BACKGROUND Irritable bowel syndrome(IBS) is a common functional gastrointestinal disorder. Dysregulation of the gut–brain axis plays a central role in the pathophysiology of IBS. It is increasingly clear that the microbiome plays a key role in the development and normal functioning of the gut–brain *** To facilitate the identification of specific areas of focus that may be of relevance to future research. This study represents a bibliometric analysis of the literature pertaining to the microbiome in IBS to understand the development of this *** The data used in our bibliometric analysis were retrieved from the Scopus database. The terms related to IBS and microbiome were searched in titles or abstracts within the period of 2000–2019. VOSviewer software was used for data *** A total of 13055 documents related to IBS were retrieved at the global level. There were 1872 scientific publications focused on the microbiome in IBS. There was a strong positive correlation between publication productivity related to IBS in all fields and productivity related to the microbiome in IBS(r = 0.951, P ed States was the most prolific country with 449(24%) publications, followed by the United Kingdom(n = 176, 9.4%), China(n = 154, 8.2%), and Italy(n = 151, 8.1%). The h-index for all retrieved publications related to the microbiome in IBS was 138. The hot topics were stratified into four clusters:(1) The gut–brain axis related to IBS;(2) Clinical trials related to IBS and the microbiome;(3) Drugmediated manipulation of the gut microbiome;and(4) The role of the altered composition of intestinal microbiota in IBS *** This is the first study to evaluate and quantify global research productivity pertaining to the microbiome in IBS. A field study was performed at rivers in Gunung Jerai forest reserve(Kedah,Malaysia) to assess seasonal changes in mayfly community structure and abundance in relation to altitude and water *** at lower(Batu Hampar River) and higher(Teroi River) elevations were visited through dry and wet seasons in September 2007 to August *** visits were made to 20 sites on each river,and water and aquatic insects were sampled using D-pond aquatic *** was warmer,more acid,and more turbid in Teroi River during wet *** was the only nutrient exhibiting significant seasonal variations(greater during wet season).Chemical oxygen demand content was higher in Teroi River where biochemical oxygen demand content was low during wet *** richness was higher in Batu Hampar River,but displayed seasonal variations only in Teroi *** the eight families encountered,Baetidae was the *** abundance was usually high during wet season,and those belonging to the dominant genus(Baetis) were more abundant in Teroi *** was the second commonest family;its predominant genus,Thalerospyrus was more abundant in Teroi River during dry ***,Leptophlebiidae and Oligoneuriidae were only found in Batu Hampar River where their abundances peaked during dry season,i.e.,Habrophlebiodes *** Isonychia *** and Teloganodidae occurred only in Teroi River,with the first found only during dry *** were recorded under very distinct physicochemical conditions,illustrating their potential usefulness for assessing water ***,leptophlebids,oligoneurids ephemerellids and teloganodids seem to be particularly sensitive to temperature,acidity,turbidity,chemical oxygen demand and biochemical oxygen demand,parameters that varied with river altitude.

Prognosis of Breast Cancer Patients Underwent Surgery in a Developing Country

Background: We aim to review different factors associated with the relapse and the mortality in breast cancer patients in restricted-resource settings over a five-year period. Method: A retrospective cohort study including 133 women underwent breast cancer surgery was performed at the Surgical Oncology Unit of the Conakry University Hospital in Guinea. Socio-demographical characteristics, clinical information, treatment and data on relapse and survival were retrieved from medical files. Predictor factors of relapse were analyzed by using univariate and multivariate logistic regression. Factors associated with mortality were analyzed by Kaplan Meier survival and Cox models. Results: Breast cancer surgery was conservative in 13 cases (9.8%) and radical in 120 cases (90.2%). Five-year relapse was 33.8% ± 8.0% and the predictor factors of this relapse were age and stage III (p = 0.005). The overall 5-year mortality was 42.1% and the risk factors independently associated with death were body mass index (p = 0.01), origin of patients (p = 0.02), radiotherapy (p = 0.01) and cancer relapse (p = 0.001). Conclusion: The proportions of relapse and death were higher in breast cancer patients. The age and the stage of cancer at the time of surgery were the predictor factors of relapse. Body mass index, origin of patients, radiotherapy and cancer relapse were associated with death.

