Progression of Minimally Invasive Urological surgery at Hôpital Général Idrissa Pouye in Dakar in 20 Years of Practice

Introduction: Open surgery is gradually being supplanted by minimally invasive surgical techniques worldwide. Our study aimed to describe the place of minimally invasive surgery at the Hôpital Général Idrissa Pouye (HOGIP) in Dakar. Materials and Methods: This is a descriptive cross-sectional study over a 20-years period from 1sup>stsup> June 2000 to 31sup>stsup> December 2021 in the urology department of HOGIP in Dakar. The list of all surgical procedures performed was computed. We evaluated the distribution of minimally invasive procedures (endoscopy, PCNL, laparoscopy) overall and over the years. We made calculations of proportions and statistical significance was considered for alpha = 0.05. Results: In 20 years, 14,855 surgical procedures were performed, of which 5344 (36%) were minimally invasive surgeries. The average age of men was 53.19 years (standard deviation: 21.77) vs 47.32 years (standard deviation: 18.43) for women. Minimally invasive procedures in the lower urinary tract accounted for 71.93% and involved 1033 cystoscopies (26.87%), 1020 Trans Urethral Resection of Prostate (TURP) (26.53%), 931 Direct Vision Internal Urethrotomy (DVIU) (24.21%, and 612 Trans Urethral Resection of the Bladder (TURB) (15.92%). In the upper urinary tract, 1461 (28.07%) minimally invasive procedures were performed, including PCNL in 193 cases (3.61%), laser endopyelotomy in 104 cases (1.95%), ureteroscopy in 486 cases (1.7%) and laparoscopy in 39 cases (0.46%). The proportion of minimally invasive surgery has gradually increased. Among the 5344 minimally invasive procedures, 333 (5.23%) were performed in 2000-2004 and 2332 (43.63%) in 2015-2019. Conclusion: Minimally invasive surgery represents an important part of the activity of the HOGIP urology department. Its development has improved over the years offering its advantages to the patients.

strong>Background:strong> Many patients who have had radical prostatectomy for prostate cancer may present with microscopic extraprostatic extension of the disease. Positive surgical margins are a common pathological finding in this subgroup of patients. To report the epidemiological, clinical and therapeutic aspects of PsM after radical prostatectomy (RP) and to evaluate the follow-up of patients. strong>Patients and methods:strong> A single-center retrospective descriptive study of patients who underwent radical prostatectomy between June 1, 2004 and December 31, 2019 was conducted. Patients who had radical prostatectomy with PsM on pathology report were included. The parameters studied were age, initial prostate specific antigen (PsA), Gleason and International society of Uropathology (IsUP) scores, cTNM and pTNM stages, operative technique, PsA levels after surgery, adjuvant treatment and patient survival. strong>Results:strong> Eighty-six (86) radical prostatectomies were performed. PsM was found in 23 patients (26.7%). The mean age of the patients was 63.7 ± 6.1 years. The mean preoperative total PsA was 31.5 ng/mL (6.31 - 146 ng/mL). Prostate biopsy showed only prostatic adenocarcinoma. Thoracic-abdominopelvic CT was performed in all patients. Prostate cancers were found at the localized stage in 12 patients and locally advanced in 11 patients. A classification adjustment was obtained after pathological examination of the surgical specimen. The IsUP score 3 and 1 on the surgical specimen were in the majority with 9 and 7 patients respectively. After the recurrence, all patients who consented received hormone therapy, which was either medical with Goserelin and Triptorelin (7 patients) or surgical with testicular pulpectomy (1 patient). Wave Interactions with Multiple semi-Immersed Jarlan-Type Perforated Breakwaters

This study examines wave interactions with multiple semi-immersed Jarlan-type perforated breakwaters. A numerical model based on linear wave theory and an eigenfunction expansion method has been developed to study the hydrodynamic characteristics of breakwaters. The numerical results show a good agreement with previous analytical results and experimental data for limiting cases of double partially immersed impermeable walls and double and triple Jarlan-type breakwaters. The wave transmission coefficient Cv; reflection coefficient CR, and energy dissipation coefficient CE coefficients and the horizontal wave force exerted on the front and rear walls are examined. The results show that CR reaches the maximum value when B/L = 0.46n while it is smallest when B/L=0.46n＋0.24 （n=0, 1, 2,...）. An economical triple semi-immersed Jarlan-type perforated breakwater can be designed with B/L = 0.25 and CR and CT ranging from 0.25 to 0.32 by choosing a relative draft d/h of 0.35 and a permeability parameter of the perforated front walls being 0.5 for an incident wave number kh nearly equal to 2.0. The triple semi-immersed Jarlan-type perforated breakwaters with significantly reduced CR, will enhance the structure＇s wave absorption ability, and lead to smaller wave forces compared with the double one. The proposed model may be used to predict the response of a structure in the preliminary design stage for practical engineering.

The performance of activated carbon/NiFe_(2)O_(4) magnetic composite to retain heavy metal ions from aqueous solution

Allometric models for estimating aboveground biomass and carbon in Faidherbia albida and Prosopis africana under agroforestry parklands in drylands of Niger

This study developed allometric models to estimate aboveground biomass and carbon of Prosopis africana and Faidherbia albida. The destructive method was used with a sample of 20 trees per species for the two parkland sites. Linear regression with log transformation was used to model aboveground biomass according to dendrometric parameters. Error analysis, including mean absolute percentage of error(MAPE) and root mean square of error(RMsE), was used to select and validate the models for both species. Model 1(biomass according to tree diameter) for P. africana and F. albida were considered more representative. The statistical parameters of these models were R2 = 0.99, MAPE 0.98% and RMsE1.75% for P. africana, and R2 = 0.99, MAPE 1.19%,RMsE 2.37% for F. albida. The average rate of carbon sequestered was significantly different for the two species(P ≤ 0.05). The total amount sequestered per tree averaged0.17 × 10-3 Mg for P. africana and 0.25 × 10-3 Mg for F. albida. These results could be used to develop policies that would lead to the sustainable management of these resources in the dry parklands of Niger.

The most abundant phenolic biopolymer in the biosphere is the *** phenolic biopolymer commonly exists in combination with polysaccharides and other cell wall *** this study,the solvent system dioxane-water is used to extract lignin,which is considered as unaltered native *** dioxane lignin extracted from fig stems was characterized regarding to its structural feature,quantification of its functional groups,molecular weight,and evaluation of its thermal *** and molecular weight distribution of the studied lignin indicated that isolated lignin contained a low amount of sugar(c.a.19%)and had a high weight-average molecular weight(10068 ***-1).Lignin sample had approximately the same amounts of guaiacyl(G)and p-hydroxyphenyl(H)units with relatively fewer syringyl(s)*** isolated lignin revealed good antioxidant ***,it proved to have a high potential of application in new antioxidants formulations.
