Background: It is y.t a controversy.subject whether low birth weight and infant death are associated to human immunodeficiency.virus-1 infection. Objective: To appreciate association between low birth weights, mother to child HIV transmission and infant mortality.in HIV-1 infected pregnant women delivering between 2011 and 2016. Materials: We conducted 6 y.ars cohort study.in urban Mali. Outcome included preterm delivery. small for gestational age, infant survival status and HIV transmission. Comparison concerned women clinical WHO stage, mother viro-immunological status, and newborn anthropometric parameters. Results: HIV-1 infected women who delivered low birth weight newborn were 20.9% (111/531) versus 16.5% (1910/11.546) in HIV negative patients (p = 0.016). CD4 T cell counts low than 350 T cells count were strongly.associated to LBW (p = 0.000;RR = 3.03;95% CI [1.89 - 3.16]). There is no significant association between ART that was initiated during pregnancy.(p = 0.061, RR = 0.02;CI 95% (1.02 - 1.99)) or during delivery.(p = 0.571;RR = 1.01;CI 95% (0.10 - 3.02)) and LBW delivery. In multivariate analy.is ART regimens containing protease inhibitor (PI) were lone regimens associated with LBW ((p = 0.030;RR = 1.001;95% confidence interval [1.28 - 3.80]). Very.low birth weight was statistically.associated to women HIV infection (adjusted relative risk, 2.02;p = 0.000;95% confidence interval (2.17 - 4.10)). There is no significant difference between mother to child HIV transmission rate in the two HIV-infected pregnant women (10 infected children in group 2: MTCT rate 4.5%) and 3 infected children in group 1 (MTCT rate: 2.7%) (p = 0.56;RR, 0.59;CI 95% (0.18 - 4.39)). In multivariate analy.is, LBW was associated with infant death (p = 0.001;RR = 2.04;CI 95% [1.04 - 5.05]). The median weight of infant at the moment of death in group 1 was 851 g (IQR: 520 - 1833 g). Introduction: Congenital heart disease is an important cause of mortality. chronic morbidity. and disability.in children in poor countries. The objective of this study.was to highlight the diversity.of congenital heart defects in the neonatology.department. Methodology. This work had taken place in a neonatology.service over a period of six months from January.2019 to June 2019. It was a prospective descriptive study.with information collected from the medical records of newborns with congenital heart disease. Results: Of 1478 neonates hospitalized during the study.period, 41 had congenital heart disease, for a hospitalization rate of 2.77%. CHD accounted for 2.77% of neonatal hospitalizations. The sex ratio was 1.73 (26 boy./15 girls). Consanguinity.was found in 19% of cases. The average time to consultation was 11 day.. The main clinical signs were heart murmur (58%), respiratory.distress (56%) and extracardiac malformations (54%). The main cardiac malformations found were atrial septal defect (46%), patent ductus arteriosus (44%), ventricular septal defect (17%), complete atrioventricular canal (15%), and transposition of the great vessels (5%). The case fatality.rate was 29%. Conclusion: Delay.d diagnosis, generally.related to the long delay.in consultation, and lack of surgical management partly.explain this high case fatality.

Secondary.Stress Induced Central Serous Chorioretinopathy.in Bamako: Report of Two Cases

Introduction: Idiopathic central serous chorioretinopathy.(ICSC) is characterized by.the buildup of fluid leading to circumscribed elevation of the retina within the posterior pole. This condition affects y.ung people between the ages of 20 - 50 y.ars. The disease is seen predominantly.in men as compared to women. We report the cases of two (02) patients received in consultation at IOTA-Teaching Hospital between July.2019 and February.2020. Case Presentation: We report two cases of Idiopathic stress central serous chorioretinopathy. The first case is a 37-y.ar-old man that we received in consultation for visual acuity.decrease from a sudden installation for 1 hour. The clinical and paraclinical arguments permitted us to retain the diagnosis of Idiopathic stress central serous chorioretinopathy. The patient benefited from laser treatment and the clinical evolution was good. The second case is a 42-y.ar-old man, who consulted for a sudden reduction of visual acuity.that had been evolving for 3 weeks. The clinical and paraclinical contexts were in favour of the diagnosis of Idiopathic stress central serous chorioretinopathy. The patient was treated with spironolactone. The clinical evolution was favourable. Conclusion: Our study.confirms the existence of central serous chorioretinopathy.(CSC) in Black African patients. Studies show the connection between CSC and stress.

