Epidemiological Aspects of Diabetic Retinopathy at the Center of the Application of the Diploma of Specialised Studies in Ophthalmology (Cadeso)/Donka-Conakry

Purpose: The authors analyzed the epidemiological and clinical aspects of diabetic retinopathy at the Center for the Application of the Specialized Studies Diploma in Ophthalmology, Gamal Abdel Nasser University (CHU/ Donka) Conakry. Diabetic retinopathy (eye damage: eye and retina) is a serious complication of diabetes that affects 50% of type 2 diabetic patients. The eyes are particularly sensitive to damage to a small vessel. Diabetic retinopathy (DR) is the retinal localization of diabetic micro angiopathy resulting in impaired blood flow in the affected territories, the consequences of which will determine the clinical manifestations of the disease. DR is the leading cause of vision loss in adults of working age. Patients and Methods: This is a longitudinal prospective study carried out at CADES/O concerning 198 diabetic patients. The selection criteria were that the media be transparent and the fundus accessible over a period of six months from February to July 2018. Results: A total of 73 patients (37%) presented with diabetic retinopathy with an average age of 49.5 +/−9 years. The sex ratio (M/F) was 0.6 with a female predominance of 59%. Liberal profession patients were the most numerous 47.5%. Type 2 diabetes was the most common (85.86%). Diabetes fundus assessment was the most common reason for consultation (52%). Arterial hypertension was the most incriminated risk factor (45.45%) followed by the poor balance of diabetes (40.90%), and the age of diabetes (28.28%). 9.5% had diabetic retinopathy complicated by rubella iris, neovascular glaucoma and retinal detachment. Conclusion: Diabetic retinopathy is a common condition for which early detection and regular monitoring must be the rule to prevent, slow down or avoids irreversible blindness if possible, induced by this pathology by a good balance of diabetes and good control of associated risk factors.

Background: The rapid diagnostic tests play a pivotal role in the screening of viral markers in blood qualification for transfusion in limited resource setting. Therefore, it is important to assess their analytical performances to ensure their proper functioning. Material and Methods: We performed a cross- sectional study by successive recruitment to assess the diagnostic value of rapid diagnostic tests algorithms using ELISA as a reference test. A total of 661 blood from donors were enrolled for this study. Rapid Diagnostic Tests (RDTs) and ELISA tests were performed for each sample by a couple of double-blinded biotechnologists. Data were collected on case report form and captured in Microsoft Excel then the file was imported and analyzed using R software version 4.0.3. Results: The diagnostic accuracy for the algorithms are summarized in Table 1. For HIV-algorithm, the internal validity parameters were as follow: sensitivity (sens) 99.0% (95% CI = 97.8, 99.5);specificity (spec) 98.3% (95% CI = 90.9, 99.7);positive likelihood ratio (PLR) 57.4 (95% CI = 8.2, 401.0);negative likelihood ratio (NLR) 0.01

Cardiovascular Risk Factors among Outpatients: An Alarming Sign of the Epidemiological Transition in Developing Country?

Background: Noncommunicable diseases are the leading cause of death in the world and low and middle-income countries suffer from preventable premature death. The aim of this study was to assess the risk factors for non- communicable disease (NCDs) in general and particular cardiovascular diseases (CVDs) among the outpatients of our department of medicine. Methods: We performed a cross-sectional study from April to December 2017 by the consecutive enrollment of outpatients who attended in our department of medicine of Hôpital Sominé DOLO de Mopti, Mali. Clinical and laboratory data were measured for cardiovascular risk assessment. Framingham Risk Score (FRS) and Systemic Coronary Risk Estimation (SCORES) were computed by using Framingham and SCORE equations. Metabolic syndrome was defined using the harmonized criteria from the International Diabetes Federation (IDF) and the American Heart Association/National Heart, Lung, and Blood Institute (AHA/NHLBI). Data were captured in excel and analyzed with R version 4.0.3. The statistical significance was set at p = 0.05. Results: A total of 292 patients were enrolled in this study. The prevalence of traditional cardiovascular risk factors was 36.64%, 21.57%, 14.04%, and 13.01% for high blood pressure, hyperglycemia, smoking, and alcohol consumption, respectively. The metabolic syndrome accounted for 23.63%. The mean body mass index was 26.10 ± 7 kg/m2. The overall 10-year risk for cardiovascular events or death was 26.3% and 8.6% according to the FRS and SCORE equation, respectively. The 10-year risk of cardiovascular events according to the FRS was significantly higher in subjects aged 50 and above compared to subjects aged under 50 years, 34.46% vs 13.16%, p < 0.001. Modified Concrete Using Polyethylene Terephthalate Plastic Waste as a Partial Replacement for Coarse Aggregate

