Superior Mesenteric Artery Syndrome, a Rare Complication of Severe Malnutrition: Two Cases from a Subsaharian Endocrinology Department

Introduction: Superior mesenteric artery syndrome (SMAS), a rare diagnosis due to compression of the third duodenum between the superior mesenteric artery (SMA) and the aorta resulting in bowel obstruction, may lead to severe malnutrition. We report two cases of patients hospitalised in the Internal Medicine, Endocrinology, Diabetology, and Nutrition Department of the National Hospital Center (NHC) of Pikine. Observations: Patient 1: A 35-year-old female was referred for an aetiological diagnosis due to a rapid weight loss of 15 kilograms in one month, accompanied by persistent vomiting, following an appendectomy performed a month before admission. Upon clinical examination, she presented severe malnutrition (Buzby index of 76%), early post-prandial chronic vomiting, and a poor general condition. An abdominal CT scan revealed aortomesenteric clamp syndrome (AMCS) with an angulation between the aorta and the SMA of 13˚. The underlying cause in this patient was severe malnutrition. Fortunately, her condition improved with medical treatment. Patient 2: We report the case of a 30-year-old female hospitalized due to unusual weight-bearing post-prandial epigastric pain and intermittent vomiting over the past six months. Upon physical examination at admission, she exhibited severe malnutrition with a body mass index (BMI) of 14 kg/m2, a Buzby index of 71%, trophic disorders, and a stage IV general condition assessment according to the World Health Organization (WHO). An abdominal CT scan revealed AMCS with an angle between the aorta and the SMA of 22˚ and an aortomesenteric space of 4 mm. The outcome was poor with medical treatment failure and, unfortunately, the patient died before surgery. Conclusion: SMAS is rarely evoked in clinical practice despite the presence of contributing factors and suggestive clinical signs. The prognosis depends on management time.

Overview of Adrenals Tumors in Dakar Hospitals

Introduction: Adrenal tumours (AT) are commonly encountered in clinical practice. For any patient presenting an adrenal mass, there are two crucial points to consider: is the adrenal mass malignant and is it hormonally active? The objective of our study was to evaluate the epidemiological, diagnostic, therapeutic, and evolutionary aspects of AT in a developing country. Methodology: We conducted a retrospective study spanning 17 years (from January 2005 to October 2022) in four Dakar hospital services. We included all patient medical records with explored AT. Results: AT was diagnosed in 35 patients who had a mean age of 36.62 years (range: 12-79), and a female predominance (26 women to 9 men). Among these patients, 17.2 % had incidental AT. Hypertension was the commonest presenting symptom in 27 patients (77%), which was associated with Mnards triad in 14 patients (40%). Abdominal pain was reported by 20 patients (57%), and 14 presented with an abdominal mass. Among the patients, 29 had functional AT: 22 with pheochromocytoma, 5 with primary hyperaldosteronism, and 2 with hypercortisolism. Imaging evaluations revealed that 33 patients had unilateral AT and one had bilateral AT. Tumour sizes varied, 5 patients had tumours 10 cm. Two patients presented with metastases. The therapeutic approach involved adrenalectomy in 32 patients. Perioperative complications were observed in five patients, including haemorrhage (two patients), hypotension (two cases), and hypertensive crisis (one case). One patient with bilateral pheochromocytoma developed acute adrenal insufficiency postoperatively, followed by abdominal herniation. Histopathological examination confirmed pheochromocytoma in 21 patients, Conns adenoma in 5, adrenocortical carcinoma in 3, cortisol adenoma in 2, and adrenal cyst in 1 patient. Treatment of Kidney Stones by Percutaneous Nephrolithotomy: Evaluation of the Results of the First Series of Our Experience in the Urology Department of the Idrissa Pouye General Hospital in Dakar

The principle of PCNL is the extraction through a nephrostomy channel of kidney stones which are defined as the presence of crystalline concretions in the kidneys. Objectives: The objective of this work was to study the epidemiological, clinical and paraclinical aspects of patients with renal lithiasis treated by PCNL and on the other hand the different technical aspects of PCNL. Materials and Methods: This was a retrospective descriptive study, conducted in the urology department of HOGIP, covering the period from January 2015 to January 2019. We studied the demographic and clinical aspects of patients presenting renal lithiasis;we also studied the technical aspects and treatment outcomes. Statistical significance was set for α = 0.05. Results: Our series covered 82 PCNLs performed during the study period. The average age was 45.95 years, the age group [40 - 49 years] was more affected. Our study involved 44 men and 38 women, a sex ratio of 1.15. At the clinic, atypical lumbar pain was more frequent in 45.83% of cases. On URO-CT, the calculations were located more at the pyelic level (31.2%) and lower caliceal (27.2%). The size of stones measured between 15 and 20 mm in 30.52%. The stone density was between 500 and 1000 HU in 47.54% of cases. Intraoperatively, the patients were placed in lateral decubitus in 41.46% of cases. Only one access to the kidney was necessary in 87.8% of cases. The lithoclast was used in 65.85% of cases. The overall success rate (stone free) in our series was 96%. The majority of cases, 41.44%, drainage were done by natural means (totally tubeless). The average duration was 92 minutes. The complications encountered in our experience concerned 10 cases, a rate of 12.19%. Conclusion: Urinary lithiasis is more and more frequent in our regions. The development of Endo-urology offers several therapeutic options. Thus, PCNL occupies an important place in the management of kidney stones.

