a. increa.ed lung to hea.t ra.io(LHR) on tha.lium-201(Tl- 201) stress myoca.dia. perfusion ima.ing(MPI) is a.predictor of a.verse ca.dia. events a.d identifies people with extensive corona.y a.tery disea.e(Ca.). The implica.ions of increa.ed LHR in pa.ients undergoing stress technetium- 99m(tc- 99m) sesta.ibi a.e developing. Our a.m is to eva.ua.e the rela.ionship between increa.ed LHR a.d extent of Ca. in pa.ients undergoing tc- 99m sesta.ibi MPI. Methods: We reviewed the records a.d ima.es of 530 consecutive subjects who underwent exercise or a.enosine tc- 99 m sesta.ibi MPI. One hundred thirty-two ha. tra.sient or pa.tia.ly reversible myoca.dia. perfusion defects a.d 79(exercise=34, a.enosine=45, ma.e=43, fema.e=36, mea. a.e=61 yea.s) of these underwent corona.y a.giogra.hy(study popula.ion). The a.era.e LHR of these 79 subjects wa. compa.ed to 79 pa.ients(control popula.ion) with norma. sca.s(exercise=50, a.enosine=29, ma.e=34, fema.e=45, mea. a.e=60 yea.s). Results: The mea. LHR(± SE) in subjects with norma. sca.s wa. 0.30± 0.01. The mea. LHR for those with a.norma. sca.s a.d single vessel Ca. who underwent exercise wa. 0.32± 0.01 a.d pha.ma.ologica. stress wa. 0.31± 0.01. There wa. no sta.istica.ly significa.t difference between the LHR of those with a.norma. sca. a.d those with single vessel disea.e a.d a. a.norma. sca.. However, there wa. a.sta.istica.ly significa.t a.socia.ion between the eleva.ed LHR a.d multi-vessel Ca.. The mea. LHR for subjects with multi-vessel Ca. with exercise wa. 0.39± 0.01(p=0.000) a.d for a.enosine wa. 0.39± 0.02(p=0.000). Conclusion: a. eleva.ed LHR in pa.ients undergoing exercise or pha.ma.ologica. tc- 99m MPI correla.es with multi-vessel Ca..

Thrombotic thrombocytopenia.purpura.TTP) wa. fi rst described in 1924 a. a."pa.hologic a.tera.ion of the microva.cula.ure, with deta.hment or swelling of the endothelium, a.orphous ma.eria. in the sub-endothelia. spa.e, a.d lumina. pla.elet a.grega.ion lea.ing to compromise of the microcircula.ion". Ticlopidine induced TTP ha. been highly a.socia.ed with a.toimmune induced reduction in a.a.TS-13 a.tivity. These findings, to a.lesser extent, ha.e a.so been found in clopidogrel induced TTP. We report a.ca.e of clopidogrel a.socia.ed TTP in a.pa.ient tha. presented with a.ute stroke, rena. fa.lure, a.d non-ST eleva.ion myoca.dia. infa.ction.

Perica.dia. ca.cifica.ion a.one is a.ymptoma.ic a.d is neither necessa.y nor sufficient for the dia.nosis of perica.dia. constriction. Its presence ma. suggest of diffused perica.dia. sca.ring a.d consequently, its pa.hologica. involvement with perica.dia. constriction. Tra.sthora.ic echoca.diogra.hy(TTE) revea.ed a.dila.ed left ventricle(LV) a.d ejection fra.tion(EF 30%) with thinning a.d a.inesis of inferior/inferola.era. wa.l wa. noted with ba.a. a.d mid inferior wa.l a.eurysm, a.d thrombus wa. not identified. CMR done to a.certa.n via.ility of myoca.dium revea.ed a.mura. thrombus within ba.a. inferior a.eurysm. This wa. not visua.ized on tra.sthora.ic echoca.diogra.hy with a.d without use of contra.t. She underwent corona.y a.tery bypa.s gra.ting, bioprosthetic mitra. va.ve repla.ement, resection a.d plica.ion of posterior left ventricula. a.eurysm with remova. of mura. thrombus, a.d wa. sta.ted on a.ticoa.ula.ion with wa.fa.in postopera.ively for the a.ica. thrombi. Ca.dia. ma.netic resona.ce is a.well suited ima.ing moda.ity in detecting LVT due to its high resolution ima.es a.d is more reproducible tha. TTE. In our pa.ient, conventiona. TTE despite a.ministra.ion of echo-contra.t a.ents fa.led to dia.nose the presence of LVT in the ba.a. inferior a.eurysm a. well a. the a.ica. thrombi. Dela.edenha.cement CMR provides the grea.est sensitivity for detection of left ventricula. thrombus, superiorto sta.da.d tra.sthora.ic a.d contra.t-enha.ced tra.sthora.ic echoca.diogra.hy.

We report a.ca.e of contra.t induced neurotoxicity following corona.y a.giogra. a.d intervention using Iohexol(Omnipa.ue 350) in a. end sta.e rena. disea.e pa.ient on peritonea. dia.ysis who ha. prior exposure to iodina.ed contra.t without a.y a.verse rea.tion. Hemodia.ysis ha. to be initia.ed for ra.id remova. of the contra.t a.ent with subsequent complete resolution of neurologica. deficits. Persistent left superior vena.ca.a. usua.ly a. incidenta. finding, is the most common thora.ic vein a.a.omica. va.ia.ion dra.ning into the corona.y sinus. Centra. venous ca.heter procedures ma. be complica.ed seconda.y to the presence of a.persistent left superior vena.ca.a. lea.ing to life-threa.ening complica.ions such a. a.rhythmia., ca.diogenic shock, a.d ca.dia. a.rest. We present a.ca.e of persistent superior vena.ca.a.dia.nosed on tra.sthora.ic echoca.diogra.(TTE) in a.pa.ient with congestive hea.t fa.lure. a.dila.ed corona.y sinus wa. identified on TTE, followed by injection of a.ita.ed sa.ine into the left a.tecubita. vein resulting in filling of the corona.y sinus prior to the right a.rium-a. indica.ion of persistent left superior vena.ca.a. This a.so wa. confirmed on ca.dia. computed tomogra.hy. Such a.dia.nosis is critica. in pa.ients who ma. undergo centra. venous ca.heter procedures such a. our pa.ient's potentia. requirement for a. impla.ta.le ca.diovertor defibrilla.or due to severe globa. left ventricula. systolic dysfunction. The presence of a.persistent left superior vena.ca.a.should a.wa.s be suspected when the guidewire ta.es a.left-sided downwa.d course towa.ds the righta.rium a. the level of the corona.y sinus. Therefore, specia. a.tention should be pa.d to the ima.ing work-up prior to centra. venous ca.heter procedures.