Lipid metabolism disorders would be among the components responsible for the risk of the onset of T2DM and its vascular complications. Apolipoprotein E plays an important role in lipid metabolism. We studied the involvement of the APOE gene in the onset of T2DM and its vascular complications. Clinical and biochemical parameters were assessed in each participant. APOE genotypes were identified by PCR-RFLP. Arterial stiffness was studied using a pOpmetre® which evaluates the pulse wave velocity (ft-PWV). Endothelial dysfunction was studied using an EndoPAT2000® which measures endothelium-dependent vasodilation (RHI). In control subjects, the ε3 allele was associated with an increase in fasting blood glucose (r = 2.36, p = 0.018), and a decrease in LDL cholesterol levels (r = −2.17, p = 0.03), and ε4 was associated with an increase in total cholesterol (r = 2.59, p = 0.01), LDL cholesterol (r = 2.84, p = 0.004), and No-HDL cholesterol (r = 2.74, p = 0.006). In type 2 diabetes subjects, the ε2 was associated with a decrease in diastolic blood pressure (r = −2.25, p = 0.02). The ε3 was associated with a decrease in ft-PWV (r = −2.26, p = 0.024) while the ε4 was associated with an increase in ft-PWV (r = 2.52, p = 0.012). Carrying the ε2ε3 genotype would have in 99% a limited risk of developing T2DM, and in event of T2DM, only 1 to 2% would have a significant risk of developing atherosclerosis, which would be severe in 17%. Of the ε2ε4 genotype, 93% had a limited or even possible risk of developing T2DM, the remaining 7% had a very high risk of developing T2DM. Diabetics carrying ε2ε4 had in 7% very high risk of developing atherosclerosis. The latter had a 20% very high risk of being very severe. Subjects carrying the ε3ε4 genotype had a 67% possible or even probable risk of developing T2DM and in the event of diabetes, there was in 34% very high risk of developing atherosclerosis which will not have even the time to evolve towards severity. Polygynous males can change their mating tactics across their lifetime, but information is scarce on the flexibility of this trait within a given season and the relative costs and benefits of using different tactics. Here, we monitored individually marked male reindeer Rangifer tarandus and classified their mating tactics as harem-defense, sneaking, or mixed. The costs of the male reproductive effort were assessed using both direct （i.e. percentage of body mass lost） and indirect measures （i.e. activity patterns such as feeding, standing, and walking）, while mating group size and reproductive success were recorded as mating ef- fort benefits. Our results show that reindeer males may switch between the harem-defense and sneaking tactics throughout the same breeding season, providing further support to the notion that reproductive tactics are flexible in ungulates. The costs and benefits of male mating effort vary according to the mating tactic, reaching the highest values in harem-holders and the lowest values in sneaking males. Moreover, males who switched between the sneaking tactic and the harem-defence tactic tended to achieve higher mating success than males who consistently used the least costly tactic. Indeed, all harem-holders successfully sired offspring, whereas only two out of three mixed-tactic males sired one calf, and sneaking males did not sire any calves. In conclusion, our results show that reindeer males can modulate their mating efforts during the same breeding season by switching between the most costly harem-defense tactic and the least costly sneaking tactic, suggesting individual solutions to the balance between reproductive effort and mating opportunities [Current Zoology 61 （5）： 802-810, 2015].

We present a 27-year-old female patient received for epistaxis associated with gingivorrhagia. In her medical history, she had undergone an oesogastroduodenal fibroscopy one year earlier, which revealed a bulbar ulcer with gastritis. On admission, she presented with a bleeding syndrome, an anemic syndrome with poor hemodynamic tolerance, and epigastric sensitivity. The blood count performed in the emergency on the citrate tube showed a bicy-topenia (regenerative anemia + thrombocytopenia). The HELIKIT test was positive. Thus, the hypothesis of an acute immunological thrombocytopenic purpura associated with a Helicobacter pylori infection seemed to us the most probable with a Khellaf hemorrhagic score of 8. She had benefited from a bolus of methyl prednisone: 15 mg/kg/day for 3 days then relay per os with prednisone 1 mg/kg/day (and adjuvant treatment);the eradicating treatment of Helicobacter pylori had been simultaneously started.
