Modeling a Local Geoid: Application in Thies, Senegal

Many applications in geodesy, hydrography and engineering require determining heights linked to the geoid. Direct leveling, which is the traditional method of obtaining these elevations, is slow, time consuming and expensive. The contribution of space techniques can make it possible to overcome these constraints provided that we have a precision geoid model compatible with that obtained by the GNSS method. There are today relatively precise regional geoid models, at least outside of mountain ranges, in all developed countries, which is not yet the case in developing countries like Senegal. An alternative is to use local models restricted to a small area. Thus, this study aims to produce a geoid model by combining multi-source data for the city of Thies intended mainly to support leveling operations by GNSS. To achieve this objective, direct precision leveling and GNSS leveling (static mode) were carried out covering the study area. The reference points used are, among others, those of the RRS04 (Reference Network of Senegal 2004) and the NGAO53 (General Leveling of West Africa 1953). Additionally, gravimetric measurements were conducted using the Sensor Play-Data Recorder application. The calculation of the model was carried out by the SRBF (Spherical Radial Basis Function) method using the PAGravf4.5 software. The SRBF method uses EGM08 to first calculate height and gravity anomalies. These are then compared with the raw data in order to determine the residuals which will allow the model to be refined. In order to validate our model, control points (GNSS/leveled) were chosen based on a homogeneous geographical distribution in the area in order to evaluate their altitude. An accuracy of less than 2 cm was obtained. Comparing our model with the existing local model GGSV12v1 shows that our model is more accurate.

Relationship between Circulating Plasma Galectin-3 Levels and T-Cell Activation during Cervical Cancer Chemotherapy

Objective: Despite the existence of several therapeutic strategies, the management of cervical cancer remains challenging. Our region has very little data on the interaction between the immune system and the clinical response to chemotherapy. This work examines plasma levels of galectin-3 (Gal-3) and percentages of activated T cells in patients with cervical cancer treated with chemotherapy and investigates if there is a relationship between the rates of these two elements. Methods: We compared data from 37 patients with cervical cancer undergoing chemotherapy and 42 controls with normal cervical cytology. Plasma Gal-3 concentrations were assessed by ELISA and expression of activation markers by T cells (CD69 and HLA-DR) was assessed by flow cytometry at three different time points during chemotherapy. Results: Our results showed that patients had a significantly higher concentration of Gal-3 compared to controls (4.025 vs. 1.340, p 0.001), similarly, they had a significantly high percentage of activated lymphocytes (2.610 vs. 0.731;p 0.0001). According to the response to treatment, patients with no response to treatment had a lower concentration of circulating Gal-3 but had approximately the same percentage of activated CD4 and CD8 lymphocytes as patients with a partial or total response. In addition, we found a positive correlation between the Gal-3 level and CD4 T cells expressing the activation marker CD69 (p 0.05;rho = 0.44). Conclusion: In conclusion, our results show that there would be a relationship between circulating galectin-3 and the percentage of peripheral CD4+CD69+ cells in cervical cancer.

Lithostructural Control of the Gold Mineralization of the Douta Permit of African Star Resources (Kédougou-Kéniéba Inlier, Southeastern Senegal)

The Douta permit of African Star Resources/Thor Explorations, located in the southeast of Senegal, in the Kédougou-Kéniéba Inlier (western part of the West African Craton), is dominated to the East by metasedimentary formations such as greywackes, shales, graphitic shales, quartzites, cherts, claystones and breccias characteristic of the Dialé-Daléma basin. To the West, the mafic formations of the Mako volcanic belt are the most common. Metasedimentary rocks are associated with metavolcanosedimentary terms found at the contact zones between the two (2) Birimian groups. These different geological formations are cut by mafic dolerite and gabbro sills and/or dykes, as well as quartz and microgranite veins. The Douta gold project is crossed from North to South by the MTZ (Main Transcurrent Zone), generally oriented NE-SW and becoming N-S towards the North. The permit is characterized by several shear corridors. The rocks are affected by brittle, brittle-ductile to ductile deformations. The gold mineralization is hosted by a NE-trending shear corridor called the Makosa corridor (Makosa shear zone), therefore sub-parallel to the MTZ. It has a subvertical dip (75˚ to 85˚ to the NW). It is associated with a hydrothermal phase characterized by quartz-sericite-epidote-fine, disseminated pyrite and arsenopyrite ± albite ± chlorite paragenesis. These minerals testify to the existence of a low degree of metamorphism (greenschist facies, epizonal domain) in the area. However, metamorphism reaches amphibolite facies in some places, particularly in the vicinity of intrusive bodies, with the presence of hornblende (amphiboles) and plagioclase. The gold mineralization is mainly hosted by two (2) metasedimentary lithological units: meta-greywackes and shales.

