Objective: To describe diagnostic and therapeutic outcomes of infected vaginal *** and Methods: This is a retrospective descriptive study over a period of two years from June 2013 to May 2015 at the urological emergency department of Yalgado Ouédraogo University Hospital (CHU-YO). Results: We collected seven cases during the period of study. The average age of patients was 35 years. Two patients (2/7) were from urban and five from rural areas. Five patients (5/7) were married and two (2/7) unmarried. Six patients (6/7) were farmers and one (1/7) was civil servent. Clinical examination findings were dominated by bilateral painful scrotal swelling and fever in all cases, a scrotal necrosis in two cases. The dominant indication of the puncture-aspiration was discomfort during sexual intercourse (5/7). At the opening of the scrotum purulent fluid was aspirated in all cases, and we performed a unilateral orchidectomy in two cases. A hydrocelectomy was performed in all cases. The postoperative evolution was simple in all patients. Conclusion: The puncture-aspiration must be avoided because the risk of infection and the high recurrence rate.

Trauma of External Genitalia by Firearm: Therapeutic Strategy and Result, about a Case at CHU-YO, Ouagadougou, Burkina Faso

Introduction: Ballistic traumas of the external genitalia are rare especially in civil practice. Emergency surgical exploration is the rule. Case Report: Mr. F.Y., 29 years old, a security agent, admitted to the Urological Emergency Unit of Yalgado Ouédraogo University Hospital of Ouagadougou for trauma of the penile and left purse by firearm. The clinical examination revealed a good hemodynamic status, bladder distension. A suprapubic catheterization was performed. We noted a transfixing and bleeding wound on the penile, and on the left purse which was edematous. The patient was admitted to the operation room 07 hours after trauma. Exploration found a total destruction of the left testicle classified grade 5 of the classification of the American Association of the Surgery of Trauma (AAST). We performed a left orchiectomy. There was a partial rupture of the ventral face of the penile urethra. We performed an end-to-end anastomosis on a Foley catheter CH 18. The Foley catheter was removed after two weeks. Urination was spontaneous without dysuria. Erections were painless and without kinking of penile. Conclusion: Open traumas of the external genitalia are rare and their treatment is an extreme surgical emergency.

Acute Generalized Peritonitis Due to the Migration of a Ballpoint Pen from Bladder to Great Peritoneal Cavity: A Case Report

Foreign bodies of the bladder are rares. The presence of a foreign body in the bladder is often anecdotic. We report a case of acute generalized peritonitis due to migration of a ballpoint pen from bladder to great peritoneal cavity in a 17-year-old man. He was admitted for a peritoneal syndrome after introducing a ballpoint pen into the urethra to masturbation purposes. He has no psychiatric history. Exploration had found an agglutination of small bowel loops around the ballpoint pen. The ballpoint pen was about 13.5 cm long and was transfixing the small intestine. We had noted about 20 cm of ileal necrosis about 30 cm from the ileocecal junction. An exit port was located at the posterior wall of the bladder. We performed a resection of the necrotic portion followed by end-to-end anastomosis. Bladder foreign bodies can have serious complications such as the formation of an acute surgical abdomen after migration of the foreign body. The extraction must be quickly carried as soon as the diagnosis is made to avoid these complications.

Suprapubic Cystostomy at Urological Emergency Unit of CHU-YO, Ouagadougou, Burkina Faso: Indications, Technique and Complications

Introduction: Cystostomy allows under certain circumstances deriving the urine. This is the most performed surgery in uro-andrological emergencies. Complications can occur. We aim to analyze the indications, technical aspects and complications of suprapubic cystostomies in the urological emergency unit at CHU-YO. Patients and Methods: Between October 2013 and May 2014 we conducted a prospective study of all patients who underwent suprapubic cystostomy. Results: 86 cases of cystostomy were noted or 77.5% of surgical procedures performed in uro-andrological emergency. All patients were male. The average age of patients was 52.5 years. The AUR represented 88.4% of indications. The prostate tumor represented 65.8% of the etiologies of AUR. The cystostomy was performed under local anesthesia in 87.2% of cases. Peritoneal gap was noted in 5.8%, parietal suppuration in 12.8%. A death by generalized acute peritonitis was noted in one case. Conclusion: Suprapubic cystostomy presents a non-negligible morbidity and mortality and must be mastered.