Background:The objective of this work was to evaluate the first results of the radio ablation of the great saphenous vein in the processing of chronic venous insufficiency. Method: This is a monocentric retrospective study.of radio ablation of the great saphenous vein using VNUS Closure® FAST for superficial venous insufficiency.of the great saphenous vein, between January.2012 and December 2014. Results: 42 patients (57 limbs) were operated on with this technology. There were 33 women and 9 men (mean age: 46.5 y.ars). Thirty.nine patients (92. 8%) were sy.ptomatic. Thirty.two patients (76.1%) were classified CEAP 2. The average diameter of the saphenous vein was 8.5 ± 1.5 mm. Three patients (7.1%) had a percutaneous approach. A perfect occlusion of the saphenous vein was observed in all patients. No incident or undesirable event was observed during the procedure. One case of delay.d deep-vein thrombosis was observed. At one month of follow-up, six patients (14.2%) had induration opposite to the occluded vein. Forty.three percent of the patients stated they were very satisfied with the procedure. Conclusion: The effectiveness, security and simplicity of the closure method are real. More studies are necessary to assess the long-term results of this procedure.

Neonatal surgical pathologies in developing countries are characterized by.high mortality. The aim of this work was to describe the characteristics of the newborn and its surgical pathologies treated in the neonatal department of the Gabriel Touré University.Hospital in Bamako (Mali). Patients and method: This is a retrospective study.of 626 files of newborns hospitalized over a period of 3 y.ars (January.2016 to December 2018) for a surgical pathology. whether operated on or not. We have studied the etiological, therapeutic and evolutionary.aspects. Results: During the study.period, we collected 626 patients who had surgical pathology. i.e. a frequency.of 5.35%. The average therapeutic consultation time was 2 day. with extremes (1 and 30 day.). Pathologies of the abdominal wall and digestive tract represented 57.9% (n = 84) followed by.pathologies of the spine 26.9% (n = 39). The mean treatment time was 4.5 day.. Twenty.three point sixteen (23.16%) newborns could be operated on. The neonatal mortality.rate was 12.4% (n = 18). Mortality.was postoperative (100%) with a mean time to onset of 5 day.. Abdominal wall pathologies were responsible for 8.7% (n = 13) of deaths. Conclusion: Surgical pathologies of the newborn are characterized by.delay.d treatment with high mortality. Improving their prognosis requires early.diagnosis (prenatal diagnosis) and adequate management.

Consent and Quality.of Information for Patients in Laparoscopic Surgery.at the Academic Hospital Point“G”(Mali)

Objectives: To evaluate the quality.of the information provided to patients for their consent to be operated laparoscopically.in the “A” surgery.service of Point “G” University.Hospital in Bamako (Mali). Methodology. The study.was transversal, descriptive and prospective from November 1, 2016 to December 31, 2017 (14 months). It was performed at the general and laparoscopic surgery.department of the Point “G” University.Hospital in Bamako. Laparoscopically.operated patients in scheduled or emergency.programm whom consented the study.and answered the questions before and after the surgical procedure were included. Results: One hundred patients were retained. The sex ratio was 4.2 for women. The surgery.was scheduled in 93% (programmed surgery. of cases versus 7% (urgent surgery.. Gy.ecological and obstetric surgery.was performed in 54%, digestive surgery.in 43% and laparoscopic exploration in 3%. The information was provided by.surgeons, anesthetists, fellowship and students in respec-tively.63%, 6%, 11% and 9%. The reflection period before consenting to the act was greater or equal to 3 day. in 90%. The media used were found to be suitable in 62%. The information was considered satisfactory.preoperatively.in 54% and postoperatively.in 88%. Patients wanted additional information on post-surgery.outcomes in 21%. Conclusion: The establishment of an information sy.tem and a free and informed consent form is a prerequisite for the quality.of care in laparoscopic surgery.