Hepatic Cirrhosis: Diagnostic and Prognosis in Internal Medicine

Introduction: Cirrhosis is a public health problem and the causes are dominated by viral hepatitis and alcoholism. Deaths due to cirrhosis represented 2.4% of deaths worldwide in 2017. The aim of this work was to study the diagnostic and prognosis of cirrhosis at the department of internal medicine of the Abass Ndao hospital center. Patients and Methods: This is a retrospective, descriptive study, carried out on the basis of patient files followed from May 1, 1999 to February 28, 2018 (19 years) and included all patients for whom the diagnosis of cirrhosis was accepted. The diagnosis of cirrhosis was made on the basis of clinical, biological and ultrasound (portal hypertension, signs of hepatocellular insufficiency, a diffuse heterogeneous aspect, and irregularity of the liver contours). Results: 60 patient files were listed with an average age of 46.9 years, a sex ratio of 2.2 and alcoholism in 11.7% of the cases. The reasons for consultation were dominated by an increase in the size of the abdomen (55%), lose weight (50%). On physical examination, it was a hepatomegaly (43.3%) and jaundices (33.3%). Exploration of liver function showed a cholestasis (48%), a cytolysis (58%) and a hepatocellular insufficiency (46.2%). Anemia was found in (24%). The abdominal ultrasound found in all cases a dysmorphic, heterogeneous liver with irregular contours. Hepatic atrophy was found in 20%. Among the 34 patients who underwent upper gastrointestinal endoscopy, they were grade 2 to 3 esophageal varicose vein in 67.6%, an erosive gastropathy in 29.4%. The etiology was viral hepatitis B in 45%, alcoholism in 11.7% and undetermined in 43.3%. A hematemesis complicated the evolution in 16.7%. Carcinomatous degeneration was found in 5% and 16.7% of the patients had died. Conclusion: our experience with cirrhosis was marked by advanced forms at the stage of complications. Analysis of Maternal Deaths in Suburban of Dakar, Senegal

Objective: Maternal mortality remains a major concern in developing countries. This survey aims to suggest strategic plans that would help decrease maternal and perinatal mortality in the suburbs. It is a descriptive study that shows the different causes of maternal deaths during pregnancy and puerperium period. Methods: It is a retrospective descriptive study done between January 1st 2016 and the 31st of December 2018. We have collected information on all deaths due to mortality issues in Five (5) maternity clinics around Dakar. The record shows that teach centre have an average of 4000 deliveries per year. The data collected from the hospital records were inputted using Sphinx software (version 5). These data were analysed using Epi Info software (version 3.5). After analysing these data, recommendations were made to minimize the different causes of maternal deaths. Results: We recorded 154 maternal deaths out of 32,420 live births. The direct causes of maternal deaths were a result of preeclampsia and its complications (31.2%), postpartum haemorrhage (24.7%), abruption placentae (20.8%) and obstructed labour (7.8%). The indirect causes were mainly sickle cell disease (1.3%), heart disease (1.3%) and gynaecological cancers (1.3%). Maternal deaths were also associated with fetal loss in 47.4% of 153 maternal deaths, need for blood transfusion 59% while none of the reference structures in the suburbs of Dakar has a blood bank;with a need for admission in intensive care unit was noted as 40%.

