Impact of cytochrome P450 2C19*2 polymorphism on the clinical cardiovascular events after stent implantation in patients receiving clopidogrel of a southern Tunisian region

Introduction: The concept of clopidogrel resistance, first described in biology is being strengthened by recent data from clinical epidemiology. The cardiologists have been sensitized to this concept because of its possible involvement in the occurrence of coronary stent thrombosis. Purpose of the study: The purpose of this study was to investigate the genetic variant of the gene CYP 2C19 inour population and to assess the involvement of this genetic profile in the occurrence of major cardiovascular events (MACE) during the follow-up period. Methods: Our prospective study was conducted between May 2009 and September 2010 including 100 patients admitted to the cardiology department for percutaneous coronary stenting. The patients were divided into 2 groups: those with at least one CYP2C19*2 allele (*2 carriers) and non-carriers. Results: The mean age of our patients was 56.7 years ± 10, 5. No remarkable differences in the baseline characteristics were noted between the two groups. The prevalence of CYP2C19*2 allele in our population was 11.5%. Hospital mortality was estimated at 3%. No statistically significant differences were noted between the two groups regarding the occurrence of intra hospital MACE. The mean follow up was 7.5 ± 4.87 months for the entire study population. The rate of MACE during the follow-up of patients receiving clopidogrel was 8.2% throughout the study population: 5.3% in the *2 non-carriers versus 18.2% in the *2 carriers with a statistically significant difference (p = 0.075) at the risk of error of 10%. Concerning the occurrence of stent thrombosis, there was no significant statistical difference between the two study groups. Conclusion: From these results it is suggested that CYP2C19*2 polymorphism is associated with increase in the occurrence of MACE among Tunisian patients receiving clopidogrel. A larger study is needed to assess the role of genotyping in the evaluation of the phenomenon of clopidogrel resistance.

Anticoagulation in Pregnant Women with Mechanical Heart Valve Prostheses: 25-Year Experience at a Tertiary Care Hospital in a Developing Country

The aim of this retrospective study was to compare the different anticoagulation regimens used in pregnant women with prosthetic heart valves. We reviewed 86 pregnancies in 57 women from 1987 to 2011. The patients were divided into two groups: group A (39 pregnancies) had oral acenocoumarol throughout pregnancy;and in group B (47 pregnancies), acenocoumarol was replaced by subcutaneous heparin during the first trimester. Both groups received heparin at the time of delivery. The valves replaced were mitral (59.65%), aortic (12.28%), or both (28.07%). 74 pregnancies (86.04%) resulted in live births, 9 (10.46%) had stillbirths, 1 (1.16%) had spontaneous abortion and 2 (2.32%) underwent therapeutic abortions. The live birth rate was higher in women on heparin (87.23%) compared with those on acenocoumarol (84.61%). No malformations appeared in the 74 newborns, except for one case of hydrocephalus. There was one maternal death due to acute mitral valvular thrombosis while on heparin in the first trimester. Hemorrhagic complications occurred in 13 patients in the postpartum period, 4 of whom required transfusion. No anticoagulant regimen can be said to be entirely safe for use during pregnancy as there is a degree of risk with each regimen. Heparin does not offer a clear advantage over oral anticoagulation in the pregnancy outcome.

Adapted European seabass(Dicentrarchus labrax L.)larvae at 25 days post-hatching(dph)with a primary weight and length of 23 mg and 9.92 mm,start the weaning up to 46 dph using the weaning micro-diet(54%crude protein from fish meal,powder milk,and poultry egg)concurrently with Artemia enriched with fortification emulsions(0.6 g/L)of DHA selco®as a control group(DHAS)or Fish oil+20%Vitamin C(FOVC),or Fish oil+20%Vitamin E(FOVE),or Fish oil+10%Vit.C+10%Vit.E(FOCE),or Fish oil only(FO).At 46 dph,groups of larvae fed enriched Artemia with DHAS and FOVE exhibited the highest final body weight,weight gain,average daily gain,specific growth rate,feed intake,protein efficiency ratio,and survival%as well as the lowest feed conversion ***,larvae in FOVC displayed the lowest final body weight,total length,weight gain,average daily gain,specific growth rate,feed intake,protein efficiency ratio,and survival%as well as the highest feed conversion *** histological assessment exhibited no pathological alteration as well as an improvement in the structure with DHAS,FOVE,FOVC,and FOCE co-additives compared to *** peroxidase enzyme(GPx)recorded the highest rates(P<0.05)in groups fed Artemia supplemented with DHAS and FOVE followed by FOCE and *** the lowest record for GPx activity was noted in the FO *** conclusion,Using augmented Artemia nauplii with fish oil+vitamin E in single or in companion with vitamin C as cheap antioxidants support *** larval growth,survival,and antioxidant efficacy during the critical weaning period.

Firefly-CDDL:A Firefly-Based Algorithm for Cyberbullying Detection Based on Deep Learning

There are several ethical issues that have arisen in recent years due to the ubiquity of the Internet and the popularity of social media and community *** them is cyberbullying,which is defined as any violent intentional action that is repeatedly conducted by individuals or groups using online channels against victims who are not able to react *** alarmingly high percentage of people,especially teenagers,have reported being cyberbullied in recent years.A variety of approaches have been developed to detect cyberbullying,but they require time-consuming feature extraction and selection ***,no approach to date has examined the meanings of words and the semantics involved in *** past work,we proposed an algorithm called Cyberbullying Detection Based on Deep Learning(CDDL)to bridge this *** eliminates the need for feature engineering and generates better predictions than traditional approaches for detecting *** was accomplished by incorporating deep learning—specifically,a convolutional neural network(CNN)—into the detection *** this algorithm shows remarkable improvement in performance over traditional detection mechanisms,one problem with it persists:CDDL requires that many parameters(filters,kernels,pool size,and number of neurons)be set prior to *** parameters play a major role in the quality of predictions,but a method for finding a suitable combination of their values remains *** address this issue,we propose an algorithm called firefly-CDDL that incorporates a firefly optimisation algorithm into CDDL to automate the hitherto-manual trial-and-error hyperparameter *** proposed method does not require features for its predictions and its detection of cyberbullying is fully *** firefly-CDDL outperformed prevalent methods for detecting cyberbullying in experiments and recorded an accuracy of 98%within acceptable polynomial time.