Acute Pneumonia Characteristics in Children under Five y.ars of Age in Bamako, Mali

Pneumonia is the most common cause of mortality.in child under five y.ars of age. The objective of the study.was to assess socio-demographic and clinical characteristics of children under 5 hospitalized for pneumonia. Material and method: We conducted a 6 months prospective study.from June 1st, 2016 to December 31st, 2016 in the general pediatric service in the Pediatric Department of Gabriel Touré Teaching Hospital in Bamako, the capital city.of Mali. Result: We have selected 63 cases of pneumonia according to our definition criteria, i.e. 2.2% of hospitalizations for children aged 1 to 59 months. The average age was 14 months. Infants under 2 y.ars accounted for 82.53%. The sex ratio was 1.2. Seventy.eight percent came from an unfavorable socio-economic background. The majority.of mothers were uneducated (71.42%). Breastfeeding was exclusive up to 6 months in 50.79% of patients. Vaccination according to the national program was not up to date in 27% of patients. The average consultation time was 18 day.. On admission, 81% of the patients had a fever, 93.64% had a tachy.nea and 58.73% had crackling rales at pulmonary.auscultation. Hy.oxemia was present in 58.73%. Severe anemia was present in 79.36%. Radiologically. opacity.was found in 42 patients (66.66%). The blood culture was positive in 8.3%. Beta-lactams were first-line prescribed in all patients. The case fatality.rate was 9.52%. The factors associated with mortality.were age less than 14 months (p = 0.08), adverse socio-economic conditions (p = 0.0003) and the presence of hy.oxemia at the entrance (p = 0.01). Conclusion: Pneumonia remains major cause of morbidity.and mortality.in our context. Emphasis should be put on preventive measures.

Hematological Profile of Newborns Hospitalized for Neonatal Bacterial Infection in the Neonatology.of the Pediatric Department of Gabriel Toure Teaching Hospital Bamako, Mali

The blood count is an easily.achievable routine exam and will it have specifics in the event of a neonatal bacterial infection? Hence, the present study.with the objective of determining the profile of the hemogram of newborns hospitalized for early.bacterial neonatal infection. Material and methods: This was a cross-sectional study.that took place from June 27 to September 03, 2016 in the neonatology.department of teaching hospital Gabriel Toure. Included were all neonates hospitalized for early.neonatal bacterial infection (ENBI) and who had a blood count. Results: We included 227 patients, 64.8% of whom were premature. The sex ratio was 1.4. The infants were less than 24 hours old in 93.6% of the cases. The mean hemoglobin level was 16.435 g/dl [8.8 - 22.26]. Ery.hrocy.openia was found in 18.5% of cases. Anemia was present in 17% of newborns. The average leukocy.e was 15.228·103/mm3 [1.4 - 72]. Hy.erleukocy.osis and leukopenia were found in 12.32% and 6.6% respectively. Neutropenia and ly.phopenia were present in 14.5% and 30.8%. There was a correlation between leukocy.osis of negative blood cultures (23/27) (p = 0.030). For Neutrophils, neutrophilia was more observed in term neonates and neutropenia in premature infants (p = 0.03). Monocy.osis was present in 13.6% of cases. One quarter (25.5%) of newborns had thrombocy.openia. Conclusion: Hematological variations did not allow a specific profile of newborns hospitalized for early.neonatal bacterial infection to be identified.