The present work evaluated the properties of modified concrete using polyethylene terephthalate (PET) bottle waste as a partial replacement for coarse aggregate. Modified concrete samples were designed using a water/cement (W/C) ratio of 0.50 and varying percentages of PET replacement (3%, 7%, 10%, and 15% by volume). Dreux Gorisse’s formulation approach was used to make the final products, and the mechanical properties of the samples were determined using Controlab presses. This modified concrete with PET chips has shown that with a 10% replacement of PET chips, the fresh density decreases by 3.56%, and the hardened state density decreases by 2.01%. The water absorption and thermal conductivity of the formulated concretes decreased. However, the results showed that the slump of these fresh concretes increased as the percentage of plastic aggregate replacement increased. Based on the results, incorporating PET aggregates into concrete contributes to good workability, and lightweight concrete structures, and provides some thermal comfort in concrete structures.

Urinary Sphincter Disorders of Neurological Origin: Prognostic Aspects in the Neurology Department of the Ignace Deen National Hospital

Introduction: Complications of Urinary sphincter disorders of neurological origin can be life threatening. The objective of this study was to describe the prognosis of urinary sphincter disorders during neurological conditions. Methods: This was a six-month analytical study conducted at the neurological unit of the Hôpital national Ignace Deen. Including patients with urinary sphincter disorders following a neurological condition;Chi-square, Fischer, and Student’s t-tests were used for variables with a p value less than 0.10 and then included in a logistic model with a significance level set at 0.05 and a 95% confidence interval. Results: We collected 1081 patients among whom, 324 presented, that is to say a frequency of 30%, which concerned subjects aged 57.3 ± 16.4 years with a slight female predominance 50.3%. Urinary incontinence (80.6%) was associated with complications such as urinary tract infection with a high proportion of cerebral damage (92.3%). HIV infection (P = 0.015), bedsores (P = 0.049), and inhalation pneumonia (P = 0.001) were the main poor prognostic factors. Conclusion: Urinary sphincter disorders are elements of poor prognosis, both vital and functional, concerning elderly subjects with a predominance of urinary incontinence. HIV infection, bedsores, pneumopathy are poor prognostic factors.

Purpose: The purpose of our study was to describe the technique of scanopelvimetry used in the radiology department of the Point G University Hospital, to specify the indications of scanopelvimetry in the department and to evaluate the cost of radiopelvimetry. Patients and Method: This was a prospective, cross-sectional study that took place over a 12-month period from January 1 to December 31, 2018. It focused on pregnant women in whom scanopelvimetry was performed in the medical imaging department of Point G University Hospital during the study period. Results: Out of 8615 CT examinations performed in the radiology department, we collected 65 cases of scanopelvimetry, i.e., 0.7% of the CT examinations. Primigravida and paucigravida dominated the study (40% each). In 60% of the pregnant women the height was less than 150 cm. The most frequent indication for CT scanning was clinical pelvic narrowing (50%). In 80% of the cases, the scanopelvimetry was performed after 37 weeks of amenorrhea. A predominance of narrowing of the superior strait was noted (40%). 4.6% of the pregnant women had a Magnin index of less than 20. CT scanopelvimetry is a feasible examination in our context but the level of demand is low because of the high cost of the examination (49500 fca). Conclusion: In light of the results obtained, we can say that CT pelvimetry allows us to predict the probable route of delivery and thus we can reduce the risk of fetomaternal morbidity and mortality to improve maternal and child health.

Introduction: Acute intestinal obstruction is a serious pathology, a surgical emergency for which medical imaging plays an important role in the management. We initiated this work in order to study the contribution of imaging in the diagnosis of acute intestinal obstruction at the Point-G University Hospital. Patients and Methods: This was a prospective, descriptive and analytical study of 96 patients collected at the radiology and medical imaging department of CHU Point-G from January 2018 to January 2019. Results: The age of our patients varied from 11 to 86 years, with an average of 36 years old. There was a male predominance of 64.6% against 35.4% for women, i.e., a sex ratio of 1.82. Previous surgery was found in 61.5% of our patients. The pain was present in all patients. An unprepared abdominal X-ray was performed in 89.6% of patients. Hydroaerobic levels were found in 96.5% of patients. Abdominopelvic CT scans were performed on 12 patients, all of whom were diagnosed with occlusion. These positive diagnostic findings were consistent with intraoperative findings in 92% of cases. The causes were dominated by bridges in 46 patients and tumors in 9 patients. Signs of severity on CT were dominated by signs of distress of the upstream bile ducts in 8.3%. Exactly 8% of our patients spontaneously resumed transit, 91% received surgical treatment and 1% died before surgery. The outcome was favorable in 80 patients (83.3%) and poor with death in 16 patients (16.7%). Conclusion: Acute intestinal obstruction remains a serious pathology for which the X-ray of the PSA is often the only radiological examination performed in an emergency. However, abdominopelvic CT seems to us to be widely indicated thanks to its contribution both to the positive diagnosis and to the diagnosis of severity and etiology. However, this imaging technique is widely underused in our practice because of its high cost and lack of availability.