Dysthyroidism in Elderly Subjects

Introduction: Aging is associated with an anatomical and functional heterogeneity of the thyroid which varies according to the iodized status. The aim was to describe the clinical, etiology and therapeutic profile of dysthyroidism in elderly subjects in the internal medicine at the Abass Ndao University Hospital. Patients and methods: This was a retrospective, descriptive and analytical study, including all subjects aged 60 and over followed from January 1, 2010 to December 31, 2019 (10 years) for thyroid disease. Results: 371 Patients were collected with a prevalence of 3.8%. The sex ratio was 0.15 and the mean age was 65.3 years. The circumstances of discovery of the disease were a grade 2 - 3 goiter (62.5%), exophthalmos (24.5%), thyrotoxicosis (56.6%), a symptom of low metabolism (5.4%), a cervical compression (10.8% including 8% of dysphasia), and a cardiothyreosis (9.4%). It was hyperthyroidism (65.2%), and hypothyroidism (7.2%). The main etiologies were toxic multinodular goiter (33.2%), Graves’s disease (29.6%), euthyroidism nodular goiter (26.8%), toxic adenoma (2.4%), and Hashimoto’s thyroiditis (6.7%). The compressive manifestations were exclusive of nodular goiter. Among the 35 cases of cardiothyreosis, there was a rhythm and conduction disorder in 25 cases (6.7%) and the underlying thyreopathy was a toxic nodular goiter in 57.1%. Thyroidectomy involved 44.7% of patients, including 32.6% of Graves’ disease, 48.3% of toxic nodular goiter, and 61.8% of nodular goiter in euthyroidism. We found 02 cases of papillary carcinomas on multinodular goiter. Conclusion: In our hospital series, there is variability in the clinical manifestations of dysthyroidism in the elderly. Complications, mainly cardiac and compressive, remain a major reason for consultation. Toxic nodular goiter is preponderant and its management, especially radical, must be multidisciplinary and according to the profile.

From Lysis to Hemolysis

Introduction: Sickle cell disease (SCD) is the most common genetic disease in the world. Vitamin D deficiency has been described in several patients with this disease. We report the case of a patient in whom osteomalacia revealed the disease. Observation: The patient was 35 years old, not an alcoholic, not exposed to toxic products, with a family history of sickle cell disease in her 4 brothers and sisters. She reports the appearance of diffuse bone pain but which predominates in the axial skeleton, the ribs and the rhizomelic zones having motivated several hospitalizations. These pains are recurrent with new attacks on the lumbar spine, and sacroiliac joints with intense pain. Paraclinical examinations revealed: normal long bone radiographs without bone lysis, a CT scan of the sacroiliac joints without any aspect of sacroiliitis, but revealing multiple vertebral condensing bone lesions in streaks, diffuse at the level of the iliac wings with osteolysis surrounded by non aggressive osteocondensation. Densitometry was normal, vitamin D deficiency was noted at 29 ng/mL with elevated alkaline phosphatase, normal concentrations of Calcemia and Phosphoremia respectively at 96 mg/L and 36 mg/L, contrasting with hyperparathyroidism with a blood level of parathyroid hormone elevated to twice the normal level at 104.4 pg/mL It should be noted that the patient had received per os vitamin D supplementation before coming to us. The diagnosis of osteomalacia secondary to vitamin D deficiency complicated by secondary hyperparathyroidism was retained. Given the family history, we looked for the existence of sickle cell disease which was finally confirmed on hemoglobin electrophoresis with an AS profile. Conclusion: The association between vitamin D deficiency and sickle cell disease is not uncommon, and is explained by the ethnic origin, race, skin color, genetics and physiological features of patients with this disease rather than the disease itself.

Purpose: In Sub-Saharan Africa, an important proportion of incident HIV cases occur among heterosexual serodiscordant couples (HSDC) but the majority of HIV negative partners can remain seronegative. These are called HIV-exposed seronegative (HESN). We aimed to compare immune activation (IA) levels between HESN, their HIV infected counterparts (HIV+ partners) and HIV unexposed uninfected individuals (HIV-neg Controls) and to evaluate the association between sexual exposure to HIV (SEHIV) and IA. Methods: We conducted a cross-sectional study in Dakar, Senegal on 148 participants recruited between November 2013 and February 2014: 40 HIV-neg Controls, 54 HESN and 54 HIV+ Partners. SEHIV was evaluated individually using questionnaires. IA level was measured by plasma level of β2-microglobulin (β2m). Logistic regression was used to estimate odds ratios (OR) and 95% confidence intervals (CI) for the different associations. Results: The median levels of β2m were 1.57 mg/l (IQR: 1.37 - 1.77), 2.14 mg/l (IQR: 1.76 - 2.43) and 2.24 mg/l (IQR: 1.80 - 3.17) for HIV-neg Controls, HESN and HIV+ partners, respectively. After adjustment, HESN had similar levels of IA with HIV+ partners but significantly higher than that of HIV-neg Controls (adjusted OR = 6.28;95% CI: [2.19 - 18.00]). The association between IA and SEHIV was evaluated in the HIV negative individuals. High frequency of SEHIV was associated with a β2m > 2.2 mg/l (OR = 6.56;95% CI: [1.71 - 25.21]);significantly more than median cut off value of >1.81 mg/l. Conclusions: Our study shows that, despite being uninfected with HIV, HESN individuals show a high level of IA, which was depended on the level of SEHIV.

