OA *** proliferation,cell cycle,apoptosis rate,and western blotting were detected to explore the effects on MC and OA in human hepatocarcinoma SMMC-7721 ***:In vivo results demonstrated that MC inhibited HCC progression while OA promoted tumor *** vitro results demonstrated that MC effectively inhibited the growth of SMMC-7721 cells by arresting the cell cycle at the G2/M phase with inhibiting Cdc25C and activating the phosphorylation of the Cdc2 *** cytometry results further showed that MC increased ***,the expression of phosphorylated ERK1/2 was lower in the MC group but higher in the OA *** docking results showed that MC docked well with ERK1/***:MC inhibited HCC progression by suppressing the growth and activating the apoptosis of cancer cells and suppressing the expression of PP2A and ERK1/2.

Exploring mechanism of hidden,steep obliquely inclined bedding landslides using a 3DEC model:A case study of the Shanyang landslide in Shaanxi Province,China

Catastrophic geological disasters frequently occur on slopes with obliquely inclined bedding structures(also referred to as obliquely inclined bedding slopes),where the apparent dip sliding is not readily *** phenomenon has become a focal point in landslide ***,there is a lack of studies on the failure modes and mechanisms of hidden,steep obliquely inclined bedding *** study investigated the Shanyang landslide in Shaanxi Province,*** field investigations,laboratory tests of geotechnical parameters,and the 3DEC software,this study developed a numerical model of the landslide to analyze the failure process of such *** findings indicate that the Shanyang landslide primarily crept along a weak interlayer under the action of *** landslide,initially following a dip angle with the support of a stable inclined rock mass,shifted direction under the influence of argillization in the weak interlayer,moving towards the apparent dip *** slide resistance effect of the karstic dissolution zone was increasingly significant during this process,with lateral friction being the primary resistance force.A reduction in the lateral friction due to karstic dissolution made the apparent dip sliding characteristics of the Shanyang landslide more ***,deformations such as bending and uplift at the slope’s foot suggest that the main slide resistance shifts from lateral friction within the karstic dissolution zone to the slope foot’s resistance force,leading to the eventual buckling failure of the *** study unveils a novel failure mode of apparent dip creep-buckling in the Shanyang landslide,highlighting the critical role of lateral friction from the karstic dissolution zone in its failure *** insights offer a valuable reference for mitigating risks and preventing disasters related to obliquely inclined bedding landslides.

BACKGROUND Autoimmune enteropathy(AIE)is a rare disease whose diagnosis and long-term prognosis remain challenging,especially for adult AIE *** To improve overall understanding of this disease’s diagnosis and *** We retrospectively analyzed the clinical,endoscopic and histopathological characteristics and prognoses of 16 adult AIE patients in our tertiary medical center between 2011 and 2023,whose diagnosis was based on the 2007 diagnostic *** Diarrhea in AIE patients was characterized by secretory *** common endoscopic manifestations were edema,villous blunting and mucosal hyperemia in the duodenum and *** blunting(100%),deep crypt lymphocytic infiltration(67%),apoptotic bodies(50%),and mild intraepithelial lymphocytosis(69%)were observed in the duodenal ***,there were other remarkable abnormalities,including reduced or absent goblet cells(duodenum 94%,ileum 62%),reduced or absent Paneth cells(duodenum 94%,ileum 69%)and neutrophil infiltration(duodenum 100%,ileum 69%).Our patients also fulfilled the 2018 diagnostic criteria but did not match the 2022 diagnostic criteria due to undetectable anti-enterocyte *** patients received glucocorticoid therapy as the initial medication,of which 14/16 patients achieved a clinical response in 5(IQR:3-20)*** were administered to 9 patients with indications of steroid dependence(6/9),steroid refractory status(2/9),or intensified maintenance medication(1/9).During the median of 20.5 months of followup,2 patients died from multiple organ failure,and 1 was diagnosed with non-Hodgkin’s *** cumulative relapse-free survival rates were 62.5%,55.6%and 37.0%at 6 months,12 months and 48 months,*** Certain histopathological findings,including a decrease or disappearance of goblet and Paneth cells in intestinal biopsies,might be potential diagnostic criteria for adult *** long-term prognosis is still unsatisfac

Glaucoma drainage device implantation and cyclophotocoagulation in the management of refractory glaucoma after Descemet-stripping automated endothelial keratoplasty

AIM:To compare the surgical outcomes of glaucoma drainage device implantation(GDI)and trans-scleral neodymium:YAG cyclophotocoagulation(CPC)in the management of refractory glaucoma after Descemetstripping automated endothelial keratoplasty(DSAEK).METHODS:This retrospective study on observational case series enrolled 29 patients who underwent DSAEK and posterior anti-glaucoma surgery(15 with GDI and 14 with CPC).The main outcome measures were intraocular pressure(IOP),glaucoma surgery success rate(defined as IOP of 6–21 mm Hg without additional anti-glaucoma operation),number of glaucoma medications,endothelial graft status,and best-corrected visual acuity(BCVA).RESULTS:The mean follow-up time was 34.1 and 21.0mo for DSAEK or glaucoma surgeries,both for the GDI and CPC *** groups showed significant IOP reduction after glaucoma *** GDI group presented a significantly higher success rate in IOP control than the CPC group(60%vs 21.4%,P=0.03).Both procedures significantly decreased the number of glaucoma medications(P=0.03).Forty percent and 57%of cases in the GDI and the CPC group,respectively,experienced endothelial graft failure during follow-up(P=0.36).Significantly worse BCVA after surgery was observed in the CPC group but not in the GDI ***:Both GDI and CPC significantly decrease IOP in eyes with glaucoma after *** is preferable to CPC in refractory glaucoma cases after DSAEK,as it manifests a significantly higher success rate for IOP control,similar endothelial graft failure rate,and relatively preserves BCVA than CPC.