Introduction: Malformations of the excretory tract are abnormalities due to a disorder of embryogenesis. According to some authors, their frequency varies from 0.30 to 5.25 per 1000. The aim of this study was to highlight the CT uroscan aspect of pyelic bifidity with intravesical ureteral ectopic junction revealed by a urinary calculus and to explain the interest of CT uroscan in the management of urinary pathologies. Result: We report the observation of a 37-year-old overweight patient with a body mass index equal to 28, a history of surgery and extracorporeal litho-thipsis dating back 10 years. He was referred to the Imaging Department in the context of renal colic. The uroscanner made it possible to highlight a left pyelic bifidity associated with an ectopic abutment of the left ureter on the superolateral wall of the bladder, a calculation in the left ectopic ureteral meatus measured at 4 mm in axial diameter. He noted upstream of these lesions, a uretero-pyelo-calicielle dilation on double homolateral pyelon. On late cuts, a slight delay in left renal excretion was noted. The right kidney was also the site of a hyperdense calculation of 1257 HU density and measured 4 mm in transverse diameter with a non-obstructive appearance. Due to a lack of means, our patient was subjected to oral medication, hygiene and dietary rules and rigorous monitoring. Conclusion: At the end of this study, the uroscanner is presented as the reference examination to better explore urinary pathologies. At the same time, it makes it possible to carry out a study of the organs surrounding the urinary tract. Always think about the association of several urinary pathologies each time you see a patient for renal colic.

Introduction: Spondylolisthesis is defined as a permanent anterior sliding of the vertebral body accompanied by the pedicles, the transverse processes and the posterior joints in relation to the underlying vertebra. The prevalence of spondylolisthesis varies between 4% and 9%, according to different authors in France, China and Japan. In Africa, some African authors find a spondylolisthesis rate of 9.93%. The objective of this study was to study the clinical and radiological aspects of spondylolisthesis at the Mother-Child Hospital of Bamako. Methods: Prospective and retrospective descriptive study carried out in the neurosurgery department of CH Mother-Child Luxembourg in Bamako from 2010 to 2018. Result: We collected 42 patients operated on for spondylolisthesis out of 145 patients, i.e., 29% of the department’s activities. The average age of our patients was 51 years old, with extremes of 20 and 75 years old. The sex ratio was 0.81 in favor of women. 61.9% of our patients performed work requiring physical strength. Neurogenic claudication was the predominant clinical sign with 88.1% of cases. CT was the most common way of exploration in 57.1% of cases and CT myelo in 47.6% of cases. Disc herniation was associated with spondylolisthesis in 23% of cases. Spondylolisthesis was grade 1 in 59.6% of cases and was responsible for mixed stenosis in 73.82% of cases. All our patients underwent laminectomy. The evolution at 6 months postoperative was favorable in 64.3% of cases. Conclusion: The diagnosis of spondylolisthesis is radiological and makes it possible to define the type of lesion according to its etiology and its severity, but also to highlight the conflicts with the nervous system which will have to be cured. Field and household work, pregnancy, carrying loads or children on the back could be incriminated.

Our study was carried out at Robert BALLANGER Hospital, its aim was to determine the contribution of the scanner in the diagnosis of COVID-19. It was a prospective descriptive study during the first wave of the COVID-19 epidemic in France. Patients were referred to the medical imaging department for a thoracic CT scan without contrast injection or an angioscanner in the context of a suspected COVID-19 pneumopathy or pulmonary embolism. The study involved both sexes, the male sex was with 55.20% and sex ratio of 1.24. In our study 50% of our patients had a comorbid factor, of which diabetes and hypertension were the most represented with 33% and 17%. The RT PRC test is considered the gold standard in the diagnosis of COVID-19 disease. This test was positive in 63.62% of our patients. CT imaging played a key role in the management of COVID-19 pneumonia because chest CT scans found lesions consistent with COVID-19 pneumonia in 71% of patients. The depoliated lung lesion was present in 87% of our patients, with sub pleural topography and minimal parenchymal involvement predominating. In our study, the chest CT scan had a higher sensitivity and a lower specificity than the PCR test.