Influence of Topography on the Distribution and Structure of Woody Plants in the Senegalese Sahel (Sandy Ferlo)

This study describes the floristic composition and structure of a woody stand in the Senegalese Sahel, paying particular attention to the edaphic factors of its floristic composition. A stratified inventory considering the different relief units was adopted. Woody vegetation was surveyed using a dendrometric approach. The results obtained show that the flora is dominated by a few species adapted to drought, such as Balanites aegyptiaca (L.) Del., Calotropis procera Ait. and Boscia senegalensis (Pers.). The distribution of this flora and the structure of the ligneous plants are linked to the topography. In the lowlands, the flora is more diversified and the ligneous plants reach their optimum level of development compared with the higher relief areas. In the lowlands, there are a few woody species which, in the past, were indicative of better climatic conditions. These are Anogeissus leiocarpus (DC.), Commiphora africana (A. Rich.), Feretia apodanthera Del., Loeseneriella africana (A. Smith), Mitragyna inermis (Willd.) and Sclerocarya birrea (A. Rich). It is important that their reintroduction into reforestation projects takes account of their edaphic preference.

Surgical Treatment of the Upper Urinary Tract Lithiasis at the Urology Department of the Hopital General Idrissa Pouye

Upper urinary tract lithiasis is a condition characterized by the presence of stones which is a stony concretion of crystallized substances in the kidney and/or ureter. Objective: The objective of this work is to study the epidemiological, clinical and therapeutic aspects of upper urinary tract stones on the one hand, then to compare the different types of surgical treatments and their results. Materials and Methods: This is a retrospective and descriptive study focusing on the surgical management of upper urinary tract stones, between January 2017 and December 2020, at the Urology department of the Hopital General Idrissa Pouye. Results: During the study period, 7.59% of surgical interventions performed were related to the treatment of upper urinary tract stones. Average age was 45.20 ± 16.4 years, the age group [41 - 60] years was more affected with 42.68%. A history of urolithiasis was present in 25.52% of cases. Lower back pain was present in 97.48% of cases. On URO-CT scan, the stone was located more at the level of the ureter (49.57%). The size between 10 to 15 mm was more frequent (30.96%) and the density greater than 1000 HU was more frequent (33.47%). The most used therapeutic procedure was ureteroscopy (52.08%), followed by PCNL (40.83%) and open surgery (7.08%). The success rate (stone free) was 93.68% for the URS, 89.36% for the PCNL. The length of hospital stay for open surgery was 6.76 ± 4.25, for PCNL 2.62 ± 2 days and for URS 1.75 ± 1.62 days. Postoperative complications were present in 23.52% for open surgery, 11.22% for PCNL and 8% for URS. Conclusion: Urolithiasis is constantly growing in our regions. Effective endourological treatment is increasingly replacing open surgery. However, global access to these new techniques in our regions is slow to be effective.

Ultra-Violet (UV): A Good Bacterial Sterilizer?

Introduction: The aim of this study was to improve the DISTER-UV and to perform microbiological quality control at the biomedical laboratory of the West African Polytechnic University from January 2022 to November 2022. Methodology: During this eleven-month prospective study, we set up a quality control device (QCD). For microbiological quality control, we performed different cultures of bacteria with different bacteriological and morphological characteristics at T0 (no sterilization) and at T30 (after 30 minutes of sterilization under DISTER-UV). Results: After the realization, the DCQ attached to the DISTER-UV1 allows to display of the UV wavelength present in the light box. This device also displays and alerts when the UV intensity emitted by the lamps is below 250 nm. During microbiological quality control, the cultures carried out at T30 and incubated for 24 hours did not reveal any bacterial colonies. This shows the bactericidal character of DISTER-UV-2. Conclusion: The improvement and the microbiological quality control allowed us to switch from DISTER-UV1 (without sensor) to DISTER-UV-2 (with sensor or DCQ). The biological control allowed us to affirm that the DISTER-UV-2 is bactericidal.

