Chronic Lymphocytic Leukemia of del 17p in a Young Subject: About a Case and Reviewed a Literature

Objective: To report a case of Chronic Lymphoid Leukemia in a 28-year-old young subject, with variable clinical features and a TP53 mutation, diagnosed and followed up in the Onco-Hematology department of the HNN. Observation: 28-year-old patient, having consulted for polyadenopathy and physical asthenia, whose clinical examination found a conscious patient, submaxillary, laterocervical, axillary and inguinal lymphadenopathy, bilateral, symmetrical, painless and non-compressive whose largest measures 3 cm in diameter. Hepato-splenomegaly and epistaxis. Predominantly lymphocyte hyperleukocytosis, immunophenotyping revealed low CD19+, CD5+, CD23+, CD20 monoclonal B lymphoid proliferation. The Matutes score was 4. A karyotype showed a three-chromosome translocation;the short arm of a chromosome 2, the long arm of a chromosome 11 and the long arm of a chromosome 13, and a translocation between the long arm of a chromosome 6 and the long arm of a chromosome 18. A FISH objectified a led 17p. The diagnosis of Binet Stage C CLL with positive del 17p and complex karyotype was retained. Despite the poor prognosis, the R-C (Rituximab-Chlorambucil) protocol was instituted with once-weekly transfusions. The patient is still alive in partial clinical and biological remission. Conclusion: Despite therapeutic progress, the presence of the deletion of chromosome 17p with TP53 mutation and the young age of the patient does not change the patient’s prognosis.

Evolution of the Spatial Distribution of Basic Infrastructures and Services in the Niakhar Area (Fatick), Senegal

Basic infrastructures and services present an important socio-economic and territorial equity issue, because the availability of these makes it possible to improve the living conditions of populations, makes territories attractive and stimulates their development. The objective of our study has been achieved, so our results provided information on the rate of propagation of basic infrastructures and services, as well as the evolution of the forms of spatial distribution of the latter. The data used comes from the census and geolocation of basic infrastructures and services in the Niakhar area, carried out by the IRD in 2018. Centrographic measurements were used to describe the overall spatial distribution and the use of chi-square statistics confirmed the existence of a preferential direction of distribution. To verify the existence of a spatial structure of infrastructure and service seeding, the Ripley statistic is used. Our results can be useful for land use planning and spatial resource allocation policies. Indeed, the identification of different types of spatial aggregates and the highlighting of the preferred directions of the distribution of basic infrastructures and services, make it possible to correct disparities in the spatial distribution of basic resources.

在萨赫勒,临时池塘在保护物种多样性方面起着重要作用。本研究考察了塞内加尔北部费罗地区的两个放牧池塘以及一个未放牧池塘,探讨了人类活动与生态水文因素对池塘周边植物多样性的影响。每个池塘建立两条植物生态样带,确定并记录每条样带上平均植被单元情况(包括植被覆盖度、土壤暴露度、土壤质地、土壤湿度和酸碱度、平均高度以及坡度等)。研究结果表明,3个池塘周边共有草本植物22科41属55种。放牧池塘周边多为禾本科植物,尤其是画眉草属(Eragrostis tremula)与肠须草属(Enteropogon prieurii)植物,未放牧池塘周边则主要为其他科类植物,如牛膝属(Achyranthes sp.)、鸭跖草属(Commelina forsskaolii)、黄麻属(Corchorus sp.)植物等。Wendu Marodi池塘(未放牧池塘)的香农物种多样性指数(H′)为4.9,略高于Belel Lougal池塘和Belel Sow池塘(放牧池塘),这两个池塘的香农物种多样性指数分别为4.8和4.5。3个池塘的种类平均性指数相对都较高,分别为0.85、0.83和0.78。根据生态因子可分出4个物种群:蒺藜(Tribulus terrestris)种群,集中在限牧区与过度放牧区;平枝马唐(Digitaria horizontalis)种群与孟仁草(Chloris barbata)种群,见于过度带;以及土牛膝(Achyranthes aspera)种群,生长在人类活动较少且草本植物丰富的湿地。本研究强调了维持未放牧池塘或半保护池塘与放牧池塘景观镶嵌结构的重要性,以保护塞内加尔中心与萨赫勒地区草本植物的多样性。