Introduction: Self-mutilation of the external genitalia is rare. It occurs most often on a psychotic ground. Case Report: Mr. P.O. 25-year-old, a dress maker, admitted to the urological emergency for self-section of the penile. The patient had cut his penile with a razor blade and the amputated distal extremity had been thrown in the water closet. The patient was admitted 19 hours after the act of self-harm. Clinical examination had found a stable hemodynamic status, a section of the penile at its root. Psychiatrically schizophrenia evolving for 08 months was diagnosed. An attempt of penile reconstruction was performed. The patient benefited serum and tetanus vaccine, antibiotic and neuroleptics. The parents asked to go home against medical advice the 5th day. Conclusion: The management of self-mutilation of external genitalia is multidisciplinary including surgery and psychiatry.

Factors Associated with Non-Adherence to Treatment in Sickle Cell Patients Monitored at the National Reference Center for Sickle Cell Disease in Niger

Introduction: Sickle cell disease is a real public health problem in the world and particularly in Niger where the prevalence of the S gene is estimated at 25% and that of the homozygous forms at between 1% and 2%. Treatment combines quarterly follow-up of patients and management of complications. The objective of this study was to identify the potential explanatory factors of non-adherence to treatment in sickle cell patients followed at the national reference center for sickle cell disease in Niger. Methods: This is a cross-sectional study of sickle cell cases followed at the CNRD in Niger. The population consisted of all sickle cell patients followed in this center in 2021. The data collection techniques were individual interviews and documentary reviews. Non-adherence was assessed with the Girerd test. Descriptive statistical tests and simple and multiple logistic regression models were performed. Results: A total of 368 patients were enrolled. The median age is 7 years (4;10) and the sex ratio is 1.04. Ninety-eight (98) or 26.6% were compliant and 270 (73.4%) were non-compliant. In multivariate analysis, the factors independently and negatively associated with non-adherence to treatment were schooling (adjusted OR [95% CI], p-value), 0.17 [0.10 - 0.30];p Conclusion: The factors influencing treatment compliance identified in this study are all modifiable. To prevent the complications of sickle cell disease, we must fight against ignorance, make care services accessible and make care free.

Delivery on Scarred Uterus at Souro Sanou Teaching Hospital, Burkina Faso (about 531 Cases)

Introduction: Therapeutic conduct for delivery via a scarred uterus is controversial in modern obstetrics. Some authors recommend a ceasarean section. The purpose of this study was to analyse the conduct and prognosis of delivery via scarred uterus at Souro Sanou Teaching Hospital in Bobo-Dioulasso. Methods: We conducted a descriptive cross-sectional study from January 1 to December 31, 2017. Data were collected from medical, birth, and operating room records. Result: In total, 531 scared uterus deliveries and 5293 deliveries have been recorded in our study;the frequency of deliveries via a scarred uterus was 9.96%. The average age of the patients was 28.02 years old, with extremes of 17 and 44 years. The average parity was 2.34, with extremes of 1 and 8. Patients with a spacing interval between births of at least 24 months accounted for 86.6% of observed patients. Patients with a single scar uterus made up 70.6% of the population. There were 349 (65.73%) patients who had an emergency caesarean section during a previous delivery. The trial of vaginal delivery via a scarred uterus was conducted on 182 patients with a success rate of 89.56%. There was no maternal death. However, we noted 23 foetal deaths (4.33%). Conclusion: More than 50% of parturient women with a single caesarean uterine scar who underwent the uterine test gave birth vaginally in our department. However, like most previous studies on the subject, we recommend vaginal delivery in the presence of a prior caesarean-scarred uterus whenever possible.