Diabetes Mellitus and COVID-19 at Abass Ndao Hospital Epidemic Treatment Center (ETC)

Introduction: SARS-COV2 infection is a major public health problem and diabetes mellitus is a serious factor. The objective was to determine the characteristics of COVID-19 in diabetics at the Epidemic Treatment Center (ETC) of the Abass Ndao Hospital Center. Observation: This was a retrospective, descriptive and analytical study carried out at Abass Ndao Hospital from March 01, 2021 to September 31, 2021. We evaluated the epidemiological, clinical and evolutionary characteristics. Results: Among 333 patients infected with COVID-19, 203 were diabetic (60.96%). The sex ratio was 0.70 and the mean age 60.18 years. Diabetes was type 2 (84.2%) and inaugural (15.8%). Arterial hypertension was the main comorbidity (86.96% of cases). The RT-PCR test was positive in 86.21% of cases. The main clinical signs were: asthenia (90.14%), dyspnea (78.81%), cough (52.21%) and body aches (52.21%). Oxygen saturation was alanced in 71.43% with an average blood sugar of 2.02 ± 4 g/l. At the paraclinical, the D dimer was ≥1000 ng/l (71.52%) and the C-reactive protein ≥100 mg/l (32.01%). Chest CT scan found mild to moderate (55.17%), severe (21.67%) and critical (17.24%) lung lesions. Pulmonary embolism was found in 4 diabetics. The treatment of diabetes was represented by insulin therapy (85.82%) and oral antidiabetics (24.11%). Anti-COVID-19 treatment was dominated by azithromycin (89.11%), corticosteroid therapy (92.11%), anticoagulants (99%), hydroxychloroquine (21.29%). Mortality was found in 14.36% (n = 29) and significantly correlated with age ≥ 60 years (OR = 3.013 [1.266 - 7.173], P = 0.005), short duration of hospitalization (OR = 3.154 [1.149 - 8.663], P = 0.009), C-reactive protein > 100 mg/l (OR = 6.370 [2.704 - 15.006], P < 0.001), lung saturation < 90% (OR = 6.331 [2.633 - 15.222], P [3.367 - 18.322], P Conclusion: Diabetes is one of the most important comorbidities linked to the severity of SARS-CoV-2 infection. A Hospital-Based Cross-Sectional Study Assessing the Relation between Time of Birth and Perinatal Outcome

Objectives:?Investigating the relation between perinatal outcomes and?hospital working shifts.?Methods:?We conducted a cross-sectional study at Philippe Maguilen Senghor health center (PMSHC) in Dakar, Senegal from January, 1st?2011 to December, 31th 2018. The study population was comprised of all mothers who had delivered at PMSHC and their newborns after completing 22 weeks of gestation. Time of delivery was?divided into three periods of working hours: morning shift (deliveries occurred between 7 am and 4:59 pm);evening shift from 5 pm to 10:59 pm and night shift from 11?pm to 6:59 am.?Maternal outcomes were assessed by mode of delivery, epsisotomy and perineal injuries.?The Apgar scoring system was used to assess newborns at first minute after they were born. Other adverse perinatal outcomes included fresh stillbirth, neonatal referral and early neonatal death. Data were analyzed using Statistical Package for Social Science software (SPSS 24, Mac version).?Results:?A total of 48,270 mothers and their newborns met eligibility criteria. Caesarean section deliveries were less likely to occur during evening (OR 0.84, 95% CI;0.79?-?0.89, p = 0.001) and night shifts (OR 0.45, CI;0.47?-?0.53, p = 0.001).?Evening shift deliveries had 1.1 the odds of poor perinatal outcome (Apgar score ?- 1.18, p = 0.012). No significant difference was found in the odds of neonate referrals and deaths across the three shifts.?Night shift deliveries had 1.1 the odds of perineal injuries compared to morning shift deliveries (OR 1.11, 95% CI;1.04?- 1.18, p = 0.001, for episiotomy and OR, 1.14;95% CI, 1.04?- 1.26, p = 0.008, for perineal tears). Conclusion:?Off-hours deliveries, particularly during the night shift, were significantly associated with higher proportions of perineal injuries compared to morning shift.?However, no significant difference was found in the odds of neonate referrals and deaths across the three shifts.?Our findings suggest to set up a Neonatology unit at the CSPMS as