Discrete spread model for COVID-19: the case of Lebanon

Background: Mathematical models are essential to predict the likely outcome of an epidemic. Various models have been proposed in the literature for disease spreads. Some are individual based models and others are compartmental models. In this study, discrete mathematical models are developed for the spread of the coronavirus disease 2019 (COVID-19).Methods: The proposed models take into account the known special characteristics of this disease such as the latency and incubation periods, and the different social and infectiousness conditions of infected people. In particular, they include a novel approach that considers the social structure, the fraction of detected cases over the real total infected cases, the influx of undetected infected people from outside the borders, as well as contact-tracing and quarantine period for travelers. The first model is a simplified model and the second is a complete ***: From a numerical point of view, the particular case of Lebanon has been studied and its reported data have been used to estimate the complete discrete model parameters using optimization techniques. Moreover, a parameter analysis and several prediction scenarios are presented in order to better understand the role of the ***: Understanding the role of the parameters involved in the models help policy makers in deciding the appropriate mitigation measures. Also, the proposed approach paves the way for models that take into account societal factors and complex human behavior without an extensive process of data collection.

BACKGROUND The impact of type 2 diabetes mellitus(T2DM)on acute respiratory distress syndrome(ARDS)is debatable.T2DM was suspected to reduce the risk and complications of ***,during coronavirus disease 2019(COVID-19),T2DM predisposed patients to ARDS,especially those who were on insulin at *** evaluate the impact of outpatient insulin use in T2DM patients on non-COVID-19 ARDS *** We conducted a retrospective cohort analysis using the Nationwide Inpatient Sample *** patients diagnosed with ARDS were stratified into insulin-dependent diabetes mellitus(DM)(IDDM)and non-insulindependent DM(NIDDM)*** applying exclusion criteria and matching over 20 variables,we compared cohorts for mortality,duration of mechanical ventilation,incidence of acute kidney injury(AKI),length of stay(LOS),hospitalization costs,and other clinical *** Following 1:1 propensity score matching,the analysis included 274 patients in each ***,no statistically significant differences emerged between the IDDM and NIDDM groups in terms of mortality rates(32.8%vs 31.0%,P=0.520),median hospital LOS(10 d,P=0.537),requirement for mechanical ventilation,incidence rates of sepsis,pneumonia or AKI,median total hospitalization costs,or patient disposition upon *** Compared to alternative anti-diabetic medications,outpatient insulin treatment does not appear to exert an independent influence on in-hospital morbidity or mortality in diabetic patients with non-COVID-19 ARDS.

Spatio-temporal dynamics of cyanobacterial abundance and toxicity in a Mediterranean hypereutrophic lake

The Mediterranean basin is considered one of the most vulnerable areas in the world under the impacts of global warming and changes in precipitation ***,a shallow and polymictic freshwater lake located in northeastern Algeria(36°50′N,08°23′E),has recently experienced a significant proliferation of harmful cyanobacteria resulting in the generation of *** carried out this study in order to understand the succession patterns of dominant cyanobacteria and cyanotoxin production and the factors driving this in Oubeira Lake.A total of 26 cyanobacterial genera were identified,and among them Microcystis and Planktothrix accounted for more than 60%of the overall cell *** summer/fall period was dominated by Microcystis,Planktothrix,and in lesser extend by Cylindrospermum,*** the fall/winter transition,Dolichospermum,Pseudanabaena,and Aphanizomenon were the dominant ***,the bloom-forming cyanobacteria showed significant differences between months but not between *** microcystins(MC-LR)was detected in all collected samples(0.62-and 19.14-μg MC-LR equivalent/L),but appeared in high concentrations throughout the period of dominance of Microcystis and *** was more sensitive to nutrients than to *** was more dependent on temperature than on nutrients,which explains their coexistence during summer-fall ***,both genera are positively correlated with MC-LR and would probably be the main producers of ***,Dolichospermum,and Aphanizomenon co-occurred at the end of Planktothrix dominance period(December).Aphanizomenon and Pseudanabaena were correlated negatively with temperature and positively with water *** showed a strong positive correlation with *** Lake,could serve as a model of how cyanobacteria blooms may develop in lakes within Mediterranean climates.

Snow depth estimation is an important parameter that guides several hydrological applications and climate change *** advances in remote sensing technology and enhanced satellite observations,the estimation of snow depth at local scale still requires improved accuracy and *** advances in ubiquitous and wearable technology promote new prospects in tackling this *** this paper,a wearable IoT platform that exploits pressure and acoustic sensor readings to estimate and classify snow depth classes using some machine-learning models have been put ***,the results of Random Forest classifier showed an accuracy of 94%,indicating a promising alternative in snow depth measurement compared to in situ,LiDAR,or expensive large-scale wireless sensor network,which may foster the development of further affordable ecological monitoring systems based on cheap ubiquitous sensors.