Frequency and Factors Associated with Depression in Rheumatoid Arthritis in African Black Patients: Case-Control Study

Objective: To study the frequency and factors associated with depression in rheumatoid arthritis (RA) among Black African patients. Patients and methods: It was a case-control study with a group of patients with RA (case) and a group of healthy subjects without rheumatoid arthritis (con-trols) matched by age and sex. All patients admitted to the rheumatologic department during February 2015 to July 2015 for rheumatoid arthritis meeting the criteria ACR/EULAR 2010, were included. Patients with high blood pressure, diabetes mellitus, renal failure or HIV infection have been excluded. Depression was assessed by the Hamilton Rating Scale for Depression. Results: Fifty patients with RA were included and compared to 100 healthy controls. For RA patients, it was 39 (78%) female and 11 (22%) male with a sex ratio of 0.28. In the control group, it was 77 (77%) female and 23 (23%) male with a sex ratio of 0.29. Twenty-seven (54%) patients with RA had depression compared with 17 (17%) subjects in the control group (p = 0.000). Impaired quality of life and a DAS 28 score greater than 3.2 were statistically associated with depression during RA with a p value respectively of 0.021 and 0.0000231. Conclusion: Depres-sion is significantly more common during RA than in non-RA controls. RA patients should be screened routinely for depression in order to ensure improved treatment and management.

Forest and landscape restoration (FLR) practices have been reported to improve soil organic carbon stocks (SOCs) and contributing to climate change mitigation. This study aims to evaluate the impact of combined FLR practices, mainly developed in semiarid regions, on SOCs. The SOCs, soil texture, bulk density (ρ), pH, CO2 emissions, and herbaceous biomass were determined at a 0 - 30 cm depth. The experimental design comprised degraded land without FLR practices and three sets of combined FLR practices. These practices included “zaï” + stone bunds + organic manure + assisted natural regeneration (ANR) used to convert degraded land into forest (GF) and cropland (PARL);“zaï” + stone bunds + crop rotation + crop/fallow successions + ANR used to convert degraded land into cropland (OARL) and “zaï”+ stone bunds + organic manure used to convert degraded land into cropland (KARL). SOCs were highest (20.02 t C ha−1) under OARL compared with the other combinations of FLR practices. SOCs increased by 99% (+0.2 t C ha−1⋅yr−1), 58% (+0.3 t C ha−1⋅yr−1) and 13% (+0.2 t C ha−1⋅yr−1) under GF, OARL and KARL, respectively, and decreased by 15% (−0.1 t C ha−1⋅yr−1) under PARL. This study provides additional information explaining SOC variation in restored degraded land through the implementation of a combination of FLR practices. This is useful for recommending the combination “zaï” + stone bunds + crop rotation + crop/fallow successions + ANR to improve SOCs in the semiarid agroecosystem.

Epidemiological, Radio-Clinical and Histological Correlations of Breast Lesions ACR 3, ACR 4 and ACR 5 in the Ouagadougou’ City

Objective: To correlate the epidemiological and histological data of breast le-sions ACR 3, ACR 4 and ACR 5 biopsied on ultrasound in the city of Ouaga-dougou. Materials and methods: This was a retrospective study of data for 02 years (2015-2017) in the city of Ouagadougou. With the approval of the Ethics Committee for Health Research, the study focused on the histological results and radiological reports of biopsied lesions. Results: A total of 150 patients files had been collected. Breast lesions classified as ACR 4 were 50%, ACR 3 40% and ACR 5 10%. The risk factors were related to the ACR class. Ultra-sound was performed in all patients. The masses were the most common ele-mentary lesions found in breast imaging. On histology 98 lesions were benign, 16 intermediate lesions and 36 malignant lesions. Fibroids were the most common benign lesions of the ACR 3 and ACR 4 classes. Invasive ductal car-cinoma was the most common histologic type among malignant lesions in all ACR classes. Conclusion: Cancerous lesions are discovered late in our context. The ACR classification must always take into account discriminative ele-ments.
