Introduction: malaria remains a public health priority in Senegal, particularly in Tambacounda, where it is one of the main causes of child mortality. The objective of this work was to evaluate the care of children under 10 years of age hospitalized at the Tambacounda Health Center and the factors associated with recovery. methods: An analytical, retrospective, and descriptive cross-sectional study with exhaustive recruitment of children 0 to 120 months hospitalized at the Tambacounda reference health center for severe malaria (according to WHO criteria) between 1 January 2018 and 31 December 2021 was performed. Data collection was done through a questionnaire. Records, hospitalization records, and treatment records were the sources of collection. Data entry and analysis were performed on Epi Info 7.2 and R. Results: A total of 481 children hospitalized with severe malaria were recruited. The highest number of cases was recorded in 2018 (33.05%). In the four years of the study, peaks were always observed between October and November and the highest peak in November 2020 with 95 cases. The mean age was 65.64 months with a standard deviation of 29.28 months and a predominance of male (53.43%). The majority of people were admitted from the outpatient clinic (57.79%) and the rest (42.21%) on the recommendation of a peripheral health post. All hospitalized patients had a positive RDT and/or a positive thick drop. However, the sharp decline at admission or during hospitalization was positive in 93.80% of patients in our series, negative in 5.20% and not achieved in 1.00%. Seizures and severe anemia topped the list of signs of severity with 45.94% and 8.11%, respectively. In terms of evolution, for all hospitalized patients, there were 81.29% recovery, 10.19% referral to the Tambacounda regional hospital center for hospitalization, 4.99% death, 0.83% discharge and 2.70% unknown evolution. Introduction: The improvement of survival in patients with cancer and the expansion of therapeutic options have led to the emergence of a new profile of cardiotoxicity, specifically associated with antimitotic agents. Our study aimed to assess the incidence of chemotherapy-induced myocardial toxicity in patients with cancer. Patients and methods: We conducted a looking-forward longitudinal cohort study including all patients admitted to the Cardiology departments of Aristide le Dantec Hospital and Dalal Jamm National Hospital Centre for apre-chemotherapy check-up. The included patients did not undergo any pre-existing cardiopathy. Results: Over a period of two years ranging from January 2019 to December 2021, a total of 37 patients were included in the study. Notably, there was a female predominance (92%) with an average age of 49.7 years ± 13.69. Breast cancer accounted for 70% of the neoplasms. Laboratory findings revealed moderate anemia in 19 patients (51%). At inclusion, the left ventricle (LV) was of normal size (LV diastole at 44.46 ± 4.97 mm). The systolic function of the left ventricle was normal in all patients, with an average ejection fraction (EF) of 63.1% ± 5.80 and a mean global longitudinal strain (GLS) of −20.4% ± 2.58. The most commonly used agents were anthracyclines. During follow-up, 3 patients (8.1%) developed clinical symptoms of left heart failure, and LV dysfunction on echocardiography was observed in 5 (13.5%) patients, with a significant decrease in EF Conclusion: The incidence of cardiac toxicity is not negligible, hence the importance of early screening. Strain imaging is an essential tool that should be performed as part of the assessment before chemotherapy and re-evaluated during treatment.

Study of the Variation in Total and Reducing Sugars Contents According to the Variety and According to the Position Where the Fruit (mango) Was Harvested from Five mango Varieties Exploited in Senegal

In this study, the results show that total sugar contents depend on the variety and the position where the fruit (mango) has been harvested. This variation in total sugar contents is observed according to the following varieties: the Bk (Boukodiekhal) variety (bottom position 45 mg/100 mL) then Bk (intermediate and bottom position 40 mg/100 mL) and with the Dr (Diourou) variety the content according to the intermediate position is 63 mg/100 mL while the content according to its bottom position is 56 mg/100 mL. The same is observed for variety Knt (Kent). For the variety factor, we see that the Dr and Sl (Sierra Leone) varieties are the richest in total sugars. Compared to the variation in reducing sugar content, the effect of position is less pronounced. Nevertheless, the statistical results show that the contents vary according to the varieties. There is a very significant variation in reducing sugar content between certain varieties. The variety Kt (Keitt) (45 mg/100 mL) is the richest in reducing sugars but compared to the lower position of the variety Bk, they are statistically identical. Variety Knt remains the poorest in reducing sugars 20 mg/ 100 mL.

Study of the Biochemical Composition of Senegalese mango Varieties Intended for Export and Local Consumation