White Blood Cells Detection Using Spectral Tresholding

The study of the morphology of White Blood Cells (WBCs) further contributes to the clinical diagnosis of blood diseases. In this research paper, we come up with an image segmentation enhancement by combining Fourier Fast Transform on smear blood capture and classical thresholding. The Fast Fourier Transform (FFT) is a very powerful tool in image processing and it was used to segment and extract the WBCs. Our image processing method uses a Fast Fourier Transform combined with filtering and an Inverse Fast Fourier Transform for the extraction and visualization of the high frequency region of the image. In order to remove residual Red Blood Cells acting as noise in the expected result, a final thresholding step is added at the end of the processing. The results presented in this article report the tests performed using our mathematical implementation. Moreover, we were able to detect and differentiate the sub-families of WBCs.

Evolution of the Spatial Distribution of Basic Infrastructures and Services in the Niakhar Area (Fatick), Senegal

Basic infrastructures and services present an important socio-economic and territorial equity issue, because the availability of these makes it possible to improve the living conditions of populations, makes territories attractive and stimulates their development. The objective of our study has been achieved, so our results provided information on the rate of propagation of basic infrastructures and services, as well as the evolution of the forms of spatial distribution of the latter. The data used comes from the census and geolocation of basic infrastructures and services in the Niakhar area, carried out by the IRD in 2018. Centrographic measurements were used to describe the overall spatial distribution and the use of chi-square statistics confirmed the existence of a preferential direction of distribution. To verify the existence of a spatial structure of infrastructure and service seeding, the Ripley statistic is used. Our results can be useful for land use planning and spatial resource allocation policies. Indeed, the identification of different types of spatial aggregates and the highlighting of the preferred directions of the distribution of basic infrastructures and services, make it possible to correct disparities in the spatial distribution of basic resources.

Hierarchy of Rural Localities in the Niakhar Area (Fatick, Senegal)

Although the study of the hierarchy of localities presents an important social and economic stake, studies focused on the hierarchy of rural localities are relatively weak, especially in Africa. It is important to highlight the hierarchy of rural areas in order to reveal poles that can support sustainable growth. Our main objective is to study the hierarchy of rural localities in the Niakhar area, using a geographic approach. The data come from the Niakhar demographic, epidemiological and environmental observatory database and the Niakhar Atlas project of the French Research Institute for Development (IRD). The net flow method, the primacy index, the median population threshold method and the measurement of the correlation between level of centrality and population size are used to analyze the data. Our study allowed us to determine the main types of centrality and to establish the hierarchy of rural localities in the Niakhar zone. It also highlighted the strong correlation between the rank of localities and the size of the population of the latter. The main contribution of our work is to have highlighted, in order, the structuring localities of the rural space of the Niakhar area, by combining several indicators of centrality.

Postoperative Analgesia for Abdominal Laparoscopic Surgery: Tap Block vs Peri-Orificial Infiltrations

Introduction/Purpose: The “transverse abdominal plane block” or TAP block was described by Rafi in 2001. It describes an approach to the neurofascial plane of the transverse abdominal muscle via the Jean-Louis Petit triangle and provides analgesia of the entire homolateral hemi abdomen. The aim of our study was to compare post-operative analgesia and post-operative morphine requirements between transverse abdominal plane block (TAP) and peri-orificial infiltration during laparoscopic abdominal surgery. Material and method: Prospective, randomized study conducted over a 2-year period. The study was conducted in the operating theatre of the Saint Louis Regional Hospital in Senegal. All adult patients undergoing laparoscopic abdominal surgery were included. Clinical aspects, pain scales and morphine consumption were analyzed. Results: A total of 60 patients were enrolled: 30 patients in the TAP group and 30 patients in the infiltration group. The average age was 32.9 years. The indications for laparoscopy were acute appendicitis in 50% of cases, gallbladder stones in 16% and inguinal hernia in 8%. For the TAP group, the mean numerical pain scale was 3.9 at 6 hours post-operatively and 2.1 at 24 hours post-operatively. For the infiltration group, the mean numerical pain scale was 4.3 at 6 hours post-op and 3 at 24 hours post-op. Morphine consumption at 6 hours post-op was on average 0.4 mg/patient for the TAP group and 0.9 mg/patient for the infiltration group. Discussion/conclusion: Analgesia provided by ultrasound-guided TAP block for laparoscopic abdominal surgery appears to be identical to periorificial infiltration. However, the simplicity and reproducibility of ultrasound-guided TAP block gives it a definite advantage.