Physico-Chemical Characterization of Sludge from the Goudel Drinking Water Production Plant in Niamey (Niger)

Population growth and increasing needs make our current societies a considerable source of environmental threats. Going towards sustainable cities where harmony exists between economic, socio-cultural and environmental issues is a necessity that is essential if we want to bequeath a livable world to future generations. Cities produce huge quantities of domestic and industrial waste, the management of which is becoming a growing problem for city managers. Located on the banks of the Niger River, the city of Niamey, capital of the Republic of Niger, is supplied with drinking water from this river. Significant quantities of sludge are produced by the plant following the treatment of this water. This study focuses on the physico-chemical characterization of this sludge which is now directly discharged into the Niger River. A total of 12 samples of pasty sludge taken from the pre-settling ponds were analyzed. The samples were previously dried, crushed, sieved and packaged. Physical (pH, EC and particle size), chemical (Ca, Mg, Na, K, CEC, AE, P, C, MO and N) and metallic trace elements (Pb, Ni, Cu, Zn, Cd, Mn) parameters and Al) were analyzed in the laboratory. The results show that the sludge is weakly acidic (pH between 4.16 and 5.71), conductive (<0.35 mS/cm) and rich in fine elements (12.1% to 77.71% clay). The nutrient content is low in nitrogen (<0.25%), phosphorus (<2.5 ppm), potassium (between 1 to 188 Mèq/100g) and organic matter (less than 2.88%). The concentrations of ETM comply with the values admissible in residual materials (French decree of 08/01/1998). This sludge can be recovered, especially in agriculture.

长期以来煤炭一直是中国的主体能源,今后相当长时间内仍占主导作用。矿井水害是煤炭资源开发过程中的主要灾害形式之一,开展矿井水文地质条件探查研究,制定科学合理的防治方案,对于提高资源开发效率、避免水灾发生、减弱灾害影响均具有重要价值。山东万福煤矿-820水平井底车场多条主要巷道穿越三灰布置,该区域三灰初期探查发现,其水文地质条件较资源勘查阶段获取的水文地质参数有较大出入,呈现水压高、富水丰富的特征。为此本项目采用资料分析、室内测试、现场试验、数值分析等方法,就万福煤矿-820井底车场区域三灰的水文地质条件开展综合研究,并以此为基础制定穿层巷道水害防治方案。项目研究主要成果如下:(1)万福矿井-820井底车场区域三灰水文地质条件研究主要采用井下放水试验进行,获得了三灰的渗透系数、单位涌水量等数据。放水试验采用井上、下11个孔组合进行,历经三个试验阶段,根据放水试验成果,-820井底车场区域三灰的渗透系数为3.40～7.75m/d,单位涌水量为0.036～0.276 L/(s·m)。(2)根据研究区地质资料及放水试验成果,获得了-820水平井底区域三灰的厚度、产状、成分、强度及溶裂隙发育特征;研究发现试验区域三灰富水及渗透性均为中等;三灰疏放水位降深与涌水量比值在87～191之间,属于易于疏降级别;三灰与五灰、新近系底部含水层、奥灰具有水力联系,研究构建了三灰补给的模式。(3)鉴于三灰含水层与奥灰含水层水力联系不密切,三灰具有易于疏降的特点,-820水平井底车场三灰水害治理设计采用“疏堵结合”的技术方案,此方案具有降低注浆难度、大幅减小注浆工程量、三灰水完全可控、改善井下作业环境、提升掘进效率的优点。(4)在三灰水压大幅疏降情况下,开展大巷三灰帷幕注浆工作,根据安全隔水层厚度数据确定了注浆帷幕厚度及钻孔布置方式,选用普通水泥浆液,合理控制了注浆压力及注浆孔施工顺序;注浆后大巷已安全掘进穿过三灰,均无明显出水现象,注浆效果高于预期。

47XYY syndrome is a rare sex chromosome variant with an extra Y chromosome. Most patients with a 47XYY karyotype have a normal phenotype. This disorder seems to be associated with a higher risk of developing behavior and cognitive problems, tall stature and infertility in adulthood. We report here a rare case of 47, XYY syndrome associated with an 11-beta-hydroxylase deficiency revealed by a stature advance along with precocious puberty after obtaining informed consent from parents. To our knowledge, this is the first case reported in the literature.