Biochemical characterization was carried out on five mango varieties grown in Senegal, two of which are intended for export (Kent and Keitt) and three to local consumption, on the one hand, and to national marketing, on the other. It can be seen that the flavanols composition is between “0.74 ± 0.03 mg/100 g” to “4.70 ± 0.33 mg/100 g” and the total polyphenols composition is “265.83 ± 33.70 mg gallic acid/100 g” to “834.29 ± 25.78 mg gallic acid/100 g”. This analysis shows that the variety Bk is richer in polyphenols followed by the variety Dr then Knt and finally the lowest proportions are noted with the varieties Kt and SL. The analysis of antioxidant activity shows us percentages ranging from “4.16% ± 2.68% inhibition” to “50.21% ± 3.91% inhibition”. It was noted that the SL and Knt varieties give the highest inhibition percentages of “50.21 ± 3.91” and “33.97 ± 0.36” respectively. Variety Bk gives the lowest percentage followed by variety Kt. The Dr variety is the richest in flavonoids “11.75 ± 0.27” followed by the SL variety “11.27 ± 0.10” and the lowest composition is noted in the Knt variety “1.91 ± 0.04”. For total sugars, the most important contents are found in the varieties: Bk “12.71 ± 0.32 mg/100 g” followed by Dr “11.5 ± 0.33 mg/100 g” and SL “9.86 ± 0.06 mg/100 g”. This time the variety Kt stands out with also important proportions. For quantification, there is a slight difference in the values obtained for the different varieties studied. However, the highest values are observed in local varieties: “0.36 ± 0.06 g/100 g” for Bk and Dr followed by SL “0.31 ± 0.01 g/100 g”. The study of the mineral composition shows that the local varieties are the richest and the Knt variety is the poorest. The difference in composition noted within the same variety can be explained by the fact that some varieties have an advanced maturity compared to others.

Sexual and Urinary Disorders after Treatment of Rectal Cancer by Radiotherapy and Surgery at the Dantec University Hospital of Dakar

We performed a descriptive retrospective study of sexual and urinary disorders after treatment of rectal cancer by radiotherapy and/or surgery at the Dantec University Hospital in Dakar from 2008 to 2015. The objective of the study was to evaluate these sexual and urinary complications and the factors influencing it. We have collected 50 patients. The average age of is 55.7 years with a sex ratio of 0.78. The dominant clinical signs are rectorrhagia (66.0%). Endoscopy (94.0% of patients) showed an ulcerative-budding appearance in 84.0% of cases. The preferred location was the lower rectum 66.0%). The predominant histologic type is adenocarcinoma lieberkunien (82.0%). Computed tomography is performed in 78% of cases and mRI in 30%. Stage III accounts for 70.0% of cases. Thirty-two patients (64.0%) were treated with conventional 2-beam 2D radiation therapy with or without chemotherapy. The total dose of 46 Gy in 23 sessions was the most used, found in 22 patients;30 Gy in 10 sessions in 9 cases. And 16 Gy in 10 sessions, found in 1 case. Surgery performed was abdominoperineal amputation (58.0%) and conservative surgery (42.0%). We note a complete response in 28.0% of patients;8.0%, an increase of 16.0% and a stabilization of 4.0%. The sexual disorders are more important after radiotherapy compared to non-irradiated patients: 31.3% vs 5.6% (p = 0.035). We observe respectively that 2%, 6% and 8% of our patients had urinary disorders in the form of acute retention, urinary incontinence, and urinary burning. Patient follow-up time was between 0 and 42.83 months with an estimated average of 34.9 ± 3.37. The evolution is marked at 6 months by a persistence of sexual disorders in 63.8% of cases and urinary dysfunction in 4% of cases.

Chronic Kidney Disease in Sub-Saharan Africans: A Study of 462 Patients

Chronic kidney disease is a global public health problem due to its increasing prevalence as well as its main risk factors such as hypertension and diabetes. However, in Africa, few studies have been done on chronic kidney disease. The aim of our study is to describe the epidemiological, clinical, paraclinical and therapeutic aspects of chronic kidney disease. It was a retrospective and descriptive study carried out from the first of January 2004 to the 31st of December 2013 at Principal hospital in Dakar. Records of any patient aged 18 and over with chronic kidney disease were included. Chronic kidney disease was defined according to the KDIGO 2012 recommendations. Among the 8873 patient records used during our study, 462 presented with chronic kidney disease, which was a hospital prevalence of 5.2%. The sex ratio was 1.61. The mean age of the patients was 58.37 ± 19.97 years. There were 75.32% of the patients who were aged 50 and over. The mean serum creatinine was 49.14 ± 56.83. The mean glomerular filtration rate was 27.47 ± 19.86 ml/min/1.73 m2. Chronic renal failure was diagnosed in 92% of patients, including 34.9% in the end-stage of the renal disease. The mean proteinuria was 3.07 ± 4.92 g/24 h. Leukocyturia was present in 34.17% of patients. Hematuria was present in 25% of patients. Hypertension and diabetes were the most common causes, found in 61.25% and 35.93% of patients, respectively. Hemodialysis was performed in 49 patients. Peritoneal dialysis was performed in 2 patients. One patient had undergone a kidney transplant. This study establishes the relatively high prevalence of chronic kidney disease and its risk factors including hypertension and diabetes. Radiotherapy of Esophageal Carcinomas at the Dantec University Hospital of Dakar from 2010 to 2015

