Efficacy of different doses of sugammadex after continuous infusion of rocuronium

AIM: To evaluate the effects of two different doses of sugammadex after maintenance anesthesia with sevofluorane and remifentanil and deep rocuroniuminduced neuromuscular blockade(NMB).METHODS: Patients between 20 and 65 years of age, with American Society of Anesthesiologists physical status classification Ⅰ-Ⅱ, undergoing gynecological surgery were included in a prospective, comparative and randomized study. NMB was induced with an injection of 0.6 mg/kg of rocuronium followed by continuous infusion of 0.3-0.6 mg/kg per hour to maintain a deep block. Anesthesia was maintained with sevofluorane and remifentanil. Finally, when surgery was finished, a bolus of 2 mg/kg(group A) or 4 mg/kg(group B) of sugammadex was applied when the NMB first response in the train-of-four was reached. The primary clinical endpoint was time to recovery to a train-of-four ratio of 0.9. Other variables recorded were the time until recovery of train-of-four ratio of 0.7, 0.8, hemodynamic variables(arterial blood pressure and heart rate at baseline, starting sugammadex, and minutes 2, 5 and 10) and adverse events were presented after one hour in the post-anesthesia care ***: Thirty-two patients were included in the study: 16 patients in group A and 16 patients in group B. Only 14 patients each group were recorded because arterial pressure values were lost in two patients from each group in minute 10. The two groups were comparable. Median recovery time from starting of sugammadex administration to a train-of-four ratio of 0.9 in group A and B was 129 and 110 s, *** estimated difference in recovery time between groups was 24 s(95%CI: 0 to 45 s, Hodges-Lehmann estimator), entirely within the predefined equivalence interval. Times to recovery to train-of-four ratios of 0.8(group A: 101 s; group B: 82.5 s) and 0.7(group A: 90 s; group B: 65 s) from start of sugammadex administration were not equivalent between groups. This article summarizes the state of stocks of sardine,sardina pilchardusin Moroccan waters at the end of 2016 and *** stocks varies ac-cording to the region,They are now in a sustainable exploitation situation:Atlantic sardine,but also stocks that have reached or are still in over-ex-ploitation levels:Mediterranean *** fishing pressure exerted onthe sardine stock exceeds the optimal level by 40%.Due to the degrada-tion of the fragile marine environment of the Mediterranean region,gen-eraled by pollution of various origins,the overexploitation of juveniles inthe coastal strip,non-responsible practices and the use of non-fishing gearselective,it is imperative to take the necessary measures to protect themarine environment and ensure rational and sustainable exploitation ofthe resource.(High commission plan 2006 report).

Redo Cardiac Surgery: Bleeding Control

Redo cardiac surgery increases?mortality and morbidity. The aim of this study was to determine if aprotinin was superior to tranexamic acid concerning control bleeding loss after redo valve surgery. A retrospective study was conducted from January 1994 until December 2014. 221 patients underwent redo cardiac valve surgery and separated into two groups: aprotinin group (n?=?85) and tranexamic acid group (n?=?136). Univariate tests were applied for data analysis. A total of 221 patients were enrolled in this study. This cohort was separated into two groups: aprotinin group (n?=?85) and tranexamic acid group (n?=?136). Euroscore in tranexamic acid group was higher: 5.96 ± 3.04 vs.?5.17 ± 2.83 in aprotinin group?(p?=?0.055). There was no statistical difference in postoperative mortality between the two groups (p?= 0.153). No statistical differences were reported concerning: total blood loss (p?= 0.51), red blood cells transfusion (p?= 0.215), reexploration for bleeding (p?= 0.537) and postoperative renal failure (p?= 0.79). There were statistical differences concerning mechanical ventilation time, which is longer in tranexamic acid group (p?= 0.008) and the use of inotropic drug support, which is more frequent in the tranexamic acid group (p?= 0.001). Our results demonstrated that tranexamic acid and aprotinin reduce transfusion requirement and blood loss. Due to financial reason, we chose tranexamic acid in preventing blood loss in redo valve surgery.

Long-term operational stability remains the primary concern for perovskite solar cells. Consequently, there is a quest for searching for new compositions that enable stable and efficient perovskites. We report a new molecular-level interface engineering strategy using a multifunctional ligand that augments long-term operational and thermal stability by chemically modifying the formamidinium lead iodide rich photoactive layer. The surface derivatized solar cells exhibited high operational stability(maximum powering point tracking at 1 sun) with a stabilized T80(the time over which the device efficiency reduces to 80% of its initial value of post-burn-in) of ≈5950 h at 40 oC and stabilized efficiency over 23%. The origin of high device stability and performance is correlated to the nano/subnanoscale molecular level interactions between ligand and perovskite layer, which is corroborated by comprehensive multiscale characterization. Our results provide key insights into the modulation of the grain boundaries, local density of states, surface bandgap, and interfacial recombination. Chemical analysis of the aged devices showed that interface passivation inhibited ion migration and prevented photoinduced I2 release that irreversibly degrades the perovskite. This study shows that passivating ligands have the potential to overcome stability issues associated with the high performing hybrid perovskite compositions, thus allowing a step closer to achieving long-standing stability of perovskitebased solar cells.

Sheep pox is an infectious viral disease that affects specifically sheep and it is caused by the Capripoxvirus *** clinical signs include fever,diarrhea,difficulty breathing,nodules,lung lesions and *** Morocco,the 2010 epidemic of sheep pox was characterized by the emergence of a nodular form of the *** local strain was isolated and the analysis of affected animals was positively confirmed by virus isolation and real-time polymerase chain reaction(RT-PCR).The epidemiological analysis of 911 data records showed that the virus is endemic in the country;an average of 350 cases per year with an epizootic evolution was observed in *** incidence varies depending on provinces and the disease appears confined to the central and the eastern regions of the country where a very intensive sheep breeding activity is taking *** statistical analysis showed that there is a positive correlation between the endemicity and the significant factor of the rural market(p=0.006).The annual average morbidity and mortality rates were 2.96%(1.26%to 4.32%)and 0.71%(0.41%to 0.94%),*** clinical findings associated to the epidemiological data analysis confirmed the presence of sheep pox in its nodular form and suggest that new pathogenic strains may have been introduced from *** purpose of this work was to provide a better description of the spatiotemporal evolution of sheep pox disease based on some epidemiological indicators and to put forward plausible hypotheses regarding the emergence of the virus in order to implement an adequate control strategy.