Establishment of Local Diagnostic Reference Levels in Conventional Radiography: A Pilot Study in Dakar, Senegal

Diagnostic Reference Levels (DRLs) are indicators that allow assessing the quality of equipment and procedures from the point of view of the doses delivered to patients and subsequently initiate corrective actions if necessary. The purpose of this study is to encourage health professionals to investigate patient radiation doses and to determine whether those doses comply with the principles of radiation protection in medical fields so as to improve practices by reducing patient exposure without reducing clinical effectiveness. To perform this work, we have investigated patient doses for different radiological examinations from six (6) medical centers in Dakar, including the following nine routine types: chest (PA), abdomen (AP), pelvis (AP), cervical spine (AP), lumbar spine (AP, Lat), hip (AP), thoracic spine (AP, Lat). Three types of data were collected, i.e., X-ray tube machine data, patient data and output measurements. The data were analyzed statistically and the median, minimum, maximum, and third quartile values were calculated and displayed throughout boxplots graphs for all exams and medical centers. The two sigma range (95% confidence interval) was also checked. Comparison of third quartiles of Entrance Surface Dose (ESD) and Dose Area Product (DAP) by type of examination with recommended international DRLs was performed. The third quartile of ESD for pelvis (AP) and thoracic spine (AP) was up to 16% and 38% higher, respectively than their corresponding DRLs in the European Commission Report RP 180 Part 2. For all exams, except thoracic spine (lat), the third quartiles of the dose area product were higher than the corresponding DRLs in the above report. The source of dose variability between medical centers was related to many parameters such as poor radiographic techniques, lack of modern X-ray machines and adequately documented radiation protection practices. Hydrodynamic Characterization of the Water Table of the Upper Dallol Maouri Basin (Niger)

The Dallol Maouri is a valley of an ancient tributary of the Niger River. The groundwater in this area of the upper Dallol Maouri is little known because of the weakness of the data. The purpose of this study is to make a hydrodynamic characterization of the water table of the upper dallol Maouri by piezometric approach. A piezometric monitoring network made up of 65 points (54 wells and 11 piezometers) was set up. Six (6) piezometric measurement campaigns were carried out from June 2018 to June 2021. The piezometric survey is carried out once every six months: in January and June corresponding respectively to the high and low water period. The results show that the water table is multi-layered in places with a combination of alluvial and Continental Terminal 3 (CT3) aquifers, or perched aquifers and inferoflux aquifers around permanent ponds. The flow direction of the Continental Terminal 3 (CT3) aquifer is mainly North-East/South-West. The piezometric levels are around 219 m (in the South-West part) and 281 m (in the North-East part). The hydraulic gradient varies from 0.018‰ to 1.83‰. The recharge zones are located in the outcrop zone of the aquifer and in the valleys. A lack of recharge is observed on the plateaus despite the presence of numerous permanent ponds. The recharge coefficient in 2020 is estimated at 25% of the total rainfall recorded.

Posterior Urethral Valves without Reflux Associated with Bilateral Renal Cortical Atrophy: Diagnosis and Management

The objective of this case report was to highlight the characteristics of posterior urethral valves observed in a 3-year-old patient and to describe their management. The case of a 4-year-old patient treated for a posterior urethral valve with bilateral renal cortical atrophy was reviewed. The diagnosis was confirmed by abdominal ultrasound, cystography and abdominal CT. He was referred to Martinique (a tertiary health establishment) for effective support. From the acute situation to the fortuitous discovery, its understanding must be deepened because of the potential immediate symptomatic impact in the form of renal colic which can be associated with sepsis, as well as in the long term on renal function. This observation is intended to help the attending physician to initiate his diagnosis and treatment.