We performed a descriptive retrospective study of radiotherapy of esophageal carcinomas at the Dantec University Hospital in Dakar from 2010 to 2015. The aim was to describe the type of patient, and to evaluate the indication and the results of the irradiation. We consulted 117 cases and selected 26 cases. The average age of patients is 46.1 years (Extremes 17 - 83 years and peaks 30-39 and 50 - 59 years). The sex ratio is 1.67. Smoking was reported in 38.4% of cases and alcoholism in 15.3%. Dysphagia is the predominant sign (100% of patients). The general condition is classified WHO 1 for 42.3% of cases. The tumor was localized in 61.5% in the lower third and is stenosing in 84.6% of our patients. Squamous cell carcinoma is the most common type (88.5% of cases). The size of the tumor, specified in 50% of cases, varies from 2.6 to 11.2 cm. Computed tomography is performed in more than 50% of cases. Endoscopic ultrasonography is not performed and surgical exploration has contributed to the staging of 7.7% of irradiated patients. The tumor is localized in 47.8% of cases;at an advanced stage in 52% and is metastatic in 4.3%. We note a transhiatal excision and a gastrostomy in 15% and 69% of the cases and no surgery of excision in healthy margin. Chemotherapy or neoadjuvant radiotherapy is not performed. The radiotherapy was: exclusive (11%), adjuvant after surgery in margin R2 (15%), palliative in 3 × 5 Gy per 3 week (4%) and concomitant with chemotherapy cisplatyl 5FU (70%). The spread ranges from 2 to 6 weeks. The partial and complete response rate is 38.4% and 11.5%. The stabilization is 30.7% and the progression is 19.4%. Grade I-2 toxicity was noted in 46.1% (asthenia, radiodermatitis, cough) and grade 3 radiodermite in 4%. The average follow-up time is 198.19 days with extremes of 2 days and 32 months.

Nasosinus metastases from Breast Cancer: A Review of Two Cases

Context: Breast cancer affects more than 2 million women a year worldwide. Improved treatments have made it possible to increase survival rates with more and more patients having time to develop secondary locations. Nasosinus metastases from this cancer are rare. Two recently treated breast cancer patients developed metastases to the sphenoidal and maxillary sinuses. These cases gave rise to an examination of the incidence of this disease, its diagnosis, the therapeutic means envisaged and the fate of these patients. Method: We report the diagnostic and therapeutic data of two patients suffering from breast cancer and presenting metastases at the nasosinus level, with a focus on the interest of radiotherapy in palliative care. Results: The two patients aged 56 and 60 presented in the course of their breast cancer secondary localizations in the sphenoidal and maxillary sinus confirmed by endoscopy and histology. All the cases presented massive lesions associated with other secondary bone locations in particular. After local radiotherapy and systemic chemotherapy treatment, one patient died 5 months after her diagnosis and the other was alive 16 months later. Conclusion: Nasal and sinus metastases from breast cancer are rare and have a poor prognosis as disseminated disease, they do not respond well to conventional systemic therapies even if palliative radiotherapy allows local control.

background: Radiation therapy is an essential treatment for rectal cancer. In 2018, Senegal experienced a transition from two-dimensional to three-dimensional radiotherapy (RC3D). We are evaluating for the first time the impact of this RC3D in the treatment of rectal cancer in Senegal. Objective: To describe the epidemiological, clinical and diagnostic profile of rectum cancer in Senegal, to list the different radiotherapy techniques used, to evaluate the response as well as the outcome of patients. Method: We conducted a retrospective descriptive study on preoperative radio-chemotherapy of rectal adenocarcinoma at the Dalal Jamm University Hospital in Dakar from July 2018 to June 2020. Resultsmily:Verdana;">: The 42 patients included, including 18 men and 24 women (Sex Ratio: 0.75), had an average age of 55.9 years. We found a family history of CRC (Colorectal cancer) in 7 cases and smoking in 5 cases. The clinical signs were dominated by rectal bleeding, noted in 92.8% of cases, with or without pain, and changes in transit in 57.1% and 50% respectively. The tumor was perceptible on digital rectal examination (DRE) in 76.1%. All patients had undergone endoscopy, which revealed a budding aspect in 69% of cases. Aim: We endeavored to describe the epidemiological profile of prostatic cancer, and to evaluate its diagnostic and therapeutic aspects. Method: We conducted a descriptive retrospective study on the conformational radiotherapy of prostatic adenocarcinoma at the Dalal Jamm University Hospital, Senegal from June 2018 to December 2019. We included 62 consecutive patients. The average age of the patients was 68.9 years. The average consultation time was 7.6 months. Results: Symptom manifestation and high PSA triggered the diagnosis of cancer in 74.2% and 25.8% of patients, respectively. Symptoms: pollakiuria (72.3%), bone pain (8%) and erectile dysfunction (4.8%). The digital rectal examination: normal (33.8%), nodular (30.6%), induration (24.1%), and shielding (11.3%). The mean PSA level was 90.6 ng/ml. Histology: adenocarcinoma was the most common (98.6%). The Gleason score: 7 (19.4%). The majority of patients were in the high-risk group (70.9%) and 7 patients (11.2%) were metastatic at diagnosis. Therapy: first radical prostatectomy (20.9%), first-line curative radiotherapy (67.8%), adjuvant (21%) and palliative radiotherapy in 7 patients (11.2%): patients having received palliative radiotherapy had an estimated decline in symptoms of 80%. Hormone therapy was performed in 88.8% of patients, with average duration of 12.5 months.
